Abstract:OBJECTIVE Adiposity and the gut microbiota are both related to the risk of type 2 diabetes. We aimed to comprehensively examine how changes induced by a weight-loss diet intervention in gut microbiota–related metabolites, such as trimethylamine N-oxide (TMAO) and its precursors (choline and l-carnitine), were associated with improvements in adiposity and regional fat deposition. RESEARCH DESIGN AND METHODS This study included 510 overweight and obese individuals who were randomly assigned one of four diets varying in macronutrient intake. We examined associations of 6-month changes in blood metabolites (TMAO, choline, and l-carnitine) with improvements in body weight (BW), waist circumference (WC), body fat composition, fat distribution, and resting energy expenditure (REE). RESULTS Individuals with a greater reduction of choline (P < 0.0001) and l-carnitine (P < 0.01) rather than TMAO showed significant losses of BW and WC at 6 months. The reduction of choline was significantly predictive of decreases in body fat composition, fat distribution, and REE. Results of sensitivity analysis showed that the baseline diabetes risk status, such as the presence of hyperglycemia (31% of the total participants) and fasting glucose levels, did not modify the associations. Early changes in choline and l-carnitine were significantly predictive of weight loss over 2 years (P < 0.05 for all). Individuals with increases in choline or l-carnitine were 2.35-times (95% CI 1.38, 4.00) or 1.77-times (1.06, 2.95) more likely to fail to lose weight (–5% or more loss) at 2 years. CONCLUSIONS Overweight and obese individuals who showed decreases in circulating choline or l-carnitine levels achieved greater improvements of adiposity and energy metabolism by eating a low-calorie weight-loss diet, suggesting that such metabolites are predictive of individuals’ response to the treatment. Further investigations are necessary to confirm our findings, particularly in a population with prediabetes that is more representative of the U.S. population with obesity.

We investigated whether changes in gut microbiota-dependent metabolite of trimethylamine N-oxide (TMAO) and its nutrient precursors (choline and L-carnitine) were associated with improvements in glucose metabolism and diabetes-related amino acids in a weight-loss diet intervention.DESIGN:We included 504 overweight and obese adults who were randomly assigned to one of four energy-reduced diets varying in macronutrient intake. The 6-month changes (Δ) in TMAO, choline and L-carnitine levels after the intervention were calculated.RESULTS:Greater decreases in choline and L-carnitine were significantly (p<0.05) associated with greater improvements in fasting insulin concentrations and homeostasis model assessment of insulin resistance (HOMA-IR) at 6 months. The reduction of choline was significantly related to 2-year improvements in glucose and insulin resistance. We found significant linkages between dietary fat intake and ΔTMAO for changes in fasting glucose, insulin and HOMA-IR (pinteraction <0.05); a greater increase in TMAO was related to lesser improvements in the outcomes among participants who consumed a high-fat diet. In addition, ΔL-carnitine and Δcholine were significantly related to changes in amino acids (including branched-chain and aromatic amino acids). Interestingly, the associations of ΔTMAO, Δcholine and ΔL-carnitine with diabetes-related traits were independent of the changes in amino acids.CONCLUSION:Our findings underscore the importance of changes in TMAO, choline and L-carnitine in improving insulin sensitivity during a weight-loss intervention for obese patients. Dietary fat intake may modify the associations of TMAO with insulin sensitivity and glucose metabolism.TRIAL REGISTRATION NUMBER:NCT00072995.

Abstract:Purpose of ReviewThis paper reviews the genetic and non-genetic factors that provided predictions of, or were associated with, weight loss and other metabolic changes in the POUNDS Lost clinical trial of weight loss. This trial randomized 811 individuals who were overweight or obese to one of four diets that contained either 15% or 25% protein and 20% or 40% fat in a 2 × 2 factorial design. A standard behavioral weight loss program was available for all participants who were followed for 2 years with an 80% completion rate.Recent FindingsNineteen genes and five genetic risk scores were used along with demographic, behavioral, endocrine, and metabolic measurements. Genetic variations in most of the genes were associated with weight loss, but this association often varied with the dietary assignment. A number of demographic and behavioral factors, including attendance at behavioral sessions and food cravings were predictive of weight changes. A high baseline level of free triiodothyronine or free thyroxine predicted the magnitude of weight loss. Several perfluoroakyl compounds predicted more rapid weight regain.SummaryGenetic evidence from POUNDS Lost provides guidance toward selection of a personalized weight loss diet and improvement in metabolic profile. There is still room for additional research into the predictors of weight loss.

Abstract:CONTEXT:Compelling evidence suggests that the beneficial effects of weight-loss diet interventions on improvement of cardiometabolic risk factors may be partly through modulating secretion of adiponectin from adipose tissue.OBJECTIVE:To investigate the effects of long-term weight-loss diets with different compositions of macronutrients on longitudinal changes in circulating adiponectin concentrations and how such changes, if they exist, affect cardiometabolic risk.DESIGN, SETTING, AND PARTICIPANTS:In the 2-year Preventing Overweight Using Novel Dietary Strategies trial, 811 overweight or obese adults were randomly assigned to 1 of 4 diets varying in macronutrient intakes. The current analysis was restricted to participants who had baseline adiponectin measurement (n = 768). Circulating concentrations of adiponectin and cardiometabolic outcomes were repeatedly measured at baseline, 6 months, and 2 years.MAIN OUTCOME MEASURES:Circulating concentrations of adiponectin and cardiometabolic risk factors.RESULTS:Weight-loss diet interventions significantly increased circulating adiponectin concentrations over 2 years, similarly in 4 diet groups (P value for difference >.05). We found that the increase of adiponectin was significantly associated with reduction of waist circumference and low-density lipoprotein cholesterol, but associated with increase of high-density lipoprotein cholesterol (P < .001 for each), after adjusting for age, sex, ethnicity, follow-up time, diet group, baseline body mass index, baseline level of respective outcome trait, and concurrent weight change.CONCLUSIONS:Our findings indicate that long-term interventions by weight-loss diets varying in macronutrients similarly increase circulating adiponectin, which may particularly improve abdominal fat distribution and lipid metabolism independently of weight change.

Abstract:ObjectiveIdentifying predictors of weight loss and clinical outcomes may increase understanding of individual variability in weight loss response. We hypothesized that baseline multiomic features, including DNA methylation (DNAme), metabolomics, and gut microbiome, would be predictive of short‐term changes in body weight and other clinical outcomes within a comprehensive weight loss intervention. MethodsHealthy adults with overweight or obesity (n = 62, age 18‐55 years, BMI 27‐45 kg/m2, 75.8% female) participated in a 1‐year behavioral weight loss intervention. To identify baseline omic predictors of changes in clinical outcomes at 3 and 6 months, whole‐blood DNAme, plasma metabolites, and gut microbial genera were analyzed. ResultsA network of multiomic relationships informed predictive models for 10 clinical outcomes (body weight, waist circumference, fat mass, hemoglobin A1c, homeostatic model assessment of insulin resistance, total cholesterol, triglycerides, C‐reactive protein, leptin, and ghrelin) that changed significantly (P < 0.05). For eight of these, adjusted R2 ranged from 0.34 to 0.78. Our models identified specific DNAme sites, gut microbes, and metabolites that were predictive of variability in weight loss, waist circumference, and circulating triglycerides and that are biologically relevant to obesity and metabolic pathways. ConclusionsThese data support the feasibility of using baseline multiomic features to provide insight for precision nutrition–based weight loss interventions.

Objectives: To investigate effects of weight-loss diets with different compositions of macronutrients on long-term changes of circulating adiponectin concentrations and cardiometabolic risk factors, as well as dietary interactions with variation in the adiponectin gene. Methods: In the 2-year Preventing Overweight Using Novel Dietary Strategies (POUNDS Lost) trial, 811 overweight or obese adults were randomly assigned to 1 of 4 diets; the targeted percentages of energy derived from fat, protein, and carbohydrates in the 4 diets were 20, 15, and 65%; 20, 25, and 55%; 40, 15, and 45%; and 40, 25, and 35%. The current analysis was restricted to participants who had baseline plasma adiponectin measurement (n=768). Circulating adiponectin and cardiometabolic outcomes were repeatedly measured at baseline, 6 months, and 2 years. We genotyped single-nucleotide polymorphism rs266171 in the adiponectin gene. Linear mixed models were used to compare changes in adiponectin concentrations across diet groups over the intervention and predict trajectory of changes in cardiometabolic risk factors. Results: Circulating adiponectin concentrations significantly increased over 2 years, similarly in 4 weight-loss diet intervention groups. Change of plasma adiponectin was significantly associated with decreases in weight, waist circumference, triglycerides, fasting insulin, and insulin resistance measured by homeostasis model assessment of insulin resistance (HOMA-IR), and increase in HDL cholesterol, after adjusting for age, gender, ethnicity, follow-up time, diet group, and baseline level of respective outcome trait. The associations remained similar after further controlling for concurrent weight change. In addition, we observed significant interactions between rs266717 genotype and dietary fat on changes in fasting insulin and HOMA-IR (P for interaction=0.03 for both) at 6 months with adjustment for covariates. Participants who are homozygous for T allele of rs266717 had more reductions in fasting insulin and HOMA-IR than those with other genotypes in response to the high-fat diet, but such differences were not observed in the low-fat diet group. Conclusions: Regardless of macronutrient compositions, weight-loss diets beneficially increased circulating adiponectin, which was related to improvement of cardiometabolic profiles. Dietary fat intakes modified the effect of adiponectin variant rs266717 on improvement of insulin resistance.
Plasma Signature Metabolites of Dietary Fat Intake Characterize Associations with Prevalent Metabolic Syndrome

Abstract:The metabolic response to dietary fat intake and its relation with metabolic syndrome (MetS) remains unclear. Here, we identified dietary fat related signature metabolites and characterized their associations with MetS prevalence. We enrolled 236 participants from the Precision Nutrition and Food Safety for Dietary Prevention of Chronic Disease study in China. Nontargeted metabolomics was conducted to investigate plasma metabolome. The signature metabolites in relation to dietary fat intake were assessed using elastic net regression with a 10-fold cross-validation. We identified multi-metabolite profiles comprising of 28, 19, 23, 31, and 25 metabolites, which were robustly correlated with dietary intake of saturated fatty acids, monounsaturated fatty acids, polyunsaturated fatty acids (PUFAs), n - 3 PUFAs, and n - 6 PUFAs, respectively (r = .47-.54; p < .001). After adjustment for potential risk factors, metabolic signatures of PUFAs and n - 3 PUFAs were inversely associated with 31% and 48% lower MetS prevalence, respectively. Dietary intake of n - 3 and n - 6 PUFAs ameliorates key metabolites involved in the glyoxylate and dicarboxylate metabolism, arginine biosynthesis, and tricarboxylic acid cycle. Our findings revealed plasma metabolic signatures characterizing dietary fat intake and supported the beneficial role of PUFAs especially n - 3 PUFAs in MetS prevention.
Metabolomic characterization of unintentional weight loss among community‐dwelling older Black and White men and women

Abstract:Metabolomic analysis of older adults reveals distinct signatures associated with incident unintentional versus intentional weight loss. Data suggests mitochondrial dysfunction and inflammation may precede such unintentional weight loss. This study aims to understand the metabolic mechanisms of unintentional weight loss in older adults. We investigated plasma metabolite associations of subsequent weight change over 2 years in 1536 previously weight stable participants (mean age 74.6 years, 50% women, 35% Black) from the Health, Aging and Body Composition (Health ABC) Study. Multinomial logistic regressions were used to examine associations of the 442 metabolites with weight loss with/without an intention and weight gain >3% annually relative to weight stability. The metabolite associations of unintentional weight loss differed from those of intentional weight loss and weight gain. Lower levels of aromatic amino acids, phospholipids, long‐chain poly‐unsaturated triglycerides, and higher levels of amino acid derivatives, poly‐unsaturated fatty acids, and carbohydrates were associated with higher odds of unintentional weight loss after adjusting for age, sex, race, and BMI categories. Prevalent diseases attenuated four and lower mid‐thigh muscle mass and poorer appetite each attenuated 2 of 77 identified metabolite associations by >20%, respectively. Other factors (e.g., energy expenditure, diet, and medication) attenuated all associations by <20%. While 16 metabolite associations were attenuated by 20%–48% when adjusting for all these risk factors, 47 metabolite associations remained significant. Altered amino acid metabolism, impaired mitochondrial fatty acid oxidation, and inflammaging implicated by identified metabolites appear to precede unintentional weight loss in Health ABC older adults. Furthermore, these pathways seem to be associated with prevalent diseases especially diabetes, lower muscle mass, and poorer appetite.

Longitudinal Profiling of Fasting Plasma Metabolome in Response to Weight-Loss Interventions in Patients with Morbid Obesity

Abstract:It is well recognized that patients with severe obesity exhibit remarkable heterogeneity in response to different types of weight-loss interventions. Those who undergo Roux-en-Y gastric bypass (RYGB) usually exhibit more favorable glycemic outcomes than those who receive adjustable gastric banding (BAND) or intensive medical intervention (IMI). The molecular mechanisms behind these observations, however, remain largely unknown. To identify the plasma metabolites associated with differential glycemic outcomes induced by weight-loss intervention, we studied 75 patients with severe obesity (25 each in RYGB, BAND, or IMI). Using untargeted metabolomics, we repeatedly measured 364 metabolites in plasma samples at baseline and 1-year after intervention. Linear regression was used to examine whether baseline metabolites or changes in metabolites are associated with differential glycemic outcomes in response to different types of weight-loss intervention, adjusting for sex, baseline age, and BMI as well as weight loss. Network analyses were performed to identify differential metabolic pathways involved in the observed associations. After correction for multiple testing (q < 0.05), 33 (RYGB vs. IMI) and 28 (RYGB vs. BAND) baseline metabolites were associated with changes in fasting plasma glucose (FPG) or glycated hemoglobin (HbA1c). Longitudinal changes in 38 (RYGB vs. IMI) and 38 metabolites (RYGB vs. BAND) were significantly associated with changes in FPG or HbA1c. The identified metabolites are enriched in pathways involved in the biosynthesis of aminoacyl-tRNA and branched-chain amino acids. Weight-loss intervention evokes extensive changes in plasma metabolites, and the altered metabolome may underlie the differential glycemic outcomes in response to different types of weight-loss intervention, independent of weight loss itself.

Abstract:The obesity epidemic among children has become a major public health issue, and the presence of childhood insulin resistance (IR) has been demonstrated prior to the onset of type 2 diabetes mellitus. However, it is unclear whether the metabolomic signature is associated with weight loss interventions in obese children with IR. Thirty-six obese children with IR were selected from the weight loss camp (Shenzhen Sunshine Xing Yada health Technology Co., LTD). Clinical parameters were collected before and after weight loss intervention. Targeted metabolomics of plasma samples was performed by ultra-performance liquid chromatography coupled to the tandem mass spectrometry, and principal component analysis, variable importance in projection, and orthogonal partial least squares discriminant analysis were used to obtain the differentially expressed metabolites. Pathway analysis was conducted with the Homo sapiens (HSA) sets in the Kyoto Encyclopedia of Genes and Genomes. We used machine learning algorithms to obtain the potential biomarkers and Spearman correlation analysis to clarify the association between potential biomarkers and clinical parameters. We found that clinical parameters and metabolite clusters were significantly changed in obese children with IR before and after weight loss intervention. Mechanistically, weight loss intervention significantly changed 61 metabolites in obese children with IR. Furthermore, 12 pathways were significantly changed. Moreover, the machine learning algorithm found 6 important potential biomarkers. In addition, these potential biomarkers were strongly associated with major clinical parameters. These data indicate different metabolomic profiles in obese children with IR after weight loss intervention, providing insights into the clinical parameters and metabolite mechanisms involved in weight loss programs.

Abstract:Introduction: Weight-loss diet interventions affect gut microbiota-related metabolites, such as trimethylamine N-oxide (TMAO) and its precursors (choline and L-carnitine), and improve cardiometabolic status. However, little is known about how these metabolites may affect bone mineral density (BMD) during weight loss. Hypothesis: We assessed the hypothesis that changes in gut microbiota-related metabolites are related to changes in BMD during weight loss. Methods: A total of 264 overweight and obese participants with measurement of BMD by dual-energy X-ray (DEXA) scan were included in the present analysis. The participants were randomly assigned to one of four diets varying in macronutrient intakes. We investigated the associations of changes in plasma levels of TMAO, choline, and L-carnitine from baseline to 6 months with changes in BMD. Results and Conclusions: We found that changes in plasma levels of TMAO from baseline to 6 months were positively related to changes of whole body ( P =0.011) and spine ( P =0.002) BMD over the 2-year intervention, independent of weight change. The association with whole body BMD was also significant at 6 months. The correlations between changes in TMAO and BMD were not modified by dietary macronutrients; while changes in TMAO precursor L-carnitine showed interactions with dietary fat intake on changes of spine and trochanter BMD (all P <0.05). Participants with the least L-carnitine decrease showed less BMD reduction by eating a low-fat diet ( P spine =0.031 and P trochanter =0.017). In conclusion, our results suggest that decrease in TMAO during weight loss may be related to reduction in BMD.

Abstract:Objective: Gut microbiota metabolite Trimethylamine-N-oxide (TMAO) has been related to obesity and various cardiometabolic disorders. TMAO is involved in lipid metabolism in the liver; however, little is known about whether changes in TMAO affect hepatic fat and abdominal fat distribution, and whether genetic variations modify such effects. Research Design and Methods: The present study included 92 overweight and obese participants from the POUNDS Lost trial having complete data on computed tomography (CT) scans and a genetic risk score (GRS) for nonalcoholic fatty liver disease (NAFLD). We analyzed the associations of changes in plasma TMAO with changes in hepatic fat and fat distribution from baseline to 6 months; we also assessed the interactions between the GRS and changes in TMAO. Results: Larger decreases in TMAO from baseline were significanly associated with a reduction in subcutaneous adipose tissue (SAT), visceral adipose tissue (VAT), and total adipose tissue (TAT) at 6 months, p=0.01, 0.02, and 0.003, respectively. In addition, we found that larger decreases in TMAO were associated with increases in HDL-cholesterol and decreases in triglycerides, p=0.02 and 0.03, respectively. Moreover, we observed significant interactions between 6-month changes in TMAO and the NAFLD GRS on the concurrent changes in liver fat (P interaction =0.007) and VAT (p interaction =0.018). Among individuals whose TMAO decreased, a lower NAFLD GRS tends to predict a greater improvement in hepatic fat and a reduction in VAT; while in those whose TMAO increased, no differences in hepatic fat or VAT were observed across the tertiles of NAFLD GRS. Conclusions: Our data indicate that changes in TMAO are related to changes in hepatic and visceral fat in weight loss diet interventions; and such relations are modified by genetic variation. Abstract:CONTEXT:Recent evidence has related circulating branch-chained amino acids (BCAAs) to ectopic fat distribution.OBJECTIVE:To investigate the associations of changes in plasma BCAAs induced by weight-loss diet interventions with hepatic fat and abdominal fat, and potential modification by different diets.DESIGN, SETTING, AND PARTICIPANTS:The current study included 184 participants from the 2-year Preventing Overweight and Using Novel Dietary Strategies (POUNDS Lost) trial with repeated measurements on plasma BCAAs, hepatic fat, and abdominal fat over 2 years.MAIN OUTCOME MEASURES:Repeated measurements of hepatic fat, abdominal fat distribution, including visceral adipose tissue (VAT), subcutaneous adipose tissue (SAT), and total adipose tissue (TAT).RESULTS:Over 2 years, a decrease in total plasma BCAAs was significantly associated with improvement in hepatic density (a marker for hepatic fat; P = 0.02) and reductions in abdominal fat, including VAT, SAT, and TAT (all P < 0.05) in the main analyses. Additionally, we observed that decreases in BCAAs were associated with decreased insulin, homeostasis model assessment of insulin resistance, and triglycerides, independent of weight loss (all P < 0.05). Moreover, we found that dietary protein intake significantly modified the relation between changes in total plasma BCAAs and hepatic density at 6 months (Pinteraction = 0.01). Participants with a larger decrease in total BCAAs showed a greater increase in hepatic density when consuming a high-protein diet, compared with those with a smaller decrease or increase in total BCAAs.CONCLUSIONS:Our findings indicate that weight-loss diet-induced decrease in plasma BCAAs is associated with reductions of hepatic and abdominal fat. In addition, dietary protein intake may modify these associations.

Abstract:As obesity develops, metabolic changes increase the risk of non-communicable diseases such as type 2 diabetes (T2D). Weight loss is crucial for improving health in T2D and cardiometabolic conditions. However, weight loss rates vary between individuals, even with identical diets or energy restrictions, highlighting the need to identify markers or predictors of weight loss success to enhance intervention outcomes. Using nuclear magnetic resonance (NMR) spectroscopy-based metabolomics, we investigated the change in serum polar metabolites in 28 women with overweight or obesity and prediabetes who completed an 8-week low-energy diet (LED) as part of the PREVIEW (PREVention of diabetes through lifestyle intervention and population studies in Europe and around the World) clinical trial. We aimed to characterize the metabolic shift in substrate oxidation under fixed energy intake (~4 MJ/day) and its relation to weight loss success. Nine of the thirty-four serum metabolites identified significantly changed during the LED phase: 3-hydroxybutyrate, O-acetylcarnitine, 2-hydroxybutyrate, mannose, dimethyl sulfone and isobutyrate increased, whilst choline, creatine and tyrosine decreased. These results confirmed a shift towards lipid oxidation, but no metabolites predicted the response to the LED-induced weight loss. Further studies in larger populations are required to validate these metabolites as biomarkers of diet exposure.

Abstract:To investigate the metabolic biomarkers of predicting the transition from pre-diabetes (pre-DM) to normal glucose regulation (NGR) and diabetes (DM) in a longitudinal cohort study. 108 participants with pre-DM were followed up for ten years and divided into 3 groups according to different glycemic outcomes. 20 participants progressed to DM, 20 regressed to NGR, and 68 remained at pre-DM. Alterations in plasma metabolites in these groups were evaluated by untargeted ultra-performance liquid chromatography-quadrupole time-of-flight mass spectrometry (UPLC-QTOF-MS). Twenty three metabolites related to glycerophospholipid metabolism, oxidation and antioxidation were associated with the process from pre-DM to NGR, while twenty two metabolites related to amino acid metabolism, glycerophospholipid metabolism and mitochondrial β-oxidation played important roles in the progression to DM. Results from stepwise logistic regression analysis showed that five biomarkers (20-Hydroxy-leukotriene E4, Lysopc(20:4), 5-methoxytryptamine, Endomorphin-1, Lysopc(20:3)) were good prediction for the restoration to NGR, and five biomarkers (Iso-valeraldehyde, linoleic acid, Lysopc(18:1), 2-Pyrroloylglycine, Dityrosine) for the development of DM. The findings suggest that the combination of these potential metabolites may be used for the prognosis of pre-DM. Targeting the pathways that involved in these prognostic biomarkers would be beneficial for the regression to NGR and the early prevention of DM among pre-DM.

Abstract:Background: Circulating amino acids, such as branched-chain amino acids (BCAAs) and aromatic amino acids (AAAs), have been associated with diabetes risk; however, little is known about how a long-term dietary intervention for weight loss affects circulating amino acids.Objectives: We examined the effects of weight-loss diets on long-term changes in plasma amino acids and the associations of these changes with weight loss and the improvement of insulin resistance.Design: We repeatedly measured plasma amino acid profiles over 2 y in overweight or obese participants from 2 randomized, dietary intervention, weight-loss trials [774 subjects from the POUNDS LOST (Preventing Overweight Using Novel Dietary Strategies Trial) and 318 subjects from the DIRECT (Dietary Intervention Randomized Controlled Trial)].Results: Intervention diets consistently lowered most of the amino acid concentrations, including BCAAs and AAAs, in both trials. In the POUNDS LOST, average-protein diets (15% of daily energy) showed stronger effects than did high-protein diets (25% of daily energy) on reducing concentrations of the diabetes-associated BCAA valine at 6 mo independent of the weight change. In both trials, weight loss was directly related to the concurrent reduction of the BCAAs leucine and isoleucine, the AAAs tyrosine and phenylalanine, and 4 other amino acids. For example, per kilogram of weight loss, there was a 0.04-SD decrease in log tyrosine (similar to 0.6 mu mol/L) in both trials. In addition, we showed that reductions in alanine and the AAA tyrosine were significantly related to improved insulin resistance (measured with the use of the homeostasis model assessment of insulin resistance), independent of weight loss, in both trials (both P < 0.05). For example, per 1-SD decrease in log tyrosine (similar to 17 mu mol/L), there was a 0.04-SD (similar to 3%) improvement in insulin resistance in the POUNDS LOST and a 0.13-SD (similar to 8%) improvement in insulin resistance in the DIRECT.Conclusion: Our findings underscore the potential importance of dietary interventions in improving amino acid profiles (i.e., reducing diabetes risk enhancing amino acid concentrations) along with and beyond weight loss. The POUNDS LOST and the DIRECT were registered at clinicaltrials.gov as NCT00072995 and NCT00160108, respectively.

Abstract:Abstract Background One of the major challenges in obesity treatment is to explain the high variability in the individual’s response to specific dietary and physical activity interventions. With this study, we tested the hypothesis that specific DNA methylation changes reflect individual responsiveness to lifestyle intervention and may serve as epigenetic predictors for a successful weight-loss. Methods We conducted an explorative genome-wide DNA methylation analysis in blood samples from 120 subjects (90% men, mean ± SD age = 49 ± 9 years, body mass-index (BMI) = 30.2 ± 3.3 kg/m 2 ) from the 18-month CENTRAL randomized controlled trial who underwent either Mediterranean/low-carbohydrate or low-fat diet with or without physical activity. Results Analyses comparing male subjects with the most prominent body weight-loss (responders, mean weight change − 16%) vs. non-responders (+ 2.4%) ( N = 10 each) revealed significant variation in DNA methylation of several genes including LRRC27 , CRISP2 , and SLFN12 (all adj. P < 1 × 10 −5 ) . Gene ontology analysis indicated that biological processes such as cell adhesion and molecular functions such as calcium ion binding could have an important role in determining the success of interventional therapies in obesity. Epigenome-wide association for relative weight-loss (%) identified 15 CpGs being negatively correlated with weight change after intervention (all combined P < 1 × 10 − 4 ) including new and also known obesity candidates such as NUDT3 and NCOR2 . A baseline DNA methylation score better predicted successful weight-loss [area under the curve (AUC) receiver operating characteristic (ROC) = 0.95–1.0] than predictors such as age and BMI (AUC ROC = 0.56). Conclusions Body weight-loss following 18-month lifestyle intervention is associated with specific methylation signatures. Moreover, methylation differences in the identified genes could serve as prognostic biomarkers to predict a successful weight-loss therapy and thus contribute to advances in patient-tailored obesity treatment.

Abstract:Background: Weight loss through lifestyle interventions, notably low-energy diets, offers glycemic benefits in populations with overweight-associated prediabetes. However, more than 50% of these individuals fail to achieve normoglycemia after weight loss. Circulating lipids hold potential for evaluating dietary impacts and predicting diabetes risk. Objective: This study sought to identify serum lipids that could serve as evaluative or predictive biomarkers for individual glycemic changes following diet-induced weight loss. Methods: We studied 104 participants with overweight-associated prediabetes, who lost ≥8% weight via a low-energy diet over eight weeks. High coverage lipidomics was conducted in serum samples before and after the dietary intervention. The lipidomic recalibration was assessed using Differential Lipid Abundance Comparisons and Partial Least Squares Discriminant Analyses. Associations between lipid changes and clinical characteristics were determined by Spearman's correlation and Bootstrap Forest of Ensemble Machine Learning model. Baseline lipids, predictive of glycemic parameters changes post-weight loss, were using Bootstrap Forest Analyses. Results: We quantified 439 serum lipid species and 9 related organic acids. Dietary intervention significantly reduced diacylglycerols, ceramides, lysophospholipids and ether-linked phosphatidylethanolamine. In contrast, acylcarnitines, short-chain fatty acids, organic acids, and ether-linked phosphatidylcholine were significantly increased. Changes in certain lipid species (e.g. saturated and monounsaturated fatty acid-containing glycerolipids, sphingadienine-based very long-chain sphingolipids and organic acids) were closely associated with clinical glycemic parameters. Six baseline bioactive sphingolipids primarily predicted changes in fasting plasma glucose. In addition, a number of baseline lipid species, mainly diacylglycerols and triglycerides, were predictive of clinical changes in hemoglobin A1c, insulin and HOMA-IR. Conclusions: Newly discovered serum lipidomic alterations and the associated changes in lipid-clinical variables suggest broad metabolic reprogramming related to diet-mediated glycemic control. Novel lipid predictors of glycemic outcomes could facilitate early stratification of individuals with prediabetes who are metabolically less responsive to weight loss, enabling more tailored intervention strategies beyond one-size-fits-all lifestyle modification advice.
