Study on the Pressure During Arthrocentesis in Upper Compartment of TMJ

Abstract:Objective: The study aims to find out the fluctuating curve of the intra-articular pressure intemporomandibular joint with anterior disc displacement without reduction during arthrocentesis. This experiment studiedthe biomechanical difference between two methods of arthrocentesis and compared their influences on the intra-articularpressure. Methods: 10 affected discs in 8 patients. Were involved A No.8 syringe needle was used to pierce into the uppercompartment of TMJ. Method one: we increased hydraulic pressure on the inflow intermittently during the arthrocentesis.Method two: Close the outflow, add hydraulic pressure within the patient' tolerance and then, open the outflow needle andthe saline in the joint spurt out. During the operation IAP was measured, Method of measurement was the same as theexperimental one. Intra-articular pressure was taken by the pressure transducer during arthrocentesis. The curve and meanvalue were documented. Results: The average intra-articular pressure in method one was 27.575±5.657kPa and it droppedinto15.555±8.158kPa when the operation stopped, the pressures dropped by 12.020±4.342kPa. The average pressurein method two was 42.903±5.508kPa, and it dropped into 22.512±3.929kPa when the operation stopped., the pressuresdropped by 20.391±3.310kPa.Conclusions: In order to get better curative effect on patients with TMD, we can achievedifferent pressure levels through different methods during arthrocentesis.
