Current Opinion in the Diagnosis and Treatment of Pulmonary Cryptococcosis: a Report of Two Cases and Literature Review

Abstract:Objective To learn more about pulmonary cryptococcosis and improve the ability in dealing with the disease. Methods To reach the current opinion in the diagnosis and treatment of pulmonary cryptococcosis by reporting two cases and reviewing the literature. Results The patient of case 1 had close contact with a parrot, and had mild cough for 4 months.The CT scan revealed several nodules in the left lung with mediastinal lymphadenopathy.The patient was diagnosed by CT-guided percutaneous biopsy of the lung. The symptoms were improved significantly after treating with fluconazole for 10 days, and the imaging improved four months later. The patient of case 2 had a history of rheumatoid arthritis using methotrexate and prednisone, and had had a fever and shortness of breath for one month. Various appearance was found in CT scan, including consolidation with small cavities, ground-glass shadow, small nodules with a small burr, lobular, and the helical around the lesions thickened.The patient was diagnosed by transbronchoscopy lung biopsy. Symptoms were improved after treating with fluconazole for 10 days. The lesions were absorbed obviously 6 months later. Conclusion Close contact with parrots and immunosuppression are the two risk factors of pulmonary cryptococcosis,and the symptoms are nonspecific. Symptoms are seriouser in secondary cases, and the imaging varied. Histopathology is the most important way in diagnosis. Fluconazole was effective and safe for pulmonary cryptococcosis. The course of treatment should be longer, especially for immunosuppressions.
