Abstract:Objective To evaluate the outcomes and prognostic factors of image-guided 125I seed implantation for locally recurrent soft tissue sarcoma(RSTS).Methods A total of 60 patients with RSTS who received image-guided 125I seed implantation in Peking University Third Hospital,from September 2002 to December 2015,were retrospectively analyzed.The enrollment criteria: KPS ＞60 points,refused or could not tolerate surgery or radiotherapy,the expecting survival time ＞3 months,relapsed after multiple treatment of soft tissue sarcoma,and underwent CT or ultrasound guided 125 I seed implantation treatment.In all,the median activity of seeds was 25.9×106Bq(range,11.1×106-29.6×106Bq),median number of implanted seeds was 58(range,3-133),and the median D90was 120 Gy(range,36.50-460.97 Gy).The local progression-free survival(LPFS)and overall survival(OS)were calculated using the Kaplan-Meier method.The log-rank test and Cox regression model were used for the univariate and multivariate analyses.Results The median follow-up was 18.75 months(range,1-146).The median OS was 18.5 months(95％CI 13.1-23.9).The 1-,3-and 5-year OS rate were 63.3％,33.0％and 29.5％,respectively.The 1-,3-and 5-year LPFS rate were 72.5％,63.7％and 59.7％,respectively.The general rate of pain relieving was 100％(6/6).8.3％(5/60)presented grade Ⅳskin toxicity.No fatal complications ocurred.The univariate analysis suggested that tumor size,tumor volume,KPS score,D90were prognostic factors of OS and LPFS.The multivariate analysis demonstrated that previous chemotherapy history and distant metastases were independent prognostic factors of survival.Conclusions Image-guided 125I seed implantation for recurrent soft tissue sarcoma is a safe treatment option with high efficacy and low morbidity.Tumor size and D90were the prognostic factors of OS and LPFS.

Abstract:11073 Background: To observe the effect of ultrasound / CT guided radioactive 125I seed implantation in the treatment of recurrent soft tissue sarcoma and its relationship with physical dosimetry prognostic factors. Methods: The data of 37 patients with recurrent soft tissue sarcoma who received ultrasound/CT-guided 125I seed implantation from November 2005 to December 2015 were retrospectively analyzed. The local progression-free survival rate and overall survival rate were evaluated. The relationship of local progression-free survival rate and overall survival with physical dosimetric parameters was analyzed. Results: Thirty-seven patients, 20 males and 17 females, with a median age of 53 years (16-79 years), received a median radiation dose of 60 Gy (28 Gy-120 Gy). The median tumor volume was 46.8 cm3 (0.5-252.2 cm3), the median particle activity was 0.67 mCi (0.4-0.84 mCi), and the median implanted particle number was 60 (3-158). The median follow-up time was 20 months (range: 1~144 months). The median overall survival time was 20.0 months (95% CI 16.4-23.6 months). The overall survival rates of 1, 3 and 5 years were 62.2%, 34.3% and 27.7% respectively. The median local progression-free time was 63.0 months. The 1-year, 3-year and 5-year local progression-free survival rates were 68.9%, 55.0% and 47.1%, respectively. Correlation analysis showed that HI was positively correlated with total survival and local progression-free survival (P = 0.001). Multivariate analysis showed that HI ( > 0.25) was an independent prognostic factor for long overall survival (P = 0.048, HR 0.39), and D90 ( > 110 Gy) was an independent prognostic factor for long local progression-free survival (P = 0.024, HR 0.17). Conclusions: Ultrasound/CT guided 125I seed implantation is a safe and effective method for the treatment of recurrent soft tissue sarcoma with high local control rate. The HI and D90 of the postoperative plan maybe affect the therapeutic efficacy.

Abstract:Soft tissue sarcomas are marked by a poor outcome, especially recurrence. This study aimed to explore whether the adoption of interstitial 125I seed implantation in patients with recurrence soft tissue sarcoma after surgery combined with radiation. We conducted a retrospective review of 31 patients (median age, 64 years; range, 6-79 years; 14 male, 17 female) with local recurrence after surgery combined with radiation of soft tissue sarcoma, who underwent 125I seed implantation in our department from 2003 to 2012. Details of tumor volumes were assessed using CT 1 to 2 days prior to seed implantation, and pretreatment planning was received. The seeds were implanted using a Mick applicator (Mick Radio-Nuclear Instruments Inc., Mount Vernon, NY), and the distance between the centers of any 2 seeds was maintained at approximately 1.0 cm. Tumor response was initially evaluated at 4 weeks and thereafter at intervals of 2 to 3 months. Disease status was assessed by physical examination, liver function tests, and full blood and platelet counts. Radiological measurement of disease progression, which included ultrasound examination and CT scans, was performed. All the patients were followed up until expiration. The median followup period, which was measured from the time of seed implantation, was 21 months (range, 3-53 months). All statistical analyses were onducted using SPSS version 19.0 (SPSS, Chicago, IL). The activity of the 125I seeds ranged from 0.5-0.8 mCi (median, 0.7 mCi). The prescribed doses delivered to 90% of the gross tumor volume as defined by treatment planning system using a dose-volume histogram (D90) was 143-184 Gy (median, 170 Gy). The median overall survival time was 34 months (3-43 months), and 1- and 3- year survival rates were 86.3% and 65.3%, respectively. There was no serious complication was observed. Of the 31 patients, 8 (25.8%) patients still alive, 17 (54.8%) died of multi hematogenous metastasis, and 6 (19.4%) patients died of pneumonia. No serious complications were observed following seed implantation. There was no evidence of bone or soft tissue necrosis, carotid rupture, neuropathy, blood vessel damage, or late complications. In conclusion, the results of our study suggest that percutaneous 125I seed implantation is a safe and effective salvage treatment for recurrent soft tissue sarcoma after surgery combined with radiation.

Abstract:The purpose of this study was to evaluate the safety and efficacy of percutaneous iodine-125 (I-125) seed implantation using computed tomography (CT) or ultrasound guidance in the treatment of recurrent soft tissue malignancies after surgery and radiotherapy. From February 2002 to September 2009, 18 patients with recurrent soft tissue sarcomas were treated under ultrasound or CT guidance. The actuarial median number of I-125 seeds implanted was 35 (range, 6-129), and the actuarial D90 of the implanted I-125 seeds ranged from 107.9 to 204.4 Gy (median, 147.1 Gy). The activity of the seeds ranged from 0.4 to 0.8 mCi (median, 0.7mCi). Follow-up times ranged from 4 to 78 months (median, 20 months). The median local control was 41 months (95% CI, 15.9-66.1 months). The 1-, 2-, 3-, 4-, and 5-year local controls were 78.8%, 78.8%, 78.8%, 26.3%, and 0%, respectively. The median survival was 32 months (95% CI, 16-48 months). The actuarial 1-, 2-, 3-, 4-, and 5-year survivals were 76.6%, 61.3%, 39.4%, 39.4%, and 39.4%, respectively. Seven (7) patients (38.9%) experienced recurrence after seed implantation. Six (6) patients (33.3%) died of distant metastases and 1 died of stroke. Two (2) patients developed ulceration, 1 case caused by recurrence and another by a reaction of the skin to radiation. Percutaneous I-125 seed implantation for recurrent soft tissue malignancies under CT or ultrasound guidance is safe and is associated with high efficacy and low morbidity.

Abstract:OBJECTIVES: The objective of this study was to evaluate the efficacy and safety of CT-guided radioactive I-125 seed implantation as a salvage treatment for locally recurrent head and neck soft tissue sarcoma (HNSTS) after surgery and external beam radiotherapy. METHODS AND MATERIALS: From December 2006 to February 2018, 25 patients with locally recurrent HNSTS after surgery and external beam radiotherapy were enrolled. All the patients successfully underwent CT-guided I-125 seed implantation. The primary end points included the objective response rate (ORR) and local progression-free survival (LPFS). The secondary end points were survival (OS) and safety profiles. RESULTS: After I-125 seed implantation, the ORR was 76.0%. The 1-, 3-, and 5-year LPFS rates were 65.6%, 34.4%, and 22.9%, respectively, with the median LPFS of 16.0 months. The 1-, 3-, and 5-year OS rates were 70.8%, 46.6%, and 34.0%, respectively, with the median OS of 28.0 months. Furthermore, univariate analyses showed that the recurrent T stage and histological grade were prognostic factors of LPFS, whereas only the histological grade was a predictor of OS. The major adverse events were skin/mucosal toxicities, which were generally of lower grade (<= Grade 2) and were well tolerated. CONCLUSIONS: Radioactive I-125 seed implantation could be an effective and safe alternative treatment for locally recurrent HNSTS after failure of surgery and radiotherapy. Effects Of Radioactive Iodine-125 Seed Implantation On Therapeutic Efficacy, Serum Vegf, Cea Levels, And Prognosis Of Patients With Ocular Malignant Tumors

Abstract:Objective: The aim of the current study was to investigate the effects of radioactive iodine-125 seed implantation on therapeutic efficacy, serum vascular endothelial growth factor (VEGF), carcinoembryonic antigen (CEA) levels, and prognosis of patients with ocular malignant tumors. Methods: A total of 86 patients with ocular malignant tumors were randomly allocated into the adjuvant chemotherapy group or iodine-125 seed implantation group, based on a random number table, with 43 patients in each group. The chemotherapy group underwent ocular tumor resections and routine postoperative adjuvant chemotherapy, while the iodine-125 group was treated with tumor resections and intraoperative iodine-125 seed implantation. At 3 months after surgery, after the chemotherapy group had received 2 cycles of chemotherapy, total effective rates were accessed in both groups. Serum CEA levels, before and after treatment, were detected by automatic electrochemiluminescence. Serum VEGF levels were also measured, before and after treatment, in both groups via ELISA. Correlation levels between CEA and VEGF were analyzed. Incidence rates of side effects, during treatment, were recorded and compared between the two groups. One-year progression-free survival and 3-year survival rates of the two groups were compared via regular follow-ups. Results: The total effective rate of the iodine-125 group was higher than that of chemotherapy group (P<0.05). In addition, expression levels of serum CEA and VEFG in the two groups, after treatment, were lower than those before treatment (all P<0.05). Decreases in the iodine-125 group were more significant than those in the chemotherapy group (both P<0.05). Serum CEA levels and VEGF levels were positively correlated. Incidence of side effects in the iodine-125 group was significantly lower than that in the chemotherapy group (P<0.05). Furthermore, 1-year progression-free survival and 3-year survival rates in the iodine-125 group were higher than those in the chemotherapy group. Differences were statistically significant (both P<0.05). Conclusion: Radioactive iodine-125 seed implantation in patients with ocular malignant tumors can effectively improve therapeutic efficacy, reduce serum levels of tumor markers CEA and VEGF, and improve 1-year progression-free survival and 3-year survival rates. This method shows a high level of safety. Thus, it is highly recommended for clinical practice.

Abstract:Objective To evaluate the efficacy of computed tomography ( CT ) image?guided 125 I radioactive seed implantation for locally recurrent rectal cancer ( LRRC ) , and to analyze the relationship between the dosimetry and prognosis. Methods A retrospective analysis was performed on the clinical data of 36 patients with LRRC who received CT image?guided 125 I seed implantation in our hospital from 2003 to 2011. Dosimetric verification was performed using CT scan immediately after 125 I seed implantation. The D90 , D100 , V100 , and V150 values were evaluated. In all the patients, the median activity of seeds was 0?7 mCi (0?4?0?8 mCi) and the median number of implanted seeds was 74(33?137). The local control (LC) and overall survival ( OS ) rates were calculated using the Kaplan?Meier method. The log?rank test and Cox regression model were used for the univariate and multivariate analyses, respectively. Results The median OS time was 16?2 months ( 95% CI= 13?5?18?9 months ) . The median LC time was 10?0 months (95% CI=6?2?13?8 months). The D90 and V100 values were (118.6±25?1) Gy and (90.0±0?3)%, respectively. The univariate analysis suggested that D90 was correlated with the LC time ( P=0?048) and V100 was correlated with the OS time ( P=0?035) . The multivariate analysis showed that a V100 value higher than 90% was a prognostic factor of OS (P=0?044). Conclusions In the treatment of LRRC using CT image?guided 125 I radioactive seed implantation, a D90 value larger than 140 Gy and a V100 value higher than 90% in the postoperative verification plan help improve the LC and OS rates. The D90 and V100 values in the postoperative verification plan may predict treatment outcomes in patients.

Abstract:Background The management of pediatric recurrent or metastatic soft tissue sarcoma after multimodal treatment remains challenging. We investigated the feasibility, efficacy, and morbidity of permanent interstitial 125 I seed implantation under image guidance as a salvage treatment for pediatric patients with recurrent or metastatic soft tissue sarcoma. Methods This was a retrospective study of 10 patients who underwent percutaneous ultrasound or computed tomography (CT) guided permanent 125 I seed implantation. Postoperative dosimetry was performed for all patients. Actuarial D90 was 121–187.1 Gy (median, 170.3 Gy). The number of 125 I seeds implanted was 6–158 (median, 34.5), with a median specific activity of 0.7 mCi per seed (range, 0.62–0.8 mCi); total activity was 4.2–113.76 mCi. Follow-up time was 6–107 months (median, 27.5 months); no patients were lost to follow-up. Results The overall response rate (complete response + partial response) was 8/10 (80 %), including two patients with complete response (CR) (20 %) and five patients with partial response (PR) (60 %). Local control rates after 1 and 2 years were 70.1 and 62.3 %, respectively, with a mean local control time of 70.6 months (95 % confidence interval (CI) 45.1–96.0). Survival rates after 1 and 2 years were 68.6 and 57.1 %, respectively, with a mean survival time of 65.3 months (95 % CI 34.1–96.5). Three patients died from distant metastasis; one died from local recurrence 12 months after seed implantation. Three patients suffered a grade I skin reaction and one developed ulceration. No severe adverse neurologic sequelae or blood vessel damage occurred. Conclusions Image guided permanent interstitial 125 I seed implantation as a salvage treatment appears to have a satisfactory outcome in children with recurrent or metastatic soft tissue sarcoma.
Interstitial implantation of ~(125)I seeds for the treatment of soft tissue tumors

Abstract:Objective To investigate the efficacy and safety of ~(125)I seeds interstitial implantation for soft tissue tumors. Methods Interstitial implantation of ~(125)I seeds was performed on 12 patients with soft tissue tumors (15 lesions including 2 primary lesions and 13 local recurrent or metastatic lesions). A mean of 60 (7-80) seeds were implanted into each lesion. The radioactivity of the ~(125)I seeds ranged from 0.5 to 0.8 mCi (mean, 0.6 mCi), and the matched peripheral doses (MPD) was 140-160 Gy. Quality check was carried out after the implantation. And then, supplemental radiation therapy was performed on 2 lesions with a dosage of 30 and 60 Gy respectively (2.0 Gy/f). Results The patients were followed up for a median of 17 months (3-34 months). The rate of pain relief was 83%(5/6); actual clinical effective rate (CR+PR+SD>2 months) was 93%(14/15); both 1- and 2-year local control rates were 83%; and 1- and 2-year survival rates were 72.9% and 62.5%, respectively. The median survival time was 32 months. Three patients had distant metastasis. Five patients died of lung metastasis (1 case) or multiple organ failure (4 cases). No complication or side-effect was observed in the patients, except skin ulceration in one patient. Conclusions Interstitial implantation of ~(125)I seed combined with or without external beam radiation therapy is convenient, effective, safe, and minimally invasive for soft tissue tumor, especially for metastatic or recurrent tumors.
Efficacy And Dosimetry Analysis Of Image-Guided Radioactive I-125 Seed Implantation As Salvage Treatment For Pelvic Recurrent Cervical Cancer After External Beam Radiotherapy

Abstract:Objective: To investigate the efficacy of image-guided radioactive I-125 seed (IGRIS) implantation for pelvic recurrent cervical cancer (PRCC) after external beam radiotherapy (EBRT), and analyze the influence of clinical and dosimetric factors on efficacy.Methods: From July 2005 to October 2015, 36 patients with PRCC received IGRIS. We evaluated local progression-free survival (LPFS) and overall survival (OS).Results: The median follow up was 11.5 months. The 1- and 2-year LPFS rate was 34.9% and 20%, respectively. The multivariate analysis indicated recurrence site (central or pelvic wall) (hazard ratio [HR]=0.294; 95% confidence interval [CI]=0.121-0.718), lesion volume (HR=2.898; 95% CI=1.139-7.372), D-90 (HR=0.332; 95% CI=0.130-0.850) were the independent factors affecting LPFS. The 1- and 2-year OS rate was 52.0% and 19.6%, respectively. The multivariate analysis suggested pathological type (HR=9.713; 95% CI=2.136-44.176) and recurrence site (HR=0.358; 95% CI=0.136-0.940) were the independent factors affecting OS. The dosimetric parameters of 33 patients mainly included D-90 (128.5 +/- 47.4 Gy), D-100 (50.4 +/- 23.7 Gy) and V-100 (86.7%+/- 12.9%). When D-90 >= 105 Gy or D-100 >= 55 Gy or V-100 >= 91%, LPFS was extended significantly, but no significant difference for OS. The 79.2% of 24 patients with local pain were suffering from pain downgraded after radioactive I-125 seed implantation.Conclusion: IGRIS implantation could be a safe and effective salvage treatment for PRCC after EBRT, which could markedly release the pain. Recurrence site, tumor volume and dose were the main factors affected efficacy. Compared with central recurrence, it was more suitable for patients with pelvic wall recurrent cervical cancer after EBRT.
CT-Guided 125i Seed Implantation for In-Field Recurrence of NSCLC with Prior Radiation

Abstract:To assess the feasibility, efficacy, and morbidity of CT image-guided radioactive iodine 125 (125I) seed implantation for in-field recurrence of NSCLC with prior radiation. 14 patients received CT-guided percutaneous permanent implantation of 125I seeds for in-field recurrence of NSCLC between October 2010 and August 2014. All patients received radiotherapy before seed implantation. The tumor volumes were measured using CT scans at 5-mm intervals one week before seed implantation. Postoperative dosimetry was routinely performed for all patients. The D90 (the doses delivered to 90% of the target volume defined by CT using dose-volume histogram) of the implanted 125I seeds ranged from 83Gy to 224Gy (median: 111 Gy). The activity of 125I seeds ranged from 0.58mCi to 0.75mCi (median: 0.72mCi). The total number of seeds implanted ranged from 16 to 61 (median: 39). The survival and local control probabilities were calculated by the Kaplan-Meier method. The median local control was 7months (95% CI, 4.26-9.74 months). The 1- and 2-year local control rates were 33.3%, and 33.3%, respectively. The estimate median overall survival is 18 months (95% CI, 11.91-24.09 months). The 1- and 2-year overall survival rates were 67.5%, 27.0% respectively. 71.4% of the symptoms (10/14) had disappeared or improved. No patients had radiation pneumonia, or radiation esophagitis in follow-up. One patient developed lethal hemoptysis 18 months after seed implantation. Interstitial permanent implantation of 125-I seeds under CT guidance is feasible, efficacy and safe method for in-field recurrence of NSCLC with prior radiation.

Abstract:The management of inoperable lung cancer remains a challenge. It has been proven that computed tomography (CT)-guided iodine-125 (125I) seed implantation is a safe and efficient method for treating lung cancer. Computed tomographic fluoroscopy (CTF) is superior to traditional CT for percutaneous management of lung lesions, due to the real-time guidance and accurate localization of the lesions. The aim of the present prospective study was to evaluate the feasibility, safety and efficacy of CTF-guided percutaneous permanent implantation of 125I seeds for the treatment of selected patients with inoperable stage T1-3N0M0 non-small-cell lung cancer (NSCLC). A total of 24 patients with resectable but inoperable stage T1-3N0 NSCLC, with a total of 28 lesions, underwent CTF-guided percutaneous implantation of radioactive 125I seeds. A prescription dose of 100-120 Gy was delivered to each lesion. The complications and local tumor control rates were documented. Survival was estimated using the Kaplan-Meier method. All the patients successfully completed the procedure, with a mean procedure duration of 45.7 min (range, 30-75 min). No severe complications occurred. Small asymptomatic pneumothorax with lung volume compression of <10% and minor hemorrhage along the needle track without hemoptysis occurred immediately after the procedure in 3 (12.5%) and 4 (16.7%) of the 24 patients, respectively. At a median follow-up of 31.5 months (range, 8-46 months), the local control rate (LCR) of the lesions was 78.6% (22/28). The 1-, 2- and 3-year overall survival rate was 95.8, 78 and 55%, respectively. In conclusion, CTF is the favourable imaging guidance method for the percutaneous implantation of 125I seeds. CTF-guided brachytherapy with implantation of 125I seeds is a safe, feasible and effective modality for the treatment of inoperable early-stage NSCLC and may be considered an alternative option in selected patients with medically inoperable NSCLC.

Abstract:Abstract To observe the safety and efficacy of CT-guided 3D template-assisted radioactive 125 I seed implantation in the treatment of unresectable advanced soft tissue sarcoma (STS). Sixty-two patients who underwent continuous 3D template-assisted radioactive 125 I seed implantation for the treatment of unresectable advanced STS from August 2017 to August 2018 were selected from four tumor treatment centers for retrospective analysis. The postoperative adverse reactions and tumor response were recorded, and the postoperative complications were observed and treated at the same time. The overall survival (OS) rate was determined. All patients successfully completed 125 I seed implantation. In practice, the median number of puncture needles used during the operation was 20, and the median number of 125 I particles was 88. There were no statistical differences in the relative dosimetry parameters before and after the operation (P > 0.05). Tumor evaluation was performed 6 months after the operation. The effective rate was 61.3%, and the local control rate was 93.5%. As of March 2020, the 1-year survival rate was 85.2%, and the 2-year survival rate was 49.0%. The OS was 23 months. CT-guided 3D template-assisted 125 I seed implantation for the treatment of unresectable STS has a high local control rate, thereby further prolonging the OS of patients with unresectable STS.

Abstract:Background A preliminary assessment was conducted of the feasibility, efficacy, and morbidity of 125 I seed implantation for recurrent head and neck carcinoma after surgery and external beam radiotherapy. Methods Nineteen patients with recurrent head and neck carcinomas underwent 125 I seed implantation under ultrasound or computed tomography guidance. The actuarial D90 of 125 I seed implantation ranged from 90 to 160 Gy (median, 131 Gy). The follow-up period ranged from 3 to 44 months (median, 11 months). Results The median local control was 24 months (95% confidence interval, 10.2 to 37.8). The one- year, two-year and three-year local controls were 73.3%, 27.5% and 27.5%, respectively, whereas the one-year, two-year and three-year survival rates were 53.0%, 18.2% and 18.2%, respectively, and the median survival was 13 months (95% confidence interval, 6.6 to 19.4). A total of 26.3% of patients (5/19) died of local recurrence and 21.1% of patients (4/19) died of metastases. One suffered from a grade 1 skin reaction. Conclusions 125 I seed implantation is feasible and safe as a salvage treatment for patients with recurrent head and neck cancers. The high local control results and low morbidity merits further investigation.

Abstract:Objective: To evaluate the relationship of dosimetry parameters and efficacy of (125)I seeds implantation for pelvic recurrent cervical cancer (PRCC) after external beam radiotherapy(EBRT) under CT guidance. Methods: A retrospective analysis was made on 30 PRCC patients after EBRT in Peking University Third Hospital with (125)I seeds implantation under CT guidance. Postoperative plans were made to evaluate the dosimetric parameters. Kaplan-Meier method was used to calculate local progression free survival (LPFS) rate and overall survival (OS) rate, and Log-rank test and Cox regression were used for univariate and multivariate analysis. Results: The 1-year and 2-year LPFS rate was 39.4% and 22.5%, respectively. The 1-year and 2-year OS rate was 57.3% and 27.4%, respectively. On postoperative plan, D(90) was (132±47) Gy, D(100) was (51±24) Gy, V(100) was 88%±10%, V(150)was 69%±15%, V(200) was 51%±18%.LPFS time would be longer while D(90) ≥105 Gy or D(100) ≥ 55 Gy or V(100) ≥ 91% (all P<0.05). D(100) was significantly related to LPFS (P<0.05). But these dosimetry parameters got no effect on OS. Conclusions: LPFS time of (125)I seeds implantation for PRCC after EBRT under CT guidance would be longer when D(90)≥105 Gy or D(100)≥ 55 Gy, or V(100)≥ 91%. D(100) is an independent factor related to LPFS.

Abstract:PURPOSE: To analyze factors influencing the efficacy of 125I seed implantation in the treatment of in-field cervical lymph node recurrence of esophageal squamous cell carcinoma (ESCC) after external beam radiation therapy.MATERIAL AND METHODS: We conducted a retrospective review of 36 patients with in-field cervical metastatic lymph nodes recurrence of esophageal squamous cell carcinoma (CML-ESCC) after external beam radiation therapy treatment, who underwent 125I seed implantation in our department from 2013 to 2019. Previous cumulative external irradiation dose ranged from 20 to 66 Gy (median, 60 Gy). The post-implant efficacy was evaluated by response evaluation criteria in solid tumors (RECIST) version 1.1, and an adverse event was evaluated by the Radiation Therapy Oncology Group (RTOG)/European Organization for Research and Treatment of Cancer Late Radiation Morbidity Score (EORTC). COX proportional hazards model was used to analyze risk factors affecting effectiveness.RESULTS: Among 36 patients, 31 patients (86.1%) received fluorouracil-based chemotherapy (1-6 cycles) after 125I seed implantation. Local control rates at 3, 6, 12, and 24 months after implantation were 51%, 30%, 30%, and 18%, respectively, with a median of 9 months (95% CI: 6.106-11.894); survival rates after 3, 6, 12, and 24 months were 55%, 41%, 22%, and 22%, respectively, with a median of 8 months (95% CI: 5.753-10.247). Multivariate analysis showed that D90 and short-term efficacy were independent factors related to local control and survival rate (p = 0.005, < 0.001, 0.010, < 0.001). There were 2 cases (5.6%) with grade 1 skin toxicity, 1 case (2.8%) with grade 4 skin toxicity, 3 cases (8.3%) with grade 1 mucosal ulcer, and 3 cases (8.3%) with grade 1 xerostomia.CONCLUSIONS: 125I seed implantation as an effective salvage treatment shows definite efficacy and safety for patients with in-field cervical lymph node recurrence of ESCC after external beam radiation therapy.

Abstract:Objective To preliminarily evaluate the feasibility and therapeutic effect of 125 I seed implantation under computed tomography (CT) guidance in the treatment of recurrent or metastatic chest wall tumor.Methods A retrospective analysis was performed on the clinical data of 23 patients with recurrent or metastatic malignant chest wall tumor who underwent 125I seed implantation under CT guidance from July 2004 to October 2012.A computerized treatment planning system was used for three-dimensional treatment planning before operation to determine the number and spatial distribution of 125I seeds.CT scan dosimetry was performed immediately after operation.The dose received by 90％ of the target volume was 100-160 Gy (median:130 Gy).The activity of 125I seeds was (1.85-2.89) × 107 Bq (median:2.63 × 107 Bq).The total number of seeds implanted was 8-269 (median:53).The local control rate and survival rate were calculated by the Kaplan-Meier method.Results The follow-up rate was 100％.Two patients were followed up for at least 3 years,and 2 for at least 5 years.Of the 23 patients,4 had a complete remission,and 12 had a partial remission.The 1-,3-,and 5-year local control rates were 82％,62％,and 62％,respectively; the 1-,3-,and 5-year tumor-specific survival rates were 61％,45％,and 45％,respectively;the 1-,3-,and 5-year overall survival rates were 58％,58％,and 39％,respectively.After treatment,mild or moderate brachial plexus injury was seen in one patient,and grade 1 or 2 skin injury was seen in 6 patients.Conclusions 125I seed implantation under CT guidance is safe and effective in the treatment of recurrent or metastatic chest wall tumor,according to the preliminary study.

Abstract:This study aimed to analysis outcome and prognosis of interstitial I-125 seed implantation in patients with cervical lymph node recurrence after multimodal treatment of thoracic esophageal squamous cell carcinoma (ESCC). We conducted a retrospective review of 19 patients with 32 cervical lymph nodes recurrences after multimodal treatment (lymphadenectomy, radiotherapy, chemotherapy, and various combinations of these treatments) of thoracic ESCC, who underwent 125I seed implantation in our department from 2003 to 2011. All the patients were followed up until expiration and the median duration of follow up was 7 months (range, 3-44 months). Syndromes significantly improved after implantation. The local control rates after 3, 6, 12, and 24 months were 84.2%, 63.2%, 32.0%, and 26.0%, respectively, with a median of 10 months. The median overall survival time was 7 months (95% CI, 5.6-8.4), and 1- and 2-year survival rates were 31.6% and 10.5%, respectively. Among these patients, there were 11 died of progression of disease (PD) 3-44 months after implantation. One patient presented grade IV skin toxic effect and repaired by free flap transplantation. No fatal complications such as massive bleeding happened. In univariate analysis, N stage, number of recurrent nodes, recurrence interval time, and D90 were prognostic factors of the tumor local control and survival (p = 0.131 vs. 0.029, 0.129 vs. 0.071, 0.042 vs. 0.042, and 0.056 vs. 0.065, respectively). Multivariate analysis demonstrated that N stage, number of recurrent nodes, and recurrence interval time were independent prognostic factors of the tumor local control (p = 0.022, 0.019, and. 0.001, respectively), and recurrence interval time was prognostic factor of the survival (p < 0.001). Interstitial I-125 seed implantation is a safe and effective salvage treatment for cervical lymph node recurrence after multimodal treatment. The N stage, number of recurrent nodes and recurrence interval time are factors influencing tumor local control, and the recurrence interval time is independent factor influencing survival after percutaneous I-125 seed implantation in ESCC with cervical lymph node recurrence.

Abstract:Background Management of locally recurrent rectal cancer (LRRC) after surgery or external beam radiotherapy (EBRT) remains a clinical challenge, given the limited treatment options and unsatisfactory outcomes. This study aimed to assess long-term outcomes of computed tomography (CT)-guided radioactive 125 I seed implantation in patients with LRRC and associated prognostic factors. Methods A total of 101 patients with LRRC treated with CT-guided 125 I seed implantation from October 2003 to April 2019 were retrospectively studied. Treatment procedures involved preoperative planning design, 125 I seed implantation, and postoperative dose evaluation. We evaluated the therapeutic efficacy, adverse effects, local control (LC) time, and overall survival (OS) time. Results All the patients had previously undergone surgery or EBRT. The median age of patients was 59 (range, 31–81) years old. The median follow-up time was 20.5 (range, 0.89–125.8) months. The median LC and OS time were 10 (95% confidence interval (CI): 8.5–11.5) and 20.8 (95% CI: 18.7–22.9) months, respectively. The 1-, 2-, and 5-year LC rates were 44.2%, 20.7%, and 18.4%, respectively. The 1-, 2-, and 5-year OS rates were 73%, 31.4%, and 5%, respectively. Univariate analysis of LC suggested that when short-time tumor response achieved partial response (PR) or complete response (CR), or D 90 >129 Gy, or GTV ≤ 50 cm 3 , the LC significantly prolonged (P=0.044, 0.041, and <0.001, respectively). The multivariate analysis of LC indicated that the short-time tumor response was an independent factor influencing LC time (P<0.001). Besides, 8.9% (9/101) of the patients had adverse effects (≥grade 3): radiation-induced skin reaction (4/101), radiation-induced urinary reaction (1/101), fistula (2/101), and intestinal obstruction (2/101). The cumulative irradiation dose and the activity of a single seed were significantly correlated with adverse effects ≥grade 3 (P=0.047 and 0.035, respectively). Conclusion CT-guided 125 I seed implantation is a safe and effective salvage treatment for LRRC patients who previously underwent EBRT or surgery. D 90 and GTV significantly influenced prognosis of such patients.
