Endoscopic Submucosal Dissection for Esophageal Lesions

Abstract:Objective: To approach the clinical value of endoscopic submucosal dissection (ESD) for esophageal lesions. Method: ESD was carried out with the needle knife as following: (1) Injecting fluid into the submucosa to elevate the lesion from the muscle layer, (2) pre-cutting the surrounding mucosa of the lesion,(3) dissecting the connective tissue of the submucosa beneath the lesion. Results: The 20 cases esophageal lesions were ranged from 0.5 to 7.0 cm (mean size 2.6 cm) ,18 of them were successfully resected with ESD and enbloc resection rate was 90.0% (18/20) . All 18 lesions were resected with lateral and basal resection margins free of tumor. The mean ESD procedure time (from fluid injection to complete dissection) was 45 minutes (ranged from 15 to 150 minutes) . Minor bleeding occurred in all of the lesions;1 patient had massive hemorrhage which could not be controlled with gastroscope and required balloon compression during ESD,the bleeding rate was 5.0% (1/20) . Subcutaneous emphysema occurred in one case during the dissection of the esophageal cyst,and recovered after several days' conservative treatment.The perforation rate of ESD was 5.0% (1/20) . 11 cases were followed up after ESD,with confirmed healing of the artificial ulcer with no residue or recurrence. Conclusion: ESD is a novel endoscopic surgical procedure to treat esophageal lesions,which makes it possible to resect whole large lesion and provide pathological information about the whole lesion.
