The Value of Lactate Level in Risk Stratification and Outcomes Evaluation in Patients with Acute Myocardial Infarction Referred for Percutaneous Coronary Intervention

Abstract:Objective To investigate the value of lactate(LA)level in risk stratification and outcomes evaluation in patients with acute myocardial infarction referred for percutaneous coronary intervention(PCI).Methods From July 2003 to June 2004,according to the admission LA level,92 myocardial infarction patients with ST elevation who underwent PCI were divided into 2 groups:LA-increase group with 50 cases(≥2 mmol/L)and LA-normal group with 42 cases(2 mmol/L).The clinical characteristics and major cardiovascular events(MACE)were analyzed with follow-up period of mean 7.2±2.9 months.Results The percentage of older patients and cardiac troponin I(TnI)level was higher(P0.01),and left ventricular ejection fraction(LVEF)decreased more significantly in LA-increase group than those in LA-normal group(P0.05).In LA-increase group,more patients had persistent ST-elevation 2 hours after PCI than those in LA-normal group(P0.01).Complications of myocardial infarction and total rates of MACE increased significantly in LA-increase group than those in LA-normal group(P0.05).Conclusion LA level at admission is associated with the increase in MACE(OR:7.62 95% CI:1.92-40.13,P0.05),and it is an another independent predictor of MACE besides patients with age 70 years or LVEF value.
