Reconstruction of Bone Defects Following Resection of Malignant Tumors in the Proximal Humerus

Abstract:Objective To explore the limb salvage strategy and the clinical outcome in reconstructing bone defects following malignant tumors resection of proximal humerus.Methods 22 patients underwent bone defects reconstructive procedures after intra-articular resection of malignant tumors in the proximal humerus.A variety of reconstructive procedures were performed after tumor resection.3 cases underwent reconstruction with reimplantation of tumor-bearing bone after alcohol devitalization,6 cases with free periosteum vascularized clavicle,5 cases with an osteoarticular allograft and 8 cases with endoprosthesis.Results 19 cases were followed up for 9~96(50.0±8.2)months,4 patients had local recurrence,8 patients died.According to Enneking function evaluation standard,the mean post-operative function score of the upper extremity was 22.8±1.4 points in reimplantation of tumor-bearing bone after alcohol devitalization group,23.9±1.5 points in osteoarticular allograft group,24.2±1.6 points in free periosteum vascularized clavicle group,26.1±1.8 point in endoprosthesis group respectively.Conclusions There are a variety of reconstructive procedures for the bone defects following resection of malignant tumors in the proximal humerus,the reconstructive method with endoprosthesis is the first choice for the adult patients,the reconstruction with an free periosteum vascularized clavicle is suitable for the children and young patients.More attention must be payed on the reconstruction of the rotator cuff and the abduction mechanisms.Most of patient can reserve the function of joint.
