Multiple Recurrence Parathyroid Carcinoma One Case Report

Abstract:Multiple combined with recurrence parathyroid carcinoma is rarely reported in literature. Compared with solitary parathyroid carcinoma, it is no doubt that multiple combined with recurrence parathyroid carcinoma would increase the difficulty of diagnosis, increase misdiagnosis and the risk of reoperation. A 64 years old male patient was diagnosed of parathyroid carcinoma in our department in july 2005. The patient underwent surgery three times. He has three parathyroid carcinoma lesions, two on the right side of neck (one shuold be recurrence parathyroid carcinoma lesion), one on the left side. In order to improve the prognosis of patients, there are some treatment experience worth sharing. En-bloc resection shuold be recommended. Preoperative FNA is not recommended. Intraoperative frozen pathology has certain reference value, but the misdiagnosis rate remains high. For single cancer, we should perform tumor resection together with ipsilateral thyroid lobe resection, lymph node dissection, including thymic tongue and lymph nodes around the trachea. At the same time we should explore other parathyroid gland. We should not only attach importance to treat the primary disease, but also attach great importance to the complications. Postoperative monitoring of PTH and blood calcium concentration is necesary. If Postoperative PTH and blood calcium concentration remain high it suggest tumors have not been completely removed or exist other lesions, even exist distant metastasis and it has poor prognosis.
