Radiologic development of cryptogenic organizing pneumonia during follow-up:an analysis of 23 cases

Abstract:Objective To conduct a retrospective analysis on the radiologic development during follow-up in patients who had cryptogenic organizing pneumonia(COP) treated or not treated with corticosteroids.Methods We recruited 42 patients with COP diagnosed clinically and radiologically in addition to laboratory examinations and biopsy from November 2004 to July 2011.The radiologic and clinical development during follow-up visits in 23 patients was analyzed.Results Twenty-three patients(12 males and 11 females) who were aged 51(range:16~80) years were followed-up for an average of 6.2 months(range:30 days~14 months).On initial presentation,common features of pulmonary anomaly on high-resolution computed tomography were multiple patchy attenuation,patchy air-space consolidation and reticulation.The consolidation lesions,distributed randomly or along with bronchial vessels,were mainly located in subpleural regions and bilateral lower lobes where ground-glass opacification,traction bronchiectasis and stripe-like opacities might be common while migratory lesions could be found in certain cases.Of 20 cases treated with corticosteroids,1 had complete resolve,17 presented diminished high-density images including consolidation and ground-glass opacification,3 showed decreased region of reticulation,1 did not show evidence of changes in reticulation,2 developed emerging reticulation and 2 relapsed.All three cases not treated with corticosteroids presented significantly diminished lesions.Conclusion Twenty cases(91%) with COP respond well to corticosteroids.The majority of patients(82%) have remaining foci on CT during follow-up.Certain corticosteroid-naive patients may resolve spontaneously.Follow-up chest CT may be useful to assessment of treatment outcomes and prognosis.
