The Measurement and Clinical Significance of Pro-hepcidin in Patients Having Renal Anemic Before and after Renal Transplantation

Abstract:Objective:Hepcidin(Hepc)was a central regulator of iron homeostasis recently confirmed by lots of fundamental and clinical research.To explore whether Hepc played a role in the pathogenesis of renal anemia and the effect of renal transplant on it,the patient's serum propeptide(pro-Hepc)and other hematologic parameter concentrations before and after renal transplantation were determined.Methods:Six patients with anemia from chronic renal failure(CRF)before and after renal transplantation and 30 normal individuals were studied,and also ELISA,radioimmunoassay and etc.were used for serum pro-Hepc,EPO,ferritin,serum iron,TIBC and other parameters concentration.Results:Serum pro-Hepc concentration before renal transplantation(352.75±109.5ng/ml)was lower than that after renal transplantation(806.88±438.73ng/ml,P0.05).Serum Hb,and Hct concentration before renal transplantation was lower significantly than that after renal transplantation,respectively(P0.01).Serum Urea,Crea concentrations before renal transplantation were higher significantly than those after renal transplantation.(P0.05,P0.01).During the observed time,the difference of the levels of EPO and iron parameters showed no statistical significance between before and after renal transplantation(P0.05).The serum pro-Hepc,iron,and TIBC concentrations before renal transplantation were lower than that of the control.The serum iron and TS concentrations after renal transplantation were lower than that of the control.The serum EPO and ferritin concentrations before and after renal transplantation showed no statistically significant difference between that of the control respectively.Conclusion:The changes of serum pro-Hepc concentration before and after renal transplantation showed that down-regulation of Hepc by anemia and hypoxia was stronger than the other up-regulation factors,this was probably the body's protective response and good for iron absorption and mobilization to compromise the iron need for erythropoiesis because when faced with the increased need for iron as anemia and hypoxia,and at the same time when faced with the decreased need for iron to defend infection,the body may be chiefly prone to the former.
