Abstract:Diabetic cerebrovascular disease is one of the common complications of diabetes .The collater-al disease is widely existed in the diabetic chronic complications .It is the important pathological mecha-nism of diabetic cerebrovascular disease .The basic pathogenesis is qi and yin deficiency in liver and kid-ney and with wind, fire, phlegm, blood stasis blocking the brain collateral .The basic pathological processes are qi deficiency of collaterals , the blood stasis resistance in the brain collateral , contraction of the brain collateral , blood stasis in brain collateral .To discuss the ideas about syndrome differentiation and treatment of diabetic cerebrovascular disease , the following points are set forth .The determination of etiologic factors should be based on differentiation .The reinforcing method and reducing method are com-bined together .The insect drugs should be made the most of to eliminate pathogens and drege collaterals . Abstract:Objective:To analyze the "disease-syndrome-treatment-medicine" experience of professor DING for diabetic peripheral neuropathy by using "Traditional Chinese Medicine(TCM) inheritance support system(V2.5)".By analyzing the data,we summarized the Professor DING's academic experience.Methods:Through collecting the standard clinical cases,combined with the software analysis system,we analyzed the composing principles by using datamining methods such as frequency statistics,revised mutual information,association rules analysis,cluster analysis,improvement of mutual information,complex system entropy clustering,unsupervised hierarchical clustering entropy and so on.Results:Characteristics of diabetic peripheral neuropathy:Based on the analysis of 31 cases of diabetic peripheral neuropathy internal wind syndrome.The highest-frequency used insect drugs were Scorpion,bombyx batryticatus,cicada slough.The highest frequency used drug combinations were folium mori and the root bark of white mulberry,scorpion and bombyx batryticatus,rhizoma coptidis,folium mori and the root bark of white mulberry,radices rehmanniae,bombyx batryticatus and scorpion and so on.Conclusion:Treating diabetic peripheral neuropathy focuses on wind.Extinguishing wind and dredging collaterals is important.

Abstract:Diabetic peripheral neuropathy (DPN) is one of the most common chronic complications of diabetes. Symptoms of DPN mainly include spontaneous intractable pain that is diffuse and continuous and can last from several weeks to several months. DPN is associated with a high mortality rate and poor prognosis. Its pathogenesis is not fully understood, and clinical treatment is focused on relieving its clinical symptoms, as well as improving blood sugar control and cardiovascular risk factors. DPN and its clinically effective treatments need to be studied. This study discusses the treatment methods and pathogenesis of DPN, summarizes the related research progress, and attempts to provide a reference for DPN research.

Abstract:Diabetic peripheral neuropathy (DPN) refers to the development of peripheral nerve dysfunction in patients with diabetes when other causes are excluded. Diabetic distal symmetric polyneuropathy (DSPN) is the most representative form of DPN. As one of the most common complications of diabetes, its prevalence increases with the duration of diabetes. 10-15% of newly diagnosed T2DM patients have DSPN, and the prevalence can exceed 50% in patients with diabetes for more than 10 years. Bilateral limb pain, numbness, and paresthesia are the most common clinical manifestations in patients with DPN, and in severe cases, foot ulcers can occur, even leading to amputation. The etiology and pathogenesis of diabetic neuropathy are not yet completely clarified, but hyperglycemia, disorders of lipid metabolism, and abnormalities in insulin signaling pathways are currently considered to be the initiating factors for a range of pathophysiological changes in DPN. In the presence of abnormal metabolic factors, the normal structure and function of the entire peripheral nervous system are disrupted, including myelinated and unmyelinated nerve axons, perikaryon, neurovascular, and glial cells. In addition, abnormalities in the insulin signaling pathway will inhibit neural axon repair and promote apoptosis of damaged cells. Here, we will discuss recent advances in the study of DPN mechanisms, including oxidative stress pathways, mechanisms of microvascular damage, mechanisms of damage to insulin receptor signaling pathways, and other potential mechanisms associated with neuroinflammation, mitochondrial dysfunction, and cellular oxidative damage. Identifying the contributions from each pathway to neuropathy and the associations between them may help us to further explore more targeted screening and treatment interventions.

Abstract:Through analyzing the pathological mechanisms and current treatment of some brain diseases belonging to the category of in Chinese medicine,the authors found that the pathological basis of brain collateral disease is the disorder of endothelial-glial cell information network consisted of cerebral microvascular endothelial cells,astrocytes and cytokines for transmitting information mutually.It is also a crosspoint of understanding on collateral disease in brain diseases by Western and Chinese medicine,and a pointcut of research on theory of brain collaterals.The study will give a more reasonable explanation on collateral disease in TCM brain diseases,and provide the proof for the kernel pathology aimed at by treatment of freeing collaterals.

Abstract:Objective:To observe the therapeutic effects of replenishing qi and nourishing yin,activating blood and dredging col-laterals on the treatment of early diabetic nephropathy (DN).Methods:A total of 58 DN patients conforming to the diagnostic cri-teria were randomly divided into the treatment group (29 patients)and control group (29 patients).Besides the routine therapy including controlling diet,lowing blood glucose and orally taking fosinopril,10 mg daily,the treatment group was given Chinese medicine of replenishing qi and nourishing yin,activating blood and dredging collaterals for 12 weeks.The changes in UAER,cur-ative effect of renal function,TCMsyndrome score and Ccr were observed in the patients.Results:Curative effect of renal func-tion comparison,the total effective rate of the treatment group was 82.76% and that of control group was 62.07%.Syndromes curative effect comparison,the total effective rate of the treatment group was 86.21% and that of control group was 58.62%.The effect of treatment group was better than the control group in lowing UAER and improving Chinese medicine Syndromes,the differ-ence was significant(P <0.05).There were no obvious side effects found during the treatment.Conclusion:The method of re-plenishing qi and nourishing yin,activating blood and dredging collaterals showed better therapeutic efficacy in the treatment of early DN.

Abstract:Foot ulceration is one of the most serious complications of diabetes mellitus and may lead to amputation of the lower extremity. Timely prophylaxis and treatment of diabetic foot ulceration are important to maintain a good quality of life. This article reports a complicated diabetic patient with severe limb-threatening necrotizing infection. We successfully applied endovascular stent insertion, digit amputation, negative pressure wound therapy, and advanced dressings in different wound phases to achieve definitive wound healing after 12 months of treatment. Based on this case report, we would like to emphasize the importance of combined multiple therapies and patient compliance for severe diabetic foot ulcers.

Abstract:Coronary collateralization is substantially impaired in patients with type 2 diabetes and occlusive coronary artery disease, which leads to aggravated myocardial ischemia and a more dismal prognosis. In a diabetic setting, altered serum lipid profiles and profound glycoxidative modification of lipoprotein particles induce endothelial dysfunction, blunt endothelial progenitor cell response, and severely hamper growth and maturation of collateral vessels. The impact of dyslipidemia and lipid-lowering treatments on coronary collateral formation has become a topic of heightened interest. In this review, we summarized the association of triglyceride-based integrative indexes, hypercholesterolemia, increased Lp(a) with its glycoxidative modification, as well as quantity and quality abnormalities of high-density lipoprotein with impaired collateral formation. We also analyzed the influence of innovative lipid-modifying strategies on coronary collateral development. Therefore, clinical management of diabetic dyslipidemia should take into account of its effect on coronary collateralization in patients with occlusive coronary artery disease.

Abstract:According to the self-discipline of traditional Chinese medicine, vessel-collateral theory was constructed systematically, which was important to improving prevention and treatment level of vasculopathy. The hypothesis of "homeostasis (Cheng), compensatory auto-adaptation (Zhi), regulation (Tiao) and equilibrium (Ping)" based on the "qi-yin-yang-five elements" coupled with the ying (nutrients)-wei (defense) theory, has become the core content of the vessel-collateral theory. Clinical and laboratory trials have been developed to further confirm the scientific connotations of the hypothesis, such as Tong Xin Luo capsule, as the representative drugs of vessel collateral theory, showed good efficacy in protecting the vascular endothelium, stabilizing the vulnerable plaque and reducing the blood vessel spasm. "Sou, ti, shu, tong" was the characteristics of Tong Xin Luo capsule in treating "microvascular damage" as the core mechanism of acute myocardial infarction, cerebral infarction and microvascular complications of diabetes. Shen Song Yang Xin capsules in the treatment of arrhythmia have made integrated adjustment advantage. Qi Li Qiang Xin capsules have been made treating both manifestation and root cause of chronic heart failure. These research have improved prevention and treatment level of major vascular system diseases.

Abstract:AbstractPeripheral arterial disease (PAD) increases the risk of lower extremity amputation. It is also an independent predictor of cardiovascular and cerebrovascular ischemic events, affecting both the quality and expectancy of life. Many studies have demonstrated that the prevalence of PAD in patients with diabetes mellitus (DM) is higher than in non‐diabetic patients. In diabetic patients, PAD occurs early with rapid progression, and is frequently asymptomatic. Multiple metabolic aberrations in DM, such as advanced glycation end‐products, low‐density lipoprotein cholesterol, and abnormal oxidative stress, have been shown to worsen PAD. However, the role of DM in PAD is not completely understood. The purpose of the present article is to review and discuss the pathophysiology of PAD in DM.

Abstract:Diabetic peripheral neuropathy (DPN) is one of the neurological diseases with high morbidity, and its course is migratory and progressive. Currently, the effectiveness of acupuncture therapy in the prevention and treatment of diabetes and its complications has been confirmed by clinical and basic research. Acupuncture treatment modality has the advantages of low cost, high safety, no toxic side effects, and good efficacy, and is easily accepted by patients in treatment. The authors have documented the inflammatory response, glycolipid metabolism disorders, oxidative stress, blood rheology and nerve conduction in function. Abstract:A diabetic foot is any disorder directly attributable to peripheral arterial disease (PAD) and/or the sensory neuropathies influencing the diabetic foot; it is a long-term (or "chronic") complication of diabetes. Generally, it is a permanent (or "chronic") comorbidity of diabetes. The swallowing of peripheral neuropathy and peripheral vascular disease leads to damage to the soft tissue, bone, and joint system of the foot. In addition, diabetic feet are also susceptible to infection, which can lead to lameness and even amputation. The management of the diabetic foot can be both challenging and chronic; it may comprise orthoses, surgery, antibacterial medication, and topical dressings. Traditional Chinese medicine (TCM) has a variety of methods for the treatment of diabetic foot with obvious advantages, which can be divided into internal treatment and external treatment of TCM. These treatments include external foot bath, fumigation and washing stains, internal administration of traditional Chinese medicine, syndrome differentiation and staging treatment, empirical prescription, self-made prescription, internal use of proprietary Chinese medicine, massage, acupuncture, and paste powder application, which have significant value in the prevention and treatment of diabetic foot. Some studies have shown that modern medicine combined with herbal therapy can significantly improve the prognosis of diabetic foot. Therefore, this study summarized the difficulties in the therapeutic approaches to diabetic foot and the recent research progress in combining Chinese and Western medicine in the treatment of diabetic foot.

Abstract:People with diabetes are prone to develop peripheral vascular and nerve abnormalities which, in extreme cases, can lead to limb amputations. Although numerous theories have been advanced for these complications, no firm explanation is yet available. Recently, evidence has appeared suggesting that these vascular and nerve abnormalities may involve transition metals; administration of chelators such as desferrioxamine has been shown to prevent or actually reverse slowed peripheral nerve conduction and neuronal blood flow, as well as impaired endothelium-dependent arterial relaxation. Here, we argue that (i) the heavily glycated proteins known to accumulate in people with diabetes gain an increased affinity for transition metals such as iron and copper, (ii) as a result, proteins such as elastin and collagen within the arterial wall-which are known to be particularly heavily glycosylated in diabetes-may accumulate bound metal, especially copper, (iii) the bound metal causes the catalytic destruction of endothelium derived relaxing factor (nitric oxide or a derivative thereof), thereby engendering a state of chronic vasoconstriction. The resulting impairment of blood flow to peripheral nerves restricts the delivery of oxygen and nutrients and, in extremis, nerve death eventuates. If this hypothesis is proved correct, there are important implications for the development of novel pharmaceuticals for the treatment of diabetic peripheral neuropathy.

Abstract:The extent of coronary collateral formation is a primary determinant of the severity of myocardial damage and mortality after coronary artery occlusion. Type 2 diabetes mellitus (T2DM) represents an important risk factor for impaired collateral vessel growth. However, the mechanism of reduced coronary collateralization in type 2 diabetic patients remains unclear. With the reference to the recent researches, this review article describes the pathogenic effects of T2DM on collateral development and outlines possible clinical and biochemical markers associated with reduced coronary collateralization in type 2 diabetic patients with chronic total occlusion (CTO). Diffuse coronary atherosclerosis in T2DM reduces pressure gradient between collateral donor artery and collateral recipient one, limiting collateral vessel growth and function. An interaction between advanced glycation end-products and their receptor activates several intracellular signaling pathways, enhances oxidative stress and aggravates inflammatory process. Diabetic condition decreases pro-angiogenic factors especially vascular endothelial growth factor and other collateral vessel growth related parameters. Numerous clinical and biochemical factors that could possibly attenuate the development of coronary collaterals have been reported. Increased serum levels of glycated albumin, cystatin C, and adipokine C1q tumor necrosis factor related protein 1 were associated with poor coronary collateralization in type 2 diabetic patients with stable coronary artery disease and CTO. Diastolic blood pressure and stenosis severity of the predominant collateral donor artery also play a role in coronary collateral formation. T2DM impairs collateral vessel growth through multiple mechanisms involving arteriogenesis and angiogenesis, and coronary collateral formation in patients with T2DM and CTO is influenced by various clinical, biochemical and angiographic factors. This information provides insights into the understanding of coronary pathophysiology and searching for potential new therapeutic targets in T2DM.

Abstract:Diabetic retinopathy(DR) is one of the most common and serious complications of diabetes.Through the study of ancient medicals and based on many years of clinical experiences,Professor GAO Yan-bin infers that DR is particularly closely related to the liver and kidney.Using these two organs as the theoretical foundation,the root of eye diseases can be grasped.The theory of collateral disease coincides and is responsible throughout the whole process of DR.Using syndrome differentiation,he holds the idea that with deficiency as the basic pattern and excess as the syndrome,the main reason for DR is qi,blood,yin and yang deficiency,while blood stagnation in the eye meridian is pathogen,The rules of treatment are tonifying qi and enriching yin,supplementing and nourishing liver and kidney,removing blood stasis and promoting circulation.By combining overall symptoms with fundus changes,together with dynamic differentiation and flexible treatment,satisfactory results have been obtained in clinical practice.
