Abstract:The polycystic ovary syndrome(PCOS)is one of the most common anovulatory infertility among reproductive-age women.Hyperandrogenism,insulin resistance,ovarian dysfunction and obesity are features of this disease.Metformin,as a representative of the insulin sensitizing agent,can improve the clinical symptoms and endocrine indicators of PCOS patients to a certain extent,and is applied in clinics extensively,but there are some disputes on the clinical effects of metformin in patients with PCOS,to which this article aims to state.

Abstract:Polycystic ovary syndrome (PCOS) is a common and heterogeneous disorder of women of reproductive age, characterized by hyperandrogenism and chronic anovulation. Metformin may lower the androgen level in follicular fluid, affecting local levels of IGF (Insulin like growth factor)s, and may produce improved ovarian stimulation. Metformin is used in PCOS cases in order to decrease plasma luteinizing hormone (LH), hyperinsulinemia, and ovarian androgen levels. Metformin increases the ovulatory rate, cervical scores and pregnancy rate. This review article focuses on the combined effect of metformin administration in PCOS patients.

Abstract:The polycystic ovary syndrome (PCOS) is characterized by increased secretion of LH, insulin and androgens. The main clinical complaints of women with PCOS include: oligo- or amenorrhea, dysfunctional uterine bleeding, hirsutism, obesity and/or anovulatory infertility. The first line treatment for these problems include: 1) estrogen-progestin therapy for oligomenorrhea and dysfunctional uterine bleeding; 2) estrogen-progestin therapy and/or antiandrogens for hirsutism; 3) lifestyle changes such as diet and exercise for obesity and 4) weight loss or clomiphene for anovulatory infertility. However, clinical trials have indicated that metformin is effective second line therapy when first line therapy has not been effective, is not acceptable to the patient or is medically contraindicated. The addition of metformin to the armamentarium of the gynecological endocrinologist represents an important advance.

Abstract:Objective: This study aimed at elucidating the therapeutic effectiveness of metformin in polycystic ovary syndrome (PCOS) patients with normal clinical indices of insulin sensitivity. Methods: This study was a retrospective cohort study. Based on their homeostasis model of assessment of insulin resistance (HOMA-IR) results, PCOS patients (n = 113) were allocated to either the IR Group (IR, HOMA-IR >2.69) or the non-IR Group (NIR, HOMA-IR ≤2.69). Metformin was administered to all patients, and their medical history, menstrual status, and endocrine and metabolic characteristics were obtained before and after treatment. The one-way analysis of variance was used to determine the differences between the IR and NIR groups. The paired t-test was used to determine the differences between pre- and posttreatment results. Results: Analyses of baseline characteristics showed that the incidence of IR and glycolipid metabolism is higher within elder PCOS patients. Metformin improved menstrual cycle and ovulation rate in NIR patients, similar to those in IR patients. In the NIR group, metformin decreased luteinizing hormone (LH) levels, free androgen index (FAI) levels, body mass index, and waist circumference, but had no obvious effect on glucose, insulin, and lipid profiles. Whereas in the IR group, metformin not only decreased LH and FAI levels, but also improved glycolipid metabolism. Conclusions: The effect of metformin on PCOS patients with normal insulin sensitivity index suggests that the mechanism of menstrual improvement may not be related to IR. In addition, metformin can improve glycolipid metabolism. Therefore, metformin is suitable for long-term treatment of PCOS and should be more widely used.

Abstract:Objective To explore the efficacy of metformin for improving the endocrine and reproductive function in polycystic ovary syndrome(PCOS) patients with obese and without obese. Methods Eighty four infertility patients with PCOS(48 cases with obese and 36 cases without obese) were treated with metformin at a dose of 1 500 mg daily for 3 menstrual cycles. The clinical and endocrine changes were compared before and after treatment. The ovulation rate and pregnancy rate of subjects treated by metformin were recorded, and compared with the ovulation rate and pregnancy rate of 35 PCOS cases(18 cases with obese and 17cases without obese) treated by clomiphene citrate(CC). Results After treatment of metformin for three menstrual cycles, the BMI,WHR, total testosterone and fast insulin and luteinizing hormone(LH)/follicle stimulate hormone(FSH) in PCOS patients with obese decreased significantly(P<0.05 or P<0.01), but the decreases of those indexes in PCOS patients without obese were not obvious(P>0.05). After treatment for three menstrual cycles, although the cumulative ovulation rate in metformin group was higher than that in CC group, there was no significant difference(P>0.05). The cumulative pregnancy rates in metformin group with obese and CC group with obese were 54.17%(26/48) and 27.78%(5/18), respectively; the cumulative pregnancy rates in metformin group without obese and CC group without obese were 58.33%(21/36) and 29.41%(5/17), respectively. The cumulative pregnancy rate in metformin group was significantly higher than that in CC group(P<0.05). Conclusion Metformin could improve the endocrine condition,boost the ovulation and increase the pregnancy rates in PCOS patients.

Abstract:PurposesPolycystic ovary syndrome (PCOS) is a major cause of female infertility, being present in up to 20% of women of childbearing age. Insulin resistance (IR) plays an important role in the pathophysiology of PCOS; therefore, its treatment may benefit women with the syndrome. The main drug used for IR management is metformin (MT). We aim to review the literature on the use of metformin in women with PCOS.MethodsUsing the terms “metformin” and “polycystic ovary syndrome,” we conducted a search the PubMed, EMBASE, and Google Scholar databases. The research was restricted to articles published in English. Initially, only published meta-analyses were included, in the absence of meta-analyzes, RCT and well-designed prospective studies were used.ResultsMetformin increases success rates and decreases complication rates when used as an adjunctive medication for ovulation induction during low complexity assisted reproduction treatments and during ovarian stimulation for in vitro fertilization in women with PCOS. Evidence about the effect of metformin on fetal and obstetric complication rates is conflicting. Metformin is associated with high incidence of gastrointestinal symptoms; however, serious adverse effects are rare and there is no evidence of teratogenicity.ConclusionFor women with PCOS, metformin is a good adjunctive medication for ovulation induction/stimulation for high and low complexity assisted reproduction therapies. The adverse effects are mostly mild, and there is no risk of teratogenicity, but the risk of long-term complications for the offspring is not yet defined. High heterogeneity of the studies limits extrapolation of findings, and further research is needed to determine which women will benefit most from the medication.

Efficacy of metformin combined with vitamin D in the treatment of polycystic ovarian syndrome: A meta-analysis

Abstract:Polycystic ovary syndrome (PCOS), as an endocrine and metabolic disorder, affects approximately 6% -20% of women of childbearing age. This study aims to assess the therapeutic effects of Metformin combined with vitamin D in PCOS patients. Eight databases were searched to obtain randomized controlled trials, both domestically and internationally, on the effects of Metformin combined with vitamin D in patients with PCOS. Data analysis was performed using RevMan 5.3 software. Nine studies were ultimately included in this meta-analysis. Six studies reported the homeostatic model assessment for insulin resistance of the test group and the control group, which was significantly lower (SMD: -0.23; 95% Cl: -0.42,-0.04; P<0.05) than the control group, body mass index (BMI) (SMD: -1.86; 95% Cl: -2.77,-0.96; P<0.01), Serum 25 (OH) D (SMD: 14.28; 95% Cl: 12.26,16.29; P<0.01), testosterone (SMD: -0.11; 95% Cl: -0.15,-0.07; P<0.01) and regulated menstrual cycles (OR: 1.27; 95% Cl: 0.99,1.63; P=0.063). Our meta-analysis of nine trials demonstrates significant reductions in insulin resistance, BMI, and testosterone levels, along with increased serum vitamin D levels and improved menstrual cycle regulation after Metformin and vitamin D treatment. These findings suggest the potential of this combined therapy in managing the multifaceted aspects of PCOS.
Efficacy of Metformin for Polycystic Ovary Syndrome:Evidence-based Evaluation

Abstract:OBJECTIVE:To evaluate the efficacy of metformin in the treatment of polycystic ovary syndrome (PCOS)-induced infertility. METHODS:Literature analysis and interview on doctors based on evidence-based medicine were performed. The databases of controlled clinical trials including CMB,CNKI,MEDLINE and EMbase and Cochrane library were retrieved to collect clinical research documents on the efficacy of metformin in the treatment of polycystic ovary syndrome (PCOS)-induced infertility. RESULTS:Of the pertinent literature collected about the using metformin for treatment of PCOS,four were systematic reviews (SR) and five were randomized controlled trials (RCTs). The quality of five RCTs were evaluated according to Jadad scale,of which three scored 5 points,one 4 points,and one 3 points. Of the 4 SR papers,2 supported that metformin was effective for ovulation rate and pregnancy rate in patients with PCOS,1 supported that clomiphene citrate was more useful than the metformin for PCOS,and 1 supported that further RCT remain to be done to confirm the clinical efficacy of metformin for polycystic ovary syndrome (PCOS)-induced infertility. CONCLUSION:It has been demonstrated in research that Metformin is effective for polycystic ovary syndrome (PCOS)-induced infertility,however,there is limitation in protocol of the research,and well-designed larger sample study is necessary.

Secondary outcomes included pregnancy-induced hypertension (PIH), intrauterine growth restriction (IUGR), fetal malformation, vaginal delivery (VD), cesarean section (CS), and metformin's side effects, such as nausea or gastrointestinal discomfort. Certainly, data about neonatal death and macrosomia were analyzed if data available. Results: Finally, 13 studies including 5 randomized controlled trials (RCT) and 8 cohort studies involving 1606 pregnant women with PCOS were analyzed. The pooled OR of EPL was 0.19 with obvious statistical significance, manifesting that metformin help to lower the rate of EPL (95% CI 0.12–0.28, P < 0.00001). Simultaneously, metformin showed the advantage of reducing the prevalence of preterm delivery (OR 0.37, 95% CI 0.20–0.68, P = 0.002). In addition, metformin could promote term delivery greatly and the pooled OR was 5.23 with sharp statistical difference (95% CI 3.12–8.75, P < 0.00001). Conclusion: Metformin treatment in women with PCOS throughout pregnancy could increase the possibility of term delivery, VD and reduce the risk of EPL, preterm labor, pregnancy complications such as GDM and PIH, with no serious side effects. Moreover, metformin was not teratogenic based on the limited data. So we may recommend metformin treatment for women with PCOS during the whole pregnancy period for it is quite beneficial and safe for both mothers and babies.

Effect of Metformin During Early Pregnancy on Pregnancy Outcome of Women with Polycystic Ovarian Syndrome

Abstract:Objective To investigate the effect of metformin used during early pregnancy on pregnancy outcome of pregnant women with polycystic ovary syndrome (PCOS).Methods Two hundred and fifteen pregnant women with PCOS history were selected into our investigation from March 2007 to February 2010,among which,58 patients were taken as metformin group as they had taken metformin during early pregnancy,or with impaired glucose tolerance complicated by marked hyperinsulinemia and history of recurrent miscarriage.All patients in the study group received metformin (1.0～1.5 g/d) during early pregnancy.The rest 157 gravidas with PCOS history did not receive metformin as control group.The outcomes of pregnancy in the two groups were compared with x2 or t test.Results The early spontaneous miscarriage rate of metformin group was significantly lower than that of control group (5.2 ％ vs 28.7 ％,x2 =13.476,P＜0.01).There was no statistical difference in the rate of late abortion (6.90％ vs 6.4 ％ ),hypertension during pregnancy (3.9％ vs 4.9％),preeclampsia (3.9％ vs 3.9％),preterm birth (13.7％ vs 20.6％) and oligohydramnios (5.9％ vs 3.0％) between control group and metformin group (P ＞ 0.05 respectively).And there was no significant difference in neonatal birth weights [(2925±530) g vs (2910± 659) g],the rate of asphyxia (3.7％ vs 6.8％),malformation (0.0％ vs 0.9％),hypoglycaemia ( 5.6％ vs 6.8％),fetal macrosomia ( 1.9％ vs 3.4％),fetal growth restriction (5.6％ vs 6.0％) and rate of neonatal intensive care unit admission (18.0％ vs 27.4％) between the two groups (P＞0.05,respectively).The infants were followed up for 3 years and all are physically and mentally normal.Conclusions Metformin administration during early pregnancy might reduce the early spontaneous miscarriage rate of PCOS patients,and the adverse effects of metformin have not been identified on fetals and infants.

Abstract:Purpose Metformin is widely used as an insulin sensitizer in polycystic ovary syndrome (PCOS) patients. However, previous studies have found that the effect of metformin on the level of homocysteine were not consistent in PCOS patients. The aim of this review was to analyze the effect of metformin on homocysteine levels in patients with PCOS patients. Methods The Cochrane Library, Pubmed, and Web of Science were searched according to predefined search terms. There is no restriction for publication time and language. Results Eleven studies were included and the data were extracted. The homocysteine level in PCOS patients was significantly increased after taking metformin (mean difference [MD] −1.33; 95% confidence interval [CI] −2.16 to −0.49, p = 0.002). Subgroup analysis showed that the level of homocysteine was generally increased in PCOS patients with body mass index (BMI) ≥25 after taking metformin alone (MD −1.82; 95% CI −2.56 to −1.07, p < 0.00001). There was no significant change in homocysteine level in PCOS patients with BMI <25 (MD 0.69; 95% CI −0.41 to 1.79, p = 0.22). Subgroup analysis showed that there was no significant difference when taking metformin >3 months or taking metformin ≤3 months ( p = 0.84). Taking metformin ≥1700 mg/days significantly increased homocysteine levels in PCOS patients (MD −2.05; 95% CI −2.40 to −1.70, p < 0.00001). When taking metformin <1700 mg/days, there was no significant difference in homocysteine level in PCOS patients (MD 0.15; 95% CI −1.06 to 1.37, p = 0.80). The difference between the two subgroups was significant ( p = 0.0006). There was no significant difference in vitamin B12 level before and after metformin treatment (MD 24.70; 95% CI −22.54 to 71.93, p = 0.31). There was a decrease in serum folic acid level after metformin administration (MD 1.03; 95% CI 0.80 to 1.26, p < 0.00001). Conclusion Taking metformin alone increased homocysteine levels and decreased folic acid levels in nonpregnant PCOS patients. And, it was suggested that the dosage of metformin should be less than 1700 mg/days. The supplement of folic acid and B vitamins during metformin administration may be essential in nonpregnant PCOS patients. We should pay much attention to the potential effect of metformin in PCOS patients.

Abstract:Introduction: Polycystic ovary syndrome (PCOS) is an endocrine-metabolic disorder that occurs due to a hormone imbalance in the ovaries. It can cause the development of fluid-filled cysts on the follicles, causing infertility. Further, PCOS can cause insulin resistance and hyperandrogenism, increasing the amount of glucose and testosterone levels in the body. As a result, PCOS symptoms could include irregular or absent periods, weight gain, hirsutism, and more. PCOS patients are also at higher risk of developing type 2 diabetes, depression, and high blood pressure. Currently, there are no cures for PCOS, but it can be managed with lifestyle changes, contraceptive pills for irregular periods, and fertility treatments. Two of these treatments include inositol and metformin. The objective of the study is to ascertain how these therapies affect PCOS symptoms such as hyperandrogenism, insulin sensitivity, and infertility. Methods: Primary articles were found through the PubMed and OVID-Medline databases to investigate the effect of inositol or metformin on PCOS. Keywords such as PCOS, inositol, metformin, myo-inositol, insulin sensitivity, androgens, and fertility were used. Research published before 2010 was excluded. Results: The results found that metformin and inositol decreased the amounts of free testosterone and glucose in women with PCOS. Further, oocyte quality and production were improved in women taking metformin and inositol. These results were seen in women of all ages and of all BMIs. Discussion: Both metformin and MI are shown to significantly improve PCOS symptoms of hyperandrogenism, insulin sensitivity, and fertility issues. Although some limitations were present within the review and the studies examined due to exclusions, the review presents a comprehensive outlook on the impact of the interventions mentioned. Establishing a foundation for clinical trials would allow for the integration of these interventions into healthcare settings. Conclusion: A combination of both metformin and inositol, as well as diet and lifestyle changes can significantly improve the symptoms and life quality of individuals with PCOS. Future research would benefit from further studies done into investigating the combined effects of metformin and MI, as well as further assessment of secondary symptoms, as well as regarding the dosage of myo-inositol and metformin.

Abstract:Patients with polycystic ovary syndrome ( PCOS) are often confronted with adverse pregnancy outcomes. There is accumulating evidence demonstrating that metformin increases the rate of pregnancy and reduces the occurrence of miscarriage. However, the lack of sufficient evidence for metformin in reducing gestational diabetes mellitus ( GDM) and preeclampsia as well as its long-term safety data for offspring were also noted. The application of metformin in pregnant women with PCOS remains controversial. PCOS is a disease of great heterogeneity. With an increasing understanding of the pathogenesis of PCOS, the individualized treatment is a future trend as the patients with PCOS should be accurately classified to achieve the maximum benefit when metformin is added during pregnancy.

However, previous studies have found that the effect of metformin on the level of homocysteine were not consistent in PCOS patients. The aim of this review was to analyze the effect of metformin on homocysteine levels in patients with PCOS patients. Methods The Cochrane Library, Pubmed, and Web of Science were searched according to predefined search terms. There is no restriction for publication time and language. Results Eleven studies were included and the data were extracted. The homocysteine level in PCOS patients was significantly increased after taking metformin (mean difference [MD] -1.33; 95% confidence interval [CI] -2.16 to -0.49, p = 0.002). Subgroup analysis showed that the level of homocysteine was generally increased in PCOS patients with body mass index (BMI) >= 25 after taking metformin alone (MD -1.82; 95% CI -2.56 to -1.07, p < 0.00001). There was no significant change in homocysteine level in PCOS patients with BMI <25 (MD 0.69; 95% CI -0.41 to 1.79, p = 0.22). Subgroup analysis showed that there was no significant difference when taking metformin >3 months or taking metformin <= 3 months (p = 0.84). Taking metformin >= 1700 mg/days significantly increased homocysteine levels in PCOS patients (MD -2.05; 95% CI -2.40 to -1.70, p < 0.00001). When taking metformin <1700 mg/days, there was no significant difference in homocysteine level in PCOS patients (MD 0.15; 95% CI -1.06 to 1.37, p = 0.80). The difference between the two subgroups was significant (p = 0.0006). There was no significant difference in vitamin B12 level before and after metformin treatment (MD 24.70; 95% CI -22.54 to 71.93, p = 0.31). There was a decrease in serum folic acid level after metformin administration (MD 1.03; 95% CI 0.80 to 1.26, p < 0.00001). Conclusion Taking metformin alone increased homocysteine levels and decreased folic acid levels in nonpregnant PCOS patients. And, it was suggested that the dosage of metformin should be less than 1700 mg/days. The supplement of folic acid and B vitamins during metformin administration may be essential in nonpregnant PCOS patients. We should pay much attention to the potential effect of metformin in PCOS patients.

Abstract:Polycystic ovary syndrome (PCOS) is the most common disorder of ovarian function in premenopausal women. PCOS is characterised by chronic anovulation and androgen excess with clinical manifestation of irregular menstrual cycles, hirsutism and/or acne. Insulin resistance with resultant hyperinsulinaemia, irrespective of excess weight or frank obesity, has been reported in patients with PCOS, and, as insulin has a direct effect on ovarian androgen production in vitro, insulin resistance may play a crucial role in the physiopathology of PCOS. Although the molecular mechanism(s) of insulin resistance in PCOS is unclear, excessive insulin-independent serine phosphorylation of the β subunit of the insulin receptor, as reported in some patients with PCOS, has been put forward as a new mechanism for insulin resistance.Insulin-sensitising agents have recently been investigated for their role in the short term treatment of insulin resistance in PCOS. Controlled studies have shown that metformin administration, by promoting bodyweight loss, can decrease fasting and stimulated plasma insulin levels. However, other studies have shown metformin 500mg 3 times daily to decrease insulin secretion and to reduce ovarian production of 17α-hydroxyprogesterone with recovery of spontaneous or clomifene-induced ovulation, independently of weight loss. These findings suggest a new indication for metformin and present insulin-sensitising agents as a novel approach in the treatment of ovarian hyperandrogenism and abnormal ovulation in PCOS. They also suggest that long term administration of metformin might be helpful in treating insulin resistance, thus reducing risks of type 2 (non-insulin-dependent) diabetes and cardiovascular disease in these patients.

Abstract:Objective: To evaluate the efficacy of metformin administration throughout pregnancy on pregnancy-related complications in women with polycystic ovary syndrome (PCOS).Study design: MEDLINE and ScienceDirect were searched to retrieve relevant trials. The endpoint was the incidence of complications of pregnancy, gestational diabetes mellitus (GDM), pre-eclampsia (PE), miscarriage and premature birth included.Results: Five studies with 502 PCOS patients with metformin administration throughout pregnancy and 427 controls who used metformin just to get conception were included in our meta-analysis. In study group, a significantly lower change of emerging miscarriage and premature birth was observed, the pooled relative risk (RR) was 0.32 (95% confidence interval (CI): 0.19-0.56) for miscarriage and 0.40 (95%CI: 0.18-0.91) for premature birth. No significant difference was demonstrated in emerging GDM and PE.Conclusions: Metformin therapy throughout pregnancy can reduce the RR of miscarriage and premature birth incidence in PCOS patients with no serious side effects.

Abstract:Objectives:To evaluate the clinical effect of estrogen combined with metformin in the treatment of adolescent polycystic ovary syndrome (PCOS).Methods:95 patients with adolescent PCOS in our hospital from January 2014 to November 2015 were chosen and divided into observation group (48 cases) and control group (47 cases) randomly.The control group was treated with single medicine of estrogen,while the observation group was treated with estrogen and mefformin.The changes in sex hormone index and metabolic index before and after treatment in the two groups were compared and the clinical curative effect and adverse reactions were evaluated in the two groups.Results:After treatment,the sex hormone index (T,FSH,LH,DHEAS) of the two groups were lower than those before treatment,the level of SHBG was higher than that before treatment,and the indexes of the observation group were better than those of the control group (P ＜ 0.05).After treatment,the metabolic indexes (FBS,TG,FIN) in the observation group were significantly lower than those of before treatment and those of the control group,and the level of HDL-C was significantly increased (P ＜ 0.05).The total effective rate of the observation group and the control group were 91.67％ and 72.24％ respectively (P ＜0.05).There was no significant difference in the incidence of adverse reactions between the two groups (P ＞ 0.05).Conclusion:Estrogen and metformin is effective in treating patients with adolescent PCOS,through effectively controlling the levels of sex hormones and metabolic hormones.With remarkable effect,it is worth popularizing.

Abstract:Objective To observe the effect of metformin in th e treatment of patients with polycystic ovary syndrome (PCOS) who are resistant t o clomiphene citrate(CC). Methods In a double-blind, randomiz ed, placebo-controlled trail, 56 PCOS patients with CC-resi sta nt were randomly assigned into two groups. Patients in group A(n=28) randoml y received metformin 500-mg t.i.d and a placebo on day 5th-9th of menstruation . Group B(n=28) was rand omized to receive metformin 500-mg t.i.d with CC 50-mg q.d on day 5th-9th of menstruation.The d uration of the study was 3 months. The levels of serum FSH,LH T, glucose and i nsuli n at fasting (FG and FINS) were measured in all-patients. Transvaginal-ultraso nography was used for monitoring .Results IngroupA and B ,the levels of serumLH ,T ,FIN Sand HOMA IRweresignificantlyde creasedwhencomparedwiththoseofpre treatmentlevels(P 0.001) .The ovulationrateingroupBwereremarkablyhigher thanthatingroupA(P 0.01) .Conclusions Met for minthe rapyinwomen with PCOS can induce ovulation .Met for minplus CC ovulation is very effective in the treatment of CC-resistant PCOS .
