Abstract:Objective: To investigate the clinical effects of frozen shoulder treated by ultrasound-guided injection.Methods: Thirty-three patients received this treatment with ultrasound guided injection of triamcinolone acetonide 40mg,2% lidocaine 5ml.VAS and Constant & Murley were used to assess before treatment,and review within 7～14 days after treatment.Four cases droped out,and completed a full assessment of 29 cases.Paired t-test was used for statistical analysis,significance level was P<0.05.Patients droped out were treated by intention to treat Results: After ultrasound-guided injection for frozen shoulder,VAS decreased from 6.8±1.9 to 3.5±2.3(P<0.01);Constant & Murley shoulder function score increased from 33.7±12.0 to 45.2±12.2(P<0.01).Conclusion: Ultrasound-guided injection on frozen shoulder obtained good clinical efficacy.It is a kind of promising treatment in pain medicine.

Abstract:Hemiplegic shoulder pain (HSP), which occurs in most patients with hemiplegia, causes considerable distress and worsens outcomes in rehabilitation. Although they have received the treatments such as anti-inflammatory drugs or physical therapy, many of the individuals remain suffering from shoulder pain 6 months after acute stroke event. In this retrospective study, we evaluated the effectiveness of ultrasound guided subacromial-subdeltoid (SASD) bursa injections with botulinum toxin type A (BoNT/A) compared to steroids for refractory HSP. The data were collected retrospectively by reviewing the patient's medical records and pain questionnaires in our rehabilitation center. In total, 38 patients who received ultrasound guided SASD bursa injection (BoNT/A group, n=18; corticosteroid group, n=20) were included. The pain visual analog scale (VAS) score at rest and during arm passive abduction, Fugl-Meyer score of upper limbs (F-M score) were evaluated before, 2, 4, 8, and 12 weeks after injection. Both 2 groups obtained a significant improvement of VAS score at rest or during arms passive abduction compared to baseline score (within group compare, P<.05). There were no significant differences of pain score improvement between two groups at week 2, 4, 8, and 12 after injection either at rest or during passive arm abduction (between 2 groups compare, P>.05). There were also no differences in results of the post treatment F-M score between 2 groups (between 2 groups compare, P>.05). Similarly, during the follow-up period no collateral effects were reported after BoNT/A injection. SASD bursa BoNT/A injection can substantially reduce the pain as corticosteroid in patients with HSP. BoNT/A injection could be a useful strategy for replacing steroids as a treatment for refractory HSP especially in the patients who cannot tolerate the steroids injection.

Abstract:Frozen shoulder, a condition characterized by the limitation of active and passive shoulder movements, is predominantly managed conservatively. Corticosteroid injection stands as a widely utilized and effective method for treatment. Nevertheless, the optimal approach for administering the injection remains a subject of controversy. We hypothesized that the injection of corticosteroid through anterior into the extra-articular area would yield superior results than the posterior intra-articular injection. This prospective, randomized, single-blind, and single-center clinical study aimed to compare the therapeutic effectiveness of ultrasound (USG)-guided injection techniques in patients with primary frozen shoulder. We continued the study with 54 patients who met the inclusion criteria. We evaluated patients pre-injection and at the first, third, and sixth weeks after injection. Two distinct approaches were employed: (1) Injection through the anterior extra-articular area around the coracohumeral ligament (CHL), and (2) injection through the posterior route into the glenohumeral joint. Evaluation parameters, including Visual Analog Scale (VAS) scores, active range of motion (ROM), American Shoulder and Elbow Surgeons (ASES) scores, and Quick Disabilities of the Arm, Shoulder, and Hand (QuickDASH) scores, were assessed pre-injection and at the first, third, and sixth weeks post injection. No significant differences in the demographic and clinical characteristics, laboratory, or radiological findings were observed between the groups. It was observed that both groups exhibited similar and significant improvement in all measured parameters within their respective groups. Regarding pain scores, a notable decrease in VAS scores was observed in both groups (P = .000). There was no difference between the groups in terms of pre-injection VAS, QuickDASH, and ASES scores and active ROM measurements. The study concluded that there was no significant difference between ultrasound-guided injection of posterior intra-articular injection and anterior extra-articular injection concerning pain, ROM, and functional scores. Both techniques can be considered as viable options in the early stage of the disease. Level I, Therapeutic study.
Study of Functional Outcome of Intra-articular Steroid Injection and Manipulation of Shoulder Joints in Frozen Shoulder

Abstract:Introduction: Frozen shoulder, also known as adhesive capsulitis, is characterized by the insidious onset of pain and progressive loss of global active and passive mobility of the glenohumeral joint, which can be treated by non-surgical or surgical methods. This study was done to evaluate the functional outcome of intra-articular (I/A) steroid injection and manipulation of the shoulder joint manipulation under anesthesia (MUA) in frozen shoulder. Methods: A cross-sectional study was done in a tertiary care hospital in Pokhara, Nepal. A total of 35 patients over 40 years were included in the study. All the patients underwent I/A steroid injection and MUA. The functional outcome was evaluated using a Constant-Murley shoulder score (CSS) and a visual analogue scale (VAS) scoring system. Result: The mean age was 53.46±7.524 years. Twenty-two were female, while 13 were male. Twelve patients were associated with diabetes. Minor complications were noted in 15 patients (14 with transient pain and one with facial flushing). At 12 weeks, the mean VAS score was 2.16±1.33 cm, and the mean CSS score was 82.66±7.008. At 24 weeks, the mean VAS was 1.534±1.195 cm, and the mean CSS score was 85.77±6.998. Conclusion: This study shows that patients with frozen shoulder treated with I/A steroid injection and MUA give excellent functional outcomes in most patients.

Abstract:Objective To investigate the efficacy of corticosteroid injection methods for frozen shoulder. Data Sources PubMed, Embase, and Cochrane Library were searched up to 6th May 2023. Study Selection Randomized controlled trials (RCTs) that investigated corticosteroid injection methods for frozen shoulder were included. Data Extraction Data were extracted independently by two authors. Risk of bias was assessed using the RoB 2 tool. Data synthesis A random-effects network meta-analysis was performed within a frequentist framework. The certainty of evidence was evaluated using the Grading of Recommendations, Assessment, Development, and Evaluations approach. A total of 66 RCTs involving 4491 patients were included. For short-term outcomes, 4-site injection (vs placebo: SMD = −2.20, 95% CI: −2.81 to −1.59 in pain; SMD = 2.02, 95% CI: 1.39 to 2.65 in global function) was the most effective (low certainty). Rotator interval injection was the optimal treatment with moderate to high certainty (vs placebo: SMD = −1.07, 95% CI: −1.51 to −0.64 in pain; SMD = 0.94, 95% CI: 0.49 to 1.40 in global function). For midterm outcomes, 4-site injection was most effective (vs placebo: SMD = −1.71, 95% CI: −2.41 to −1.01 in pain; SMD = 2.22, 95% CI: 1.34 to 3.09 in global function; low certainty). Distension via rotator interval was the optimal treatment with moderate to high certainty (vs placebo: SMD = −1.10, 95% CI: −1.69 to −0.51 in pain; SMD = 1.46, 95% CI: 0.73 to 2.20 in global function). Distension and intra-articular injection via anterior or posterior approaches produced effects equivalent to those of rotator interval injection and distension via rotator interval. Conclusions Rotator interval injection, distension, and intra-articular injection had equivalent effects on symptom relief. More RCTs are required to validate the superiority of multisite injections.

The Use of Corticosteroid/Anesthetic Injections in Conjunction With Physical Therapy in the Treatment of Idiopathic Frozen Shoulder: A Case Series

Abstract:Background Fifty percent of people diagnosed with frozen shoulder (FS) experience pain and/or stiffness for years after onset, and the best conservative treatment for this condition remains uncertain. This case series evaluated the clinical results of intra-articular corticosteroid and anesthetic injections, combined with a specific physical therapy program, in the treatment of FS. Case Presentation The research is based on 26 patients with idiopathic FS. The patients were treated with unguided intra-articular corticosteroid and anesthetic injections, painful/end-range mobilization techniques, and painful/end-range home stretching exercises. Range of motion was measured and the Disabilities of the Arm, Shoulder and Hand questionnaire (DASH), and the Medical Outcomes Study 36-Item Short- Form Health Survey (SF-36) were administered at 5 follow-up sessions over 3 months. Outcome and Follow-Up The 90% confidence intervals of the mean range-of-motion scores for flexion, extension, internal rotation, and external rotation at 90° and at 0° of abduction did not show significant differences at 3-month follow-up between the affected shoulder and the healthy one. Between baseline and 3-month follow-up, the average DASH difference was 37%, and all scores exceeded the minimal clinically important difference of the DASH. The score on the bodily pain subscale of the SF-36 improved by 43% from baseline to 3-month follow-up. Discussion From our clinical experience and the analyzed data, intra-articular corticosteroid and anesthetic injections, combined with the above-mentioned mobilizations and a simple home exercise program, are effective at improving shoulder pain, range of motion, and disability in the short term. The main treatment for adhesive capsulitis is a trial of conservative therapies, including analgesia, exercise, physiotherapy, oral nonsteroidal anti-inflammation drugs, and intra-articular corticosteroid injections. Previously, it was reported that intra-articular corticosteroid lead to fast pain relief and improvement of range of motion (ROM). The objective of this study was to determine whether corticosteroid injections would lead to better pain relief and greater improvement in ROM. Methods: We searched PubMed, Medline, and the Cochrane library. We included 5 articles of the 1166 articles identified. Totally injection group included 115 patients and placebo group included 110 patients. We calculated the weighted mean differences to evaluate the pain relief as the primary outcome. We determined the ROM as the secondary outcome. Study quality was evaluated using the 12-item scale. We also used the criteria of the Grading of Recommendations Assessment, Development and Evaluation to evaluate the quality of evidence. Results: In total, 5 studies were included, 4 of which were randomized clinical trials, with a sample size of 225 patients with adhesive capsulitis of the shoulders. The overall pooled data demonstrated that, compared with placebo as control treatment, intra-articular corticosteroid injections were more effective in reducing the pain score at 0 to 8 weeks, but there was no difference between the injection group and the control group at 9 to 24 weeks. Improvement of ROM in the injection group was greater than that of the control group both at 0 to 8 and 9 to 24 weeks. Conclusions: Intra-articular corticosteroid injections were more effective in pain relief in the short term, but this pain relief did not sustain in the long term. Intra-articular corticosteroid injection resulted in greater improvement in passive ROM both in the short and the long terms.
Hydrodilatation and Intra-articular Steroid Injection are Both Effective in Management of Frozen Shoulder: A Case Series

Abstract:Background: Frozen shoulder is a common problem in general orthopaedic practice, affecting about 2% of the population. Intra-articular corticosteroids (IA) and hydrodilatation have been reported as more effective among other conservative treatments. However, it is unclear which treatment is superior for frozen shoulder, and the hydrodilatation procedure leads to more discomfort in patients since it involves stretching of joint capsule. In this case series, we present 10 cases of frozen shoulder that were treated with hydrodilatation or IA steroid injection. The purpose of this study is to show the effectiveness of hydrodilatation and IA steroid injection in managing patient with frozen shoulder. Methods: This study was a retrospective case series of patients who received IA steroid injection or hydrodilatation. Five patients underwent IA steroid injection, and another five patients underwent hydrodilatation. The American Shoulder and Elbow Score (ASES) was used to evaluate each patient before and six months after treatment. Result: Hydrodilatation and IA steroid injection showed significant improvement in ASES score assessed at 6-month follow-up. Conclusions: Hydrodilatation and IA steroid injection are both effective to treat frozen shoulder in long term follow up.
A retrospective comparative study on the effectiveness of multisite injection versus arthroscopic capsular release for idiopathic frozen shoulder

Abstract:Arthroscopic capsular release is a most well-known technique with favorable outcomes for frozen shoulder. However, considering the surgical trauma and the improvement of multisite injection, we design a study to compare the pain relief and safety of multisite injection (MI) versus arthroscopic capsular release (ACR) for frozen shoulder. A total of 80 patients with unilater al frozen shoulder were enrolled in this study. Group RBT ( n = 40) received multisite injection (0.5% lidocaine and triamcinolone acetonide, once a week, no more than 2 injections), while Group ACR received arthroscopic capsular release. The following parameters were employed to compare: visual analog scale (VAS), range of motion (ROM), the Disability of Arm, Hand, and Shoulder (DASH) score and Oxford shoulder score (OSS). Side effects were also recorded. The VAS, ROM, DASH and OSS all improved significantly ( P < 0.001). Internal rotation and external rotation at 1 month after operation were better in ACR group (40.35 ± 4.79 Vs 36.58 ± 7.49, 40.55 ± 4.37 Vs 38.63 ± 4.01, P = 0.009,0.043). However, no significance in terms of functional results and ROM was found at 6 months after operation. The OSS, DASH and VAS in patients with diabetes were 44.25 ± 3.05, 2.29 ± 1.12 and 0.50 ± 0.72, compared with 43.89 ± 3.09, 2.34 ± 1.49 and 0.29 ± 0.56 in patients without diabetes ( P = 0.636, 0.889, 0.157). Multisite injection and arthroscopic capsular release are both effective treatments in the treatment of frozen shoulder. However, multisite injection is a simple, cost effective and superior alternative.

Comparison of Outcomes of Two Different Corticosteroid Injection Approaches for Primary Frozen Shoulder: A Randomized Controlled Study

Abstract:Objective: Corticosteroid injection is a common treatment for primary frozen shoulder, but controversy remains regarding whether different injection approaches to the glenohumeral joint have similar clinical benefits.Design: Randomized controlled clinical trial.Patients: A total of 60 patients with primary frozen shoulder were divided randomly into either anterior or posterior approach groups.Methods: Both groups received a 5-mL drug injection, including 1 mL 40 mg/mL triamcinolone acetonide and 4 mL 2% lidocaine. Follow-up time-points were 4, 8 and 12 weeks post-injection. Outcome measures included visual analogue scale score, Constant-Murley score, and passive range of motion of the shoulder joint.Results: All outcome measures improved over the follow-up period compared with those of previous follow-up time-points within the groups. The primary finding was that the visual analogue scale score in the anterior group was better than that in the posterior group at each follow-up time-point (all p < 0.05). In addition, improvement in function score and external rotation was faster and significant in the anterior group in the early stages (p = 0.02).Conclusion: The anterior approach achieves more satisfactory results in pain control and offers better recovery of functional activity than posterior approach in the early period for primary frozen shoulder. LAY ABSTRACTAlthough many studies have been published in recent years on corticosteroid injection in the glenohumeral joint for primary frozen shoulder, the injection approaches described by current reports are not consistent and not described in detail, which is not conducive for their use to be repeated by clinicians. Meanwhile, controversy remains regarding whether different approaches have similar clinical outcomes. This study used a modified anterior approach based on anatomical landmarks, taking the acromioclavicular joint as a reference, to accurately and quickly complete the injection process. The study showed that the anterior approach could reduce pain severity more significantly than the regular posterior approach, and this advantage can be maintained for 3 months. Meanwhile, the function score of the anterior approach could recover to the maximum more quickly. In addition, recovery of external rotation in the anterior approach was faster and greater that for the posterior approach.

Comparison of the Efficacy of Ultrasound-Guided Suprascapular Nerve Blocks and Intraarticular Corticosteroid Injections for Frozen Shoulder: A Randomized Controlled Trial

Abstract:Background: The current mainstream treatment for frozen shoulder is a combination of physiotherapy and intraarticular corticosteroid injections (IACIs). Recently, the ultrasound-guided suprascapular nerve block (SSNB) has developed as a notable alternative option to the mainstream treatment. Objective: We aimed to compare ultrasound-guided SSNBs' effectiveness to IACIs' as treatments for frozen shoulder. Study design: This study was conducted as a prospective single-blind, randomized controlled trial. Setting: Department of Physical Medicine and Rehabilitation, Shin Kong Wu Ho-Su Memorial Hospital, a medical center in Taipei, Taiwan. Methods: Patients with frozen shoulder (n = 76) were enrolled as participants and allocated to either an SSNB group (n = 38) or an IACI group (n = 38). Both groups received 2 injections of 20 mg of triamcinolone and 3 mL of 1% lidocaine at 2-week intervals and underwent the same physiotherapy protocol for 3 months. The primary outcome measure was the Shoulder Pain and Disability Index (SPADI). The secondary outcome measures were the Shoulder Disability Questionnaire (SDQ), the active and passive range of motion (ROM) of each patient's affected shoulder, and the 36-item Short Form Health Survey (SF-36). Evaluations were performed at baseline and at 4 and 12 weeks after starting treatment. Results: Both groups achieved significant improvements in all outcome measures, except the general health subscale of the SF-36 at 4 and 12 weeks after starting treatment. For time and group interaction, the results for the SDQ (P = .047) and SF-36 (bodily pain, P = .025) indicated significant differences that favored IACIs. Additionally, the IACI group achieved more favorable outcomes than did the SSNB group on the SPADI (P = .094) and in ROM (i.e., abduction [P = .190] and external rotation [P = .081]) as well as on 2 subscales of the SF-36: bodily pain (P = .059) and role-emotional (P = .072). Limitations: Our study is limited by the lack of participant stratification based on the stages of frozen shoulder and the 12-week follow-up period. Conclusions: A combination of ultrasound-guided IACIs and physiotherapy should be attempted first as a frozen shoulder treatment.
High-dose short-course oral corticosteroid protocol for treatment of primary frozen shoulder: a retrospective cohort study

Abstract:Objective To evaluate the effect of high-dose prednisolone on the functional outcome of patients with early-stage primary frozen shoulder. Methods Eighteen patients treated with oral prednisolone at an initial dose of 1 mg/kg/day for primary frozen shoulder were retrospectively evaluated. The patients’ range of motion, Disabilities of the Arm, Shoulder, and Hand (DASH) score, Constant–Murley score, American Shoulder and Elbow Surgeons (ASES) score, and visual analog scale score were recorded at baseline and at 4 weeks and 6 months after treatment. Results Rapid recovery of shoulder motion was noted at 4 weeks with the exception of abduction, which was maintained at 6 months. Significant improvement in pain perception and the Constant–Murley score was evident at 4 weeks and extended to 6 months. The DASH and ASES scores did not show significant improvement in the first 4 weeks but were significantly improved at 6 months. Conclusion High-dose oral prednisolone treatment provides rapid symptom resolution that persists long after drug discontinuation. The early treatment period is characterized by marked reduction in pain and rapid recovery of shoulder motion. Improvements in functional outcomes and disability indices tend to be more subtle in the early period but significantly improve during late treatment.

Effects of intra-articular steroid injection before pan-capsular release in patients with refractory frozen shoulder

Abstract:Purpose The purpose of the study was to retrospectively determine the effects of intra-articular steroid injection on the clinical outcome of patients who underwent arthroscopic pan-capsular release for refractory frozen shoulder. Methods Between 2000 and 2010, 34 patients (20 males, 14 females) who underwent an arthroscopic pan-capsular release for frozen shoulder were included. Intra-articular steroid injections were administered from April 2006, and just before surgery and at the final follow-up, ranges of motion (ROM) and scores of the Shoulder Rating Scale of the University of California, Los Angeles (UCLA) scoring system were evaluated. Results Intra-articular steroid injection significantly improved the UCLA scores of pain just before surgery and at the final follow-up (1.7 ± 0.5 vs. 6 ± 2, p < 0.0001). We subdivided the patients into those with and without diabetes mellitus. Steroid injection had a significant effect on the ROM in forward flexion (166.6 ± 8.6 vs. 140 ± 36.1, p = 0.026) and the scores of pain (10 vs. 9.3 ± 1.2, p = 0.046) at the final follow-up in the group without diabetes mellitus compared with those with it. Conclusion Intra-articular steroid injection improves pain just before an arthroscopic pan-capsular release and at the final follow-up in all the patients with refractory frozen shoulder. However, it improves the ROM in forward flexion and the UCLA scores of pain at the final follow-up only in those without diabetes mellitus. Level of evidence Therapeutic studies, Level IV.
Multisite Injection of Corticosteroids is more Efficient than a Single Glenohumeral Injection in Treating Acute Frozen Shoulder

Abstract:Objective: To compare the effectiveness of a single glenohumeral injection versus multisite injection of corticosteroids in treating acute frozen shoulder. Study design: Randomized controlled trial Setting and duration: Department of Orthopedic Surgery Civil Hospital Bahawalpur from January 2022 to December 2022. Material and methods: Total 74 patients with acute frozen shoulder having onset of symptoms within the last 12 months were included. This study compared the functional ratings, Mobility, and pain alleviation of patients with primary Frozen Shoulder (FS) after different period of follow up in our OPD. In multisite group, triamcinolone acetonide, bupivacaine and saline were injected into four different sites. In the single glenohumeral injection group, only the glenohumeral joint was injected. Both groups received physiotherapy after injection, including soft tissue therapy, joint mobilization and a home exercise program. The outcome measures were assessed through Range of motion, function and pain scores at a follow up of every three months, six months and then after one year. Results: According to the study, the patients receiving injections at multisite outperformed those receiving single-injection on the visual analogue scale (VAS) at all follow-up intervals (P = 0.01). Moreover, compared to the single-injection group, the multisite-injection group had substantially superior active and passive ROM restoration (P 0.05) and higher functional scores (P 0.05). Practical Implication: It suggests multisite injections as a treatment for frozen shoulder, in contrast to prior studies. Positive characteristics of the study include the use of a blinded clinical examination and the presence of a sufficient number of participants Conclusion: In conclusion, multisite injection of corticosteroids has more potency than a single injection in reducing pain in patients with acute frozen shoulder. Clinicians may consider multisite injection as a preferred method of treatment for primary frozen shoulder, especially in patients with severe pain. The Disease-Modifying Effects of a Single Intra-Articular Corticosteroid Injection during the Freezing Phase of Frozen Shoulder in an Animal Model

Abstract:Although frequently prescribed for frozen shoulder, it is not known if corticosteroid injections improve the course of frozen shoulder. This study aimed to assess the disease-modifying effects of an intra-articular corticosteroid administration at the freezing phase of frozen shoulder. Twenty-four Sprague-Dawley rats were divided into four groups. Their unilateral shoulders were immobilized for the first 3 days in all groups, followed by an intra-articular corticosteroid injection in Group A, an injection and the cessation of immobilization in Group B, no further intervention in Group C, and the cessation of immobilization in Group D. All rats were sacrificed in Week 3 of study, at which point the passive shoulder abduction angles were measured and the axillary recess tissues were retrieved for histological and Western blot analyses. The passive shoulder abduction angles at the time of sacrifice were 138° ± 8° (Group A), 146° ± 5° (Group B), 95° ± 11° (Group C), 132° ± 8° (Group D), and 158° ± 2° (Control). The histological assessments and Western blots showed greater fibrosis and inflammation in the groups that did not receive the corticosteroid injection (Groups C and D) compared to the corticosteroid-injected groups (Groups A and B). These findings demonstrate the anti-inflammatory and disease-modifying effects of corticosteroid injections during the freezing phase of frozen shoulder in an animal model.
Comparison of Intra-Articular Injection and Arthroscopic Capsular Release in Stages I and II of Primary Frozen Shoulder, a Randomized Clinical Trial

Abstract:Background: To compare the effects of conservative steroid injection and arthroscopic capsular release during early intervention of stages I and II of primary frozen shoulder (PFS). Methods: A randomized controlled clinical trial was conducted. Thirty-six stage I and 32 stage II patients were randomly allotted to a steroid injection and an arthroscopic treatment groups and followed up for 12 months. The outcome was evaluated using visual pain scale (VAS), the American Shoulder and Elbow Surgeon (ASES) score, shoulder active range of motion (ROM) prior to treatment, and 3 weeks and 3 and 12 months post-treatment. Results: All the subjects in stages I and II PFS groups showed similar shoulder pain and function scoring before treatment. Among stage I patients, conservative treatment showed higher VAS, ASEA and ROM scores 3 weeks post-treatment. Better VAS and ASEA scores were observed in the arthroscopic treatment group with 3 months post-treatment. Patients treated by arthroscopy showed better performance on all the clinical outcomes with 12 months post-treatment, compared with conservative treatment group. In stage II patients, conservative treatment showed better therapeutic effects compared with arthroscopic treatment with 3 weeks post-treatment. Arthroscopic treatment led to better performance on backward extension and forward flexion with 3 months post-treatment. Arthroscopic patients so treated showed better performance on all the clinical outcomes with 12 months post-treatment, compared with conservative treatment group. Conclusion: In early PFS patients, especially for stage II cases, arthroscopic treatment can better improve shoulder joint ROM, relieve pain, and enhance function, compared with steroid injection treatment. The Effect of Intra-articular Injection of Hyaluronic Acid in Frozen Shoulder: a Systematic Review and Meta-analysis of Randomized Controlled Trials

Abstract:Abstract Background Frozen shoulder (FS) is a common progressive disorder that causes restricted motion and refractory pain undermining quality of life. Intra-articular hyaluronic acid (HA) injection is a widely adopted conservative therapy relieving symptomatic FS, whereas the effect of which were contradictory and unclear in current literatures. The aim of the present study is to investigate whether intra-articular HA administration facilitates symptomatic pain relief and functional improvements in patients diagnosed with shoulder FS. Methods The PubMed, Embase, Cochrane Library electronic databases and Google scholar were searched, from inception to 15th Jan 2022. Randomized controlled trials (RCTs) comparing intra-articular HA administration with any other non-surgical treatment in patients with FS were included. Risk of bias was evaluated using the Cochrane risk-of-bias tool and meta-analyses were undertaken to pool the data of visual analog scale for pain, range of motion (ROM) in external rotation, abduction, and flexion, as well as Shoulder Pain and Disability Index (SPADI), Constant score and American Shoulder and Elbow Surgeons (ASES). Results The present study included 7 RCTs involving 504 patients. The results provided no support for superior pain control in patients undergoing HA injection compared with any other treatment ( p = 0.75). Furthermore, HA group failed to exert superior improvements to other treatments in ROM concerning abduction ( p = 0.69) and flexion ( p = 0.33). However, HA injection was observed to facilitate functional recovery in external rotation ( p = 0.003). In addition, the pooled data showed a significant higher SPADI score in control group than in HA group ( p = 0.01), while no statistical significance between two groups was observed in Constant score ( p = 0.36) and ASES ( p = 0.76). Conclusions The current meta-analysis suggested that HA is a beneficial treatment procedure in improving the ROM of the shoulder for patients with FS, whereas the effect in relieving pain may be equal to the existing therapy. In conclusion, Intra-articular HA injection is recommended for FS patients.

Abstract:Objective To detect the effectiveness of ultrasound-guided glenohumeral joint injection combined with distension for treatment of frozen shoulder.Methods Fifty-two patients diagnosed as frozen shoulder by physical and ultrasound examination were performed the ultrasound-guided glenohumeral joint injection combined with distension.Finally,48 patients completed the continuously three times'treatment were included in this study.Patients were assessed using Visual Analogy Score(VAS),the influence of sleeping and the Active Range of Motion(AROM)at the beginning of each time's treatment and at 2 weeks after the third treatment in order to compare the effectiveness of each time's treatment.Results The pain and sleeping influence of the first treatment were significantly improved compared with before treatment(all P <0.05),but the AROM was not significantly improved(P >0.05).The pain,sleeping influence and AROM of the second treatment were significantly improved by comparing with the first treatment respectively(all P <0.05).The pain and AROM of the third treatment were further improved than the second treatment(all P <0.05).However there was no significant improvement of sleeping influence for the third treatment(P >0.05).Conclusions The pain,sleeping influence and AROM of frozen shoulder patients are significantly improved after the treatment of ultrasound-guided glenohumeral joint injection combined with distension for continuous three times,which is an effective treatment for frozen shoulder.
Efficacy of Intra-articular Triamcinolone and Hyaluronic Acid in a Frozen Shoulder Rat Model

Abstract:The American Journal of Sports Medicine, Ahead of Print. Background:Intra-articular corticosteroid or hyaluronic acid (HA) is commonly prescribed for frozen shoulder. However, few studies have investigated histological and molecular changes after injection.Purpose:To compare the effectiveness of intra-articular injections of triamcinolone and HA in a frozen shoulder rat model and verify a greater effect of triamcinolone in passive shoulder abduction compared with HA.Study Design:Controlled laboratory study.Methods:Twenty male Sprague-Dawley rats were randomly allocated into 4 groups (n = 5 in each): control group, which did not receive cast immobilization or injection, and 3 experimental groups, which received 3 weeks of unilateral shoulder immobilization followed by intra-articular injections (normal saline, triamcinolone, or HA) at the immobilized shoulder. Passive shoulder abduction angle, histological and immunohistochemical staining, and Western blotting results were assessed 2 weeks after injection. The intensity and extent of staining were converted to semiquantitative scores for further analysis.Results:Shoulder abduction angles before sacrifice were 153.0°± 2.7° (control group), 107.0°± 5.7° (saline group), 139.0°± 9.6° (triamcinoline group), and 110.0°± 10.6° (HA group), showing significant differences between control and saline groups, control and HA groups, saline and triamcinoline groups, and triamcinoline and HA groups (P < .001) but not between control and triamcinoline groups (P = .053). Histologic evaluation revealed an increase in synovial folds and thickening of the capsular membrane in the saline and HA groups; this change was not evident in the triamcinolone group. A comparison of semiquantitative scores revealed greater expression levels of proteins involved in fibrosis and angiogenesis in the saline and HA groups compared with the control and triamcinolone groups. In Western blotting, the expression of inflammatory cytokines and the receptor for advanced glycation end products was significantly lower in the triamcinolone and HA groups than in the saline group.Conclusion:Triamcinolone injection was more effective than normal saline or HA injection in improving range of motion and reversing fibrotic and angiogenic features of frozen shoulder. Both triamcinolone and HA injections elicited anti-inflammatory effects.Clinical Relevance:The antifibrotic and antiangiogenic properties of triamcinolone and the anti-inflammatory properties of both triamcinolone and HA should be considered when performing injections in clinical settings.
