Abstract:function contributes to overall RV function and would provide prognostic insight into disease severity in patients with known or suspected PH.Methods: We retrospectively studied 331 consenting adult patients with known or suspected PH.We assessed the relationship between the quantitative RA measurement and severity of PH in our pre-capillary PH registry at our Clinic between January 2012 and January 2014.The components of RA function and the interaction of RA and RV contractility was assessed through the comprehensive quantitation of RA and RV deformation mechanics.Results: Of 331 patients, 22.6% had WHO functional class (Fc) I, 41.2% had Fc II, 32.9% had Fc III, and 3.1% had Fc IV.The mean age was 61±15 years, and 65.6% were women.We recorded 57 deaths (17.2%) during follow-up period.There was an increasing trend (p<0.05) in RA pressure, severe RA and RV dilatation, and PVR among patients with increased pulmonary artery systolic pressure (PASP).Interestingly, deceased patients had significantly lower in overall RA function; RA reservoir function (48±16% vs. 58±12%); RA passive conduit function (20±10% vs. 28±12%); and RA active pumping function (35±14% vs. 42±13%) (all p<0.0001).The mean value of RA longitudinal strain in PH patients was 51±20 (36±20% in deaths vs. 53±19% in alive group).Deceased patients had significantly lower in systolic atrial strain rate (1.43±0.55s -1 vs. 2.07±0.65 s -1 ), early diastolic strain rate (-0.87±5 s -1 vs. -1.24±0.55s -1 ) and late diastolic strain rate (-1.38±0.71s -1 vs. -1.86±0.78s -1 ) compared with alive group at follow up (all p<0.0001).A reduction in RA functions (reservoir and conduit), RA strain and strain rate were significantly associated with those with increased PASP and mortality (all p<0.0001).Although RA active pumping function is not significantly decreased with severity of PA systolic pressure (p=0.08), the contribution of RA active pumping function is increased with the severity of PASP (p<0.0001).Multivariate Cox regression analysis showed that RA reservoir function is statistically significant with mortality in PH patients even after adjusting for age, gender, functional capacity and RV function.Conclusions: All components of RA function and RA mechanics were decreased with RV dysfunction and severity of PA systolic pressure.A reservoir function were significantly associated with poor clinical outcomes in PH patients, regardless of RV function and functional capacity.
