Abstract:Background The choice of postoperative weight bearing protocol after uncemented total hip arthroplasty (THA) remains controversial. The aim of this study was to assess the efficacy and safety of immediate unrestricted weight bearing (UWB) compared with partial weight bearing (PWB) in patients undergoing uncemented THA. Methods Relevant articles were retrieved from electronic databases. Both randomized controlled trials (RCTs) and non-RCTs were included but analyzed separately. All functional and clinical outcomes with at least 2 independent study outcomes were meta-analyzed. Results A total of 17 studies were investigated. No adverse effect was found regarding micromotion of the femoral stem with immediate UWB following uncemented THA. There was also no correlation between immediate UWB and failure of ingrowth fixation and higher risks of femoral stem subsidence and surgical revision in RCTs. Harris hip score was better in patients with immediate UWB than those with PWB at 1 year post surgery, but the difference was not statistically significant. Conclusions Immediate UWB did not have extra harm compared with PWB in patients undergoing uncemented THA. UWB was not superior to PWB. Considering the improvement of Harris score and the compliance of patients, UWB can be encouraged in THA rehabilitation.

Abstract:After years' development and improvement, THA ( total hip arthroplasty ) has now become the most effective operation in the treatment of hip diseases and functional reconstruction. With the continuous development and improvement of surgery, patients are move demanding to their postoperative limb functions. It has reached a broad consensus that the use of bone cement fixed prosthesis allows patients to bear weight early, but it is still controversial for non-cement fixed THA. In recent years, literature shows that the early weight-bearing of non-cemented THA has no adverse effects on the stability of the prosthesis. Weight-bearing exercises are beneficial to the stability of the prosthesis, the increase of bone mineral density and the early recovery of hip functions. The following is a review to the feasibility of early weight-bearing after THA and the corresponding weight training methods.
Cementless Porous Tantalum Monoblock Tibia Vs Cemented Modular Tibia in Primary Total Knee Arthroplasty: A Meta-Analysis

Abstract:Background With the introduction of new fixation systems and designs, there has been a recent reemergence of interest in cementless fixation of the tibial component in total knee arthroplasty. However, little is known regarding the clinical features and survivorship of the cementless porous tantalum monoblock tibial component compared to the conventional cemented modular tibial component. Methods We conducted a literature search of multiple databases for comparative studies published before June 2015 that investigated the outcomes of cementless porous tantalum monoblock tibia vs conventional cemented modular tibia. A pooled analysis was performed. The outcomes of interest were postoperative functional score, range of motion, Western Ontario and McMaster University Osteoarthritis Index, total complications, reoperation, radiolucent lines, loosening of the tibial component, and length of operation. Results Six studies involving 977 patients were eligible for the meta-analysis. The use of a cementless porous tantalum monoblock tibial component may associate with a slightly higher functional score, fewer radiolucent lines, and shorter operation. No significant difference was seen in regard to the range of motion, Western Ontario and McMaster University Osteoarthritis Index, total complications, reoperation, and loosening of the component between the 2 groups. Conclusion However, due to variation among the included studies, the use of cementless porous tantalum monoblock tibia seems to achieve no substantial superiority over that of the conventional cemented modular tibia at 5-year follow-up. Data concerning the long-term prognosis of this novel implant should continue to be collected and analyzed.

Abstract:Objective To systematically review the effectiveness and safety of total hip arthroplasty(THA) vs.total hip resurfacing arthroplasty(THRA) in patients with osteoarthritis of the hip joint.Methods We electronically searched databases including PubMed,The Cochrane Library(Issue 3,2012),EMbase,PloS,national institutes of clinical test library of the United States,national joint replacement registration network of Australia,British national joint registration network,British orthopaedic association network(BOA),Canada orthopaedic association(COA),American Academy of Orthopedic Surgeons network(AAOS),German Institute of Medical Documentation and Information network(DIMDI) for randomized controlled trials(RCTs) on the comparison between THA and THRA for osteoarthritis of the hip joint from inception to November,2012.References of the included studies were also retrieved.Two reviewers independently screened literature according to the inclusion and exclusion criteria,extracted data and assessed the quality of the included studies.Then,meta-analysis was performed using RevMan 5.1 software.Results Six RCTs(10 articles) involving 518 cases of surgery(THA:n=259;THRA:n=259) were identified.The risk of bias of 8 articles was moderate and that of the other 2 articles was low.The results of meta-analysis showed that,compared to THRA,THA brings greater improvements of femoral offset within 2 years after surgery(MD=6.60,95%CI 5.53 to 7.68,P=0.25).There were no significant differences between the two groups in Merle d'Aubigné Postel,WOMAC score,UCLA score,SF-36 Health Survey Questionnaire,walking speed,step length,walking cadence,postoperative leg length discrepancy,and complication rates.All these conclusions are based on early postoperative results.Conclusion Current evidence shows that,THA brings greater improvements of femoral offset than THRA,and no significant differences between the two groups were found in the other indicators.
Immediate weight bearing after uncemented total hip arthroplasty with an anteverted stem: a prospective randomized comparison using radiostereometry

Abstract:Background: In uncemented total hip arthroplasty with hydroxyapatite coating, early weight bearing is frequently practiced but there is still not much evidence to support this recommendation. Method: In a prospective randomized study we evaluated the effect of partial and full weight bearing after cementless total hip arthroplasty (ABG; Stryker-Howmedica) using radiostereometric analysis (RSA). Between February 1996 and February 2000, 43 consecutive patients (mean age 53 (41-63) years, 23 women) with hip osteoarthrosis received an uncemented and hydroxyapatite-coated prosthesis with an anteverted stem. All patients were operated in a standardized way by three experienced surgeons and they were randomized to partial (P) or full (F) weight bearing during the first 6 weeks after surgery. The patients in the partial weight bearing group were equipped with a pressuresensitive insole signaling when their load exceeded the prescribed weight limit. Results: At 3-month follow-up, the mean proximal (+)/ distal (-) migration of the stem was -0.14 mm (-1.93- 0.11) in group P and -0.31 mm (-4.30-0.16) in group F (p=0.6). At 1-year follow-up, the mean migration was -0.17 mm (-2.18-0.21) and -0.28 mm (-4.31-0.11), respectively (p=0.9). There was no significant difference in stem rotations either (p<0.2). The cup translations, rotations, and femoral head penetration were similar in the two groups (p<0.1). There were no re-operations during the first year. Interpretation: We did not find any adverse effect of full weight bearing immediately after operation, which justifies use of this regimen after uncemented total hip arthroplasty of the ABG type.

Abstract:Objective To investigate the feasibility of early weight bearing after uncemented total hip arthroplasty.Methods 56 patients less than 70 years old were divided randomly into early(27 cases) or late(29 cases) weight bearing group.The patients belonging to early weight bearing group were instructed to walk with one or two crutches within one week after operation,and patients belonging to late weight bearing group were allowed to walk in 6 weeks after operation.All patients were followed up in 6 weeks,3,6,12 and 24 months clinically and radiologically.Harris hip score and statistic analysis were performed to evaluate clinical outcome.Results Comparing with mean Harris score of late weight bearing group in six weeks and three months(60.3±9.1,74.0±12.0),that(71.9±10.2,87.6±9.4) of early weight bearing group was statistically significantly improved(P0.01).No loosening and subsidence sign was found in final radiograph,but 2 patients in late weight bearing group developed deep venous thrombosis of operated leg.Conclusions With primary stability of prosthesis,early weight bearing after uncemented total hip arthroplasty can facilitate functional rehabilitation of limb,and reduce the incidence of deep venous thrombosis as well.

Abstract:Background We performed an updated systematic review and meta-analysis which enrolled 25 prospective randomized controlled trials (RCTs) to compare the outcomes between total hip arthroplasty (THA) and hemiarthroplasty (HA) in patients with femoral neck fractures (FNFs). Methods We searched English databases which included PubMed, Embase (vis OvidSP), The Cochrane Library, and Web of Science, and Chinese databases Chinese National Knowledge Infrastructure (CNKI), VIP, Wang Fang, and China Biology Medicine Disc (CBM) in July 2020. The quality of each study was assessed according to the Cochrane Collaboration’s Risk of Bias. Risk ratios (RRs) and weighted mean differences (WMDs) with 95% confidence intervals (95% CIs) were pooled with random-effects models. Data regarding baseline characteristics, hospital and surgery outcomes, clinical outcomes, patients’ quality of life, common complications, prothesis-related complications, mortality, and costs were reported. Results A total of 25 RCTs involving 3223 patients (1568 THA and 1655 HA) were included. THA had longer hospital length (WMD = 0.721, P < 0.0001) and surgery time (WMD = 20.044, P < 0.0001), and more blood loss compared with HA (WMD = 69.109, P < 0.0001). THA showed better ratings in the Harris Hip Score during follow-up periods between 1 and 5 years while no differences within 6 months and after 9 years. THA was associated with higher quality-of-life EuroQol-5 Dimension (EQ-5D) scores after 2 years of surgery but no difference within 1 year. There was no difference in common complications. THA had significant higher rate of dislocation (WMD = 1.897, P = 0.002) and lower acetabular erosion (WMD = 0.030, P = 0.001). For mortality, there was no difference during all the follow-up periods except for slightly higher 2-year mortality after surgery. Conclusion This meta-analysis demonstrates that THA has better medium-term functional results and quality of life and lower acetabular erosion rate, while HA shows better in reducing hospital stay, surgery time, and blood loss and also has lower dislocation rate.
Research on Early Inpatient Rehabilitation after Cementless Total Hip Arthroplasty

Abstract:Objective:To study the early rehabilitation by comparing the outcomes in two matched groups of patients after cementless total hip arthroplasty, according to International Classification of Functioning, Disability and Health.Method: Thirty patients with 32 hips received cementless total hip arthroplasty. The patients were randomly divided into early full weight-bearing group and late full weight-bearing group.The preoperative and postoperative rehabilitation of the patients was similar except weight-bearing. The patients were evaluated clinically and radiographically before, and after surgery at 12 week. Result:At 12 week after surgery the patients in early full weight-bearing group gained the ability to walk faster than those in late full weight-bearing group(P0.001), and got better scores and improvement both in the Harris hip scores and WOMAC scores(P0.001). For the SF-36 scores at 12 week, the patients in early full weight-bearing group got higher scores in physical function, social function, and social role(P0.001). Early full weight-bearing after cementless total hip arthroplasty under some conditions was safe without showing the signs of implant instability or failure to roentgenographly at 12 week. Conclusion: The patients in early full weight-bearing group got better activities of daily living, social function and quality of life. Early full weight-bearing rehabilitation after cementless total hip arthroplasty can be carried out in patients under certain situations.

Abstract:Background: Femoral neck fractures are among the most common types of fractures and particularly affect elderly patients. Two of the most common treatment strategies are total hip arthroplasty (THA) and bipolar hemiarthroplasty (BA). However, the role of the different treatment strategies in the postoperative weight-bearing ability in the early postoperative phase is still not entirely clear. Methods: Patients who underwent either THA or BA were consecutively included in our prospective cohort study. Gait analysis was performed during the early postoperative period. The gait analysis consisted of a walking distance of 40 m coupled with the turning movement in between. During the gait analysis, the duration of the measurement, the maximum peak force and the average peak force were recorded. Results: A total of 39 patients were included, 25 of whom underwent BA and 14 of whom underwent THA. The maximum peak force during the gait analysis was, on average, 80.6% ± 19.5 of the body weight in the BA group and 78.9% ± 21.6 in the THA group. The additionally determined average peak force during the entire gait analysis was 66.8% ± 15.8 of the body weight in the BA group and 60.5% ± 15.6 in the THA group. Conclusions: Patients with femoral neck fractures undergoing THA and BA can achieve sufficient weight bearing on the operated leg in the early postoperative period. In our study, BA did not allow for a significantly higher average and maximum loading capacity compared with THA.

Abstract:Objective To compare the early rehabilitation effects of total hip arthroplasty (THA) with the direct anterior approach (DAA)versusthe posterior approach (PA). Methods We searched PubMed, Embase, Web of Science, the Cochrane Library, and Google databases from inception to June 2019 to select studies that compared the DAA and PA for THA. Only randomized controlled trials (RCT) were included. Two researchers independently screened studies for inclusion, extracted data, and assessed the methodological quality. A meta-analysis was conducted using RevMan 5.3 software provided by Cochrane Assisted Network. Results A total of 932 patients underwent THA. There were 467 cases in group DAA and 465 cases in group PA. There was a significant difference in the incidence of lateral femoral cutaneous nerve injury between DAA and PA groups (RR = 38.97, 95% CI: 7.89-192.57,P < 0.05). DAA was associated with less pain compared with PA [WMD = -0.65, 95% CI (-0.91-0.38),P < 0.05]. There was no significant difference in operation time, hospitalization stay, and intraoperative bleeding volume. Moreover, in supplementary data, the number of acetabular prostheses in Lewinnek's safety zones in DAA was more than that in the PA group (RR = 1.20, 95% CI [1.04-1.39],P < 0.05), and the time of discontinuation of walking aids in the DAA group was earlier than that in the PA group (WMD = -11.05, 95% CI [-17.79-4.31],P < 0.05). Conclusion The DAA total hip arthroplasty has comparable results with PA, with earlier postoperative functional recovery, less postoperative pain scores, and higher incidence of lateral femoral cutaneous nerve injury. The results need to be validated by large-sample, high-quality RCT studies, and long-term follow-up of complications.
A Comparison of Clinical, Radiographic and Surgical Outcomes of Total Hip Arthroplasty Between Direct Anterior and Posterior Approaches: a Systematic Review and Meta-Analysis

Abstract:A systematic review and meta-analysis were used to directly evaluate the direct anterior approach (DAA) and the posterior approach (PA) for early efficacy and safety of total hip arthroplasty (THA).Control-led trials assessing DAA and PA for the efficacy and safety of THA were searched in the database. Articles were reviewed according to predetermined inclusion and exclusion criteria; the quality of the methodology included in a given study was strictly assessed before data extraction. Moreover, meta-analysis was performed for outcomes that can be combined; otherwise, descriptive analysis was performed.There were 20 articles included, with a total of 7377 patients. Among these, 3728 and 3649 cases were in the DAA and PA groups, respectively. There was no difference between the DAA and PA groups at postoperative week 2 in the number of patients using the assistive ambulatory device or in time needed to completely discontinue all assistive ambulatory devices. Descriptive analysis found that DAA was slightly better than PA regarding early functional recovery and activity after surgery, as well as postoperative pain relief. Hospitalisation stay in the DAA group was shorter than in the PA group, in which the patients had a shorter operative time. Radiographic outcomes showed that there was little difference in prosthetic position between the DAA and PA groups. The proportions of intraoperative fractures and postoperative lateral cutaneous nerve of the thigh (LCNT) neuropraxia were significantly higher in the DAA group than in patients who underwent PA.Compared with PA, DAA presents superior early recovery following THA.
Comparison of Outcomes in Dual-Mobility Versus Fixed-Bearing Implants in Primary Total Hip Arthroplasty: A Systematic Review and Meta-Analysis

Abstract:Total hip arthroplasty (THA) is often regarded as one of the most successful surgical techniques developed in the twenty-first century. However, it is associated with complications such as prosthetic instability, dislocations, or infections. Dual-mobility (DM) implants have been developed with the goal of reducing the incidence of dislocations by increasing the femoral head-neck ratio, maximising hip stability, and improving the range of motion (ROM) before impingement and dislocation. This systematic review aims to comprehensively compare the safety and efficacy of DM versus fixed-bearing (FB) implants in primary THA patients. A comprehensive search strategy of PubMed, Embase, Scopus, and Web of Science Core Collection databases was executed to identify pertinent literature comparing DM and FB implants in THAs. Eligible studies underwent independent screening, and data were systematically extracted. The analysis employed pooled risk ratios (RR) for dichotomous outcomes and mean differences (MD) for continuous variables, each accompanied by their respective 95% confidence intervals (CI). Our systematic review and meta-analysis included nine studies encompassing 22,277 patients. The DM group had a significantly reduced incidence of dislocations compared to the FB group (RR 0.25, 95%CI [0.13, 0.47]; p-value <0.0001) and a significantly shorter length of stay (MD -9.92, 95%CI [-15.53, -4.32]; p-value = 0.0005). The FB group, however, had a significantly shorter operative time compared to the DM group (MD 10.41, 95%CI [7.64, 13.17]; p-value < 0.00001). We did not identify any significant statistical differences between the DM and FB groups regarding patient-reported outcome measures, the incidence of all-cause readmissions, the incidence of peri-prosthetic fractures, the incidence of infections, or the incidence of groyne pain.
[Medium term follow up outcomes of uncemented total hip arthroplasty for traumatic arthritis after intramedullary nail fixation of femoral intertrochanteric fracture]

Abstract:Objective: To evaluate the effect of uncemented total hip arthroplasty(THA) on treatment of traumatic arthritis caused by intramedullary nailing interfixation of intertrochanteric fractures. Methods: Total of 22 patients treated with THA due to traumatic arthritis caused by intramedullary nailing interfixation of intertrochanteric fractures from January 2012 to January 2017 were studied retrospectively, including 10 males and 12 females with a mean age of (72.5±9.8) years old ranging from 61 to 84 years old. Initial internal fixation method:14 patients were treated with Gamma nails and 8 patients were treated wit PFNA.The time from internal fixation surgery to THA was 10 to 68 months with an average of (32.2±21.3) months.Harris scores of the hip joint before and after surgery were compared, and the position of the prosthesis through postoperative imaging at 3, 6, 12 months and the last follow-up were evaluated. Results: One patient was died due to heart failure 1 year after operation. Two patients was died to advanced tumor 2 years after operation. The other 19 patients were followed up for 36 to 64 months with an average of (48.5±11.9) months. At final follow up, 14 patients regained the ability to walk independently, 4 patients needed support of a cane, 1 patient needed assistance of a walker. No serious complications such as joint dislocation, periprosthetic fracture and deep venous thrombosis occurred during follow-up. There were no signs of loosening and subsidence of the prosthesis at the final follow-up. Mean Harris hip score increased from (29.2±12.9) points preoperatively to (74.2±11.2) points at the final follow up(P<0.05);the score was excellent in 9 patients, good in 7 and fair in 3. Conclusion: Uncemented total hip arthroplasty for traumatic arthritis after intramedullary nail fixation of femoral intertrochanteric fracture can significantly improve hip function and effectively avoid bone cement implantation syndrome. The medium-term effect is satisfactory.

Abstract:Background: Impaired functional performance and decreased muscle strength and muscle atrophy generally persist for a long period after total hip arthroplasty (THA). Aim: To investigate the effects of weight-bearing (WB) and non-weight-bearing (NWB) exercises on functional performance, Harris hip score (HHS), muscle strength and muscle thickness in women after THA. Design: A randomised, controlled trial. Setting: Community. Population: Sixty-five women who had undergone unilateral or bilateral THA at least 6 months before enrolment in the study. Methods: Participants were randomly allocated to the following groups: WB (n = 22), NWB (n = 21) and control (n = 22) groups. Participants in the WB and NWB groups performed daily home exercise programs for 8 weeks. Functional performance (timed up and go, sit-to-stand, stair climbing, walking speed and 3-min walk test), HHS, isometric muscle strength of the hip and knee muscle and gluteus and quadriceps muscle thickness were measured at baseline and after 8 weeks. Results: An intention-to-treat analysis revealed that both the intervention groups exhibited significant improvements in almost all functional performance measures compared with the control group. Furthermore, the WB group showed significantly greater pre-post changes in the sit-to-stand and 3-min walk test compared with the NWB group. In terms of HHS, only the WB group showed significantly greater improvement compared with the control group. Relative to the control group, improvements in all isometric strength measures were observed in both the intervention groups. Significant improvement in quadriceps muscle thickness was observed in the WB group compared with the control group. However, no significant difference was observed among the three groups in pre-post changes in gluteus muscle thickness. Conclusion: WB and NWB home exercise programs were both effective for improving functional performance and muscle strength in women after THA. However, the WB exercise was more effective than the NWB exercise for improving sit-to-stand ability and walking endurance. Clinical rehabilitation impact: In this study, we demonstrated that the WB exercise was more effective than the NWB exercise for improving functional performance in patients after THA.
Immediate Full Weight-Bearing Versus Partial Weight-Bearing After Plate Fixation of Distal Femur Fractures in Elderly Patients. A Randomized Controlled Trial

Abstract:Introduction After surgery for distal femur fractures in elderly patients, weight-bearing is commonly restricted. Immediate non-restrictive weight-bearing might have beneficial effects. There are no randomized studies on the topic. The purpose of this study was to compare the functional outcome between immediate full weight-bearing (FWB) as tolerated and partial weight-bearing (PWB) during the first 8 weeks following plate fixation of distal femur fractures in elderly patients. Methods Patients aged 65 years or older with distal femur fractures of AO/OTA types 33 A2, A3, B1, B2, C1, and C2 were included. Exclusion criteria were impaired cognitive function, concomitant injuries, or inability to follow the postoperative regimen. Internal fixation was achieved with an anatomical lateral distal femur plate applied as a strictly bridge-plating construct. The primary outcome measure was the function index of the short musculoskeletal functional assessment (SMFA) after 52 weeks from injury. Results Thirty-two patients were randomized to FWB (n = 11) or PWB (n = 21). After 16 and 52 weeks, there were no differences in the mean SMFA function index between FWB and PWB (36 vs 43, P = .42 and 52 vs 40, P = .18, respectively) nor in the mean EuroQol 5-dimension index or range of motion (ROM). Overall, the SMFA function index was higher at 52 weeks compared with before injury (44 vs 30, P = .001) as was the mean bothersome index (37 vs 21, P = .011). There was no clear difference in the occurrence of adverse events between the treatment groups. Conclusions There were no differences in functional outcome, adverse events, or ROM between immediate FWB and PWB following plate fixation for a distal femur fracture in elderly patients. A distal femur fracture has a negative effect on the functional status of elderly patients that persists at least up to 1 year following injury.

Efficacy of cementless porous tantalum tibial components versus cemented tibial components in primary total knee arthroplasty: A meta-analysis

Abstract:Background: Total knee arthroplasty involves the use of cemented tibial components for fixation. In recent years, cementless porous tantalum tibial components have been increasingly utilized. The aim of this meta-analysis was to compare the efficacy of cementless porous tantalum tibial components with traditional cemented tibial components in terms of postoperative outcomes following total knee arthroplasty. Methods: Relevant literature was retrieved from Cochrane Library, PubMed, Embase, and Web of Science using the search terms "(trabecular metal OR Porous tantalum)" AND "knee" up to July 2023. The weighted mean difference with a 95% confidence interval was used as the effect size measure to evaluate the functional recovery of the knee joint, radiological analysis, complications, and implant revisions between cementless porous tantalum tibial components and traditional cemented tibial components after total knee arthroplasty. Review Manager 5.3 was utilized to conduct a comparative analysis of all included studies. Results: Nine studies with a total of 1117 patients were included in this meta-analysis, consisting of 447 patients in the porous tantalum group and 670 patients in the cemented group. Radiological analysis demonstrated that the porous tantalum group had better outcomes than the cemented group (P < .05). The combined results for the 5-year and 10-year follow-ups, range of motion, Western Ontario and McMaster University Osteoarthritis Index, complications, and implant revisions showed no significant differences between the porous tantalum and cemented groups. Conclusion: The results of the 5-year and 10-year follow-ups indicate that the use of cementless porous tantalum tibial components is comparable to traditional cemented tibial components, with no significant advantages observed. However, at the 5-year follow-up, the porous tantalum group demonstrated a good bone density in the proximal tibia. Future studies with a larger sample size, long-term clinical follow-up, and radiological results are needed to verify the differences between the 2 implants.

Abstract:Background This retrospective analysis compared the long-term outcomes for patients with a femoral neck fracture (AO/OTA type 31B) treated with a primary unilateral total hip arthroplasty with uncemented or cemented femoral components (UTHA or CTHA, respectively). Methods We conducted a retrospective cohort study using the South China Hip Arthroplasty Database. We identified 422 patients with femoral neck fracture (AO/OTA type 31B) who were previously treated with primary unilateral UTHA or CTHA between 2007 and 2015, with follow-up until 2019. Follow-up occurred 1, 3, 6 and 12 months postoperatively and yearly thereafter. The primary outcome was the Harris hip score (HHS). The secondary outcome was the orthopaedic complication rate. Results In total, 324 patients (UTHA n = 160, mean age 68.61 ± 7.49 years; CTHA n = 164, mean age 68.75 ± 7.04 years) were evaluated for study eligibility. The median follow-up was 73.3 months (range, 11.6–89.2 months). At the final follow-up, HHS was 74.09 ± 6.23 vs 79.01 ± 10.21 (UTHA vs CTHA, p = 0.012). Significant differences were detected in the incidence of prosthetic revision, loosening, and periprosthetic fracture between the UTHA and CTHA groups (7.5% for UTHA vs 1.8% for CTHA, p = 0.015; 17.5% for UTHA vs 8.5% for CTHA, p = 0.016; 11.9% for UTHA vs 4.9% for CTHA, p = 0.021, respectively). Conclusion In this setting, CTHA demonstrated superiority to UTHA by improving functional outcomes and decreasing complication rates.
The Comparison between Cemented and Uncemented Hemiarthroplasty in Patients with Femoral Neck Fractures: A Systematic Review and Meta-analysis of Randomized Controlled Trials

Abstract:Background: Hemiarthroplasty is the standard treatment for patients with femoral neck fractures (FNFs). Controversy exists over the use of bone cement in hip fractures treated with hemiarthroplasty. Objective: We performed an updated systematic review and meta-analysis to compare cemented and uncemented hemiarthroplasty in patients with femoral neck fractures. Methods: A literature review was conducted using Cochrane Library, ScienceDirect, PubMed, Embase, Medline, Web of Science, CNKI, VIP, Wang Fang, and Sino Med databases. Studies comparing cemented with uncemented hemiarthroplasty for FNFs in elderly patients up to June 2022 were included. Data were extracted, meta-analyzed, and pooled as risk ratios (RRs) and weighted mean differences (WMDs) with a 95% confidence interval (95% CI). Results: Twenty-four RCTs involving 3471 patients (1749 cement; 1722 uncemented) were analyzed. Patients with cemented intervention had better outcomes regarding hip function, pain, and complications. Significant differences were found in terms of HHS at 6 weeks (WMD 12.5; 95% CI 6.0-17.0; P < 0.001), 3 months (WMD 3.3; 95% CI 1.6-5.0; P < 0.001), 4 months (WMD 7.3; 95% CI 3.4-11.2; P < 0.001), and 6 months (WMD 4.6; 95% CI 3.3-5.8; P < 0.001) postoperatively. Patients with cemented hemiarthroplasty had lower rates of pain (RR 0.59; 95% CI 0.39-0.9; P = 0.013), prosthetic fracture (RR 0.24; 95% CI 0.16-0.38; P < 0.001), subsidence/loosening (RR 0.29; 95% CI 0.11-0.78; P = 0.014), revisions (RR 0.59; 95% CI 0.40-0.89; P = 0.012), and pressure ulcers (RR 0.43; 95% CI 0.23-0.82; P = 0.01) at the expense of longer surgery time (WMD 7.87; 95% CI 5.71-10.02; P < 0.001). Conclusion: This meta-analysis demonstrated that patients with cemented hemiarthroplasty had better results in hip function and pain relief and lower complication rates at the expense of prolonged surgery time. Cemented hemiarthroplasty is recommended based on our findings.

Abstract:Background:The efficacy and safety of direct anterior approach (DAA) for total hip arthroplasty (THA) is still unclear. Objective:To conduct a systematic review and meta-analysis of published studies comparing DAA and posterior ap-proach (PA) for THA. Methods:Controlled trials about comparison between DAA and PA for THA were searched in major databases. Suitable studies were selected according to inclusion and exclusion criteria. The data were extracted and the quali-ty of included studies was evaluated by two reviewers. Meta analysis was conducted using RevMan5.2 software, and the studies that could not be combined were analyzed descriptively. Results:Eleven literatures were included in this study, and a total of 1612 patients were involved. There were 817 patients in DAA group and 795 patients in PA group. All surgeons who performed DAA THA had already completed their learning curves. The patients in DAA group were able to walk earlier without the need for an assistive device than PA group (WMD=-11.05, 95%CI:-17.79,-4.31, P=0.001), and DAA was su-perior to PA in early postoperative function rehabilitation and activities. There was no statistical difference in the incidences of intraoperative and postoperative complications (OR=1.48, 95%CI:0.69, 3.20, P=0.32), intraoperative fractures (OR=1.31, 95%CI: 0.50, 3.45, P=0.58), postoperative dislocations (OR=0.34, 95%CI: 0.09, 1.28, P=0.11), heterotopic ossification (OR=1.01, 95%CI:0.26, 3.94, P=0.99) or groin pain (OR=2.73, 95%CI:0.62, 12.06, P=0.19) between two groups. Neither were the operative time (WMD=10.25, 95%CI:-6.33, 26.83, P=0.23), length of hospital stay (WMD=-0.34, 95%CI:-0.76, 0.07, P=0.10) or the percentage of acetabular cups placed within the Lewinnek safe zone (OR=2.08, 95%CI:0.65, 6.72, P=0.22). DAA could avoid postoperative dislocations as compared with PA and lead to a lower incidence of abnormality in ace-tabular cup position when using intraoperative fluoroscopy during DAA THA. Conclusions DAA is a superior approach for THA when surgeons make themselves master on the procedure of DAA THA.

Abstract:Repair of soft tissue in favor of the posterior approach for total hip arthroplasty is still under discussion and few studies are assessing this issue. Therefore we performed a meta-analysis to compare the effectiveness and safety of the posterior approach for total hip arthroplasty with and without soft tissue repair. We focus on these questions as follows: does primary posterior approach for total hip arthroplasty with soft tissue repair has better result regarding dislocation rate, Harris hip score and the sciatic nerve palsy rate compared with posterior approach without soft tissue repair. We conducted electronic literature searches using CENTRAL (Issue 1 of 12, Jan 2014), PUBMED (1980 to Jan 2014), and EMBASE (1980 to Jan 2014). Clinical studies evaluating the posterior approach for total hip arthroplasty with and without soft tissue repair were collected. After independent study selection by 2 authors, data were collected and extracted independently. The methodological quality of the studies was assessed by the Cochrane Collaboration's tool for assessing risk of bias and the Newcastle-Ottawa Scale. Seven clinical trials with 4594 hips using the posterior approach for total hip arthroplasty were included. The pooled data indicated a lower rate of dislocation (OR: 0.14, 95%CI: 0.08–0.26, P < 0.00001) and higher Harris hip score (1.75, 95%CI:1.19 to 2.32, P < 0.00001, I2 = 26%) after the posterior approach to total hip arthroplasty using soft tissue repair than without using soft tissue repair. There was no statistical difference in sciatic nerve palsy between the use of soft tissue repair and without it in posterior approach to total hip arthroplasty (OR: 5.34, 95%CI: 0.25–112.25, P = 0.28). Our meta-analysis included data from more studies than were previously available and demonstrated that the use of soft tissue repair and without it in posterior approach to total hip arthroplasty are similar in safety. Using repair resulted in a lower dislocation rate and higher Harris hip score than without repair.
