Abstract:Objective To investigate possible mechanism of neurogenic pulmonary edema(NPE) in patients with aneurismal subarachnoid hemorrhage(SAH).Methods 108 patients with aneurismal subarachnoid hemorrhage(male 51,female 57) were collected in our hospital from the March 2009 to March 2011,while 90 healthy patients were randomly included as control group. The concentrations of Catecholamine(CA) in plasma were measured by ELISA in both SAH group and control group. The correlation between CA and NPE were analyzed. The incidence of NPE and the plasma levels of Catecholamine were be compared between these subgroups including age, gender,Hunt and Hess grade,Fisher grade and the location of artery aneurysms in Willis arterial circle. Results ①The level of CA in SAH group was higher than that of control group(t = 7.314,P < 0.05).In the Hunt and Hess grade Ⅲ-Ⅴ grade group and the Fisher grade Ⅲ-Ⅳ grade group, there was a high incidence of NPE.The CA levels of the two group was 127.26 ± 37.10 μg and 124.08 ± 37.82 μg/L.②The level of CA in patients with NPE group was higher than that of patients without NPE group(P < 0.01).When aneurysm located in posterior circulation, the incidence of NPE(P < 0.05) and CA level(P < 0.05) were higher than that in anterior circulation. Conclusion Up-regulation the plasma level of CA induced the occurrence of NPE after aneurismal SAH.The severity of the disease and the position of aneurysm in Willis arterial circle may be the major factors which affected the level of CA in plasma and concurrent of NPE after SAH.
