A Prospective Study of 71 Pregnancy Patients with Systemic Lupus Erythematosus and Long-Term Follow-Up

Abstract:Objective To evaluate the risk factors of pregnancy for systemic lupus erythematosus (SLE) mothers and the fetus.Method Prospective study on pregnancy patients with SLE was carried out between 1988 and 1999 in Renji hospital,and the patients were divided into three groups:disease controlled over one year (group A),having disease activity within one year before pregnancy (group B) and new onset during the pregnancy (group C) according to the disease condition at the time of conception.Lupus activity as well as fetal outcome were observed during pregnancy and 6 months postpartum in different groups of patients followed by further long term regular follow up.Results There were total 71 patients enrolled among which 49 for group A,13 for group B and 9 for group C.One patient stopped the pregnancy due to severe lupus activity while another gave birth twins making the total 71 live infants with neither neonatal lupus nor maternal fetal death.In group A there were 9 SLE flares (18 4%),significantly lower than those in group B who had 10 flares (76 9%) and group C (100%).In group A,patients with new onset of lupus nephritis were significantly less than that of the other 2 groups ( P 0 05),and lupus nephritis activity months postpartum 6 was significantly less than that of group B ( P 0 05).The incidence of premature birth and low body weight at birth were significantly less in group A than in group B.In group A,there was no significant difference in the incidence of SLE flare during prenancy and abnormal fetal outcome between patients with past history of SLE visceral involvement and those without ( P 0 05).Conclusion Disease controlled for over one year before conception shows significant improvement of the maternal fetal outcome.The past history of SLE visceral involvement is not the additional risk for SLE patients′prenancy,provided that the disease has been quiescent over one year.SLE patients′pregnancy has a much higher maternal fetal risk if they have shown disease activity within one year before conception.
