Abstract:Background & Objective: The purpose of this study is to identify the risk factors associated with the conversion from Mild Cognitive Impairment (MCI) to Alzheimer's Disease (AD) dementia for the early detection of AD. Methods: The study comprised a prospective cohort study that included 400 MCI subjects with annual follow-ups for 3 years. Results: During the first 12 months' follow-up, 42 subjects converted to Alzheimer's dementia (21 probable AD and 21 possible AD), two subjects converted to other types of dementia and 56 subjects lost follow. The factors associated with a greater risk of conversion from MCI to AD included gender, whole brain volume, and right hippocampal volume (rt. HV), as well as scores on the Revised Chinese version of the Alzheimer's Disease Assessment Scale-Cognitive subscale 13 (ADAS-Cog-C), Clock Drawing Test (CDT), Symbol Digit Modalities Test (SDMT), and Rey-Osterrieth Complex Figure Test (ROCFT). The risk classification of the combined ADAS-Cog-C and Alzheimer Cognitive Composite (ACC) score with the rt. HV and left Entorhinal Cortex Volume (lt. ECV) showed a conversion difference among the groups. Conclusion: Early detection of AD and potential selection for clinical trial design should utilize the rt. HV, as well as neuropsychological test scores, including those of the ADAS-Cog-C and ACC.
Shanghai Cohort Study on Mild Cognitive Impairment: Study Desig N and Baseline Characteristics

Abstract:Objective: Establish cohort of patients with mild cognitive impairment (MCI) in order to identify the risk factors and validation of neuropsychological tests for early detection of Alzheimer disease. Methods: Four hundred participants with MCI were enrolled in at Huashan Hospital in Shanghai, China. All patients will be followed once annually for 3years. The primary endpoint is the time of conversion from diagnosis of MCI to Alzheimer disease (AD). Results: of the 400 subjects, 170 (42.5%) were male, and the average mean age and education was 68.7 (Standard Deviation, SD: 8.2, 45- 90 years old) and 12 years (SD: 3.1), respectively. At baseline age, education and being single influenced MMSE, ADAS-cog13 and most domains of cognition. Conclusions: Baseline characteristics of the Shanghai MCI cohort study indicate the participants were younger, less educated and had a moderate gender ratio compared to other studies conducted so far and represent the target population. 3 years later when the cohort will be finished some risk factors Associated with the conversion will be validated and some novel factors will be identified.

Abstract:BACKGROUND:To establish a prospective cohort to enumerate the prevalence, incidence and risk factors for dementia and mild cognitive impairment (MCI) among residents aged ≥60 in an urban community of Shanghai, China.METHODS:Participants received clinical evaluations including physical measurements, demographic and lifestyle questionnaires, physical and neurologic examinations, and neuropsychological testing. Urine and blood samples were collected, aliquoted, and stored. DNA was extracted for Apolipoprotein (APOE) genotyping. Diagnoses of dementia and MCI were made using standard criteria via consensus diagnosis.RESULTS:Among 3,141 participants aged ≥60, 1,438 (45.8%) were men. The average age of participants was 72.3 years (SD 8.1), and they had an average of 11.6 years (SD 4.4) of education. The most common chronic disease of participants was hypertension (56.4%). The frequencies of APOE-ε2, ε3 and ε4 were 7.9, 82.7 and 9.4%, respectively. We diagnosed 156 (5.0%, 95% CI 4.3-5.8%) participants with dementia. The prevalence rates of Alzheimer's disease and vascular dementia were 3.6% (95% CI 3.0-4.3%) and 0.8% (95% CI 0.5-1.1%).CONCLUSIONS:The Shanghai Aging Study is the first prospective community-based cohort study of cognitive impairment in China, with a comparable study design, procedures, and diagnostic criteria for dementia and MCI to most previous cohort studies in developed countries.
Sociodemographic Factors Predict Incident Mild Cognitive Impairment: A Brief Review and Empirical Study.

Abstract:OBJECTIVES:Mild cognitive impairment (MCI) is a transitional stage between normal cognitive aging and dementia that increases the risk of progressive cognitive decline. Early prediction of MCI could be beneficial for identifying vulnerable individuals in the community and planning primary and secondary prevention to reduce the incidence of MCI.DESIGN:A narrative review and cohort study.SETTING AND PARTICIPANTS:We review the MCI prediction based on the assessment of sociodemographic factors. We included participants from 3 surveys: 8915 from wave 2011/2012 of the China Health and Retirement Longitudinal Study (CHARLS), 9765 from the 2011 Chinese Longitudinal Healthy Longevity Survey (CLHLS), and 1823 from the 2014 Rugao Longevity and Ageing Study (RuLAS).METHODS:We searched in PubMed, Embase, and Web of Science Core Collection between January 1, 2019, and December 30, 2022. To construct the composite risk score, a multivariate Cox proportional hazards regression model was used. The performance of the score was assessed using receiver operating characteristic (ROC) curves. Furthermore, the composite risk score was validated in 2 longitudinal cohorts, CLHLS and RuLAS.RESULTS:We concluded on 20 articles from 892 available. The results suggested that the previous models suffered from several defects, including overreliance on cross-sectional data, low predictive utility, inconvenient measurement, and inapplicability to developing countries. Our empirical work suggested that the area under the curve for a 5-year MCI prediction was 0.861 in CHARLS, 0.797 in CLHLS, and 0.823 in RuLAS. We designed a publicly available online tool for this composite risk score.CONCLUSIONS AND IMPLICATIONS:Attention to these sociodemographic factors related to the incidence of MCI can be beneficially incorporated into the current work, which will set the stage for better early prediction of MCI before its incidence and for reducing the burden of the disease.
Early Detection of Dementia Using Risk Classification in MCI: Outcomes of Shanghai Mild Cognitive Impairment Cohort Study.

Abstract:INTRODUCTION:The purpose of this study is to identify the risk factors and risk classification associated with the conversion from mild cognitive impairment (MCI) to Alzheimer's disease (AD) dementia to facilitate early intervention and the design of clinical trials for AD.METHODS:The study comprised a prospective cohort study of 400 subjects with MCI who had annual follow-ups for 3 years.RESULTS:During an average follow-up period of 3.5 years, 109 subjects were diagnosed with all cause of dementia, of whom 104 subjects converted to Alzheimer's dementia and 5 subjects converted to other types of dementia. The cumulative conversion rate was 5.5% (95% CI: 3.4, 8.6), 16.3% (95% CI: 12.9, 21.1), and 31.0% (95% CI: 25.4, 36.5) in each of the first 3 follow-up years, respectively. The factors associated with a greater risk of conversion from MCI to AD included smoking status, ApoE4 carrier status, right hippocampal volume (rt. HV), left temporal lobe volume, and scores on the Revised Chinese version of the Alzheimer's Disease Assessment Scale-Cognitive Subscale 13 (ADAS-Cog-C). The risk classification of the ADAS-Cog-C or Preclinical Alzheimer Cognitive Composite (PACC) score combined with the rt. HV showed a conversion difference among the groups at every annual follow-up.CONCLUSION:A simple risk classification using the rt. HV and neuropsychological test scores, including those from the ADAS-Cog-C and PACC, could be a practicable and efficient approach to indentify individuals at risk of all-cause dementia.

Abstract:China is facing a demographic epidemic, with 300 million elders age 65 and over anticipated by 2030. We are planning a large community-based cohort study and tested the feasibility of conducting such a study in Shanghai. We conducted a pilot study between May and November 2008. Goals were to establish the feasibility of collecting detailed data on risk factors, diet, medical, family history, MRI, blood and urine for a large cohort study of early detection and prevention of dementia. MCI and dementia cases presented to the Memory Disorders Clinic at Huashan Hospital. Controls were recruited from the community by door-to-door approach. All subjects were seen by a study neurologist and completed a comprehensive neuropsychological battery and MRI and were diagnosed by consensus diagnosis using standard clinical criteria. 59 potential controls were approached; 3.3% were unable to be contacted and 12% refused. Of the remaining 49, 4% were demented, 18% had MCI, 73% were normal controls after examination and 4% were followed over time. The participation rate among controls was 83%. 109 cases were approached, 8% were unable to be contacted and 35% refused, with a participation rate of 57%. We analyzed an age- and sex- matched set of 32 controls, 34 MCI cases and 34 dementia cases. For controls, mean age was 73.4 (sd=5.5), education=10.8 years (sd=4.9) with 50% female. MCI cases were on average 74.9 (sd=3.9) years old with mean education=11.1 years (sd=5.9); 50% were female. Dementia cases were 74.3 (sd=5.6) years old with mean education=9.2 years (sd=5.8); 50% were female. All participants received the complete evaluation and completed detailed interviews (controls and case and control proxies). A strong participation rate is achievable from recruitment of community-based elders in Shanghai. The collection of detailed risk factor and medical histories, as well as clinical and neuropsychological data, acquisition of blood, urine and MRI is highly feasible. 83% of community-based subjects participated in detailed epidemiologic and clinical evaluations. Our planned cohort study will be fielded in a population in which a high validity can be achieved.

Abstract:This study aimed at investigating the natural history of Alzheimer disease (AD) and risk factors associated with survival We performed a 5-year retrospective cohort study on consecutive outpatients treated for clinically diagnosed Alzheimer disease between January 1, 2003, and December 31, 2006, at Huashan Hospital, Shanghai, China.The primary endpoint in this study was survival. At baseline, patients were evaluated for demographic characteristics and neuropsychological characteristics, comorbid conditions, neuroimaging findings, and neurological signs. Of the 467 patients, 398 survived. Survival analysis using the proportional hazard model with adjustments for gender and disease stage revealed that late onset of the disease was an independent predictor of short survival(HR2.18,95%CI 0.99-4.98). The cognition factors that predicted longer survival included high total score(HR0.86,95%CI 0.76-0.98)and sub-score in the verbal fluency test (animal, vegetable, and fruits) . Presence of at least 1 complication was an independent factor predictive of a short lifespan. Late onset of disease and presence of complications are indicators of poor survival in AD patients. Good performance in the verbal fluency test and stroop test at baseline was associated with increased survival in AD patients.

Therefore, understanding neuropsychological manifestations in dementia and mild cognitive impairment (MCI) is important for prevention and treatment. We conducted a pilot study recruiting age- and gender-matched 32 normal controls (NC), 34 MCI and 32 dementia (D) cases in Shanghai, China between May and November 2008. All participants were administered a battery of neuropsychological tests, including Mattis dementia rating scale(DRS), auditory verbal learning test(AVLT), Rey-Osterrich Complex Figure(R-O), Stroop test (Stroop), WAIS similarities (SIM), verbal fluency test (VFT), trail making test (TMT), clock drawing test (CDT), Boston Naming Test (BNT), logical memory(LM), digit span(DS), and bell cancellation test (BELL). One-way ANOVA showed significant differences among the three groups and between NC and D in all the above neuropsychological tests (p<0.01). There were significant differences between MCI and dementia in almost all tests (p<0.05) except SIM, DRS, AVLT delayed recall, and R-O recall, and similarity scores were significantly lower in MCI compared to NC (p<0.05). Cognitive function declined significantly from NC to MCI to dementia across nearly all domains. Verbal and non-verbal delayed recall were more sensitive in distinguishing MCI from NC than other tests.

Abstract:By 2030, China will have 300 million people aged 65 and over. Dementia is therefore a major public health challenge. Between1980-2004, we participated in several multi-site prevalence studies in China that included 88,000 people in 12 study sites age 60 and over. In these and in our studies in Shanghai, prevalence rates of dementia were 2-3%, somewhat higher in women than men, and diagnosed AD accounted for the majority of dementia cases using standard, Western criteria. The somewhat lower prevalence rate than in the West may be related to the lower frequency of the APOE-ϵ4 allele in China. We recently collected data on 64 age- and sex-matched dementia and aMCI cases and 30 controls from the community for an NIH-funded pilot study (SCOBHI). Consensus diagnostic conferences were conducted for all study subjects, and for a subset of cases with an international panel of experts. aMCI cases were very mild, with similar scores on the Cognitive Abilities Screening Instrument (CASI) as controls (85 vs. 89). However, the frequency of APOE-ϵ4 allele was higher than in controls (0.05), but similar in aMCI (0.18) and dementia (0.15). Hippocampal volumes in aMCI (0.35%) also were larger than in controls (0.39%), but similar to dementia (0.33%). Vascular risk factors distinguished aMCI from dementia, with dementia cases much more likely to have stroke on MR or by history, high Hachinski Ischemic Scores, higher fasting glucose and larger white matter hyperintensity volumes. Vascular disease appeared to be the most important contributor to the expression of dementia. Contrary to expectation, our study showed that dementia or AD was not considered a social stigma, and this may be one reason that aMCI is manifested early in our memory disorder clinic. We also found 18% of presumed controls had MCI after a detailed clinical, neurologic, neuropsychological evaluation, and that the Petersen criteria of informant or subject memory complaint should be dropped to avoid misclassification of MCI as normals.
Mortality of Alzheimer's Disease Patients: A 10-Year Follow-up Pilot Study in Shanghai

Abstract:Background: Identifying risk factors and mortality of individuals with Alzheimer's disease (AD) could have important implications for the clinical management of AD. Objective: This pilot study aimed to examine the overall mortality of AD patients over a 10-year surveillance period in Shanghai, China. This study is an extension of our previous investigation on mortality of neurodegenerative diseases. Methods: One hundred and thirty-two AD patients recruited from the memory clinics of two hospitals in Shanghai in 2007 were followed up until December 31, 2017 or death, representing a follow-up period of up to 10 years. Overall standardized mortality ratios (SMRs) were calculated, and predictors for survival at recruitment were estimated. Results: Sixty-seven patients had died by December 31, 2017, and the SMR at 10 years of follow-up was 1.225 (95% confidence interval 0.944-1.563). Employing Cox's proportional hazard modeling, lower Mini-Mental State Examination score, and comorbid diabetes predicted poor survival in this cohort. Conclusion: This pilot study suggests a similar survival trend of patients with AD compared to the general population in Shanghai urban region. Poor cognitive status and comorbid diabetes had a negative impact on the survival of AD patients.

Abstract:Previous studies have shown APOE ϵ4 allele frequencies for MCI to be intermediate between normal controls and dementia cases. Frequencies also have been found to be somewhat lower in China than in the West. We conducted a pilot study of 32 normal controls, 30 amnestic Mild Cognitive Impairment (MCI) and 34 dementia cases in Shanghai, China between May and November 2008. Cases were frequency matched to controls by age and sex. All participants had extensive risk factor evaluation in addition to a neurologic and neuropsychological evaluation and MRI. APOE testing was done by the standard Hixson-Vernier method. Mean ages were 73.4 (sd=5.5), 74.9 (sd=3.9) and 74.3 (sd=5.6) for controls, MCI and dementia cases, respectively. There were no ϵ2ϵ2 genotypes. APOE-ϵ4 allele frequencies were 0.05 for controls, 0.18 for MCIs, and 0.15 for dementia cases. One or two APOE-ϵ4 alleles were found in 6.3% of controls, 23.3% of MCIs and 26.5% of dementia cases. The odds ratios comparing dementia cases to controls and MCIs to controls were similar in strength (education-adjusted OR for dementia vs. controls=5.64, 95% CI=1.09-28.27; education-adjusted OR for MCI vs. controls =4.72, 95% CI=0.88-25.22). APOE-ϵ4 status was not associated with white matter hyperintensities (p=0.38), but was marginally negatively associated with brain volume (r=-0.20, p=0.07) and ventricle size (r=0.19, p=0.07). APOE-ϵ4 frequency was marginally higher in MCI than in dementia cases. In comparison with Caucasian samples, our Chinese elders had a lower frequency of the ϵ4 allele and a higher frequency of the ϵ3 allele. Despite this, APOE-ϵ4 remained a strong risk factor for dementia and MCI. APOE-ϵ4 status also was not associated with white matter hyperintensities but appeared to be associated with atrophy on MRI.
Progression and Predictors of Mild Cognitive Impairment in Chinese Elderly: A Prospective Follow-Up in the Shanghai Aging Study.

Abstract:INTRODUCTION:The Shanghai Aging Study is a community-based study aiming to investigate the incidence and progression of cognitive decline in Chinese elderly, with the operational procedures and diagnostic criteria similar to cohort studies in developed countries.METHODS:We prospectively evaluated 362 individuals with mild cognitive impairment (MCI) diagnosed at baseline through a clinical and neuropsychological interview. Diagnoses of dementia and MCI were made using standard criteria via consensus diagnosis.RESULTS:The conversion rate to dementia was 6.0 per 100 person-years, while the reversion rate to cognitive normal was 7.8 per 100 person-years. Amnestic MCI multiple domains was the most risky type for dementia (conversion rate: 14.2 per 100 person-years). Older age (hazard ratio [HR] = 1.09), apolipoprotein E (APOE ε4) (HR = 2.15), and low MMSE score (HR = 1.18) were predictors for dementia.DISCUSSION:Approximately 6% of elderly with MCI progress to dementia annually. Prospective studies are needed to further examine risk and protective predictors and to seek proper interventions for cognitive decline.

Abstract:The high prevalence of dementia with the rapid increase of the older population and the lack of effective therapy raise the global issues of human life with aging. All the cases diagnosed as Alzheimer's Disease (AD) in the memory clinic of Huashan Hospital Shanghai, China from Jan. 1st 2003 through Dec.31th 2006 were included in this study. The participants were followed via face-to-face clinical and neuropsychological reassessment during September 1, 2007 and July 31, 2008. Those who could not come to the hospital were followed by telephone. Demographic information, the change of cognitive function, behavior, ability of daily life, neurological symptoms, comorbid conditions (aspiration pneumonia, urinary tract infections and decubitus ulcers) were collected. Correlation analysis and logistic regression analysis were conducted to identify the predictors associated with the deterioration of cognitive function. Among the 624 initially clinical diagnosed AD patients, 16 were excluded after diagnosis verification. 608 confirmed AD cases were enrolled in final analysis. 400 cases were followed (163 came to our hospital to receive the whole neuropsychological battery which included auditory verbal fluency test (AVLT), logic memory test, Stroop color word test, Rey-Osterrich complex figure test (CFT), category verbal fluency test (VFT), trail making test (TMT), clock drawing test (CDT), five point test. 237 received telephone follow-up), 69 cases died and the rest 139 lost contact. For the 163 reassessed cases, MMSE total score (p = 0.000), AVLT short delayed recall (p = 0.019), Stroop C correct score (p = 0.002), CFT-copy (p = 0.000), VFT (p = 0.001-0.017), TMT B time(p = 0.030), CDT score(p = 0.013) decreased significantly over time. Among demographic information, previous medical history, family history and comorbid/complications no factor had significant regression with the change of cognition. Abstract:Background: Despite rapid development of epidemiology in dementia, reported prevalence in Asia varies among studies and differs from that of Western countries. Few reported the mortality and predictive factors on survival of dementia in Asia, especially China. Methods: To characterize the epidemiologic pattern of dementia in Shanghai, China, including prevalence and survival, we conducted a cross-sectional, population-based survey among Shanghai residents (>= 55 y) and followed a subsample for up to 40 months for survival analysis. A stratified, multistage cluster sampling design was adopted. Results: Crude and age-standardized prevalence of dementia was 2.99% and 2.66%, respectively, higher in women and increased with age. Prevalence of Alzheimer was higher than vascular dementia. Furthermore, Alzheimer disease was more prevalent in rural areas than in urban, whereas geographic distribution of vascular dementia was quite the opposite. Crude and age-standardized mortality of dementia was 6.06 and 5.30 per 1000 person-years. There was a striking difference in survival between the demented and nondemented, whereas no significant survival discrepancy existed between Alzheimer and vascular dementia. Factors including overall physical and cognitive function, age, and severity of disease strongly influenced survival and might be predictors for fatal events in dementia patients. Conclusions: The epidemiologic pattern of dementia in Shanghai, China is similar to that of western countries.
A Five-Year Community-Based Longitudinal Survival Study Of Dementia In Beijing, China: A 10/66 Dementia Research Group Population-Based Study

Abstract:Background: Despite the magnitude of dementia, little research on survival duration and prognosis of dementia has been reported in developing countries. This study was conducted to investigate survival times, identify related prognostic factors and construct a prognostic index (PI) for community-based dementia patients in Beijing, China.Methods: This study is part of the 10/66 Dementia Research Group study in China. One hundred and thirty-seven dementia patients identified by 10/66 dementia criteria among 2162 participants and 137 referent subjects matched by age and sex were followed up for five years.Results: Ninety-one (66.4%) dementia patients and 51(37.2%) referent subjects died during the 5-year follow-up (p < 0.01). The median survival time of dementia patients was 4.2 years (95% CI: 3.8-4.6). Severity of dementia (severe/mild, HR: 8.765, 95% CI: 4.436-17.163), substantial disability (HR: 5.503, 95% CI: 3.017-8.135), co-morbidity (HR: 4.149, 95% CI: 2.254-7.736) and age (HR: 1.079, 95% CI: 1.048-1.110) were independent predictors of survival for patients with dementia. Using the PI calculated for each dementia patient, all dementia patients were classified into three groups: low, medium and high risk groups. The median survival times of each group were 5.2 years, 4.4 years and 1.5 years (p < 0.01), respectively.Conclusions: Survival times of community-based dementia patients were significantly shorter than those of referent subjects. Severity of dementia, substantial disability, co-morbidity and age were independent predictors of survival. The PI derived from the four predictors can stratify the mortality risk and predict life expectancy for community-dwelled dementia patients, although further validation is needed.

Abstract:The incidence of dementia based on DSM-III criteria and the impact of age and education on the development of dementia using the data of a longitudinal community population survey is discussed. Study subjects were randomly sampled from the community and were assessed by a two-stage procedure twice during a 5-year period. The screening instrument was the Mini-Mental State Examination and the second stage measures include a medical history and physical examination, a neurological examination and an intensive neuropsychiatric interview and testings (i.e. Blessed Dementia Scale, Activity of Daily Living, Fuld Object Memory Test and Block Design Test. All identified cases from the second stage interview were further verified by computed tomographic scan and laboratory tests. A total of 1970 subjects of non-dementia aged 65 or older in 1987 were re-surveyed after 5 years, of whom 114 new cases of dementia were identified. The incidence of dementia in total was 1.15% annually, 0.98% for males and 1.27% for females. The incidence for Alzheimer's disease, vascular dementia and dementia by other causes was 0.74, 0.33 and 0.08%, respectively. The highest rate (2.02%) was found in illiterate individuals. The incidence of dementia in the Shanghai cohort is similar to that from other countries. Age and education are closely related to the occurrence of dementia. While age increases by each 5 years for those aged 65 and over the likelihood of developing dementia increases nearly one-fold (74%). Education shows great protective effect on the development of dementia. The non-educated has a higher risk of suffering from dementia than does the educated.

Abstract:Purpose: The Shenzhen ageing-related disorder cohort was designed to detect the associations of lifestyle, environmental and genetic factors with major ageing-related disorders, especially neurological and mental disorders. Participants: The cohort was a community-dwelling prospective study of 9411 elderly adults aged 60 to 92 years from 51 community health service centres in Luohu district of Shenzhen, China. The baseline data were collected between 2017 and 2018, including demographics and socioeconomics, lifestyles, medical history, family history of major non-communicable chronic disease, environmental exposures, clinical analysis of blood and urine, clinical imaging measurements, anthropometric measures and neurological function and mental health assessments. Blood and urinary samples were collected at baseline. All participants will be followed for physiological and psychological disorders and updated lifestyle and environmental exposures every 5 years. Findings to date: The mean age of the participants was 67.73 years at baseline, and 42.74% were males. The prevalences of individuals with unhealthy conditions were as follows: overweight/obesity (54.38%), hypertension (58.24%), diabetes mellitus (22.30%), dyslipidaemia (75.69%), chronic bronchitis (1.45%), myocardial infarction (0.55%), coronary heart disease (5.69%), stroke (1.10%), cancer (2.18%), arthritis (5.04%), Alzheimer's disease (0.18%), Parkinson's disease (0.23%), brain injury (5.75%), cognitive impairment (5.39%) and depression status (3.28%). The mean scores for the Lawton-Brody Activities of Daily Living Scale and the Social Support Rate Scale were 14.15 and 39.54, respectively. Future plans: 2000 new entrants from Luohu district will be recruited every year until 2028. The data collection is expected to be ended at the end of 2030. The data will be used to assess the causality of ageing-related disorders, especially neurological and mental disorders through integrating environmental, genetic and lifestyle factors. The data sets generated and/or analysed during the current study are not publicly available at this stage, but are available from the corresponding author on reasonable request.

Abstract:The high prevalence of dementia with the rapid increase of the older population and the lack of effective therapy raise the global issues of human life with aging. All the cases diagnosed as Alzheimer's Disease (AD) in the memory clinic of Huashan Hospital Shanghai, China from Jan. 1st 2003 through Dec.31th 2006 were included in this study. The participants were followed via face-to-face clinical and neuropsychological reassessment during September 1, 2007 and July 31, 2008. Those who could not come to the hospital were followed by telephone. Demographic information, the change of cognitive function, behavior, ability of daily life, neurological symptoms, comorbid conditions (aspiration pneumonia, urinary tract infections and decubitus ulcers) were collected. Correlation analysis and logistic regression analysis were conducted to identify the predictors associated with the deterioration of cognitive function. Among the 624 initially clinical diagnosed AD patients, 16 were excluded after diagnosis verification. 608 confirmed AD cases were enrolled in final analysis. 400 cases were followed (163 came to our hospital to receive the whole neuropsychological battery which included auditory verbal fluency test (AVLT), logic memory test, Stroop color word test, Rey-Osterrich complex figure test (CFT), category verbal fluency test (VFT), trail making test (TMT), clock drawing test (CDT), five point test. 237 received telephone follow-up), 69 cases died and the rest 139 lost contact. For the 163 reassessed cases, MMSE total score (p = 0.000), AVLT short delayed recall (p = 0.019), Stroop C correct score (p = 0.002), CFT-copy (p = 0.000), VFT (p = 0.001-0.017), TMT B time(p = 0.030), CDT score(p = 0.013) decreased significantly over time. Abstract:Objective:To describe the incidence of mild cognitive impairment(MCI) in an urban and a rural community of Beijing in five years,and to explore socio-demographic and biological risk factors of MCI incidence.Methods:Totally 1859 elderly people aged 65 and over in Xicheng District(urban) and Daxing District(rural) of Beijing were evaluated by a set of fully structured assessments in a one-stage process developed by the international cooperated 10/66 group(including the Geriatric Mental State Schedule Shortened Community Version and Automated Geriatric Examination for Computer Assisted Taxonomy(GMS-AGECAT),Cognitive test battery,Informant Interview Questionnaire,Background Socio-demographic and Risk factor Questionnaire).MCI were diagnosed according to the General MCI criteria developed by the 10/66 group.The 5-years cumulative incidence rate and average annual incidence rate were calculated to describe the status of incidence of MCI,uni-and multivariate Cox regressions were applied for analyzing risk factors.Results:In this follow-up study,193 subjects were diagnosed as new MCI patients.The 5-year cumulative incidence rate was 10.38%(95%CI:9.00-11.82%),and the average annual incidence rate was 2.17%(95%CI:1.76-2.30).There were statistical differences of MCI incidence by age,gender,urban-rural area,and education level.Older age(HR=4.66,95%CI:1.41-15.42),living in a rural area(HR=2.54,95%CI:1.72-3.75),history of stroke(HR=3.04,95%CI:1.63-5.68),history of diabetes(HR=2.00,95%CI:1.14-3.50),lager Waist Hip Ratio(HR=4.97,95%CI:3.53-7.01) were risk factors of MCI occurrence.Having spouse(HR=0.55,95%CI:0.40-0.76) and physical exercise(HR=0.34,95%CI:0.14-0.84) were the protective factors.Conclusions:The five year cumulative incidence rate of mild cognitive impairment found from this study in elderly people aged 65 and over in Beijing was 10.38% and the average annual incidence rate was 2.17%.Older age,living in a rural area,history of stroke,history of diabetes,lager Waist Hip Ratio were risk factors of MCI occurrence.This study provides a foundation for future prevention and intervention of MCI,which has significant social and clinical value.

Abstract:Background: Substantial variations in the prevalence of mild cognitive impairment (MCI) have been reported, although mostly in Western countries. Less is known about MCI in the Chinese population.Methods: We clinically and neuropsychologically evaluated 3141 community residents >= 60 years of age. Diagnoses of MCI and its subtypes were made using standard criteria via consensus diagnosis.Results: Among 2985 nondemented individuals, 601 were diagnosed with MCI, resulting in a prevalence of 20.1% for total MCI, 13.2% for amnestic MCI (aMCI), and 7.0% for non-amnestic MCI (naMCI). The proportions of MCI subtypes were: aMCI single domain (SD), 38.9%; aMCI multiple domains (MD), 26.5%; naMCI-SD, 25.0%; and naMCI-MD, 9.6%. The prevalence of aMCI-MD increased rapidly with age in women APOE 64 carriers (from 60 to 69 years to >= 80 years, 3.1%-33.3%, P < .001).Conclusions: Our findings suggest that 20% of Chinese elderly are affected by MCI.
