Abstract:AIM:To investigate the effect of apical barrier and perforation repair with Mineral Trioxide Aggregate (MTA).METHODS:Eleven cases were carried out endodontic therapy.In which MTA was used to manage clinical problems.Seven cases were treated with MTA to develop apical barrier.Each case had a pulpless permanent tooth with blunderbuss open apex.The MTA plug was created under operative microscope.The root canal space was obturated with vertical condensation.The treatment required only one or two visits.In other four cases (five teeth) the perforation in furcation or root was repaired with MTA.Eleven cases have varying follow-up periods from six months to more than four years.RESULTS:In each case,MTA allowed bone healing and elimination of clilnical symptoms.CONCLUSION:MTA may be an ideal material for perforation repair and achieving apical seal in tooth with immature apex.
Clinical Efficacy of Mineral Trioxide Aggregate for Repair of Furcal Perforation in Molars

Abstract:Objective To evaluate the clinical efficacy of mineral trioxide aggregate (MTA) for repair of furcal perforation in molars. Methods Twenty- one molars with furcal perforation diagnosed between January 2005 and January 2008 were selected. In all molars mentioned above, there were 15 with iatrogenic perforation and 6 with carious perforation of pulp floor. The pulp chamber and root canals were cleaned, followed by obturation of the root canals and MTA filling of perforation which completed the coronal repair. The treatment outcomes were evaluated within 2 weeks and 2 years after the procedure. Results Within 2 weeks after operation, X-ray follow-up showed successfully repaired perforations in 17 teeth with dense-packing MTA in place. Failed repair was note in 3 teeth with carious perforation and 1 with iatrogenic perforation. Fifteen teeth were found with over-filling but without any clinical symptoms. Two years after operation, 11 out of 12 teeth were found well-obturated. Conclusions MTA can be an effective treatment of teeth with furcal perforation. Mild over-filling of the perforation does not affect the outcomes.

Abstract:To evaluate the clinical effect of repair of lateral root canal perforation with mineral trioxide aggregate (MTA).18 teeth with root canal perforation were selected. The root canals were shaped and cleaned, and then obturated with gutta-percha before or after MTA was used to repair the perforation.The postoperative X-ray films showed that the perforation of 17 teeth was repaired well, except one tooth with large and infected perforation. 10 cases showed a little over-filling of MTA. 16 patients were recalled and declared their teeth to be asymptomatic. The recall radiographs indicated that the apical radiolucent areas of teeth with pre-existing apical lesion decreased apparently or disappeared completely. No new radiolucency was found.MTA is effective in treatment of teeth with root canal perforation, even though the material is over-filled. Immediate and mediate furcal perforation treatment in primary molars: 24-month follow-up

Abstract:Background: Furcal perforation consists of a communication between the pulp cavity and the periodontal tissues that can occur accidentally during cavity preparation or root canal instrumentation. This complication may lead to inflammatory reactions and compromise adjacent tissues irreversibly, resulting in the early loss of primary teeth. Perforation sealing with a biocompatible material is necessary to ensure a favourable prognosis. Among different materials, MTA has been employed due to its excellent healing and physical properties. Case report: The purpose of these case reports was to describe immediate and mediate cases of furcal perforation treatment using MTA in primary molars. In both the cases, after accidental perforation detection, the pulp chamber was cleaned with saline solution to clear off the debris. The wound surface was continuously irrigated with saline solution and a dry sterile cotton pellet was placed on the radicular pulp stumps and perforation area with slight pressure until the bleeding was controlled. The MTA was applied into the perforation and pulp chamber area with the aid of a sterile amalgam carrier. The teeth were restored with resin-modified glass ionomer cement. Follow-up: Clinical and radiographic follow-up examinations were carried out at 3, 6, 12, 18, and 24 months after perforation sealing. Both the cases remained asymptomatic, presenting no discomfort, mobility, swelling, or fistula after 24-months follow-up. Radiographically, the radiolucent area disappeared as a result of bone formation in the inter-radicular space, showing adjacent tissue preservation. Conclusion: Thus, MTA may be considered as an ideal option for conservative treatment of immediate and mediate furcal perforation in primary teeth, once it promoted repair with tooth maintenance in both the cases.
Comparison of the Clinical Effects of GIC、amalgam and MTA on Pulp Chamber Perforation Repair

Abstract:Objective The purpose of this study was to investigate the clinical effects of GIC、amalgam and MTA in pulp chamber perforation repair.Methods Cases were divided into three groups randomly.In total,sixty-two cases were involved in our experiment, with MTA group 21 cases,GIC group 22 and amalgam group 19.All of the cases received the pulp chamber perforation repair with one of the three materials and perfect root canal therapy,and after a 3 months' follow-up,the clinical effects were evaluated.Results The type of the materials used had significant effects on canal perforation repair.MTA was more effective than GIC and Amalgam statistically (P0.05).Conclusions MTA is a more suitable material than GIC and amalgam in pulp chamber perforation repair.

Abstract:Objective To observe the effect of mineral trioxide aggregate(MTA) in repairing pulp cavity perforation by using oral surgical microscope.Methods A total of 37 patients of 19~57 years old with pulp cavity perforation were studied.MTA was used for repairing perforation area treatment.Regular follow-up clinical evaluation included clinical symptoms,the degree of tooth looseness,and alveolar bone absorption around perforation area.Results In the observation after 12 months,all cases exhibited normal chewing function without any complaint.In X-ray film,the areas of previous alveolar bone lesion decreased.Conclusion MTA used in pulp cavity perforation restoration has accurate positioning,better able to close perforation,and prompt alveolar recovery.MTA may be one kind of ideal biomaterials for repairing perforation in dental cavity.

Abstract:Background/objectives: Mineral trioxide aggregate (MTA) is widely recognized as one of the most biocompatible materials for perforation repairs during root canal treatment (RCT). Experimental evidence has consistently demonstrated MTA's superior sealing ability and biocompatibility compared to various dental materials, including amalgam, intermediate restorative material, zinc oxide eugenol cement, and resin-modified glass ionomer cement. This study aimed to assess the efficacy of MTA as a reparative material in iatrogenic furcal perforations during RCT. Materials & methods: A descriptive cross-sectional study was conducted from May 18, 2021, to November 17, 2021, at the Department of Operative Dentistry, Nishtar Institute of Dentistry, Multan, Pakistan. Seventy-six patients aged 18-60 years, of both genders, who developed iatrogenic furcal perforations during procedures were included. Patients with fractures or endo-perio lesions identified during clinical and radiographic examinations were excluded. Isolation was achieved using a rubber dam. The perforation site was cleaned and irrigated with 1% sodium hypochlorite to control hemorrhage and enhance visualization. Following the manufacturer's recommendations, the perforation site was sealed with MTA mixed with sterile saline. Results: The age range in this study was 18 to 60 years, with a mean age of 42.09 ± 9.69 years. Most patients (56.78%) were between 41-60 years old. Out of the 76 patients, 46 (60.53%) were male, and 30 (39.47%) were female, resulting in a male-to-female ratio of 1.5:1. The study found that MTA's efficacy as a reparative material in iatrogenic furcal perforations was observed in 61 (80.26%) patients. A 6-month follow-up revealed no periodontal ligament breakdown, demonstrating the efficacy of MTA as a reparative material in iatrogenic furcal perforations. Conclusion: This study concludes that the efficacy of MTA as a reparative material in iatrogenic furcal perforations is remarkable and significant.

Abstract:Background: To evaluate and compare the sealing ability of Mineral trioxide aggregate (MTA), BiodentineTM and light cure MTA used for the repair of furcal perforations using scanning electron microscope (SEM).Material and Methods: The study sample comprised 45 extracted mandibular molars. The teeth were embedded in modeling wax. Standard access cavities were prepared in each tooth using a round bur and non-end cutting bur with a high-speed handpiece with water spray. Furcal perforations were made on the centre of the pulpal floor of each tooth using a 0.5mm round bur. The teeth were then randomly divided into 3 experimental groups of 15 specimens each based on the materials used to seal the perforation; Group A: MTA; Group B: BiodentineTM; Group C: Light cure MTA. All the sealed perforations were compacted with a moist cotton pellet, and the samples were stored in a closed container for 24 hours to allow the repair materials to set completely. After 24 hours the samples were sectioned longitudinally and the extent of marginal adaptation was measured using scanning electron microscopy. The sealing ability was evaluated by measuring the gap (in microns) between the pulpal floor and the material used for the furcal repair.Results: The overall results showed that the marginal adaptation of light-cure MTA was better than both MTA as well as Biodentine. The mean space between the pulpal floor and the repair material was least for group C (2.29). Tuckey's post hoc test showed that a significant difference (<i>p</i><0.05) existed between group C and group A & B.Conclusions: Light-cure MTA exbibits good sealing ability to dentin when compared to conventionally used Biodentine and Mineral Trioxide Aggregate. <b>Key words:</b>Calcium silicates, MTA, Biodentinte, New light-cure MTA, Perforation repair.

Abstract:Objective To explort a better matreial of repairing perforations of pulp floor.Methods One hundred forty-six teeth with perforations of pulp floor were divided into LCH group(n=22)、LCR group(n=20)、HA group(n=20)、LGIC group(n=38)and CPC group(n=46).All were treated with root canal therapy and the perforations were repaired with light-cured calcium hydroxide,light-cured composite resin,hydroxylapatite,light-cured glass ionomer cement and calcium phosphate cement.All treated teeth were followed up for 18~24 months.Results The differences of efficiency were statistically signficant among five groups(P0.01).The efficiency both LGIC and CPC groups were all superior to LCH,LCR and HA groups(P(0.01)),and the differences of efficiency between LGIC and CPC groups was not statistically signficant(P(0.05)).The differences of efficiency were not statistically signficant among LCH group,LCR group and HA group(P0.05).Conclusion Calcium phosphate cement and light-cured glass ionomer cement are considered to be a better material for repairing perforation of pulp floor.
The use of mineral trioxide aggregate in endodontics

Abstract:Mineral trioxide aggregate (MTA), composed mainly of tricalcic silicate, tricalcic alluminate, bismuth oxide, is a particular endodontic cement. It is made of hydrophilic fine particles that harden in the presence of dampness or blood. It is biocompatible, radiopaque and it is harder to infiltrate, compared to classic materials for root filling such as amalgam, cements, Super-EBA, and IRM. and SEM studies of sections and copies in resin of root neoapices filled with amalgam, IRM, Super-EBA and MTA, as well as tests of microinfiltration have shown that MTA has excellent sealing capacities. It requires a working time of about 5 min and a hardening time that varies from 2 h and 45 min to 4 h according to the density of the air entrapped during mixing and the dampness of the receiving site. The long hardening time reduces internal tensions and the incidence of marginal infiltration, but it forces to definitively fill the tooth in the following sitting, with an interval of at least 3 days from the MTA application. Clinical experience shows how MTA is a material of choice in cases not only of endodontic surgery, apicectomy and retrograde filling but also in the sealing filling of perforations of the pulp chamber and of the root, stripping, internal reabsorptions, readaptations, lacerations, and apical transports. It has been used with success also in direct cappings and in apexifications instead of calcium hydroxide, leading to quicker therapies and more predictable Results: The authors outline the operative phases of the different treatments proposed, make a survey of the most important studies published so far and hope that a new sealing cement with more reduced hardening times will soon be available.
[Study of Molar Furcal Perforation Repaired with Iroot BP].

Abstract:OBJECTIVE:To evaluate the effect of furcal perforation treated with iRoot BP.METHODS:Forty newly extracted human maxillary and mandibular molars were collected. Holes in diameter of 2 mm were prepared at the roof of the central pulp cavity of 36 samples with high speed round ball diamond in order to make perforated model, and then equally and randomly divided them into two groups. The other intact four samples were used as the negative controls. In the experimental groups, group A were repaired by iRoot BP, group B were repaired by mineral trioxide aggregate(MTA). Three samples in each group were observed under scanning electron microscope(SEM), and microleakage of others was tested by measuring the concentration of leaked glucose with the enzymatic glucose oxidase-peroxidase (GOD-POD) method.RESULTS:Microleakage of group B was higher than group A and had statistical difference (P<0.05). The dentin and iRoot BP contacted more closely in group A, some gaps existed between the dentin and MTA in group B.CONCLUSION:For repairing furcal perforation, the effect of iRoot BP is better than that of MTA.

The outcome of perforation sealing depends greatly on the tissue compatibility and bioactivity and sealing properties of the repair materials. Mineral trioxide aggregate (MTA) and Biodentine are currently the most used materials to treat this condition. The present systematic review aimed to report the treatment outcome of repaired furcal perforation using MTA and Biodentine and identify which material would yield a better outcome. Methodology: A comprehensive search was conducted using the PubMed database to identify experimental studies and case reports that describe treatment of furcal perforation. Studies and case reports that evaluated the outcome of repaired furcal perforations using MTA and Biodentine, published in English from 2018 to April 2022, were identified. Unavailable full texts were excluded. Results: Initial screening of 724 articles (670 studies and 54 case reports). After discarding the duplicated studies, we reviewed 50 studies, selecting 13 for abstract analysis. We retrieved and evaluated full texts of eight studies and five case reports. Both materials had an equivalent success rate in the first three months but by 12 months Biodentine performed better than MTA clinically and radiographically. Conclusions: Repair of furcal perforation with Biodentine yields a better outcome compared to MTA.

A clinical evaluation of MTA barrier for root?end closure of open apex teeth in patients

Abstract:Objective To evaluate the clinical efficacy of mineral trioxide aggregate(MTA)as an apical barrier material when used in teeth with incomplete root formation. Methods Twenty?eight teeth with open apices in 28 patients were treated by standard endo?dontic procedures.An absorbable collagen sponge and MTA apical barrier were placed in the apical portion of the root canal under dent?al operating microscope(DOM)after a calcium hydroxide intracanal dressing had been applied for 1 week. The remainder of canal was back?filled with AH plus sealer thermoplastisized Gutta?Percha 1 day following MTA placement and access cavity was restored with composite resin followed by all crowns. Follow?up evaluations were carried out clinically and radiographically at 6?month and 1?year in?tervals. Results Sixty?five percent of these teeth were successful at 6?month appointment. Ninety?one percent of these teeth were suc?cessful at 1?year appointment. Periapical index scores at 6?month and 1?year recall appointments were improved. Chi?square test indica?ted that successful cases in recall times did significantly influence clinical outcome. Conclusion These results indicate that the mineral trioxide aggregate apical barrier technique is a successful method for treating teeth with immature apices.
The Management of Root Perforation: A Review of the Literature

Abstract:Root canal perforation is defined as an opening or communication of the root to the outer oral tissue that occurs either accidentally during root canal therapy or due to unknown pathological causes, which generally result in complications in patients due to the passage of microorganisms to the surrounding tissues. These complications may necessitate extraction of the affected tooth. In this literature review, 42 articles were reviewed initially; 33 of them were included in the final analysis based on exclusion and inclusion criteria. Root perforation may occur due to caries, external and internal resorption, trauma, as well as iatrogenic causes. Many studies have suggested that perforation can be identified by radiographs, unexpected bleeding and pain during instrumentation, blood on paper points, electronic apex locator, and dental operating microscope. Several materials have been used in endodontic perforation repairs, such as indium foil, super 2-ethoxybenzoic acid (super EBA), as well as bioceramic materials like mineral trioxide aggregate (MTA) and biodentine. These materials must achieve biocompatibility, bacteriostatic ability, and radio-opacity. Many studies have reported the superior properties of MTA in comparison with other materials. Based on our literature review, management of root perforation should be performed based on multiple significant factors. Clinicians must possess a comprehensive understanding of tooth anatomy to prevent perforation occurrence. Furthermore, clinical management and prognosis of root perforation have been enhanced by the availability of advanced materials and sealing techniques.
Fracture Resistance of Molars With Simulated Strip Perforation Repaired With Different Calcium Silicate-Based Cements

Abstract:Background and aims A root perforation is a connection between the root canal system and the external supportive tissues. Strip perforation (SP), occurring within root canals in a strip, can worsen the prognosis of a treated tooth, reduce its mechanical resistance, and impair the tooth structure. One of the suggested methods to treat SP is to seal it with a bio-material such as calcium silicate cement. Therefore, this in vitro study aimed to assess the molar structure impairment due to SP, which requires studying the fracture resistance, and the ability of mineral trioxide aggregate (MTA), bioceramic, and calcium-enriched mixture (CEM) to repair this perforation. Materials and methods Seventy-five molars were instrumented to size #25 and taper 4%, irrigated with sodium hypochlorite and ethylenediaminetetraacetic acid (EDTA), dried, and then divided randomly into five groups (G1-G5): in G1, root canals were filled with gutta-percha and sealer (negative control sample), whereas the rest of the groups (G2-G5) had a manual simulated SP made with Gates Glidden drill at the mesial root of the extracted molar, and filled with gutta-percha and sealer up to their perforation area; in G2, SP was filled with gutta-percha and sealer (positive control sample); G3 used MTA to repair the SP; G4 used bioceramic putty; and G5 used CEM. Fracture resistance tests of the molars were conducted in the crown-apical direction using a universal testing machine. One-way ANOVA test and Bonferroni test were used to study the significance of the differences in the mean values of the tooth fracture resistance, where statistical significance was set at 0.05. Results The ANOVA test showed that there were statistically significant differences between the fracture resistance (in newtons) values among groups (p = 0.000). The Bonferroni test showed that G2 had a smaller fracture resistance mean than the other four study groups (656.53 N; p = 0.000), and that of G5 was smaller than G1, G3, and G4 (794.40 N, 1083.73 N, 1025.20 N, and 1034.20 N, respectively; p = 0.000 in each pairwise comparison). Conclusion SP reduced the fracture resistance of endodontically treated molars. SP restored using MTA and bioceramic putty was better than that treated with CEM and similar to molars without SP. Moreover, MTA and bioceramic putty enhanced the fracture resistance of endodontically treated teeth to levels similar to molars without SP.

Abstract:PURPOSE:To evaluate the clinical effect of treatment of teeth with open apices using mineral trioxide aggregate (MTA) as apical barrier under surgical operating microscope.METHODS:The root canals of 23 teeth with open apices were cleaned and shaped, and disinfected with calcium hydroxide. The MTA mixture were then carried into the apical part of the root canals to create a 3-5 mm apical barrier. The remainder of the canals were filled with AH plus and Obtura II gutta-percha. The revisit times, the period and result of the treatment were recorded for all cases.RESULTS:The treatment of all cases was accomplished within 2 or 3 weeks. Postoperative X-ray films showed that the canals of 16 teeth were obturated very well. 7 teeth showed 1-2 mm over-filling. All recalled patients declared their teeth to be asymptomatic. The recall radiographs indicated that the apical radiolucent areas of teeth with pre-existing apical lesion decreased apparently or disappeared completely. No new radiolucency was found around the roots.CONCLUSION:MTA is effective and quick in treatment of teeth with open apices, even thought there is slight over-filling of the material.

Abstract:This review article describes the methods and clinical recommendations for reinforcing traumatized anterior immature teeth with pulp necrosis treated with mineral trioxide aggregate (MTA) apexification. Traumatic injury can cause pulp necrosis and incomplete root formation in immature teeth. MTA apexification is the treatment of choice for necrotic immature teeth, particularly during the middle or late stages of root development. MTA apexification has a high success rate; however, failures due to cervical or root fractures occasionally occur. The risk of fracture is higher in immature teeth with thin root dentin, particularly those with external root resorption. Furthermore, the loading force from any parafunctional habit also increases fracture risk. Therefore, intra‐radicular reinforcement may be necessary after MTA apexification. In vitro, intraradicular restoration with a resin composite/core build‐up material or a prefabricated fiber post demonstrated better root reinforcement than root canal obturation materials (i.e., gutta‐percha and sealer). However, the root‐reinforcement effect of MTA orthograde filling in the entire root canal remains unclear. In vivo, the survival of fractured teeth with intraradicular restorations (resin composite/core build‐up material or prefabricated fiber posts) is extremely high. Moreover, the survival of teeth with gutta‐percha/sealer obturation or MTA orthograde filling and restoration with resin composite extending into the cervical third of the root canal approximately 1–2 mm below the cemento‐enamel junction is acceptably high. Based on this evidence, the remaining tooth/root structure and loading force should be carefully examined when considering intra‐radicular reinforcement of immature anterior teeth treated with MTA apexification.

Study on MTA combined with fibronectin as pulp capping agent

Abstract:AIM: This study was aimed to assess the effect of mineral trioxide aggregate (MTA) combined with fibronectin on reparative dentinogenesis and odontoblast differentiation, investigate the possible mechanism underlying traumatized pulp repair, which would facilitate the choosing of ideal pulp capping agents. METHODS: 192 teeth from 16 adult cats were used. Class V cavties were prepared on the facial surface. The pulps of cats were exposed mechanically and then capped directly with MTA, MTA combined with fibronectin, calcium hydroxide, fibronectin and starch as control. The pulpal tissue reactions were assessed by light microscopy at healing interval of 1 and 4 weeks.RESULTS: 1 week after pulp capping, no reparative dentin formation was observed in each group. 4 weeks after pulp capping, the dental pulps capped with MTA/fibronectin showed no or slight cellular inflammatory responses and odontoblast differentiation with tublar dentin bridge on the exposed sites. In MTA group, the dental pulps showed slight to moderate cellular inflammatory responses with osteodentin formation. In calcium hydroxide group, the pulps showed moderate to severe inflammatory responses with calcified matrix. In fibronectin and starch group, no dentin bridge formation was observed, the pulps showed severe cellular inflammatory responses.CONCLUSION: (1) MTA combined with fibronectin induced to form complete and tublar dentin bridge. In MTA group, most of the formed dentin bridge was irregular and osteodentin. (2) MTA combined with fibronectin is a bioactive pulp-capping agent and has a great worth in vital pulp therapy.

Abstract:Background: Perforation can occur while performing root canal treatment. Biodentine has been made utilizing MTA (mineral trioxide aggregate) as a base, and claims to improve its properties in aspects such as physical property, durability, and handling, as well as its many other applications in restorative dentistry. The chemical components of SDF (silver diamine fluoride) have been suggested to have the following benefits: fluoride aids in remineralization and prevention, silver salts induce dentin sclerosis/calcification, and exhibit germicidal actions. It works by occluding dentinal tubules thus helping in preventing microleakage. This study aims to focus on the use of SDF on Biodentine and MTA as perforation repair material using a confocal microscope. Methods : Mandibular molars will be selected for the study. The study will be divided into 4 groups: Group 1: Biodentine without application of 38% SDF; Group 2: Biodentine with the application of 38% SDF; Group 3: MTA Angelus without application of 38% SDF; and Group 4: MTA Angelus along with the application of 38% SDF. An artificial perforation 2 mm in diameter will be made directly in the middle of the pulpal floor. 38% silver diamine fluoride (FAMAGAIN) will be applied on the walls of the perforation according to the manufacturer’s instructions for the experimental groups (Groups 2 and 4). In the four groups, Biodentine will be applied in groups 1 and 2 and MTA will be applied in 3 and 4 (perforation repair material). To evaluate the sealing capacity of perforation material in this study, we will use the average values of coronal dye leakage at the perforation site and Escobar’s criteria, which are used to assess the infiltration proportions. Conclusions : It is expected that application of 38% of silver diamine fluoride on MTA Angelus and Biodentine may show better sealing ability than MTA Angelus and Biodentine alone.
