The Dosimetric Impacts of Vertebral Implants (pedicle Screw And/or Bone Cement) on Spinal Cord

Abstract:The aim of this study is to investigate the dosimetric impacts of vertebral implants on spinal cord. An entire lumbar spine specimen was used for this study. The vertebrae were implanted with pedicle screws and/or bone cement according to rational clinical method, and divided into 3 groups: A) pedicle screw; B) bone cement; and C) pedicle screw combined with bone cement. Then, the vertebrae were immobilized and placed in the water tank for CT simulation. After that, a virtual vertebral tumor and the spinal cord were contoured for treatment planning. Two 5-fields conformal treatment plans were designed with fraction dose of 2Gy and 3Gy, respectively. The planned doses of interest points (selected as the center of vertebral spinal cord in each group) were calculated by treatment planning system. To verify the accuracy of the planned dose, a semi-conductor in-vivo dose detector was put into the vertebral spinal cord, at the same position as the interest point (before measuring, kV-CBCT was used to assure the consistency of position between the interest point and the measuring point). After delivering the treatment plans, the measured doses were obtained from the in-vivo dose measuring system. To analyze the dosimetric impacts of vertebral implants, the measured doses of the interest points were compared with the planned doses for each of the three groups. The results were shown in the table. For 2Gy/f treatment plan, the measured dose was 3.26%, 5.67% and 6.56% greater than the planned dose in the Group A, B and C, respectively. The increased rate was 3.59%, 5.80% and 6.23% respectively, for 3Gy/f treatment plan. The preliminary results of our study show that the actual delivered dose of spinal cord is much higher than the planned dose for the vertebrae with pedicle screw and/or bone cement. Consequently, the dose limitation of spinal cord should be paid attention to for the patient with vertebral implants.
