The impact of TVT-O and TOT for female stress urinary incontinence on sex

Abstract:Objective To evaluate the impact of TVT-O(tension-free vaginal tape obturator) and TOT(transobturator suburethral tape) for female stress urinary incontinence on sex. Methods 44 cases of female stress urinary incontinence were divided into two groups. The 44 women in the survey do not have medical record of severe overactive bladder, pelvic organ prolapse, hysterectomy or supraubc arch sling. 24 women aged between 40 and 67(52.5±6.6) years were in the group of TVT-O, among whom 20 had experienced sex before surgery. Other 20 women aged between 38 and 67(55.4±8.4) years in the group of TOT included 15 cases that had experience of sex. There were no statistical significance between differences of two groups in general information(P>0.05). Before surgery and one year after the surgery, the 44 female patients received the Pelvic Organ Prolapse/Urinary Incontinence Sexual Questionnaire short form(PISQ-12) and the result showed that the higher the score was, the better the quality of their sex life. Results The surgeries were successful and the comparison of the two groups' scores through PISQ-12 questionnaire showed that no statistical significance was observed between the two groups(P>0.05). 2 of 9 patients(20.45%) who had no sex experience before the surgery resume their sexual life while the other 35 patients(79.55%) were still sexual active after the surgery. The female patients who had sex experience were improved in their sex life by 35% through TVT-O and 40% through TOT and there was no significant difference between two groups in improvement rate(P>0.05). Among 16 patients who had urine leakage in sex life before the surgery, the scores of 9 cases(56.25%) rose while 4 patients among the 19 patients who had no urine leakage in sex life before the surgery got their scores rise and the rate differences between their sex improvement was statistically significant(P<0.05). Conclusion TVT-O and TOT for female stress urinary incontinence have no adverse effects to the patients' sex life; neither can it enhance their sex life. There's no significance difference in the effects that the two surgical treatments exerted to the patients' sex life. However, the quality of sexual life may be enhanced with the symptoms alleviated after surgery for the patients who had urine leakage in sex life.
