The Influence of Diabetes Mellitus on Microvascular Anastomoses: an Experimental Study

Abstract:Objective To explore the healing rule of microvascular anastomoses in Diabetes mellitus. Methods Twenty animal model of Diabetes were induced via intraperitoneal injection of Streptozotocin, while twenty normal animal received injection of buffer vehicle alone. After 8 weeks of symptoms, the Diabetes groups and the control groups animals underwent bilateral exposure of the carotid arteries and microvascular anastomoses.Vascular patency, ultrastructure, scanning electron microscopy of anastomosis site were observed after 1, 3, 7, 14 days post-operation. At the same time,the serum of the TNF-α concentrion were tested. Results The intimal cells injury at the micro-arterial anastomosis site were conspicuous in the Diabetes groups compared with the control groups, while obvious inflammation and granulation tissue hyperplasia around the anastomosis site in the Diabetes groups was observed. At 7 days post-operation, the average area of exposed suture surface at micro-arterial anastomosis site was 3 745.18 μm2±1 045.31μm2 in the Diabetes group compared to 1 298.21 μm2±224.66 μm2 in the normal group (p<0.01). The degree of intimal repair was significantly lower in the Diabetes group compared to the control group. At 14 days after the operation,intima/media ratio at the anastomosis site was markedly increased in the Diabetes group than the control group (0.71 ± 0.05 versus 0.07±0.02, p<0.05). Intimal hyperplasia was obvious in the Diabetes group. Serum TNF-α, measured by Radioimmunoassay, was significantly increased in the Diabetes groups compared with the control after operation (5.48±0.34 ng/ml Verus 0.73±0.09 ng/ml on day 7, p<0.05; 5.15±0.12 ng/ml verus 0.51±0.03 ng/ml on day 14, p<0.05), and we observed a good positive correlation between the serum TNF-αlevels and the severity of intimal growth. Conclusion Diabetes mellitus changed the normal healing rule of the microvascular anastomosis site. Microvascular anastomosis site in the Diabetes animals becme a chronic inflammation and proliferation process. TNF-αwas the key inflammation factor in the pathological healing process in Diabetes animals.
