Radiomics Of Multiparametric Mri Predicts Prognosis Of Localized Prostate Cancer After Radiation Therapy

Abstract:To identify and validate multiparametric magnetic resonance imaging (MRI)-based radiomics as prognostic factors in patients with localized prostate cancer after radiation therapy. From the 2011 to 2016, a total of 93 consecutive patients with T1-4N0M0 prostate cancer were identified and randomly divided into three cohorts for the neural network modeling (training cohort: n=60; validation cohort: n=14; test cohort: n=19). All patients were treated with neoadjuvant endocrine therapy followed by radiation therapy. Four categories of features related to texture and shape were obtained from MR T1, T2 and diffusion weighted imaging (DWI) series. They include first-order statistical features (FOS), texture features, 3-D discrete wavelet transform (3D-DWT) features, and 3-D histogram of oriented gradients (3D-HOG) features. Through Wilcoxon sign rank test, a total of 50 distinct signatures were extracted from 1836 radiomics features and used in our neural network model. The neural network model was trained using training cohort, finalized by validation cohort and validated by test cohort. Among 93 patients, 27 (29%) were classified as biochemical recurrence (BCR) and 66 (71%) as non-BCR. After the neural network was trained, the correct rate of classification of training samples was 97.8% and 92.9% respectively for BCR and non-BCR groups, 88.9% and 60% for the validation cohort, 84.2% and 75% for the test cohort. The area under the curve (AUC) of receiver operating characteristic curve (ROC) indices of training and validation cohort was 0.99 and 0.92, respectively. The AUC of the test cohort was 0.89 (95%CI,0.728-0.967). In the error histogram index, the classification errors of three cohorts all showed normal distribution characteristic, and the error was negative 0.042. Attached table shows the test cohort results. To our knowledge, this is the first study to explore MRI-based radiomics to predict the prognosis of localized prostate cancer. Multiparametric MRI-based radiomics was proven to be feasible to predict the BCR of localized prostate cancer patients. The mechanism behind why radiomics features are associated with cancer prognoses has not been fully understood. However, intra-tumor heterogeneity is believed to be one of the major factors because of its important role on diagnosis and prognosis. Radiomics approach can consistently extract higher dimension image features intrinsic to shape and texture, which are highly correlated with intra-tumor heterogeneity.
