Clinicopathologic observation of follicular dendritic cell sarcoma involving systemic lymph nodes

Abstract:Objective To study the clinicopathologic features,biological behavior,therapy,and prognosis of follicular dendritic cell sarcoma (FDCS). Methods A case of FDCS occurring in multiple lymph nodes was analyzed by histopathology,immunostaining and in-situ hybridization (ISH) for Epstein-Barr virus-encoded small RNA (EBER). Results The patient had multiple lymphadenopathy,urticaria,and joint pain all over the body. Microscopically,the normal architecture of the lymph nodes was effaced by the proliferation of ill-defined tumor cells arranged in a fascicular or storiform growth pattern. The tumor cells had eosinophilic cytoplasma and ovoid vesicular nuclei with distinct nucleoli. Mitotic figures were 2-4 per 10 high-power fields. The tumor tissues were infiltrated by small amounts of lymphocytes,plasm cells and eosinophilic granulocytes. Immunohistochemical studies showed that the tumor cells were diffusely positive for clusterin,smooth muscle actin and vimentin,focally for CD21+35 and S-100,and negative for CD1a,desmin,CK8/18 and CD68. ISH for EBER was performed and the result was negative. Conclusions FDCS is a rare malignant tumor originating from follicular dendritic cells in lymph nodes or extranodal lymph tissues. Immunohistochemistry and electron microscopy can be very helpful to make a correct diagnosis. Further investigation of biological behavior will contribute to clinical therapy.
