Abstract:Objective: The aim of the current study was to investigate the effects of radioactive iodine-125 seed implantation on therapeutic efficacy, serum vascular endothelial growth factor (VEGF), carcinoembryonic antigen (CEA) levels, and prognosis of patients with ocular malignant tumors. Methods: A total of 86 patients with ocular malignant tumors were randomly allocated into the adjuvant chemotherapy group or iodine-125 seed implantation group, based on a random number table, with 43 patients in each group. The chemotherapy group underwent ocular tumor resections and routine postoperative adjuvant chemotherapy, while the iodine-125 group was treated with tumor resections and intraoperative iodine-125 seed implantation. At 3 months after surgery, after the chemotherapy group had received 2 cycles of chemotherapy, total effective rates were accessed in both groups. Serum CEA levels, before and after treatment, were detected by automatic electrochemiluminescence. Serum VEGF levels were also measured, before and after treatment, in both groups via ELISA. Correlation levels between CEA and VEGF were analyzed. Incidence rates of side effects, during treatment, were recorded and compared between the two groups. One-year progression-free survival and 3-year survival rates of the two groups were compared via regular follow-ups. Results: The total effective rate of the iodine-125 group was higher than that of chemotherapy group (P<0.05). In addition, expression levels of serum CEA and VEFG in the two groups, after treatment, were lower than those before treatment (all P<0.05). Decreases in the iodine-125 group were more significant than those in the chemotherapy group (both P<0.05). Serum CEA levels and VEGF levels were positively correlated. Incidence of side effects in the iodine-125 group was significantly lower than that in the chemotherapy group (P<0.05). Furthermore, 1-year progression-free survival and 3-year survival rates in the iodine-125 group were higher than those in the chemotherapy group. Differences were statistically significant (both P<0.05). Conclusion: Radioactive iodine-125 seed implantation in patients with ocular malignant tumors can effectively improve therapeutic efficacy, reduce serum levels of tumor markers CEA and VEGF, and improve 1-year progression-free survival and 3-year survival rates. This method shows a high level of safety. Thus, it is highly recommended for clinical practice.
Therapeutic Effect of CT-guided 125I Seed Implantation on Advanced Lung Cancer and Pulmonary Metastatic Carcinoma

Abstract:BACKGROUND:Primary lung cancer and metastatic lung cancer are common malignant tumors of the lung and are the main cause of cancer-related death. Advanced lung cancer and lung metastatic cancer are mainly treated by systemic therapy, and local treatment is also an effective treatment for the refractory or recurrent lesions in the lungs after systemic treatment. ¹²⁵I radioactive particle implantation, as an efficient conformal radiotherapy, has a certain control effect on localized lung cancer. The purpose of this study was to investigate the clinical efficacy and safety of computed tomography (CT) guided percutaneous puncture of lung tissue with ¹²⁵I radioactive particle implantation in the treatment of advanced lung cancer and lung metastatic cancer.METHODS:Continuouslycollectedthe clinical and pathological data of 105 patients with advanced lung cancer and metastatic lung cancer treated by ¹²⁵I radioactive seed implantation in the Department of Thoracic Surgery, First Affiliated Hospital of Zhejiang University School of Medicine from January 1, 2014 to November 30, 2018. The patients were followed until March, 2019. The clinical efficacy and complications of seedimplantation were analyzed.RESULTS:A total of 105 patients were included in the study, 78 patients with advanced lung cancer and 27 patients with lung metastases. The median survival time after seed therapy was 395 days. The 1-year survival rate was about 78.1%, and the 2-year survival rate was about 56.1%. Seed implantation for advanced lung cancer is equivalent to lung metastasis. Seed combined with radiofrequency ablation, microwave ablation, and chemotherapy did not improve seed treatment. However, particle combined with external radiation therapy has a significant survival disadvantage compared with simple seed therapy.CONCLUSIONS:The CT-guided ¹²⁵I radioactive seed implantation has controllable complications and can be used as a safe and effective treatment for advanced lung cancer and lung metastases.

Abstract:Objective:To investigate the value of CT-guide interstitial iodine-125 seed implantation for metastatic spine tumor.Method:14 patients with metastatic spine tumor,who did not have surgery,chemotherapy or external beam radiotherapy,underwent CT-guide interstitial iodine-125 seed implantation.Three-dimensional treatment system was used to determine the implantation plan.Different needles placement(0.5-1.0cm apart),number and distribution of seeds were determined according to the images,tumor sizes and sensitivity to radioactive rays.The mean number of seeds was 72(range,26-145),and the mean radioactive activity was 0.70mCi(range,0.50-0.80mCi).Three-dimensional treatment system was used for dose validation.Result:The mean follow-up was 12.0 months(range,1 to 75 months).The local control rate was 50.0%.The mean local control time and survival time was 26.0 months and 11.0 months repectively.The 1-year,2-year and 3-year local controls were 62.5%,50.0% and 37.5% respectively.The 1-year,2-year and 3-year survival rates were 50.0%,41.7% and 20.8% respectively.Preservation and recovery ratio of meurofunction were 92.9% and 71.4% respectively.Effect and complete pain relief ratio were 78.6% and 64.3% respectively.Conclusion:CT-guide interstitial iodine-125 seed implantation is minimally invasive and less-complication for spine metastatic tumor,which can be used as an alternative to convention surgery.

[Short- to Mid-Term Evaluation of CT-guided 125I Brachytherapy on Recurrent or Metastatic Head and Neck Cancers].

Abstract:OBJECTIVE to evaluate the short- and medium-term clinical effects of 125I seed implantation on recurrent or metastatic head and neck cancers. METHODS Thirty patients with recurrent or metastatic head and neck cancers after operation, radiotherapy, or chemotherapy, totaling 421 lesions 4.2 (2-9) cm in diameter, 23 males and 12 females, aged 56 (39-71), underwent implantation of 12-70 125I seeds (on average 33 per person) under the guidance of CT, ultrasonography, or endoscopy with an interval of 1 cm between any 2 seeds with the radioactive activity per seed of 29.6 MBq and matched peripheral dose of 90-160 Gy. Follow-up was conducted for 13 (4-40) months to observe the local control rate, overall survival rate, pain relief, and clinical complications. RESULTS Follow-up 4 months later showed that 24 of the 42 lesions obtained complete remission, 11 obtained partial remission, 5 no change, and 2 progress of disease, with a clinical response rate of 83.3% (CR+PR). The overall 1-, 2-, and 3-year survival rates were 88.4%, 72.4%, and 45.2% respectively with a median survival time of 31 months. The pain relief rate was 73.4% (17/23). The long-term complications included hyperpigmentation at operative sites (n=4), insensible feeling on lateral cheek (n=3), dryness of oral cavity (n=2), and headache combined with infection (n=1). CONCLUSION Relieving the pain, improving the life quality, CT guided radioactive 125I seed implantation is a simple, safe, and effective method in treating recurrent or metastatic head and neck cancer with minimal damage and few complications.
Clinical Efficacy of CT-guided Iodine-125 Seed Implantation for Recurrent Head and Neck Carcinoma after External Beam Radiotherapy

Abstract:Objective To evaluate the clinical efficacy and safety of CT-guided 125 I seed implantation in the treatment of recurrent head and neck tumors after radiotherapy. Methods Clinical data of 101 patients who received CT-guided radioactive 125 I seed implantation for recurrent head and neck cancer after radiotherapy from 2007 to 2015 were retrospectively analyzed. The median accumulated dose of external radiotherapy was 66 Gy and the median dose after seed implantation (D90) was 117 Gy. The local progression-free survival and overall survival were analyzed by Kaplan-Meier method. Univariate analysis was performed by log-rank test and multivariate analysis was conducted by using Cox regression model. Results The median follow-up time was 12. 2 months. The 5-year local progression-free survival rate was 26. 6％.The 5-year overall survival rate was 15. 5％. Univariate analysis demonstrated that age,pathological type,implantation site,lesion volume,D90 and short-term efficacy were correlated with local control,and KPS score,lesion volume ,D90,and short-term efficacy were associated with survival (all P<0. 05). Multivariate analysis revealed that pathological type,lesion volume ,D90 and short-term efficacy were independent factors related to local control (P= 0. 000, 0. 002,0. 003 and 0. 014).In terms of the adverse events,skin/ mucosal ulceration was observed in 25. 7％ and pain occurred in 13. 9％ of all patients. No correlation was noted between the adverse events and dose.Conclusions CT-guided radioactive 125 I seed implantation is an efficacious and safe treatment of recurrent head and neck tumors after radiotherapy. Non-squamous carcinoma,small lesion volume and high dose (D90) indicate excellent local control.
CT-guided 125 I Seed Brachytherapy for Abdominal Metastatic Lymph Nodes

Abstract:Objective To investigate the safety and clinical efficacy of CT-guided radioactive seed 125Ⅰ implantation in treating abdominal and retroperitoneal metastatic lymph nodes.Methods Twenty-seven patients with pathologically proved abdominal malignancy(17 males and 10 females) were enrolled in this study.The primary tumors included liver cancer(n = 13),pancreatic cancer(n = 8) and gastric cancer(n = 6).A total of 31 abdominal and peritoneal metastatic lymph nodes(diameter of 2.2-4.7 cm) were detected and CT-guided radioactive seed 125Ⅰ implantation was carried out.Halarism 125Ⅰ experience formula was used to design the distribution of seed 125Ⅰ.Under CT guidance,125Ⅰ seeds were implanted into the metastatic lymph nodes at a distance interval of 1.0-1.5 cm.After implantation,CT scanning was immediately performed to check the distribution of 125Ⅰ seeds and to observe the possible complications.CT or PET-CT re-examination was performed at 3 months after 125Ⅰ seed implantation.Results Of the 27 patients,125Ⅰ seed implantation was successfully accomplished in 25(92.6%) at the first time and the particle distribution met the radioactive requirements well.In the remaining 2 patients the particle distribution did not meet the requirements and replacement of 125Ⅰ seed had to be made.In this group,8 patients had severe pain before treatment and in six of them the pain was markedly relieved at 1-3 weeks after seed 125Ⅰ implantation,with a response rate of 75.0%.The patients were followed up for three months and all the 27 patients survived.According to the results of CT or PET-CT re-examination,12 of 27 patients(44.4%) achieved complete remission(CR),8(29.6%) achieved partial remission(PR),4(14.8%) showed no change(NC) and 3(11.1%) had progressive disease(PD).The overall response rate was 74.1%(20/27).No severe complications,such as acute pain,massive bleeding,infection and bone marrow suppression,occurred.Conclusion For the treatment of abdominal and retroperitoneal metastatic lymph nodes CT-guided interstitial implantation of radioactive seed 125Ⅰ is effective,minimal invasive and safe with few complications.

Abstract:Objective To preliminarily evaluate the feasibility and therapeutic effect of 125 I seed implantation under computed tomography (CT) guidance in the treatment of recurrent or metastatic chest wall tumor.Methods A retrospective analysis was performed on the clinical data of 23 patients with recurrent or metastatic malignant chest wall tumor who underwent 125I seed implantation under CT guidance from July 2004 to October 2012.A computerized treatment planning system was used for three-dimensional treatment planning before operation to determine the number and spatial distribution of 125I seeds.CT scan dosimetry was performed immediately after operation.The dose received by 90％ of the target volume was 100-160 Gy (median:130 Gy).The activity of 125I seeds was (1.85-2.89) × 107 Bq (median:2.63 × 107 Bq).The total number of seeds implanted was 8-269 (median:53).The local control rate and survival rate were calculated by the Kaplan-Meier method.Results The follow-up rate was 100％.Two patients were followed up for at least 3 years,and 2 for at least 5 years.Of the 23 patients,4 had a complete remission,and 12 had a partial remission.The 1-,3-,and 5-year local control rates were 82％,62％,and 62％,respectively; the 1-,3-,and 5-year tumor-specific survival rates were 61％,45％,and 45％,respectively;the 1-,3-,and 5-year overall survival rates were 58％,58％,and 39％,respectively.After treatment,mild or moderate brachial plexus injury was seen in one patient,and grade 1 or 2 skin injury was seen in 6 patients.Conclusions 125I seed implantation under CT guidance is safe and effective in the treatment of recurrent or metastatic chest wall tumor,according to the preliminary study.
Co-planar Template Assisted CT Guidance on the Treatment of Spinal Metastases with 125Ⅰ Seed Implantation

Abstract:Objective To explore the quality control and the short-term curative efficacy of 125Ⅰ radioactive seeds implantation in the treatment of spinal metastases by using co-planar template ( CPT ) assisted CT guidance. Methods Totally 12 cases of primary tumor were diagnosed by pathology, imaging changes for 16 lesions were consistent with the characteristics of spinal metastatic tumor. The prescription dose (PD) was 80 Gy, seed activity was 1. 48 × 107 -2. 59 × 107Bq(0. 4 -0. 7 mCi). According to preoperative plan, spinal metastases were treated with CPT assisted CT guided 125Ⅰ radioactive seeds implantation. The distribution of seeds was observed immediately after operation and the dose was assessed. Patients were followed up by CT to determine the change of tumor diameter and evaluate the efficacy. Follow-up time ranged from 3 to 29 months. And the improvement of pain was evaluated by pain grading. Results All of the 16 spinal metastases lesions were successfully implanted by the preoperative planning. After the implantation, quality verification showed the average dose of target region was (209. 21 ± 37. 16) Gy, D90 ( 115. 29 ± 7. 87 ) Gy, D100 ( 76. 59 ± 5. 53 ) Gy, V90 ( 99. 30 ± 0. 51 )%, V100 ( 98. 06 ± 1. 15 )%, conformal index (CI) 0. 981 ± 0. 012, external index (EI) 0. 012 ± 0. 007. And the average dose of the spinal cord was ( 30. 47 ± 4. 83 ) Gy. There was no significant difference in the mean dose between the target area and the spinal cord and the preoperative plan ( P>0. 05 ) . 3 months after surgery, among 16 spinal metastases lesions, complete response ( CR ) was 18. 8% ( 3/16 ) , PR ( partial response ) rate 62. 5% (10/16), PD(progressive disease) rate 6. 25%(1/16),SD(stable disease) rate 6. 25%(1/16), effective rate ( CR + PR ) 81. 3%. The were 3 cases in pain complete remission, 7 cases in partial remission, 2 cases in mild remission. The survival time range from 11 to 39 months. The median survival time was 24 months. No radiation damage of spinal cord was found. Conclusions CT guided CPT can be used to control the position and radiation dose of the seeds implantation before the operation, so as to achieve the objective of treating tumor with fewer complications and improvement of patients′tolerance.
CT-guided Transcutaneous Interstitial Implantation of 125I Seeds for Cervical Lymph Nodes Metastasis in Oral Carcinoma

Abstract:Objective To evaluate the technic,result and complications of interstitial implantation of I-125 seeds for cervical lymph nodes metastasis in oral carcinoma guided by CT aided with three dimensional treatment planning system(TPS).Methods Ten patients with cervical lymph nodes metastasis in oral carcinoma were treated with interstitial implantation of 125I seeds guided by CT with TPS monitored immediately by CT after implantation.Another CT scanning was repeated after 2 months.Results 10 cases were followed up for 12～28 months with an average of 19 months.1 patient with cervical recurrence of oral carcinoma was treated only by seed implantation,the tumor size reduced 50%.In the other 9 patients who were implanted after operation,no recurrence was seen in the target area.There were no extreme raditherapeutic side effects except for skin hyperemia occurred in 5 patients and angina occurred in 3 patients.Conclusion CT guided transcutaneous interstitial implantation of 125I seeds is an accurate,safe and valid procedure.
CT-guided Radioactive 125 I Seed Implantation for Abdominal Incision Metastases of Colorectal Cancer: Safety and Efficacy in 17 Patients

Abstract:Purpose To retrospectively evaluate the safety and efficacy of computed tomography (CT)-guided iodine-125 ( 125 I) seed implantation for patients with abdominal incision metastases from colorectal cancer . Materials and Methods Data of patients with abdominal incision metastases of colorectal cancer from November 2010 to October 2020 were retrospectively reviewed. Each incisional metastasis was percutaneously treated with 125 I seed implantation under CT guidance. Follow-up contrast-enhanced CT was reviewed, and the outcomes were evaluated in terms of objective response rate, complications, and overall survival . Results A total of 17 patients were enrolled in this study. The median follow-up was 18 months (range, 2.7–22.1 months). At 3, 6, 12, and 18 months after the treatment, objective response rate was 52.9%, 63.6%, 33.3%, and 0%, respectively. A small amount of local hematoma occurred in two patients and resolved spontaneously without any treatment. Two patients experienced a minor displacement of radioactive seeds with no related symptoms. Severe complications, such as massive bleeding and radiation injury, were not observed. No ≥ grade 3 adverse events were identified. By the end of follow-up, 14 patients died of multiple hematogenous metastases. The one-year overall survival rate was 41.6%, and the median overall survival was 8.6 months . Conclusion CT-guided 125 I seed implantation brachytherapy is safe and feasible for patients with abdominal incision metastases from colorectal cancer. Micro Background Colorectal cancer (CRC) recurrence in abdominal incisions is rare and challenging. Patients and Methods A total of 17 patients with abdominal incision metastases from CRC, who received 125 I seed implantation, were enrolled and the data of patients were retrospectively reviewed. Results All the patients successfully treated with 125 I seed implantation and no ≥ grade 3 adverse events were identified. Conclusion CT-guided 125 I seed implantation is safe and feasible in the treatment of patients with abdominal incision metastases from CRC.

Abstract:OBJECTIVE:To detect the methods and experience of CT guided interstitial mj seed implantation treatment for head and neck malignant tumor.METHODS:5 patients with head and neck malignant tumor were treated by interstitial 125I seed implantation with CT guide, 3 of them were treated with operation and adjunct seeds implantation, the other two patients were implanted seed only. According the treatment plan, register needles were punctured firstly, based on CT scan, these needle' s position and depth were adjusted, then the seeds were implanted into the target area. Another CT scan was done for the dose verification.RESULTS:The procedure of 125I seeds implantation of five patients was successful. 5 of twelve register needles were adjusted. The dose distribution of the instant verification was the same as that of treatment plan.CONCLUSION:The CT guided seeds implantation of the head neck malignant tumors can practice the treatment plan accurately, and the dose verification can be done instantly.
CT-guided 125I Seed Implantation in the Treatment of Malignant Pulmonary Metastases

Abstract:Objective To evaluate the clinical outcomes and the prognostic factors in patients with pulmonary metastases treated with CT-guided radioactive 125I seed implantation(CTRISI).Methods A total of 50 patients were treated with implantation of 125I radioactive seeds under computed tomography (CT)guidance or combined with surgical resection.There were 10(20％)patients with colorectal cancer,8(16％)patients with malignant fibrous histiocytoma,5(10％)patients with sarcoma,22 (44％)patients with other cancer,respectively.CTRISI was the only treatment in 45 patients(90％)and combined with surgical resection in 5(10％).The local control rate,the survival rate and the factor associated with overall survival were evaluated.Results The D90％ was(112.3 ± 12.2)Gy,the V90 was 91.3％ ± 8.2％,and the postoperative matched peripheral dose(MPD)was 106.2 Gy.The local control rate for 1-,2-,3-,4-,5-year were 95.8％,86.5％,56.6％,42.4％,31.1％,respectively.At a mean follow-up of 26 months(7-74 months),6 patients were alive.The median survival time was 27.1 months.The survival rate for 1-,2-,3-,4-,5-year were 83.4％,52.3％,38.7％,20.3％,13.7％,respectively.Lesion size ≤ 4 cm or ＞ 4 cm was an important prognostic factor associated with overall survival(x2=4.621,P＜0.05)and disease-free survival(x2=7.548,P＜0.05).Conclusions CTRISI is safe in these patients with pulmonary metastases,with promising results.Surgical resection remains the standard for resectable patients,while CTRISI alone or combined with surgery might offer an alternative in selected patients.

Abstract:The management of inoperable lung cancer remains a challenge. It has been proven that computed tomography (CT)-guided iodine-125 (125I) seed implantation is a safe and efficient method for treating lung cancer. Computed tomographic fluoroscopy (CTF) is superior to traditional CT for percutaneous management of lung lesions, due to the real-time guidance and accurate localization of the lesions. The aim of the present prospective study was to evaluate the feasibility, safety and efficacy of CTF-guided percutaneous permanent implantation of 125I seeds for the treatment of selected patients with inoperable stage T1-3N0M0 non-small-cell lung cancer (NSCLC). A total of 24 patients with resectable but inoperable stage T1-3N0 NSCLC, with a total of 28 lesions, underwent CTF-guided percutaneous implantation of radioactive 125I seeds. A prescription dose of 100-120 Gy was delivered to each lesion. The complications and local tumor control rates were documented. Survival was estimated using the Kaplan-Meier method. All the patients successfully completed the procedure, with a mean procedure duration of 45.7 min (range, 30-75 min). No severe complications occurred. Small asymptomatic pneumothorax with lung volume compression of <10% and minor hemorrhage along the needle track without hemoptysis occurred immediately after the procedure in 3 (12.5%) and 4 (16.7%) of the 24 patients, respectively. At a median follow-up of 31.5 months (range, 8-46 months), the local control rate (LCR) of the lesions was 78.6% (22/28). The 1-, 2- and 3-year overall survival rate was 95.8, 78 and 55%, respectively. In conclusion, CTF is the favourable imaging guidance method for the percutaneous implantation of 125I seeds. CTF-guided brachytherapy with implantation of 125I seeds is a safe, feasible and effective modality for the treatment of inoperable early-stage NSCLC and may be considered an alternative option in selected patients with medically inoperable NSCLC.

Abstract:Purpose The present study aimed to explore the safety profile and clinical efficacy of CT-guided radioactive seed implantation in treating local recurrent rectal carcinoma. Materials and methods CT-guided 125 I seed implantation was carried out in 20 patients with locally recurrent rectal carcinoma. 14 of the 20 patient had prior adjuvant external-beam radiation therapy (EBRT). The treatment planning system (TPS) was used preoperatively to reconstruct three dimensional images of the tumor and to calculate the estimated seed number and distribution. The median matched peripheral dose (MPD) was 120 Gy (range, 100-160 Gy). Results Of the 20 patients, 12 were male, 8 were female, and ages ranged from 38 to 78, with a median age of 62. Duration of follow-up was 3-34 months. The response rate of pain relief was 85% (17/20). Repeat CT scan 2 months following the procedure revealed complete response (CR) of the tumor in 2 patients, partial response (PR) in 13 patients, stable disease (SD) in 3 patients, and progressive disease (PD) in 2 patients. 75% of patients had either CR or PR. Median survival time was 18.8 months (95% CI: 3.5-22.4 months). 1 and 2 year survival rates were 75% and 25%, respectively. 4 patients died of recurrent tumor; 4 patients died of distant metastases; 9 patients died of recurrent tumor and distant metastases. 3 patients survived after 2 year follow up. Two patients were found to have mild hematochezia, which was reversible with symptomatic management. Conclusion CT-guided 125 I seed implantation appeared to be a safe, useful and less complicated interventional treatment option for local recurrent rectal carcinoma.

Abstract:The aim of this study is to evaluate the feasibility, safety, and clinical efficacy of CT-guided 125I seed interstitial brachytherapy in patients with recurrent spinal metastases after external beam radiotherapy (EBRT). Between August 2003 and September 2015, 26 spinal metastatic lesions (24 patients) were reirradiated by this salvage therapy modality. Treatment for all patients was preplanned using a three-dimensional treatment planning system 3–5 days before 125I seed interstitial brachytherapy; dosimetry verification was performed immediately after seed implantation. Median actual D90 was 99 Gy (range, 90–176), and spinal cord median Dmax was 39 Gy (range, 6–110). Median local control (LC) was 12 months (95% CI: 7.0–17.0). The 6- and 12-month LC rates were 52% and 40%, respectively. Median overall survival (OS) was 11 months (95% CI: 7.7–14.3); 6-month and 1-, 2-, and 3-year OS rates were 65%, 37%, 14%, and 9%, respectively. Pain-free survival ranged from 2 to 42 months (median, 6; 95% CI: 4.6–7.4). Treatment was well-tolerated, with no radiation-induced vertebral compression fractures or myelopathy reported. Reirradiation with CT-guided 125I seed interstitial brachytherapy appears to be feasible, safe, and effective as pain relief or salvage treatment for patients with recurrent spinal metastases after EBRT.

Abstract:Objective To investigate the feasibility and short-term efficacy of CT-guided interstitial implantation of 125I seeds for lung cancer. Methods Under local anesthesia, 16 patients with lung cancer were treated with CT-guided interstitial implantation of 125I seeds. The TPS was used to calculate the number of seeds, the space distribution, and the number of needles. The MPD of 125I seed implantation were 90-110 Gy for patients who had ever received external radiotherapy and 140-160 Gy for those who had never received external irradiation. The PTV included CTV plus 1 cm margin. The activity of each seed was 0.40-0.70 mCi. A median of 38 (range, 15-145) 125I seeds were implanted, and CT scan were performed after the operation to evaluate the quality of the operation. Three of the patients received 3D-CRT external beam radiation therapy at a total dose of 45-50 Gy. Results The patients were followed up for a median of 14 months (6-47 months), during which none of them had serious operation-related complications. All patients could well tolerated seed implantation. The 1-and 2-year local control rates were 93.8% (15/16) and 93.8% (15/16), respectively. The median survival time was 14 months (6-24 months),and the 1-and 2-year survival rates were 62.5% (10/16) and 37.5% (6/16), respectively. Among the patients, 6 cases died of systemic metastasis, 3 died of cachexia, and 1 died of cerebral hemorrhage. Conclusions CT-guided interstitial implantation of 125I seeds is a safe and minimally invasive method for lung cancer with a low rate of complications and good outcomes. The efficacy of the treatment might be improved by combining with external radiotherapy and systemic chemotherapy.

Abstract:Objective To investigate the feasibility and short-term efficacy of CT-guided implantation of interstitial 125I seed in the treatment of recurrent head and neck tumor.Methods Since December 2006 to December 2009,we performed CT-guided implantation of interstitial 125I seed on 16 patients with recurrent head and neck tumor.Before the implantation,TPS was employed to confirm the seed number and needle placement.Different puncture approach was carried out according to CT images.The seeds(0.50-0.80 mCi)were implanted at an interval of 0.5-1.0 cm.To observe the distribution of the seeds,postoperative CT-scan was carried out and then evaluation by TPS was made with the MPD ranged from 106-218 Gy.Results The procedure was completed in all the 16 cases with 17-158 seeds implanted(median,44)in each patient.The patients were followed up for 3 to 36 months(median,11.5 months);during the period we noticed a local effective rate of 68.8%(11/16),median local-controlled time of 18.0 months,and mean survival time of 25.9 months.The 1-,2-,and 3-year local-controlled rate was 67.7%,33.9%,and 33.9%,respectively.And 1-,2-,and 3-year survival rate was 76.7%,57.5%,and 57.5%.Conclusion CT-guided implantation of interstitial 125I seed is accurately controllable,minimally invasive,safe,and effective in a short term.
