Abstract:Background and Aims: Installing the transjugular intrahepatic portosystemic shunt for portal hypertension is relatively safe, but complications are still high. To explore a new method of portacaval shunt, the magnetic compression technique was introduced into the shunting procedure.Methods: A portal-inferior vena cava shunt was performed on 6 male mongrel dogs by two hemocompatible Nd-Fe-B permanent magnets, parent and daughter. The parent magnet was applied to the inferior vena cava guided by a catheter through the femoral vein. The daughter magnet was moved to the anastomosis position on the portal vein with a balloon catheter through the splenic vein. After the daughter magnet reached the target position, the two magnets acted to compress the vessel wall and hold it in place. Five to 7 days later, under X-ray guidance, the magnets were detached from the vessel wall with a rosch-uchida transjugular liver access set. One month later, histological analysis and portal venography were performed.Results: 5-7 days after the first surgery, a mild intimal hyperplasia in the portal vein and the inferior vena cava, and continuity of the vascular adventitia from the portal vein to the inferior vena cava as observed. During the second surgery, the contrast media could be observed flowing from the portal vein into the inferior vena cava. Portal venography revealed that the portosystemic shunt was still present one month after the second surgery.Conclusions: Magnamosis via a device of novel design was successfully used to establish a portacaval shunt in dogs.

Abstract:Introduction: The concept of magnetic compression technique (MCT) has been accepted by surgeons to solve a variety of surgical problems. In this study, we attempted to explore the feasibility of a splenorenal shunt using MCT in canine and cadaver.Material and methods: The diameters of the splenic vein (SV), the left renal vein (LRV), and the vertical interval between them, were measured in computer tomography (CT) images obtained from 30 patients with portal hypertension and in 20 adult cadavers. The magnetic devices used for the splenorenal shunt were then manufactured based on the anatomic parameters measured above. The observation of the anatomical structure showed there were no special structural tissues or any important organs between SV and LRV. Then the magnetic compression splenorenal shunt procedure was performed in three dogs and five cadavers. Seven days later, the necrotic tissue between the two magnets was shed and the magnets were removed with the anchor wire.Results: The feasibility of splenorenal shunt via MCT was successfully shown in both canine and cadaver, thus providing a theoretical support for future clinical application.
Establishment of Portal Hypertension Induced Esophageal Varices in Canine by Ameroid Coustrictor

Abstract:Objective To establish the canine model of esophageal variees caused by portal hyper-tension. Methods The model was established in 12 dogs with a side-to-side portacaval shunt, an ameroid constrictor around the portal vein and double ligation and cross suture of the cephalic part of the inferior vena cava (IVC), and the development of esophageal varices was confirmed 6 weeks later by endoscopy and portal vein angiography. Results The mean pre- and postoperative portal pressure were (11.0 ± 1.1) mm Hg and (22. 9 ± 1.2) nun Hg, respectively (P =0. 010). Endoscopy detected mild to moderate esophageal varices in all dogs, which was confirmed by portal vein angiography, and varices was also seen in abdominal wall. Conclusion Canine model of esophageal varices induced by portal hypertension can be established with the procedure.

Abstract:Objective In order to investigate the therapeutic effects of portal arterialization for portal hypertension,portal arterialization and complete shunt(PACS) was applied in canine model of portal hypertension,which was made by thread embolization within the portal vein.Methods A splenectomy,splenic artery and upper portal vein anastomosis,and a complete portal-caval shunt were performed on portal hypertension dogs.The blood pressure and flow of the portal vein including that towards the liver and towards the vena cava were observed.Results The postoperative hepatic inflow,PVF,increased to 180% of the former while PVP increased to 196%;the caval-inflow PVF increased to 130% of the former while PVP decreased to 45.5%.Significant difference existed(P<0.01).The hepatic function changed little after the operation without any statistic difference.Conclusion PACS can significantly increase the hepatic inflow and decrease the blood pressure of the portal system with a satisfactory dog survival.It may be a worthy attempt in the treatment of portal hypertension and needs more research work.

Abstract:BACKGROUND:The resection and reconstruction of large vessels,including the portal vein,are frequently needed in tumor resection.Warm ischemia before reconstruction might have deleterious effects on the function of some vital organs and therefore,how to reconstruct the vessels quickly after resection is extremely important.The present study was to introduce a new type of magnetic compression anastomosis（MCA）device to establish a quick non-suture anastomosis of the portal vein after resection in canines.METHODS:The new MCA device consists of a pair of titanium alloy and neodymium-ferrum-boron magnet（Ti-Nd Fe B）composite rings.The Nd Fe B magnetic ring as a core of the device was hermetically sealed inside the biomedical titanium alloy case.Twelve canines were divided into two groups:a MCA group in which the end-to-end anastomoses was made with a new device after resection in the portal vein and a traditional manual suture（TMS）group consisted of 6 canines.The anastomosis time,anastomotic patency and quality were investigated at week 24 postoperatively.RESULTS:The portal vein was reconstructed successfully in all of the animals and they all survived.The duration of portal vein anastomosis was significantly shorter in the MCA group than in the TMS group（8.16±1.25 vs 36.24±2.17 min,P＜0.05）.Portography and ultrasound showed that the blood flow was normal without angiostenosis or thrombosis in all of the canines.Hematoxylin-eosin staining and electron microscope scanning showed in contrast to the TMS group,MCA anastomotic intimal was much smoother with more regularly arranged endothelial cells at week 24 postoperatively.

Abstract:<h3 class="Heading">Background</h3><p class="Para">Vascular access port (VAP) systems are widely used in human medicine to provide long-term venous access. However, in veterinary medicine the use of VAP systems is not common practice and publications on their potential applications have been limited. A VAP system was used as part of an experimental study on liver regeneration and implanted in the canine portal vein to create direct access to the portal venous circulation of the liver. The aim of the present study is to describe the surgical technique, its use, and the complications of a VAP system in three research dogs.</p><h3 class="Heading">Results</h3><p class="Para">The VAP system was successfully used for the intraoperative measurement of portal blood pressure, the administration of cell suspensions, and the collection of portal venous blood samples. Long-term complications consisted of dislocation of the VAP system in one dog (2 months after implantation) and thrombus formation at the catheter tip in two dogs (3 months after implantation). Both complications prevented further use of the VAP but had no adverse clinical implications.</p><h3 class="Heading">Conclusions</h3><p class="Para">This pilot study suggests that the VAP system is an effective and safe technique to obtain long term access to the portal venous system in dogs. However, complications with port detachment and thrombosis may limit long term use of VAPs in the portal system of dogs.</p>

Histologic and scanning electron microscopic study for portal vein sutureless anastomosis by magnetic compression anastomosis in canines

Abstract:Objective:To compare the histological differences between magnetic anastomosis and traditional suture in canine portal vein (PV) .Methods:Eighteen healthy Chinese garden dogs, either gender, 8-12 months and weighing 13.5-18.9 kg, were randomly divided into magnetic compressive anastomats (MCA) group ( n=9) and hand-sewing (HS) group ( n=9) for PV reconstruction. The time of PV anastomosis was compared between the two groups. HE and Masson staining were performed immediately and at 4, 8, 12 and 24 weeks after operation. The ultrastructure of the anastomosis was observed using scanning and transmission electron microscopy. Results:All dogs survived. The PV anastomosis time was significantly shorter in MCA group (3.58±2.75) min than that HS group (12.89±3.12) min, P<0.01. In MCA group, the vascular wall of anastomotic stoma was well aligned immediately after operation, and the shrinkage was obvious in HS group by gross eyes. At 24 weeks, electron microscope scanning showed the re-endothelialization was smooth and endothelial cells arranged regularly at the anastomotic site of the MCA group, whereas different-sized and irregularly aligned endothelial cells and large collagenous fibers arranged in disorder were present at the HS anastomotic stoma. Representative HE and Masson staining confirmed that the magnetic device was associated with decreased infiltration of inflammatory cells and deposition of fibrotic collagen at 24 weeks explanted anastomotic stomas compared with the HS group. Conclusions:Compared with the HS, MCA produced shorter anastomosis time, smooth anastomotic intima, light fibrous tissue hyperplasia, no foreign body residue, mild inflammatory reaction and reliable technique for canines PV anastomosis.
Extrahepatic portacaval shunt via a magnetic compression technique: A cadaveric feasibility study

Abstract:AIM: To explore the anatomical feasibility of portacaval shunt using a magnetic compression technique (MCT) in cadavers. METHODS: Computed tomography (CT) images of 30 portal hypertensive patients were obtained. The diameters of the portal vein (PV), the inferior vena cava (IVC), and distance between the two structures were measured. Similar measurements were performed on 20 adult corpses. The feasibility of portacaval shunt based on those measurements was analyzed. First stage of the extrahepatic portacaval shunt using MCT was performed on five cadavers. Specifically, the PV and IVC were exposed through an abdominal incision of the cadavers. The parent magnet was introduced from the femoral vein and was delivered into the IVC by an anchor wire and a 5F Cook catheter. The daughter magnet was introduced into the PV through the splenic vein using an interventional guide wire. When the daughter magnet met the parent magnet, they automatically clipped together and the first stage of the portacaval shunt was set up. RESULTS: The average diameters of the PV and the IVC measured from the 30 CT image were 14.39 +/- 2.36 mm and 18.59 +/- 4.97 mm, respectively, and the maximum and minimum distances between the PV and the IVC were 9.79 +/- 4.56 mm and 9.50 +/- 4.79 mm, respectively. From 20 cadavers, the average diameters of the PV and the IVC were 14.48 +/- 1.47 mm and 24.71 +/- 2.64 mm, and the maximum and minimum distances between the PV and the IVC were 10.14 +/- 1.70 mm and 8.93 +/- 1.17 mm, respectively. The distances between the PV and the IVC from both the CT images and the cadavers were within the effective length of portacaval anastomosis using MCT (30.30 +/- 4.19 mm). The PV and IVC are in close proximity to each other with no intervening tissues or structures in between. Simulated surgeries of the first stage using MCT on five cadavers was successfully performed. CONCLUSION: Anatomically, extrahepatic portacaval shunt employing MCT is highly feasible in humans.
[The Development of Extrahepatic Portacaval Shunt Device Based on Magnetic Compression Technique Through the Interventional Procedure].

Abstract:A device of extrahepatic portacaval shunt is introduced. This device is composed of the daughter and parent magnets and the vascular interventional operation equipment. It is based on the principle of magnetic compression technology, through the intervention approach the daughter and mother magnet are moved to the portal vein and inferior vena cava, respectively. Then the two magnets attract and compress the vessel walls of portal vein and inferior vena cava. Two weeks later, the magnets are detached from the vessel wall with a RUPS-set and the portacaval shunt is established. It is mainly used for the treatment of portal hypertension. It belongs to the interventional operation, there is no any foreign body remaining after the portacaval shunt is established, which can maintain long-term patency. Futhermore the portacaval shunt will not be expanded, therefore it can significantly reduce the incidence of hepatic encephalopathy.

Abstract:Objective: In order to solve the problem of the decreased portal inflow,decreased metabolic function and dangerous varicosis in portal hypertension,portal arterialization and complete shunt(PACS) was applied in the treatment of canine model of portal hypertension,which was made by thread embolization in the portal vein.At the same time,traditional operations such as PCDV and SRS were also applied in comparison.Methods: The preparation of canine model.Group PCDV accepted a splenectomy and peripheral cardia divided vessel(PCDV),while the group SRS accepted a spleen-renal vein shunt.Group PACS accepted a splenectomy,splenic artery and upper portal anastomosis,and complete portalcaval shunt.The blood pressure and flow of the portal system were observed.Results: In the PCDV group,the postoperative PVF decreased in 17% while PVP decrease in 5%,the incidence of portal thrombus was 37%.In the SRS group,the postoperative PVF decreased in 51% while PVP decrease in 50%.In the PACS group,the postoperative hepatic inflow PVF increased to 180% of the former while PVP increase to 196%;the caval-inflow PVF increased to 130% of the former while PVP decrease to 45.5%.The results of PACS group have a magnificent statistic difference comparing with those two traditional operations.Conclusion: PCDV made no difference in the treatment of portal hypertension and congestion of portal system.The dangerous rebleeding was still threatening the life because of severer gastric varicosis.SRS extraordinarily decreased the PVF and PVP,while decreased the hepatic inflow at the same time.PACS can significantly increase the hepatic inflow and decreased the blood pressure of the portal system with a pleasant dog survival.It may be a worthy attempt in the treatment of portal hypertension and need more research work.
Direct Intrahepatic Portacaval Shunt: an Experimental Study

Abstract:AIM: To determine the feasibility of creating direct intrahepatic portacaval shunt (DIPS) in swine with puncture under sonographic guidance.METHODS: DIPS was created in 10 domestic swine under sonographic guidance. Liver function, blood ammonia level and portosysternic gradient (PSG) were compared before and after the procedure. Patency of shunt was followed by portography every 7 days after DIPS.RESULTS: DIPS was successfully established in all 10 swine without any complications. One day after procedure the alanine aminotransferase (ALT), aspartate aminotransferase (AST), and blood ammonia level (BAL) of swine rose from 5.40 +/- 0.69, 16.00 +/- 0.79 and 35.66 +/- 4.10 to 34.20 +/- 3.46, 59.70 +/- 2.22 and 66.94 +/- 3.44 respectively. (P<0.05). The PSG decreased from 0.59 +/- 0.20 kPa to 0.24 +/- 0.11 kPa after DIPS (P<0.05). The shunt of 10 swine was kept patent from 7-28 days (median patency time was 14 days).CONCLUSION: This initial experience demonstrated that creating intrahepatic portacaval shunt from retrohepatic segment of IVC to portal vein with puncture under sonographic guidance in swine is safe and feasible. Further studies are necessary to perform DIPS in cirrhosis patients.
Sutureless Choledochoduodenostomy with an Intraluminal Degradable Stent in Dog Model.

Abstract:Background It is difficult and time-consuming for carrying out conventional hand-sewn bilioenteric anastomosis, especially for small bile duct anastomosis and laparoscopic procedure. In order to simplify it, we have developed a novel procedure of sutureless bilioenteric anastomosis with an intraluminal degradable stent. This study aimed to evaluate the feasibility and safety of this technique with cholangioduodenostomy in dog model.Methods A patent intraluminal degradable stent tube for sutureless choledochoduodenostomy in dog model was made with polylactic acid in diameter of 3 mm or 4 mm. Thirty-eight dogs were randomly divided into to a stent group (SG, n=20) and a control group (CG, n=18). Dogs in the SG underwent sutureless choledochoduodenostomy with intraluminal stent, while the CO underwent conventional choledochoduodenostomy (single layer discontinuous anastomosis with absorbable suture). Dogs of each group were divided into 4 subgroups according to time of death (1, 3, 6, and 12 months postoperatively) to evaluate the healing of anastomosis. Operation time, intraoperative tolerance pressure of anastomosis, rate of postoperative bile leakage, bursting pressure of anastomosis were compared between the two groups. Anastomosis tissue was observed afterwards by pathology evaluation, hydroxyproline content, serum bilirubin, liver enzyme level and magnetic resonance cholangio-pancreatography (MRCP) to assess the stricture.Results All procedures were completed successfully. The surgical time of the SG was significantly less than the CG (SG: (19.2+/-4.3) minutes, vs. CG: (29.2+/-7.1) minutes, P=0.000). One bile leakage was occurred in either group. No significant difference of intraoperative tolerance pressure of anastomosis, rate of bile leakage and postoperative bursting pressure of anastomosis, anastomotic stricture, hydroxyproline content, serum bilirubin and liver enzyme level was found between the two groups. MRCP showed no anastomosis stricture and obstruction during months of follow-up.Conclusion The technique of sutureless choledochoduodenostomy with a degradable intraluminal stent is feasible and a safe procedure in this dog model. Abstract:Objective: To make magnetic compression anastomosis between the small intestines as a novel treatment, using the advances in interventional radiology. Methods: One magnet was put into the ileum of an adult dog, another magnet was put into the colon of the dog, the ileum between the two magnets was ligatured causing a complete obstruction of the small intestine. Two magnets were immediately attracted towards each other transmurally, so that the walls of intestine were compressed together. Results: Two magnets compressed the stricture transmurally, causing gradual ischemic necrosis of the stricture. The ischemic necrosis created an anastomosis between the two magnets. The magnets of 2 300～2 770 gauss were discharged from the anal 6～14 days after the procedure. There was a complete fistula between the ileum and the colon. The magnets of 3 200 gauss were discharged 3- 4 days post the procedure, and two dogs were died of perforation on colon caused by the magnets of 3 200 gauss. Conclusion: Magnetic compression anastomosis(MCA)is a novel interventional method that creates an anastomosis between the small intestine and the colon. The magnets from 2 300～2 770 gauss are safe. MCA could be applied in patients.

Abstract:BACKGROUND: The traditional hand-sewn Roux-en-Y choledochojejunostomy is technically complicated, and the incidence of postoperative complications has remained high. A set of novel magnetic compressive anastomats was introduced to facilitate choledochojejunostomy and improve the prognosis of patients.METHODS : After ligating the common bile duct for 7 days, 16 dogs were randomly divided into two groups (n=8 per group). Anastomats were used in the study group, and the traditional hand-sewn method was used in the control group for standard Roux-en-Y choledochojejunostomy. We compared the operation time, incidence of complications, gross appearance, and pathological disparity in stoma between the two groups in 1-month and 3-month follow-tip examinations.RESULTS: The time spent on constructing the anastomosis for the study group was significantly shortened. Although no anastomotic stenosis occurred in the two groups, the narrowing rate of biliary-enteric anastomosis was much higher in the control group. There was one case of bile leakage in the control group, whereas no bile leakage occurred in the study group. A smoother surface, an improved layer apposition, and a lower local inflammatory response were identified in the anastomosis of the study group.CONCLUSION: The structures of the novel magnetic compressive anastomats are simple, and they are time-saving, safe and efficient for performing Roux-en-Y choledochojejunostomy procedures in a canine model of obstructive jaundice. Abstract:AIM: To observe the effects of large caval vein and portal vein reconstruction method in typical canine liver transplantation after improving these methods on improving anastomosis speed and mass,shortening anheptic phase and reducing the severe effect of anheptic phase on body and shortening operative duration. METHODS: The experiment was conducted in Department of Animal Surgery of the First Hospital of Xi'an Jiao Tong University from February 2005 to March 2006. ①Thirty-two adult mongrel dogs were used for typical orthotopic liver transplantation. They were selected as 16 donors and 16 receptors randomly. ②Magnetic rings with titanium oxide coating were used for vascular anastomosis rapidly without veno-venous bypass. In the donors,the broken ends of vessels were fixed by purse string suture after corresponding magnetic rings were inserted into the purse through vascular ecstrophy. ③In the receptors,corresponding magnetic rings were inserted into the broken ends of vessels,and fixed with nipper ecstrophy after receptor liver liberation excision. ④The magnetic rings were connected rapidly and blood flew after checking the vascular direction. ⑤The twisted stomas could be corrected by revolving the magnetic rings to a optimal position if there was stoma twist. ⑥entire operative period,the anastomosis time of superaheptic and infraheptic inferior vena cava and portal vein,change of hemodynamics in operation,stoma status and survival situation after liver transplantation were observed. RESULTS: A total of 12 dogs were selected in preliminary experiment and 20 in the formal experiment,so 20 dogs were involved in the result analysis. ①Operation time: The entire operative period was (3.24±0.49)hours,the anhepatic phase was only (5.89±2.27) minutes and infrahepatic vena cava anastomosis time was (3.89±0.73) minutes. ②Haemodynamics changes: Haemodynamics was stable without vein bypass and no pressor agent,and fluid replacement averagely (730.56±150.56) mL. ③Stoma status: All stomas had no stenosis,errhysis,twist. The twisted stomas could be corrected by revolving the magnetic rings slightly. No thrombogenesis,the healing was good at day 27 after operation. ④Post-operative survival situation: Nine of 10 dogs in experiment were survived. CONCLUSION: Using magnetic rings for large vascular reconstruction in canine liver transplantation is simple,and the anhepatic phase is shortened obviously. But the magnetic rings are left in canine body,more research should be done before the technique used for clinical liver transplantation.
[A Canine Portal Hypertension Model Induced by Intra-portal Administration of Polyurethane-Tetrahydrofuran Solutions].

Abstract:This study was to build a canine portal hypertension model by intra-portal administration of high polymer material polyurethane and organic solvent tetrahydrofuran mixed solutions in order to evaluate the effectiveness of the model. Twelve local crossbreed dogs were selected randomly, with intra-portal administration of 8% (weight/volume) polyurethane- tetrahydrofuran solutions through an incision in the upper abdomen to build the portal hypertension model. We measured the portal vein pressure before modeling, during modeling, and four-, eight-, and twelve- weeks after modeling, respectively. Then we evaluated the effectiveness of the model comparing values of data with those data obtained before modeling started, which were regarded as the normal values. The results showed that the portal vein pressure rose by 2. 5 times after the solution administrated instantly as much as that before modeling, and maintained at 1. 5 times after 4 weeks. This method presents an easy operation, low animal mortality and reliable model of portal hypertension. Its less abdominal adhesions and its ability in keeping normal anatomic structure specially make it suit for surgical research of portal hypertension.
A Sutureless Method for Digestive Tract Reconstruction During Pancreaticoduodenectomy in a Dog Model.

Abstract:UNLABELLED:Development of pancreatic fistulas as a result of anastomotic gaps is still a major complication after pancreaticoduodenectomy, and can cause post-operative death. Therefore, safer and more effective methods of anastomosis are needed to avoid leakage and decrease mortality.MATERIALS AND METHODS:Twenty domestic dogs with body weights ranging from 15 to 25 kg were used, regardless of gender. A model of common bile duct and pancreatic duct dilatation was surgically prepared in these dogs. Pancreaticobiliary stents combined with magnetic anastomoses (PB-MA), and controls were treated with fibrin glue were studied in terms of and efficacy by measurement of serum amylase, incidence of complications, and survival times.RESULTS:The mean time required to create the fibrin glue pancreaticoenterostomy was 9 ± 2.05 min, while the mean time required to create the magnet cholangioenterostomy was 5 ± 0.9 min. The total operative time was 2.7 ± 0.6 h. Eighty percent of the dogs that underwent the operations were still alive for 15 days after the operations and none developed pancreatic fistulas. Examination by macroscopic observation, and hematoxylin and eosin staining of the pathological specimens showed that the anastomoses were completed healed.CONCLUSIONS:The use of a PB-MA in sutureless digestive tract reconstruction for pancreatoduodenectomy resulted in an elimination of pancreatic fistulas, and shortening of the stent removed time. In addition, the procedure is simple to perform, fast, and appears to be safe in this dog model.
