Impact of insulin therapy to control blood glucose on different levels on criticaly ill patients

Abstract:Objective: To evaluate the effect of different levels of blood glucose control on prognosis of the patients in surgical intensive care unit(SICU).Methods: 934 patients admitted to SICU were randomly divided into two groups.Blood glucose were controlled by insulin infusion.The blood glucose was maintained at the level of 4.44～6.10 mmol/L in intensive insulin theraphy group(478 cases).In control group(456 cases),blood glucose was maintained at the level of 6.10～8.32 mmol/L.The outcome was evaluated by systemic and local complications,ICU and hospital mortality.Results: Compared with control group,acute renal dysfunction and incisional infection in intensive insulin theraphy group were significantly reduced(respectively,5.23% vs 8.55% and 5.86% vs 9.87%,P<0.05).Although there was no significant difference between two groups,ARDS,arrhythmia and sepsis showed a descending tendency(respectively,14.02% vs 17.76%,12.97% vs 15.7% and 21.55% vs 25.22%).ICU and hospital mortality were not significantly different between two groups(P>0.05).Compared with control group,hypoglycemia in intensive insulin therapy group was very significantly increased(9.83% vs 1.54%,P< 0.01).Conclusion: The control of blood glucose in normal range with intensive insulin therapy can reduce complications of critically ill patients,but it was also increase the incidence of hypoglycemia.

Abstract:A total of 62 critically ill patients were randomly allocated to the intensive insulin therapy group and the conventional insulin therapy group. The effect of therapy on the prognosis, the activity of NF-κB in peripheral blood monouclear cells and plasma intercellular adhesion molecule-1 level were investigated. The results showed that the intensive insulin therapy may improve the prognosis and insulin had a reliable anti-inflammatory effect in the critically ill patients.

Abstract:OBJECTIVE:To evaluating the effect of different levels of blood glucose control on inflammatory response and prognosis of the patients in surgical intensive care unit (SICU). METHODS:One hundred and eighty-eight patients admitted to SICU were randomly divided into three groups, blood glucose were controlled by insulin infusion. Group A (75 cases): the mean blood glucose (MBG) was maintained at the level of 4.4 - 6.1 mmol/L. Group B (75 cases): MBG was maintained at the level of 6.7 - 8.3 mmol/L. Group C (38 cases): MBG was maintained at the level of 10.0 - 11.1 mmol/L. Blood glucose control was achieved with an effected computerized protocol. The outcome was evaluated by days in ICU, days to wean mechanical ventilation, infection, amount of red blood cell transfusion, hospital mortality and ICU cost. RESULTS:Compared with other groups, hypoglycemia (< 3.3 mmol/L) in Group A was significantly increased (P < 0.05). Compared with Group C, red blood cell transfusion and infection were significantly reduced in Group A and Group B (P < 0.05). Compared with Group C, days of mechanical ventilation and days in ICU in Group A were significantly reduced (P < 0.05). Hospital mortality and ICU cost were reduced in Group A compared with the other groups (P > 0.05). CONCLUSIONS:To maintain blood glucose in normal range with intensive insulin therapy has potential positive impact on SICU patients' outcome and can reduce days in ICU and ICU cost. Further correlation research is needed to determine the best levels of blood glucose in ICU patients.

Abstract:Background. Studies on the effect of intensive insulin therapy (IIT) in septic patients with hyperglycemia have given inconsistent results. The primary purpose of this meta-analysis was to evaluate whether it is effective in reducing mortality. Methods. We searched PubMed, Embase, the Cochrane Library, clinicaltrials.gov, and relevant reference lists up to September 2013 and including randomized controlled trials that compared IIT with conventional glucose management in septic patients. Study quality was assessed using the Cochrane Risk of Bias Tool. And our primary outcome measure was pooled in the random effects model. Results. We identified twelve randomized controlled trials involving 4100 patients. Meta-analysis showed that IIT did not reduce any of the outcomes: overall mortality (risk ratio [RR] = 0.98, 95% CI [0.85, 1.15], P = 0.84), 28-day mortality (RR = 0.66, 95% CI [0.40, 1.10], P = 0.11), 90-day mortality (RR = 1.10, 95% CI [0.97, 1.26], P = 0.13), ICU mortality (RR = 0.94, 95% CI [0.77, 1.14], P = 0.52), hospital mortality (RR = 0.98, 95% CI [0.86, 1.11], P = 0.71), severity of illness, and length of ICU stay. Conversely, the incidence of hypoglycemia was markedly higher in the IIT (RR = 2.93, 95% CI [1.69, 5.06], P = 0.0001). Conclusions. For patients with sepsis, IIT and conservative glucose management show similar efficacy, but ITT is associated with a higher incidence of hypoglycemia.
[The Influence of Intensive Insulin Therapy on the Mortality of Critically Ill Patients in Intensive Care Unit: a Meta-Analysis].

Abstract:OBJECTIVE:To assess the influence of intensive insulin therapy (IIT), as compared with conventional insulin therapy (CIT), on mortality of critically ill patients in the intensive care unit (ICU) by Meta-analysis.METHODS:Computer retrieval was conducted to search for randomly controlled trials (1966-2009) to compare the result of IIT and CIT. The quality of included studies was critically evaluated and data were analyzed by using the Cochrane Collaboration's RevMan 5.0 software.RESULTS:A total of 23 studies were included in the Meta-analyses, consisting 11 216 patients. This Meta-analysis showed that there were significant difference between IIT and CIT in the mortality (unselected in the medical, surgical and mixed ICU), with significant lowering in IIT group. There was remarkable lowering of infection rate and the complication rate in IIT group compared with CIT group. However, the incidence of hypoglycemia was slightly lower in CIT group.CONCLUSION:IIT tends to lower the mortality rate, infection rate, and complications in critically ill patients in the ICU as compared with CIT. Also with IIT the target blood glucose can be easier reached than CIT. The therapy can improve the quality of life. However, IIT is not as good as CIT in the control of hypoglycemia.
[Influence and Mechanism of a Tight Control of Blood Glucose by Intensive Insulin Therapy on Human Sepsis].

Abstract:OBJECTIVETo investigate the effect of a tight control of blood glucose by intensive insulin therapy on human sepsis, and to explore the potential mechanism of the intensive insulin therapy.METHODSEligible patients were randomized by a blinded pharmacist to receive tight control of blood glucose by intensive insulin therapy (maintenance of blood glucose at a level between 4.4 and 6.1 mmol/L) or to receive conventional treatment (maintenance of glucose at a level between 10.0 and 11.1 mmol/L). The expression of HLA-DR on peripheral monocytes was measured in 54 patients by flow cytometry on 24 h, 3 d, 5 d, 7 d, 10 d and 14 d of intensive care in parallel with serum c-reactive protein (CRP), severity of the disease (APACHE II score, SOFA score) and clinical data collection.RESULTSPatients receiving intensive insulin therapy were less likely to require prolonged mechanical ventilation. Tight control of blood glucose significantly reduced the number of days during which leukopenia or leukocytosis and the days with hypo- or hyperthermia (P < 0.05). Hypoglycemia occurred in 3 patients (10.7%) in the tight control of blood glucose group. There were no instance of hemodynamic deterioration or convulsions. Compared with the conventional treatment, tight control of blood glucose also increased the HLA-DR expression of peripheral monocytes, and there were significantly difference on 3 d, 5 d and 7 d (P < 0.05). Whereas it suppressed the elevated serum CRP concentrations, there was significantly difference on 7 d (P < 0.05).CONCLUSIONSTight control of blood glucose by intensive insulin therapy expedited healing of human sepsis, and increased the HLA-DR expression of peripheral and suppressed the elevated serum CRP. So, it is necessary to use insulin to strict control the glucose levels in human sepsis.
Efficacy and safety of intensive insulin therapy for critically ill neurologic patients: a meta-analysis.

Abstract:Background: Whether intensive insulin therapy (IIT) may improve clinical outcomes for patients admitted to intensive care units, especially critically ill neurologic patients, is still debated. In the present study, we performed a meta-analysis of literature comparing the efficacy and safety of IIT and conventional insulin therapy (CIT) for critically ill neurologic patients in terms of mortality, infection rate, neurologic outcome, and hypoglycemia. Methods: We searched for published reports of studies of randomized control trials (up to March 10, 2011) of patients admitted to neurologic intensive care units and investigated an IIT (target of blood glucose control <120 mg/dL) with a control of CIT. Data were abstracted by a standardized protocol. Results: We retrieved reports of five studies involving 924 patients. The risk of mortality, infection rate, and neurologic outcome did not differ with IIT or CIT. However, the incidence of hypoglycemic episodes was significantly higher with IIT than CIT (78.8% vs. 48.9%), with a relative risk of 2.62 (95% confidence interval [CI]: 1.07-6.43; p < 0.04). Conclusions: As compared with CIT, IIT may not benefit critically ill neurologic patients in terms of mortality, infection rate, or neurologic outcome and in fact may be associated with increased hypoglycemic complications. Therefore, IIT cannot be recommended over conventional control for critical neurologic disease, but further study is warranted.

Abstract:Hyperglycemia often occurs in critically ill patients,and continued hyperglycemia is associated with adverse outcomes. The mechanism of hyperglycemia in critically ill patients may be associated with stress response and the release of cytokines. Keeping blood glucose in a rational range with safe, effectire,economical treatment can improve the prognosis of the patients, and decrease the incidence of mortality and complications. It requires us to understand the characteristics of hyperglycemia in critically ill patients,enhance the consciousness of blood glucose monitoring,select a rational treatment of hyperglycemia, and determine the goal of glycemic control. Here reviews the mechanism and management of hyperglycemia in critically ill patients.
Effects of Intensive Insulin Therapy on Immune Factors of Obstetrics Patients with Critical Illness

Abstract:Objective To investigate the correlation of stress hyperglycemia with serum levels of C reactive protein(CRP),interleukin-6(IL-6)and Tumor Necrosis Factor-α(TNF-α)in critical obstetrics patients and to explore the effect of IIT on these immune factors.Methods 42 critical obstetrics patients,who were admitted in the ICU of the first affiliated hospital of the chongqing medical university between January 2005 and December 2007,were randomly divided into the IIT group(maintenance blood glucose at a level between 4.4 and 6.1mmol/L)and routine group(maintenance blood glucose at a level between 10.0 and 11.1mmol/L).Levels of CRP,IL-6 and TNF-α were measured before the treatment and then measured again at 1,3 and 5d after treatment.Results In the routine group,the serum levels of CRP,IL-6 and TNF-α in all cases were significantly increased and were positively correlated with the level of blood glucose.The CRP level of IIT group at 5 d after treatment was(40.92±14.46)mg/L which was significantly decreased compared with the level of routine group which was(87.50±30.20)mg/L(P0.05).The IL-6 level of IIT group at 5 d after treatment was(28.76±6.62)ng/L which was significantly decreased compared with the level of routine group which was(47.45±17.24)ng/L(P0.05).The TNF-α level of IIT group began to decrease to(254.37±39.50)ng/L at 1 d after treatment compared with the level of the routine group which was(325.17±29.47)ng/L(P0.05).Then the TNF-α level of IIT group was significantly decreased to(99.71±13.43)ng/L at 5 d after treatment compared with the level of routine group which was(254.73±19.43)ng/L(P0.05).Conclusion Intensive insulin therapy could obviously regulate the inflammatory reaction of post-injury by controlling glycemia within the normal level to inhibit excessive inflammatory reaction and have a good prognosis.

Abstract:Background Continuous insulin infusion is a method for maintaining blood glucose stability in critically ill patients with hyperglycemia. Many insulin infusion protocols have been applied in intensive care units. Understanding the content of these protocols can help clinical staff choose the most appropriate and convenient protocol and promote best practices in managing glucose levels in critically ill adult patients. Objective To examine the types of insulin infusion therapies performed for blood glucose management in critically ill patients. Methods For this scoping review, 3 Chinese-language and 8 English-language databases were searched for articles published from May 25, 2016, to October 25, 2022. Results Twenty-one articles met the inclusion criteria. Twenty-one insulin infusion protocols were examined. Most of the insulin infusion protocols were paper protocols. Fourteen glucose management indicators were included in the 21 protocols. The glucose target range for all 21 protocols ranged from 70 to 180 mg/dL (3.9-10.0 mmol/L). Nurses were primarily responsible for protocol implementation in most protocol development processes. The roles of nurses differed in nurse-led insulin infusion protocols and non–nurse-led insulin infusion protocols. Discussion This scoping review indicates an urgent need for more comprehensive glycemic control guidelines for patients receiving critical care. Because insulin infusion protocols are core aspects of blood glucose management guidelines, different population subgroups should also be considered. Conclusions Nurse-led guidelines must be based on the best available evidence and should include other variables related to glucose management (eg, patient disease type, medication, and nutrition) in addition to insulin infusion.

Intensive Insulin Therapy in Severe Acute Pancreatitis: a Meta-Analysis and Systematic Review.

Abstract:To assess the effect of intensive insulin therapy on outcomes of patients with severe acute pancreatitis.Relevant literatures cited in these electronic databases: Medline, Chinese Biomedical Literature Database, China National Knowledge Infrastructure (CNK1) database, and Excerpta Medical database (Embase) were systematically searched for randomized controlled trials (RCTs) in which intensive insulin therapy was used in severe acute pancreatitis. Length of hospitalization, acute physiology and chronic health evaluation II (APACHE II) score, incidence of complications, and adverse effects were recorded for statistical analysis. The methodological quality of the eligible studies was assessed by Jadad scale. The results were analysed by Revman 4.3 software.Three studies, which included a total of 118 cases, were finally reviewed. The methodological quality of the trials varied substantially In patients with severe acute pancreatitis, intensive insulin therapy was associated with shorter length of hospitalization (weighted mean difference (WMD) = -12.13, 95% confidence interval (CI) [-15.48, 8.78], p > 0.00001) and lower APACHE II score after 72 hours treatment (WMD = -3.80, 95% CI [-4.88,2. 72], p > 0.00001). One study reported insulin-related adverse event.In patients with severe acute pancreatitis, intensive insulin therapy could relieve the patient's condition earlier and shorten the length of hospitalization without serious adverse effect.
Is intensive glucose control bad for critically ill patients? A systematic review and meta-analysis

Abstract:Background: The monitoring and management of blood glucose concentration are standard practices in critical settings as hyperglycaemia has been shown close association with poorer outcomes. Several meta-analyses have revealed that intensive glucose control has no benefit in decreasing short-term mortality among critically ill patients, while the studies these meta-analyses have incorporated have been largely divergent. We aim to perform a more comprehensive meta-analysis addressing this problem to provide stronger evidence.Methods: We conducted comprehensive searches for relevant randomized controlled studies in online databases, including the Cochrane Library, EMBASE, and PubMed databases, up to September 1, 2018. The clinical data, which included all-cause mortality, severe hypoglycemia, need for RRT, infection resulting in sepsis, ICU mortality, 90-day mortality, 180-day mortality, and hospital and ICU lengths of stay, were screened and analyzed after data extraction. We applied odds ratios (ORs) to analyze dichotomous outcomes and mean differences for continuous outcomes with a random effects model.Results: A total of 57 RCTs involving a total of 21840 patients were finally included. Patients admitted to the ICU who underwent intensive glucose control showed significantly reduced all-cause mortality (OR: 0.89; 95% CI: 0.80-1.00; P=0.04; I2=32%), reduced infection rate (OR: 0.65, 95% CI: 0.51-0.82, P=0.0002; I2=47%), a lower occurrence of acquired sepsis (OR: 0.80, 95% CI: 0.65-0.99, P=0.04; I2=0%) and shortened length of ICU stay (MD: -0.70, 95% CI: -1.21--0.19, P=0.007, I2=70%) when compared to the same parameters as those treated with the usual care strategy. However, patients in the intensive glucose control group presented with a significantly higher risk of severe hypoglycemia (OR: 5.63, 95% CI: 4.02-7.87, P<0.00001; I2=67%).Conclusions: Critically ill patients undergoing intensive glucose control showed significantly reduced all-cause mortality, length of ICU stay and incidence of acquired infection and sepsis compared to the same parameters in patients treated with the usual care strategy, while the intensive glucose control strategy was associated with higher occurrence of severe hypoglycemic events.

Intensive Insulin Therapy on Stress Hyperglycemia in Critical Ill Patients

Abstract:Objective To observe the outcomes of the patients who had stress hyperglycemia at admission,and whose blood glucose levels were lowered to normal or near normal with insulin in ICU.Methods We prospectively analyzed patients whose admission blood glucose was over 8.0mmol/L,but without history of diabetes.Of total 105 patients were classified according to their goal blood glucose: A group,4.4-6.1 mmol/L(55);B group 50 was 6.2-8.0 mmol/L.Results The final blood glucose and average daily dosage of insulin was 5.3mmol/L vs 7.1mmol/L(P0.001)and 66.66u/d vs 49.57 u/d(P0.001),respectively.Hypoglycemia was 12.7% vs 2.0%(P=0.039).ICU morbidity and mortality were not different significantly.Conclusion Intensive insulin therapy to lower blood glucose to 6.1mmol/L and near it will obtain similar clinical outcomes in critical patients with stress hyperglycemia.The group whose blood glucose is slight above 6.1mmol/L has less hypoglycemia.

Abstract:Hyper glycemia and hypoglycemia are associated with poor outcome of critically ill patients.No consistent evidence shows that intensive insulin strategy improves health outcomes in hospitalized patients.Because of its potentially harmful and life-threatening consequences,hypoglycemia represents the main limit to the development of a tight blood glucose control strategy in critical illness.Hypoglycemia-related neurological signs may be masked.The occurrence of hypoglycemia in critical illness,especially severe hypoglycemia,is associated with a poor prognosis.So it is necessary to closely detect glycemic variability and control the blood glucose in proper level.

Abstract:Objective To evaluate the value of insulin in the treatment of severe craniocerebral injury(SCCI).Method 370 SCCI cases,incluing the treatment group(190 cases) and the control group(180 cases) were analysed.The patients in treatment group were taken insulin to control the blood sugar.The controll group was treated without insulin.The comparative parameters included rates of infection, morbidity and mortality.Results The differences in the rates of infection,morbidity and mortality between two groups were significant statistically( P 0.05).Conclusions The effect of insulin treating SCCI is evident,it is worthy to spreading clinically.

Abstract:The choice of blood sugar control technique in the intensive care unit (ICU) has long been debated. Intensive insulin therapy to achieve normoglycaemia has been shown to reduce mortality, morbidity and the length of ICU stay, but at the same time, it frequent glucose monitoring is required, as well as an adjustment to the insulin dose. There is also an increase in the medical personnel workload. However, despite its clinical benefits, intensive blood sugar control is not favoured by the intensivist because of the risk of hypoglycaemia. This article provides the reader with an interesting thought. Can intensive blood sugar control be implemented in the ICU, while avoiding hypoglycaemia, and without an increase in hospital costs, and thus change existing blood sugar control protocols in the ICU? Is this possible with the use of continuous glucose monitoring devices which have recently emerged as a tool to be used to maintain proper glucose levels? If further developed, continuous glucose monitoring technology could ultimately prove to be clinically useful in the ICU. However, further research is warranted to confirm its benefits in connection with the implementation of tight glucose control policies in the ICU.

Abstract:Objective To observe the efficacy and safety of insulin aspart in intensive insulin therapy of hyperglycemia in medical critically ill patients.Methods A total of 186 medical critically ill patients with fasting blood glucose (10.8±2.3) mmol/L were divided into multiple daily insulin (MDI)group (n = 90) and continous subcutaneous insulin infusion (CSⅡ) group (n = 96),both groups were divided into insulin aspart subgroup and human regular insulin subgroup randomly.Bonus dosage was offered by insulin aspart or human regular insulin in MDI group,base dosage was offered by Glagine.Bonus and base dosage were offered by insulin aspart or human regular insuline in CSⅡ group.The dosage of insulin was adjusted according to multi-points blood glucose levels.The course of intensive insulin therapy was 7 days with blood glucose levels 4.4 ～ 8.3 mmol/L,and it was changed to regular insulin therapy after 7 days with blood glucose levels 4.4～11.1 mmol/L.Average blood glucose level,blood glucose standard deviation,the equation of the highest and the lowest blood glucose level in one day,percentage of total hypoglycemia and serious hypoglycemia,average insulin dosage,decline of blood CRP level and APACHE Ⅱ during 7 days,and mortality during twenty eight days were observed.Results (1) There were no statistical significance in difference on lab items before intensive insulin therapy in both subgroups.(2) Compared to human regular insulin subgroup,insulin aspart subgroup was lower in average blood glucose level.MDI group:(6.17±1.32) mmol/L vs (7.6±1.6) mmol/L;CSⅡ group:(6.0±1.2) mmol/L vs (7.4±2.5) mmol/L,P ＜0.05 respectively.Standard deviation MDI group:(1.54±0.27) mmol/L vs (1.92±0.38) mmol/L;CSⅡ group:(1.24 ±0.27) mmol/L vs (1.83±0.45) mmol/L,respectively (P＜0.05).The equation MDI group:(3.0±0.5)mmol/Lvs (3.9±1.1) mmol/L;CSII group:(3.1±0.6) mmoL/Lvs (3.9±1.0)mmol/L,respectively(P＜0.05).CSⅡ group were lower in seventh day of intensive insulin therapy;average insulin dosage was less (MDI group:(38±3)U/d vs (42±4)U/d;CSⅡ group:(37±4)U/d vs (40±6)U/d,P ＜0.05 respectively),percentage of total hypoglycemia (MDI group:17.4% vs 27.3%;CSⅡ group:8.0% vs 17.4%,P＜0.05 respectively) and serious hypoglycemia (MDI group:4.3% vs 15.9%;CSⅡ group:2.0% vs 6.5%,P ＜0.05 respectively) was lower during seven days,decline of blood CRP level during seven days was more obvious (MDI group:(44.8±1.6) mg/L vs (27.8±1.2) mg/L;CSⅡ group:(57.7±2.4) mg/L vs (36.7 ±1.2) mg/L,P＜0.05 respectively),decline of APACHE Ⅱ during seven days was more obvious (MDI group:(7.5±2.1) score vs (4.3±1.1) score;CSⅡ group:(8.4±1.3) score vs (6.1±1.5) score,P＜0.05 respectively);mortality was not different in statistical during twenty eight days (MDI group:13.0% vs 13.6%;CSⅡ group:6.0% vs 6.5%,P＞0.05 respectively).Conclusion Insulin aspart can better control average blood glucose and its variability,lower the percentage of hypoglycemia,save insulin dosage and relieve inflammation than human regular insulin in treating hyperglycemia of medical critically ill patients.
