Comparison of open surgery and ultrasound-guided percutaneous injection on the cytokine response to pig islet xenotransplantation

Abstract:Background: Currently, pig islet xenotransplantation into the portal vein of monkeys is mostly performed by open surgery. However, in the clinic, percutaneous transhepatic portal vein puncture under ultrasound guidance is the approach of choice. Our goal was to compare the cytokine inflammatory response associated with each method, in order to select the optimal method. Methods: Islet donors were 2 years-old female pigs of either the GTKO or GTKO/CD55 genotype. Recipients were 3-years-old rhesus monkeys. Monkeys underwent two operation methods: one is open surgery (GTKO) and the other one is percutaneous puncture into the portal vein under ultrasound guidance (percutaneous Tx,GTKO or GTKO/CD55). Both groups received the same immunosuppressive regimen (anti-CD154mAb, tacrolimus, rapamycin), and blood drug levels and immune cell numbers were monitored to ensure the same level of immunosuppression. Serum cytokines (IL-6, IL-8, IL-1β, IP-10, TNF-a, IFN-γ, and MCP-1) were measured at various time-points and compared between the two groups. Results: In open surgery, within 15min after transplantation, the recipient serum exhibited a high level of inflammatory cytokines, including TNF-α, IL-1β, and IP-10, with a substantial release of other inflammatory cytokines, e.g., IL-6 and IFN-γ. Within 30min, IL-17A exhibited a significant increase when compared to other cytokines. At 1h, the main cytokines released were IL-8 and MCP-1. Between 1-4h, all cytokine levels fell sharply to their lowest levels. Over the first week, all cytokines displayed a similar pattern of an initial increase and a subsequent decrease. In contrast, in percutaneous Tx, there was no notable variation in cytokine levels at different time-points. Furthermore, in percutaneous Tx, there was no significant difference in cytokine release related to pig genotype, except for IP-10, which was higher after GTKO/CD55 than after GTKO islet transplantation. Conclusions: Compared to the open surgery approach, ultrasound-guided percutaneous transhepatic portal vein puncture resulted in a milder inflammatory response following pig islet xenotransplantation in immunosuppressed monkeys. The reduction in inflammatory response may be advantageous for the survival of the islets.
