Clinical Characteristics and outcomes of Follicular bronchiolitis in adult patients

Abstract:Background: Follicular bronchiolitis (FB) is an uncommon lymphoproliferative lung disease. This study was investigated the clinical feturws and outcomes of 6 adult patients with FB demonstrated by surgical lung biopsy. Subjects and Methods: Six patients were included in this study between 2009 and 2016 form our Hospital. The medical records, chest imaging and pathological findings were analyzed retrospectively. Results: The subjects included 2 men and 4 women; the average age was 42 years (range, 31-55 years). The clinical symptoms were very mild, half cases were found abnormal chest imaging by health screening. Four patients were presented with anemia and elevated CRP. ESR was increased in all subjects. Serum globulin levels were elevated in five patients. Pulmonary function tests (PFTs) showed no impairment in four cases, and mild decreased the others. Four subjects had underlying systemic diseases that included: 2 with Sjo¨gren’s syndrome, 1 multi-castlemans’ disease 1 abscess. Two cases were idiopathic FB. The main CT findings included bilateral centrilobular nodules, ground glass opacities, diffuse interlobular septum thickening bronchioectatasis, cysts and bilateral hilar and mediastinal lymphadenopath. FB was the main histological pattern. Three patients were accepted the therapy of prednisone and/or cyclophosphamide, one did the surgery, and two were just watching. Six patients were all alive at the end of follow up. Conclusion: FB is a rare pathological diagnosis that can be seen in several clinical settings. The adult patients with FB have the favourable clinical outcomes.
