Abstract:OBJECTIVE:To investigate the possibility and effect of radical vaginal trachelectomy and laparoscopic pelvic lymphadenectomy in treating women with early cervical cancers who desired to maintain fertility. METHODS:From August 2001 through May 2003, we performed vaginal radical trachelectomy with laparoscopic lymphadenectomy on 12 patients with biopsy-proven early stage cervical carcinomas. Subjects were selected for this treatment on the basis of favorable cervical tumors and a desire to maintain fertility. Pelvic lymphadenectomies were performed in all patients. Vaginal radical trachelectomy was performed immediately if lymph nodes were negative. RESULTS:Twelve women underwent this procedure. The median age was 27.6 years (range 24 - 31 years); 10 were nulligravid and 2 were multipara. Mean operative time was 142 min (115 - 178 min), with a mean blood loss of 180 ml (120 - 230 ml), and an average hospital stay of 6.7 days. There was no intra- or post-operative complication. With an average follow-up of 21.5 months, there have been no recurrences. One woman has become pregnant. CONCLUSIONS:Radical vaginal trachelectomy with laparoscopic pelvic lymphadenectomy permits preservation of fertility in selected patients. It is mini-invasive, and needs shorter recovery time.
Early Cervical Cancer Recurred with Uterine Fundus Metastasis after Laparoscopic Fertility-Sparing Surgery: A Case Report

Abstract:Abstract Purpose: Recently, the safety of minimally invasive radical hysterectomy for cervical cancer caused widespread controversy. The value of minimally invasive fertility-sparing surgery for early stage cervical cancer is still lacking evidence of randomized controlled trial (RCT).Methods: Here we present a case of stage IB3 (FIGO2018) cervical carcinoma undergone neoadjuvant chemotherapy plus laparoscopic radical trachelectomy (A clinical trial NCT02624531) because of strongly desiring to preserve the fertility. Nine months after the initial treatment, the patient was presented with a uterine fundus mass and elevated serum squamous cell antigen (SCC Ag) and OC125 antigen (CA125).Results: Cervical cancer uterine fundus involvement may occur with advanced tumor stage. Early stage cervical cancer relapsed with a uterine fundus metastasis after laparoscopic radical trachelectomy is exceptionally rare. The patient was treated and cured by a radical hysterectomy. Post-operative pathology confirmed an isolated implanted squamous cell carcinoma in uterine fundus and no previous surgical margins and endometria were involved.Conclusions: This firstly reported case indicates that the safety of laparoscopic radical trachelectomy is still uncertain for advanced stage cervical cancer even if neoadjuvant and post-surgery chemotherapy is given, and also suggests that it is needed to discuss carefully the benefit and risk of such a strategy with the patient when the fertility preservation is considered.
Curative Effect of Laparoscopic-Assisted Vaginal Radical Trachelectomy Combined with Pelvic Lymph Node Dissection on Early-Stage Cervical Cancer

Abstract:PURPOSE:The purpose of this study was to explore the safety and feasibility of laparoscopic-assisted vaginal radical trachelectomy (VRT) combined with pelvic lymph node dissection in the treatment of early-stage cervical cancer.METHODS:Patients (N=136) with early-stage cervical cancer were divided into VRT group (laparoscopic-assisted VRT combined with pelvic lymph node dissection, n=68) and abdominal radical trachelectomy (ART) group (ART combined with pelvic lymph node dissection, n=68). The operation-related indexes, incidence of postoperative complications and fertility status were compared between the two groups, and the tumor recurrence status was recorded through follow-up. The factors related to recurrence of early-stage cervical cancer were analyzed by one-way analysis of variance and Cox multivariate regression analysis.RESULTS:VRT group had significantly less intraoperative blood loss and significantly shorter postoperative ventilation time than ART group. In VRT group, the length of vaginal resection and the width of parauterine tissue resection were significantly smaller than those in ART group. The pregnancy rate in VRT group (38.6%) was far higher than that in ART group (21.2%) (p=0.041), while the fertility rate had no statistically significant difference between the two groups (90.9% vs. 81.8%) (p=0.641). The results of univariate and multivariate analysis showed that the tumor diameter was an independent risk factor for the recurrence of early-stage cervical cancer (β=0.317, p=0.029, 95% CI=0.484-0.815).CONCLUSION:VRT is safe and feasible in the treatment of early-stage cervical cancer, which can reduce surgical damage, facilitate postoperative recovery, and effectively preserve the fertility of patients. The tumor diameter is an independent risk factor for the recurrence of early-stage cervical cancer.
A Case-control Study on Laparoscopic Versus Abdominal Radical Hysterectomy with Pelvic Lymphadenectomy for Early-stage Cervical Cancer

Abstract:Objective To determine whether total laparoscopic radical hysterectomy ( TLRH ) is a feasible alternative to abdominal radical hysterectomy ( ARH ) in the treatment of early-stage cervical cancer ( stage ⅠB -ⅡA ) . Methods The outcomes of 42 patients with cervical cancer (stageⅠB-ⅡA) who underwent laparoscopic (LPS group, n=21) or laparotomic (LPT group, n=21) radical hysterectomy and pelvic lymphadenectomy in our hospital from January 2005 to March 2010 were retrospectively estimated.All of the patients from the LPT group were matched 1∶1 with the patients from the LPS group based on stage , age, histological subtype , and nodal status for case-controlled analysis .The parameters were compared between two groups including operating time, intra-operative blood loss, number of lymph nodes resected, gastrointestinal recovery time, intra-and post-operative complications , hospital stay , relapse and mortality rate . Results The operating time and complications were similar between the two groups.The LPS group had less intra-operative blood loss [(233.3 ±202.1) ml vs.(983.3 ±462.2) ml, t=-6.814, P=0.000] and shorter gastro-intestinal recovery time [(41.0 ±13.9) h vs.(55.4 ±15.0) h, t=-3.219, P=0.003] as compared with the LPT group.However, the LPS group showed less lymph nodes resected [(19.7 ±8.2) vs.(27.5 ±7.7), t=-3.170, P=0.003] and longer indwelling catheter time [(31.8 ±23.4) d vs.(14.5 ±4.7) d, t=3.177, P=0.005] as compared with the LPT group.Follow-up visits for 53-107 months (median, 53 months) in 17 patients (with 13 patients in LPS group and 12 patients in LPT group lost of follow-up) found the relapse rate was similar between the two groups [0 (0/8) vs.11.1% (1/9), P=1.000]. Conclusions It is feasible to do radical hysterectomy and pelvic lymphadenectomy by laparoscopy instead of laparotomy for early -stage cervical cancer .Laparoscopy shows more favorable operative outcomes after mid-term follow-up time.
Safety and effectiveness of laparoscopic radical hysterectomy and pelvic lymphadenectomy in the treatment of early cervical cancer

Abstract:Objective To evaluate the safety and efficacy of laparoscopic radical hysterectomy and pelvic lymphadenectomy in the treatment of early cervical cancer. Methods 88 cases of early cervical cancer were selected by lots in July 2010-2012 July and randomly divided into control group and observation group,44 cases for each group. Control group received conventional open surgery,while observation group underwent laparoscopic surgery. Measurement data were processed by t test,the result of P 0. 05 was considered statistically significant. Results In observation group,the incision of operation was small,and the operation time was not prolonged,and the average postoperative hemorrhage was( 92. 3 ± 23. 5) ml, the number of lymph nodes were( 21. 5 ± 1. 6),the time for flatus was( 1. 5 ± 0. 2) h,the operation effect was better than that in control group,the level of pain after operation was( 1. 3 ± 0. 3),which was lower than that in control group,hospital stay was( 7. 2 ± 1. 4) d,which was shorter than that in control group( P 0. 05); significant difference was found between the two groups( P 0. 05). Conclusions Hysterectomy and pelvic lymphadenectomy can be successfully performed under the guidance of laparoscope,with a higher success rate,which is effective for early cervical cancer,therefore it can be used widely in clinical practice.

Analysis of 16 Patients with Early Cervical Cancer Treated by Laparoscopic Vaginal Radical Trachelectomy

Abstract:OBJECTIVETo evaluate the therapeutic efficacies of preserving fertility treatment in patients with early cervical cancer.METHODSSixteen patients with early cervical cancer treated by laparoscopic vaginal radical trachelectomy and pre- or postoperative chemotherapy were analyzed retrospectively, focusing on the treatment indication and management of high risk patients.RESULTSThe median age was 29 years (range 26 to 34 years). Eleven were nulligravida and 4 multipara. All patients had a desire to maintain fertility. For clinical stage, 2 were stage Ia2, 13 stage Ib1 and 1 stage Ib2. Fifteen patients had squamous cell carcinoma and 1 had adenosquamous cell carcinoma. Mean operative time was 3 hours and 12 minutes, and mean blood loss was 320 ml. There were no intra- or postoperative complications. With mean follow-up time of 13 months, one patient had recurrence (6%), and no one became pregnant.CONCLUSIONSIt is possible to preserve fertility in the treatment of patients with early cervical cancer, but treatment indication should be considered carefully. The management of high risk patients should be investigated extensively.
Comparisons of Vaginal and Abdominal Radical Trachelectomy for Early-Stage Cervical Cancer: Preliminary Results of a Multi-Center Research in China.

Abstract:Background: There are limited data comparing the prognosis and fertility outcomes of the patients with early cervical cancer treated by trans -vaginal radical trachelectomy (VRT) or abdominal radical trachelectomy (ART).The objective of this study was to compare the surgical and pathologic characteristics, the prognosis and fertility outcomes of the patients treated by VRT or ART. Methods: Matched-case study based on a prospectively maintained database of patients underwent radical trachelectomy in 10 centres of China was designed to compare the prognosis and fertility outcomes of the patients treated by VRT or ART. Results: Totally 150 cases, 77 in the VRT and 73 in the ART group, were included. VRT and ART provide similar surgical and pathological outcomes except larger specimens obtained by ART. In the ART group, no patient developed recurrent diseases, but, in the VRT group, 7 (9.8%) patients developed recurrent diseases and 2 (1.6%) patients died of the tumours ( P =0.035). The rate of pregnancy in the VRT group was significantly higher than those of ART (39.5% vs 8.8%; P =0.003). The patients with tumour size >2 cm showed significant higher recurrent rate (11.6% vs 2.4%, P <0.05) and lower pregnant rate (12.5% vs 32.1%, P =0.094) compared with the patients with tumour size <2 cm. Conclusion: Patients treated by ART obtained better oncology results, but their fertility outcomes were unfavourable compared with VRT. Tumour size <2 cm should be emphasised as an indication for radical trachelectomy for improving the outcome of fertility and prognosis.
Sentinel lymph node biopsy as guidance for radical trachelectomy in young patients with early stage cervical cancer

Abstract:Background The purpose of this study was to assess the feasibility and accuracy of sentinel lymph nodes (SLNs) detection using 99mTc phytate in predicting pelvic lymph nodes status for radical abdominal trachelectomy (RAT) in patients with early stage cervical cancer. Methods Sixty-eight women with stage IA2-IB1 cervical cancer and scheduled to undergo fertility-sparing surgery enrolled in this study. 99mTc-labeled phytate was injected before surgery. Intraoperatively, SLNs were identified, excised, and submitted to fast frozen section. Systematic bilateral pelvic lymphadenectomy and/or para-aortic lymph node dissection was performed. Then RAT was performed in patients with negative SLNs. All nodes were sent for routine pathological examination and immunostained with anti-cytokeratin antibody to detect micrometastases. Outcomes of follow up and fertility were observed. Results SLNs were identified in 64 of 68 patients (94.1%). Of these, SLNs of 8 patients (11.8%) were positive on frozen sections and proved to be metastasis by final pathologic examination. The sensitivity, accuracy, and false negative rates were 100%, 100%, and 0%, respectively. All 60 patients with negative SLN underwent RAT successfully. Two relapses occurred and no one died of tumor progression during follow-up. Five of the 15 patients with procreative desire conceived 8 pregnancies (3 term delivery, 2 premature birth, 1 spontaneous abortion, and 2 were still in the duration of pregnancy) after surgery. Conclusions The identification of SLN using 99mTc-labeled phytate is accurate and safe to assess pelvic nodes status in patients with early cervical cancer. SLNs biopsy guided RAT is feasible for patients who desire to have fertility preservation.

Abstract:To investigate the possibility and value of laparoscopic radial hysterectomy with lymphadenectomy in patients with gynecology malignancies.From July 2000 through June 2002, we performed laparoscopic radial hysterectomy with lymphadenectomy on 56 patients with biopsy-proven endometrial or cervical carcinoma (21 endometrial cancer and 35 cervical cancers). Pelvic, para-aortic, and combinations of both pelvic and para-aortic lymphadenectomies were performed depending on the primary site of the disease and indication for lymph node dissection.Laparoscopic procedure required an average of 3.1 h, with an average blood loss of 198 ml. The average number of nodes was 16. Patients undergoing laparoscopic assisted vaginal radical hysterectomy and lymphadenectomy alone were admitted for 9.6 days on average. There were 5 patients with intraoperative complications. Two patients were bladder injury and other two patients with venous injury. One patient was colon injury and converted to an abdominal procedure. There were no ureteral or intestinal injuries. Postoperative complications occurred in 3% of patients and only one was considered ureteral constriction 1 month after operation. There has been one patient with trocar site recurrences 4 weeks after operation. Three patients have retention of urine.Laparoscopic assisted vaginal radical hysterectomy and lymphadenectomy was feasible and safe for treatment of cervical or endometrial cancer. Also it was a less invasive, and recovery time was reduced. This was reliable and accurate method for evaluating lymph node metastasis of cervical or endometrial carcinoma.
Vaginal-assisted Gasless Laparoendoscopic Single-Site Radical Hysterectomy for Early Cervical Cancer: a Retrospective Pilot Study

Abstract:Background Minimally invasive surgery for early cervical cancer is debated. We developed this new vaginal-assisted gasless laparoendoscopic single-site radical hysterectomy for early cervical cancer, and we aimed to evaluate the feasibility and safety of this surgical procedure and observe the early oncologic outcomes. Methods From January 2019 to August 2020, patients with early cervical cancer who underwent vaginal-assisted gasless laparoendoscopic single-site radical hysterectomy were studied retrospectively. The clinical characteristics, pathologic outcomes, perioperative outcomes, and follow-up details of the patients were recorded. Results Forty-eight patients underwent vaginal-assisted gasless laparoendoscopic single-site radical hysterectomy were included, 14 (29.2%) with stage IB1, 13 (27.1%) with stage IB2, 7 (14.6%) with stage IB3, 10 (20.8%) with stage IIA1, and 4 (8.3%) stage with IA2. The mean age at diagnosis was 50.4 (range 28–72) years old. The mean operative time was 237.3 min (range 162–393), and the mean estimated blood loss was 246.5 ml (range 80–800). No intraoperative complications occurred, and there were no patients who were readmitted. Histological types were distributed as follows: squamous cell carcinoma 72.9%, adenocarcinoma 10.4%, and adenosquamous cell carcinoma 16.7%. There were 2 patients (4.2%) with positive nodes, 20 patients (41.7%) with positive lymphovascular space invasion, and 2 patients (4.2%) with positive parametria. Twenty-eight patients (58.3%) received adjuvant therapy after the operation. With a mean follow-up of 17.7 months (range 6–26), there were no recurrent cases, and 11 patients (22.9%) suffered lower limb lymphoedema. Conclusions The vaginal-assisted gasless laparoendoscopic single-site radical hysterectomy might be a feasible technique for early cervical cancer, with promising short-term oncological outcomes and safety. A prospective study with more patients and longer follow-up periods should be performed to further evaluate the safety and oncological outcomes.
Radical abdominal trachelectomy for cervical malignancies: surgical, oncological and fertility outcomes in 62 patients.

Abstract:To report our experience of radical abdominal trachelectomy for patients with cervical malignancies.We conducted a retrospective review of a prospectively maintained database of patients undergoing fertility-sparing radical abdominal trachelectomy for cervical malignancies at our institution from 04/2004 to 09/2010.Sixty-four patients with cervical malignancies underwent laparotomy for planned radical abdominal trachelectomy. Two patients needed immediate completion of radical hysterectomy due to unfavorable intraoperative findings. Median age was 29.5 years (range, 11-41). Histology included 8 (12.9%) with adenocarcinoma, 50 (80.65%) with squamous carcinoma, 1 (1.61%) with adenosquamous carcinoma and 3 (4.84%) with botryoid sarcoma. Median number of nodes evaluated was 25 (range, 12-53); Ten (16.13%) patients with pathologic risk factors received adjuvant therapy. Fourteen of 36 IB1 cases had tumor size >2cm. No recurrences were observed at a median follow-up of 22.8 months. Five (8.06%) patients developed postoperative cervical stenosis--all occurred before we started to routinely install T-IUDs during the procedure. Thirty-eight patients completed the survey which aimed to understand what factors influenced these patients' reproductive outcomes. For various reasons, only 10 patients attempted to conceive and 2 of them succeeded. One of them delivered by cesarean section after 39 weeks and the other is currently pregnant.Radical abdominal trachelectomy seems to be a reasonable option for selected patients whose tumors are no larger than 4cm when conducted by experienced gynecologic oncologists. The main perioperative complication is postoperative cervical stenosis, which could be effectively prevented by installation of a tailed T-IUD during the surgery. Social, familial and physical factors can largely influence the patients' reproductive outcomes. The issues of reproductive concerns and quality of life require further investigation.
Application of laparoscopicope rations for malignant uterine tumors

Abstract:Objective: To investigate the feasibility of laparoscopic modifled radical hysterectomy and abdomino pelvic lymphadenectomy in the treatment of malignant uterine tumors.To explore the safety and effective of laparoscopic operation as treatment for surgical staging of endometrial carcinoma and ovarian carcinoma.Methods: Laparoscopic extensive total hysterectomy and pelvic lymphadenectomy were performed in 44 cases of early-stage malignant uterine tumors.There were 36 cases with endometrial cancer and 7 cases with ovarian,carcinoma and one with salpingian cancer.Results: The operation was completed under laparoscope in 44 cases.All the operations were successfully completed and no complication was observed.The operation time was 120～180(mean,145) min;the intraoperative hemorrhage volume was 100～200(mean,140) ml;the number of excised lymphnodes was 10～28(mean,19).The surgical staging of endometrial carcinoma: StageⅠ of endometrial adenocarcinoma 26 cases;StageⅡ 3 cases(peritoneal positive cytology).StageⅡ of endometroid sarcoma 1 case;stage Ⅰb of endometriod papilliferous adenocarcinoma 2 cases;stage Ⅰ of cervical carcinosarcoma;StageⅢ a of endometriod adenocarcinoma(partly changing into sarcoma) 26 cases;StageⅠ of endometriod adeno-acanthoma 1 case.No positive pathological finding in second-staging operations in seven cases of ovarian cancer and one case papilliferous adenocarcinoma of fallopian tube.No electric damagement,lymphatic cyst and dysfunction of lower limbs by follow-up investigation in 9~22 months in 44 cases.Conclusions: Laparoscopic modifled radical hysterectomy and pelvic/para-aotic nodes dissection and omentectomy and appendicectomyl for the endometria and Ovarian cancer is safe,feasible,effective,and reliable.
Feasibility and Safety of Radical Mediastinal Lymphadenectomy in Thoracoscopic Esophagectomy for Esophageal Cancer

Abstract:OBJECTIVE:To explore the feasibility and safety of radical mediastinal lymphadenectomy in thoracoscopic esophagectomy for esophageal cancer. METHODS:We retrospectively analyzed the clinical data of a cohort of 147 patients with esophageal cancer who underwent thoracoscopic esophagectomy. Seventy-one patients received traditional lymphadenectomy for the periesophageal and subcarinal lymph nodes (traditional dissection group), while 76 patients received radical lymph node dissection for the bilateral recurrent laryngeal nerve lymphatic chains as well as the periesophageal and subcarinal nodes (radical dissection group). The number of retrieved lymph nodes, incidence of node metastasis, the operation time, estimated blood loss, complications, morbidity and mortality were compared between the two groups. RESULTS:Thoracoscopic esophagectomy was completed in all patients with no conversion to thoracotomy. There were no significant differences of operational time and blood loss between the two groups [(89 ± 32) min versus (99 ± 32) min, P = 0.064; (152 ± 108) ml versus (107 ± 82) ml, P = 0.261]. In all patients, the total and average number of retrieved mediastinal nodes were 1644 and 11.2 ± 5.9/case, and the incidence of metastatic lymph nodes was 24.5%. The total and average number of retrieved mediastinal nodes were 999 and 13.6 ± 6.4/case in the radical dissection group, significantly higher than that in the traditional dissection group (645 and 9.1 ± 4.4/case) (P < 0.001). The average number of retrieved nodes and node metastasis rate in the radical dissection group were significantly higher than in the traditional dissection group (13.1 ± 6.4 versus 9.1 ± 4.4, P < 0.001; 35.5% versus 12.7%, P < 0.05). In the total group of 147 cases, the metastasis rates of periesophageal, subcarinal nodes and nodes along the recurrent laryngeal nerve lymphatic chains were 19.7%, 3.4% and 17.1%, respectively. There was thoracic lymph node metastasis in 27 cases with a metastasis rate of 35.5% in the radical dissection group, significantly higher than that in 9 cases and 12.7% in the traditional dissection group (P > 0.001). In the 1644 retrieved thoracic lymph nodes, there were 90 positive metastatic lymph nodes (metastasis ratio 5.5%). Among the 1644 lymph nodes, the metastasis ratio of periesophageal lymph nodes was 6.5%, subcarinal lymph modes 2.4% and the recurrent laryngeal nerve chain lymph nodes 7.5%. Among the 147 cases, complications occurred in 60 cases, with an overall morbidity rate of 40.8%. There were no significant differences of the incidence rates of overall complications, pulmonary complications and recurrent laryngeal nerve injury between the two groups (P > 0.05). CONCLUSION:Thoracoscopic esophagectomy with radical mediastinal lymphadenectomy is technically Safe and feasible.
Clinical Outcomes of Pelvic Lymph Node Dissection Before Versus after Robot-Assisted Laparoscopic Radical Cystectomy

Abstract:Objective: The purpose of this study was to compare the clinical outcomes of bladder cancer patients treated with extended pelvic lymph node dissection (ePLND) before or after cystectomy under robotic-assisted radical cystectomy (RARC).Methods: A retrospective study to identify 348 patients with bladder cancer who underwent RARC was performed. Of the patients, 152 (42.8%) underwent ePLND before radical cystectomy (RC) (group A) and 196 (56.3%) underwent ePLND after RC (group B). The clinical, pathological, and overall survival were compared.Results: The total and RC operation time in Group A (total: 130.68 +/- 29.25 minutes, RC: 59.45 +/- 28.63 minutes) were both shorter than Group B (total: 154.17 +/- 38.18 minutes, RC: 94.81 +/- 41.21 minutes) (P < .05). However, no significant difference in time of ePLND. The estimate blood loss (EBL) of RC part and total operation (RC+ePLND) in group A was less than group B (both P < .05), while the ePLND part did not show significance. The result of vascular and nerve injury and surgical drain withdrawal time were similar in two groups. The total number of lymph nodes in group A was fewer than group B (16 versus 26; P < .05). Moreover, the number of bilateral internal iliac and presacral lymph nodes of group A was fewer than group B significantly, whereas the number of bilateral external iliac, common iliac, and obturator lymph nodes was similar in two groups. The lymph node density of group A was significantly lower than group B. The median follow-up of all patients was 33.0 months. Importantly, the survival of group B was better than group A (hazard ratio: 1.412; 95% confidence interval: 1.004-1.987; P = .048).Conclusions: Performing ePLND before RC reveals better result on operation time and EBL, while, when ePLND after RC, the total number of lymph nodes dissected is more and the survival is better. It recommended ePLND be performed before RC, and it is necessary to recheck the internal iliac and presacral area after cystectomy.

Abstract:Objective:To investigate the feasibility and clinical efficacy of laparoscopic radical cholecystectomy(LRC) for gallbladder cancer.Methods:The clinical data of 247 patients with gallbladder cancer who underwent radical resection from Jan 2013 to Dec 2019 at Department of General Surgery, Sir Run Run Shaw Hospital affiliated to Zhejiang University School of Medicine was analyzed retrospectively. After propensity score matching, 54 patients were included in laparoscopic group and 103 in laparotomy group. The clinicopathological characteristics and the short- and long-term outcomes were compared.Results:Compared to the laparotomy group, patients in the laparoscopic group had less intraoperative blood loss [100(50,200)ml vs. 200(100,300) ml, Z=4.105, P<0.001], earlier postoperative oral diet[1.0(1.0,2.0) d vs. 2.0(1.0,4.0) d, Z=4.157, P<0.001]and drainage removal[6.5(4.0,12.5) d vs. 9.0(6.0,16.0) d, Z=2.769, P=0.006], shorter hospital stay[7.0(5.0,9.3) d vs. 9.0(8.0,14.0) d, Z=3.923, P<0.001]. The number of lymph node dissection in laparoscopic group was significantly lesser than that in open group [6(4,9) vs. 8(5,12), Z=2.639, P=0.008]. There were no significant differences between the two groups in postoperative complications, short-term and long-term survival outcomes. Conclusions:Laparoscopic radical surgery for gallbladder cancer is as safe and feasible, and identical survival prognosis as open surgery, and moreover a less traumatic procedure.

Abstract:The aim of the present study was to introduce and describe a novel technique, which has been termed 'laparoscopic local extraperitoneal para-aortic lymphadenectomy', and to evaluate its feasibility and safety. In this retrospective case study, a series of 21 patients were selected who underwent laparoscopic local extraperitoneal para-aortic lymphadenectomy for gynecological malignancies between March and August 2014 at the Obstetrics and Gynecology Hospital of Fudan University, Shanghai, China. All the surgical procedures were performed by the same operational team. Patients' data were collected retrospectively, and the characteristics included patient age, body mass index (BMI), tumor stage, tumor grade, blood loss, operative time (laparoscopic local extraperitoneal para-aortic lymphadenectomy), length of hospital stay, intraoperative and postoperative complications, pathology, lymph node count and lymph node status. A total of 21 patients underwent laparoscopic local extraperitoneal para-aortic lymphadenectomy. The median patient age was 52 years (range, 45-71 years); the median BMI was 24.8 kg/m2 (range, 22.2-32.4 kg/m2); the median operating time for the para-aortic lymphadenectomy was 70 min (range, 58-95 min); and the median estimated blood loss of the total surgery was 200 ml (range, 100-600 ml). No patient required a blood transfusion during the operation. The median length of hospital stay was 7 days (range, 5-9 days), and the median number of para-aortic lymph nodes was measured as 12 (range, 7-17). Postoperative complications included one patient with chylous ascites, who responded to conservative management with intravenous somatostatin. In conclusion, the present study has demonstrated that laparoscopic local extraperitoneal para-aortic lymphadenectomy, which avoids interference of the bowels during surgery, is safe and practical.

Abstract:Radical trachelectomy allows for fertility preservation in patients with early cervical cancer not qualifying as "low-risk" as defined by ConCerv. This study reports on the 10-year surgical, oncological, and obstetrical experience of patients treated by radical abdominal trachelectomy at an Austrian tertiary care center. A retrospective chart analysis and telephone survey of all patients with FIGO stage IA2-IB2 (2018) cervical cancer treated by radical abdominal trachelectomy and pelvic lymphadenectomy between 2013 and 2022 were performed. Radical abdominal trachelectomy was attempted in 29 patients, of whom 3 patients underwent neoadjuvant chemotherapy. Three cases, including one after neoadjuvant therapy, required conversion to radical hysterectomy due to positive margins; four cases had positive lymph nodes following surgical staging and were referred to primary chemo-radiotherapy. Twenty-two (75.9%) successful abdominal radical trachelectomies preserving fertility were performed. According to final histopathology, 79.3% of tumors would not have met the "low-risk"-criteria. At a median follow-up of 64.5 (25.5–104.0) months, no recurrence was observed. Eight (36.4%) patients attempted to conceive, with a live birth rate of 62.5%. Radical abdominal trachelectomy appears oncologically safe in early-stage cervical cancers that do not fulfill the "low-risk"-criteria. Strict preoperative selection of patients who might qualify for more conservative surgical approaches is strongly recommended.

Abstract:BACKGROUND:Removing more inframesenteric nodes is not only significantly increases the likelihood of finding metastasis for endometrial cancer, but also can add survival advantage. As most patients diagnosed with endometrial cancer are overweight or obesity, a high efficiency approach is important. Aim of this study was to compare the surgical outcomes of extraperitoneal laparoscopic, transperitoneal laparoscopic, and laparotomic para-aortic lymphadenectomy in endometrial carcinoma staging.METHODS:We retrospectively reviewed data of all patients diagnosed with primary endometrial carcinoma who were treated at the Department of Gynecologic Oncology, Fudan University Shanghai Cancer Center from 1 January 2017 to 31 December 2019. The numbers of para-aortic lymph nodes, surgical time, complications, blood loss and hospital stay were compared. The patients' medical records and pathological reports were carefully reviewed. Statistical significance was defined as p < 0.05.RESULTS:We retrospectively compared patients who underwent extraperitoneal laparoscopy (Group E, n = 20), transperitoneal laparoscopy (group T, n = 21), and laparotomy (group L, n = 135). The median number of para-aortic lymph nodes was significantly higher in group E than in groups T and L (9.5, 5, and 6, respectively; p = 0.004 and 0.0004, respectively). All patients in group E underwent successfully dissection to the renal vessel level. The median operation time was significantly shorter in group L than in groups T and E (94, 174, and 233 min, respectively; p < 0.0001). The median estimated blood loss volume was higher in group L than in groups T and E (200, 100, and 142.5 ml, respectively; all comparisons p < 0.001), and the length of hospital stay was significantly longer in group L than in Groups T and E (6, 5, and 6 days, respectively; all comparisons p < 0.001).CONCLUSION:The extraperitoneal laparoscopic approach for staging endometrial carcinoma harvested higher numbers of para-aortic lymph nodes which could be considered for endometrial carcinoma staging, especially for para-aortic lymph node harvest.

Abstract:Objectives. The purpose of this study was to retrospectively evaluate the technique, feasibility and oncological safety of laparoscopic radical hysterectomy with vaginectomy and reconstruction of vagina in patients with stage I primary vaginal carcinomas.Methods. Between February 2003 and July 2004, four patients, that had needs of sexual life, aging from 41 to 61 years with stage I primary vaginal carcinoma located at the upper third or 2/3 of the vagina, were submitted to laparoscopic radical hysterectomy with vaginectomy and reconstruction of the vagina using the sigmoid colon.Results. The average operative time was 305 min (range260-350 min). The average estimated blood loss was 325 ml (range 250-400 ml), and the medial number of the lymph nodes removed was 16 (range 13-20). All surgical margins and nodes removed were negative histopathologically. There were no intra-operative and postoperative complications. The mean stay day after surgery was 7 days (range 6-8 days). The mean length of a neovagina was 13 cm (range 12-15 cm) and the introitus admitted two fingers in breadth. The mean follow-up was 46 months (range 40-54 months). All patients are clinically free of disease and have satisfactory sexual life. None require dilation of the introitus. During the first 6 months, all the patients had little complaints of excessive leucorrhoea.Conclusions. To our knowledge, this is the first reported laparoscopical radical surgery combined with reconstruction of the vagina in patients with early-stage primary vaginal cancer. Our results have demonstrated the oncological safety and feasibility of the laparoscopical procedure. Intermediate-term follow-up validates the adequacy of this procedure. (c) 2007 Elsevier Inc.
