Preoperative CT analysis of multi-factors influencing the prognosis of patients with primary hepatocellular carcinoma after radical hepatectomy

Abstract:Objective: To explore the relationship between significant signs on preoperative enhanced CT and the prognosis in patients with primary hepatocellular carcinoma (HCC) after radical hepatectomy. Methods: Clinical data and preoperative enhanced CT data of 87 HCC patients who then underwent radical operation were analyzed retrospectively. The status of metastasis and (or) recurrence and survival after surgery were followed for evaluating of prognosis. Univariate survival analysis was applied with Kaplan-Meier method and Log-rank test. Then multivariate analysis was carried out with the Cox proportional hazard method. Results: The 1, 3, 5-year postoperative cumulative survival rate of the whole patient group was 80.18%, 65.48% and 42.09%, respectively. Univariate analysis indicated that tumor size, location, satellite nodule, vascular invasion and ratios of tumor to liver parenchyma density in arterial phase and portal phase on CT were risk factors for survival (P<0.05). Further multivariable Cox analysis showed that vascular invasion and ratio of tumor to liver parenchyma density in portal phase on CT were independent significant factors for prognosis (P=0.037, 0.007). Conclusion: Vascular invasion and a ratio lower than 0.85 of tumor to liver parenchyma density in portal phase on CT predict poor prognosis after radical hepatectomy in patients with primary HCC.
