Surgical Treatments of Neonatal Congenial Heart Disease

Abstract:Objective This study set out to evaluate the surgical results of neonates with severe congenital heart disease.Methods Between January 2007 to June 2011,77 neonatal patients underwent surgical treatment.In these patients,61 were male,16 were female.Diseases included transposition of great arteries in 37 cases,ventricular septal defects in 24 cases,aortic arch disease (coarctation or interruption) with ventricular septal defect in 8 cases,total anomalous pulmonary venous drainage in 3 cases,Taussig-Being in 2 cases,pulmonary atresia in 2 cases,complete atrioventricular septal defect in 1 case.One patient underwent palliative surgery,while the rest underwent radical corrective surgery.Results Overall,7 patients died,making the mortality rate of 9.1％.For post-operative complications,low cardiac output was seen in 6 cases; acute renal failure was seen in 1 case; pulmonary infection in 5 cases,arrhythmia in 12 cases.Delayed sternal closures were needed in 33 cases.Conclusions Patients who need surgical treatment in the neonatal period are usually critically ill.Their general condition needs to be improved before surgery for better outcomes.TGA is still the majority out of all the conditions.During surgery,one must try to minimize the cardiopulmonary bypass time,to use ultrafiltration technology,and to manage fluid input-output aggressively during the CPB period.Postoperative treatment should focus on respiratory,circulatory support,the use of peritoneal dialysis and delayed sternal closure technique.One-stage treatment network for complex congenital heart disease is the future direction.
