Abstract:The success of treating a wide variety of pediatric diseases with HSCT, hematologic malignancies in particular, has resulted in an increased number of long-term survivors. This study is the first large-scale, multicentre report that describes the evolution of pediatric HSCTs in China during the period of 1998-2012. Of all 1052 patients, 266 cases were treated with autologous HSCs and 786 used allogeneic HSCs. The disease indications for HSCTs mainly included leukemias, lymphoma, solid tumors, and non-malignant disorders. The total number of HSCTs, especially unrelated donor transplants, appeared to be increasing year by year. For patients with neuroblastoma, the therapeutic efficacy seemed to be poor, with a five-yr OS and DFS rate of 34.5 +/- 14.3% and 20.7 +/- 9.6%, respectively. In contrast, the survival of patients with SAA was prominently improved, and their five-yr OS and DFS rates were 82.8 +/- 4% and 80.7 +/- 4.1%, respectively. Patients who received cord blood transplants had a lower incidence of acute GVHD than that of PB and/or BM transplants from unrelated donors. This report offers us a valuable resource for evaluating the changes in HSCTs in China over the past 14yr.
Clinical outcomes of unrelated cord blood transplantation in children with malignant and non-malignant diseases: Multicenter experience in China.

Abstract:This multicenter retrospective study included 184 children with malignant and non-malignant diseases who underwent UCBT between January 1998 and August 2012. The malignant disease group included 101 children with ALL, AML, CML, JMML, and MDS, and the non-malignant disease group included 83 children with PID, -thalassemia, IMD BMF, and HLH. The median duration to neutrophil and platelet engraftment was 16 and 35days in the malignant disease group vs 15 and 38days in the non-malignant disease group. The cumulative incidence of grade II-IV aGVHD and cGVHD was 25.6% and 13.5% in the malignant disease group vs 19.7% and 11.1% in the non-malignant disease group, respectively. The median duration and cumulative incidence of neutrophil and platelet engraftment, and the cumulative incidence of grade II-IV aGVHD and cGVHD were similar between the two groups. Of the 184 pediatric patients, 114 patients survived during a median follow-up period of 14months (range 4-138). The 5-year OS and DFS were not statistically different between the two groups (56.3% and 46.1% in malignant disease group vs 68.5% and 52.8% in non-malignant disease group). The above results indicate that UCB is a viable source for HSCT for children with malignant or non-malignant diseases, especially in urgent cases.
Clinical Study of Unrelated Umbilical Cord Blood Transplantation for 25 Patients with Hematological Diseases

Abstract:This study was purposed to investigate the engraftment, graft-versus-host disease (GVHD), transplantation related mortality (TRM), relapse and survival in hematologic patients received unrelated umbilical cord blood transplantation (UCBT). A total of 25 patients with hematological disease underwent UCBT, including 8 pediatric and 17 young adult patients. Among them 3 cases received single unit of UCBT and 22 cases received double units of UCBT. For donor/recipients human leukocyte antigen (HLA) matching: HLA 6/6 loci matched in 9 cases, HLA 4-5/6 loci matched in 16 cases. There were 19 patients with hematologic malignancies, including 3 cases in the period of disease progression and 6 cases of non-hematologic malignancies. Conditioning regimens were TBI/Cy ± Flu ± ATG or BuCy ± Flu ± ATG for 21 patients and Cy+Flu+ATG for 4 patients. For prophylaxis of acute graft-versus-host disease (aGVHD) the regimen of cyclosporine (CsA) as dominant drug was used. The results showed that among 16 patients (80.0%) achieved engraftment, 20 patients survived for more than 42 d after transplantation. The cumulative neutrophil recovery rate on day 42 after transplant was 64.0%, with a median time of 17.0 d;the cumulative platelet recovery rate on day 100 after transplant was 60.0 %, with a median time of 35.0 d. The cumulative rate of grade II-IV and III-IV aGVHD after transplantation 100 d was 44.0% and 30.7%, respectively. Until the end of the follow-up, the cumulative rate of TRM was 54.3%. For all the patients, overall survival rate was 42.7%. Out of 17 evaluable patients with hematologic malignancies 7 cases (41.2%) survived to date, and only 1 case relapsed, so event-free survival rate was 35.3%. Out of 5 evaluable patients with non-hematologic malignancies, 4 patients survived and 2 patients were in stable engraftment state, 2 cases with autologous hematopoietic recovery. Among 3 cases of hematologic malignancies at advanced stage, only 1 case survived to date. It is concluded that HLA-4-6/6 loci matched UCBT is an effective option to treat hematological diseases. Double cord blood transplantation (dUCBT) can overcome the disadvantage of insufficient cells of single cord blood UCBT to treat overweight children and adult.

Abstract:Hematopoietic stem cell transplantation(HSCT) was widely used in treatment of hematologic malignancies.Sufficient hematopoietic/progenitor stem cells(HS/PCs) were critical for successful transplantation.A low cell dose would results in a delayed time to engraftment and high rates of engraftment failure.Insufficiency of cells is a main obstacle in clinical transplantation.When it is co-cultured with bone marrow-derived mesenchymal stem cells(BM-MSC) in the presence of cytokines,the cell growth was significantly enhanced: total cells by 75.2±15.0-fold,CD34+ cells by 33.4±12.3-fold.The expanded cells were collected and prepared as the expansion product of HS/PCs by resuspending cells in Leibovitz's L-15 Medium.Then,we established a series of methods to determine the contamination of bacteria,fungi and mycoplasmas,the bacterial endotoxin concentration,the residual bovine serum and SCF contents.

Abstract:This study aimed to compare the therapeutic effects of single umbilical cord blood transplantation (UCBT) and unmanipulated haploidentical hematopoietic SCT (haplo-HSCT) in childhood hematologic malignances. We enrolled 410 consecutive children who received either single UCBT (n=37) or haplo-HSCT from a family donor (n=373) during the same time period. For each UCBT recipient, three recipients matched for year of HSCT, underlying diseases, disease status and the length of follow-up were randomly selected from the haplo-HSCT cohort. Hematopoietic recovery was significantly faster in haplo-HSCT recipients than in UCBT recipients. The incidence of chronic GVHD was significantly higher in haplo-HSCT recipients. The incidence of CMV-related interstitial pneumonia was higher in UCBT recipients. The haplo-HSCT recipients had better 1-year OS (73.0% vs 56.8%, P=0.048), lower 1-year non-relapse mortality (NRM, 18.0% vs 35.1%, P=0.026) and lower 2-year NRM rates (19.9% vs 35.1%, P=0.044). The relapse- and disease-free survival rates did not differ significantly between the groups. Our results showed that compared with UCBT, unmanipulated haplo-HSCT can improve the outcomes of children with hematologic malignances.
[Clinical Observation on Haploid Hematopoietic Stem Cells Combined with Umbilical Cord Blood Double Transplantation for Aplastic Anaemia in Children]

Abstract:Objective: To explore the efficacy and safety of haploidentical hematopoietic stem cell transplantation combined with umbilical cord blood infusion for the treatment of aplastic anaemia in children. Methods: Nine cases of children with aplastic anaemia treated with umbilical cord blood combined with haploidentical hematopoietic stem cell transplantation at the People's Hospital of Henan University of Chinese Medicine from January 1, 2021 to September 15, 2023 with a median age of 11(2-13) years and a median follow up of 18(7.5-21) months were included, and the clinical data were retrospectively analyzed. Hematopoiesis reconstitution, the incidence of graft-versus-host disease(GVHD), infections and survival of the patients were analyzed. Results: All 9 children were successfully implanted. The median time to neutrophil and platelet implantation was 11.11±1.27 d and 12.44±3.36 d, respectively. One case developed acute gastrointestinal GVHD of degree I, which was improved after treatment, and the patient developed superficial gastritis and chronic gastrointestinal GVHD at a later stage, which is currently under clinical follow-up. Acute GVHD of II-IV degree was 0%. Hemorrhagic cystitis in 3 cases, CMV infection in 5 cases and bacterial and fungal infections in 5 cases improved with symptomatic treatment.All 9 children demonstrated complete donor chimerism within 1 month after transplantation, at two years of follow-up, all nine children survived without recurrence or development of grade II-IV GVHD, and there were no children with transplant-related deaths. Conclusion: Haploidentical hematopoietic stem cell transplantation combined with umbilical cord blood transfusion for aplastic anaemia in children has a low incidence and mild degree of GVHD, with significant efficacy, and can be used as a therapeutic option for children without an HLA full donor chimeric match.
Clincal Analysis of 4 Children with Severe Combined Immunedeficiency Treated by Hematopoietic Stem Cell Transplantation

Abstract:Objective To offer the experience of hematopoietic stem cell transplantation (HSCT) for children with severe combined immunodeficiency (SCID). Methods Five HSCT were performed in 4 X-linked SCID patients including 1 patient received a second HCST because of graft failure in first transplantation. One received human leukocyte antigen (HLA) matched related bone marrow, 2 received haploidentical peripheral hematopoetic stem cell from mother and another 2 received unrelated umbilical cord blood. Myeloablative conditioning and graft versus host disease (GVHD) prophylaxis were given in the two patients receiving unrelated umbilical cord blood transplant. Another 3 HSCT were performed without conditioning and GVHD prophylaxis. Results All patients received HSCT under the status of active infection. The average CD34+ cell was 6.45×107/kg in patients receiving bone marrow or peripheral hemopoietic stem cell transplantation and 1.38×106/kg in patients receiving umbilical cord blood. Three of 4 patients suffered grade Ⅰ-Ⅱ acute GVHD at about 14 d after transplantation. Two patients died of severe pneumonia, 1 patient give up treatment because of jaundice and liver dysfunction. Only 1 patient survived after experiencing primary graft failure, severe hepatic veno-occulsive disease and severe pneumonia with the nature killer cell count recovered to near normal at 78 d, immunoglobulin reached to normal at 10 months and CD3 +T lymphocyte cell count to near normal at 13 months. Conclusions HSCT is feasible for SCID. Diagnosis at earlier stage and transplant before the occurrence of severe infection is critical for the success in transplantation.
Clinical research progresses in umbilical cord blood transplantation in the treatment of hematologic malignancies

Abstract:Allogeneic hematopoietic stem cell transplantation is the only treatment method to cure hemato-logic malignancies. However, dififculty in obtaining the human leukocyte antigen (HLA) match donors is a major lim-itation in this therapeutic strategy. Many studies have demonstrated that umbilical cord blood is rich in hematopoietic stem cells and can be used as a source of stem cells of hematopoietic stem cell transplantation. Growing evidence also supports the efifcacy of cord blood transplantation (CBT) to treat patients with haematological malignancies, and the number of CBTs is rapidly increasing. Compared to other sources of stem cells, umbilical cord blood is physical cryo-preserved. Therefore, it is very convenient for transplantation, particular for those who need to receive transplantation sooner. In addition, the incidence and severity of graft versus host disease(GVHD) in CBT are usually relatively lower. Herein, we review considerations regarding status, research progresses and selection of umbilical cord blood in CBT.
Evaluation of the Efficacy and Safety of Treatment on Using Single-Unit Unrelated Umbilical Cord Blood Transplantation Based on Umbilical Cord Blood Provided by China's Public Cord Blood Bank

Abstract:Objective It mainly analyzes the umbilical cord blood used in umbilical cord blood transplantation (UCBT), which is provided by China's public cord blood bank. It made an analysis for the effect and safety of malignant hematologic diseases cured by UCBT. Methods We retrospectively analyzed 353 cases of patients with malignant hematologic diseases who accepted myeloablative single-unit unrelated donor UCBT without ATG at our center between Jan 2009 and December 2016. It made a comparative analysis for the umbilical cord blood's TNC before freezing and after thawing, the cells of CD34+, the recovery rate of cells and the clinical effect of UCBT. Results 353 units of umbilical cord blood used in UCBT respectively came from eight China's public cord blood banks. The average of 353 units of umbilical cord blood's TNC before freezing、after the tubular recovery、transfusion were respectively 18.4×108、16.27×108、15.75×108 , the number of TNC was significantly reduced after the recovery of the tubule and transfusion(P=0.000),and there was no difference for the tubular recovery and transfusion(P=0.132) . The average of CD34+ cells before freezing and after the tubular recovery were 11.16×106, 8.68×106 (P=0.000). The average recovery rate of the tubular recovery 、 umbilical cord blood transfusion and CD34+ cells after the tubular recovery was respectively 88.5%、86.5%、80.3%, the difference of recovery rate in each bank was obvious statistically significant (P=0.000,P=0.000,P=0.000). When 353 patients made the UCBT, the cumulative incidence of neutrophil 、platelet、erythrocyte engraftment was respectively 95.8%、84.9%、89.0%, it had the engraftment median time was respectively 17d (11d-42d)、38d (14d-120d)、23d (9d-60d). After CB provided by each bank was used in UCBT, it got the difference about cumulative incidence of engraftment (P=0.025,P=0.0045,P=0.00072). During 100 days after UCBT, the cumulative incidence about the Ⅱ-Ⅳ degrees of acute graft-versus-host disease (aGVHD) were 46.5%. After CB provided by each bank was used in UCBT, it got the no difference about cumulative incidence of aGVHD (P=0.904). During 3 years after UCBT, the cumulative incidence of relapse was 17.85%. After CB provided by each bank was used in UCBT, the difference about cumulative incidence of relapse was no statistical significance (P=0.654). During 3 years after UCBT, the overall survival (OS) rate、disease free survival (DFS) rate and the rate of graft-versus-host diseases/relapse-free survival (GRFS) were respectively71.1 %、66%、59.7%. After CB provided by each bank was used in UCBT, the difference about the rate of OS、DFS、GRFS were no statistical significance(P=0.689,P=0.719,P=0.477). Conclusion The umbilical cord blood provided by China's public cord blood bank was used in UCBT. It has a high safety and good efficacy in treating malignant hematologic diseases. But it needs to set up the standardized and normalized quality-control system of umbilical cord blood for China's public cord blood bank.
Outcome of 64 Aplastic Anemia Children with Allogenic Hematopoietic Stem Cell Transplant in China: A Report from Chinese Pediatric Group of HSCT

Abstract:to analysis the outcome of allogenic hematopoiteic stem cell transplant (HSCT) for children with aplastic anemia (AA) in China. To improve the evidence based HSCT indication for pediatric AA. retrospectively analysis pediatric AA received HSCT from 2002-2009 in Chinese Pediatric Group of HSCT. 64 patients from 8 hospitals enrolled in this group(16 VSAA, 44 SAA and 4 CAA). The median age was 7.5 years old (1-16.5y). Because 5 of 11 received second HSCT totally 69 transplantation were performed with 30 matched sibling donor, 24 matched unrelated donor and another 15 haploidentical family donor. 53% received CY120mg+ATG+Flu150-200mg+TBI3GY,38% received CY120mg+ ATG+ Flu150-200mg and only 9% received CY120mg+ATG condition regimen. For the sources of hematopoietic stem cell 45% with PB, 34% with PB+BM,17% with BM and only 4% with CB. After median follow up 26 months (10-95 months) 11 of 64(15.9%) suffered graft failure (3 with secondary graft failure).except 2 lost follow up post transplant 11 and 23 months, 7 patients died of transplant related mortality (6 with infection and only one with aGVHD) and another 55 (85.9%) got disease free survive. There was significant difference of overall survival (OS) for patients older or younger than 10 years(96% vs 71%, p = 0.037)and patients with BMT than PBSCT or CBT(100% vs 85% and 67%, p = 0.05). Although II-IV aGVHD was significant different between MSD, MUD or haploidentical transplant (22.2%, 45% vs 66.6%, P = 0.003)there was no any different of the OS among these three groups(93%,85% vs 80%, P = 0.526). HSCT is an effective method to rescue children with AA. Comparing with PB and CB, BM is much better for children with AA. Although aGVHD was much frequent in MUD and heploidentical donor transplant, it can be controlled and without interference with the OS. Because the outcome of MUD or haploidentical donor transplantation is the same as MSD donor transplant, which is much better than IST for pediatric AA in China. It is reasonable to consider the high resolution well matched unrelated donor transplant as the first line treatment for SAA children in our country.

Abstract:Background:Rare diseases requiring hematopoietic stem cell transplantation (SCT) present a challenge for BMT centers. The patients present at a low frequency, and there are often no established conditioning regimens for these disorders, especially for children without HLA matched sibling donors. We report the conditioning regimen using haploidentical related donors, and outcomes for the following rare disorders transplanted at our center from January 2013 through December 2015. During the past three years we had patients presented with the following: Fanconi anemia (FA), dyskeratosis congenital (DC), adrenal leukodystrophy (ALD) and Wiskott-Aldrich syndrome (WAS).

Abstract:Umbilical-cord blood (UCB) is a source of hematopoietic stem cells that has been successfully used for transplantation, primarily in children. But the low cell dose severely limits the application of unrelated umbilical-cord blood transplantation (UCBT) in adult patients, particularly in those with high body weight. We hypothesized that the combined transplantation of two partially matched UCB units would improve engraftment without crossed immunological rejection. Since May 2000, six adult patients with high risk leukemia have been transplanted in this study.
Therapeutic Effect of Unrelated Umbilical Cord Blood Transplantation in Treating Children with High Risk Acute Lymphoblastic Leukemia

Abstract:Objective To analyze the clinical outcomes of single unrelated umbilical cord blood transplantation (UCBT) in treating children with high risk acute lymphoblastic leukemia (ALL).Methods Between September 2008 and December 2014,a total of 66 consecutive pediatric patients with ALL received single UCBT treatment in Department of Hematology,Anhui Provincial Hospital Affiliated to Anhui Medical University.Myeloablative conditioning regimen was applied.The conditioning regimen consisted of total body irradiation (TBI) / Cyclophosphamide (Cy) / Cytarabine (Ara-C)in 13 patients,Busulfan(BU)/Cy/ Fludarabine(Flu) in 47 patients and BU/Cy/Ara-C in 6 patients.All patients received a combination of Cyclosporine and Mycophenolate mofetil for the prophylaxis of graft-versus-host disease (GVHD).Results The median dose of infused total nucleated cells and CD34 + cells were 4.70 (2.07-13.74) × 107/kg and 2.40 (0.73-9.89) × 105/kg.The cumulative incidence of neutrophil cells engraftment at day + 42 and platelet recovery at day + 120 was 97.0％ (64 cases) at a median of 16 (12-37) days after transplantation and 87.6％ (57 cases) with a median time of recovery of 39 (15-148) days,respectively.Incidence of grades Ⅱ-Ⅳ acute GVHD,grades Ⅲ-Ⅳ acute GVHD and chronic GVHD were 27.3％ (17 cases),17.2％ (10 cases),and 10.3％ (6 cases),respectively.The transplant-related mortality at 360 day was 21.2％ (14 cases).The 3-year cumulative incidence of relapse was 14.8％ (9 cases).The 3-year-disease-free survival and overall survival rates were 65.1％ (45 cases) and 67.6％ (46 cases),respectively.Conclusions For pediatric patients with high risk ALL,UCBT is a safe and effective treatment option.

Abstract:Objectives To study the efficacy of allogeneic stem cell transplantation in children with aplastic anemia.Methods In 10 children with aplastic anemia,5 received allogeneic peripheral stem cell transplant from HLA matched siblings,3 received unrelated allogeneic peripheral stem cell transplant,1 received bone marrow transplant and 1 received umbilical cord blood transplant.Results The child who received umbilical cord blood transplant died.Other 9 cases were successfully transplanted.The median time of reconstitution was 14 days(8 ～ 24 days).The median time for neutrophils to reach over 0.5 × 109/L was 12 days(8 ～ 19 days),while the median time for platelets to reach over 20 × 109/L was 17 days(9 ～ 40 days).Graft rejection happened in two cases.One of them received a second transplantation and the hemogram of the other one recovered automatically 3 months after transplantation.Conclusions If HLA matched sibling donor is available,allogeneic hematopoitic stem cell transplantation could be recommended as the first line of treatment for children with aplastic anemia.In particular for those patients at risk of severe infections,transplanation should not be opposed as the hematopoietic reconstitution after transplantation may in turn control infection.

Abstract:Umbilical-cord blood (UCB) is a source of hematopoietic stem cells that has been successfully used for transplantation, primarily in children. But the low cell dose severely limits the application of unrelated umbilical-cord blood transplantation (UCBT) in adult patients, particularly in those with high body weight. We hypothesized that the combined transplantation of two partially matched UCB units would improve engraftment without crossed immunological rejection. Since May 2000, six adult patients with high risk leukemia have been transplanted in this study.

Abstract:Objective: To investigate the efficacy and safety of umbilical cord blood combined with haploid HSCT (haplo-cord HSCT) in the treatment of hematological malignancies. Methods: The data of 82 patients with hematologic malignancies who received haplo-cord HSCT from January 2017 to June 2021 in the Affiliated Cancer Hospital of Zhengzhou University were retrospectively analyzed. There were 52 male and 30 female patients, aged [M(Q1, Q3)] 29 (20, 41) years. All patients received myeloablative preconditioning regimen. The day of the donor stem cell infusion was recorded as day 0 (0 d), the day before the infusion was recorded as day-1 (-1 d), and the day after the infusion was recorded as day+1 (+1 d), and so on. Eighty-two patients received transfusion of peripheral blood and/or bone marrow stem cells from unrelated cord blood and haplotype donors after the myeloablative preconditioning regimen. The graft-versus-host disease (GVHD) prophylaxis regimen was 8 mg/kg ATG combined with cyclosporine, morte-macrolide, and methotrexate. Patients were evaluated for implantation and the occurrence of transplant-related complications such as GVHD, infection, hemorrhagic cystitis, and long-term patient survival. Results: The time of neutrophil engraftment [M(Q1, Q3)] was 13 (11, 15) days and 15 (13, 21) days for platelet. The 30-day cumulative incidence of neutrophil engraftment was 98.8% (81/82) and 100-day cumulative incidence of platelet engraftment was 92.7% (76/82). The cumulative incidence of acute graft-versus-host disease (aGVHD) in degree Ⅱ-Ⅳ and Ⅲ-Ⅳ was 24.4% (20/82) and 6.1% (5/82), respectively. The cumulative incidence of chronic GVHD in+18 months was 13.5% (11/82). The follow-up time [M(Q1, Q3)] was 26 (13, 41) months, and the overall survival (OS) rate, event-free survival (EFS) rate, cumulative recurrence rate (CIR) and non-recurrence mortality (NRM) rate at 3 years after transplantation were 70.5% (95%CI: 59.7%-81.3%), 66.1% (95%CI: 56.1%-76.1%), 6.3% (95%CI: 5.7%-26.9%) and 20.8% (95%CI: 12.0%-29.6%), respectively. The cumulative incidence of cytomegalovirus and EBV reactivation was 37.8% (31/82) and 14.6% (12/82), respectively. The cumulative incidence of hemorrhagic cystitis was 32.9% (27/82). Conclusion: The efficacy of haplo-cord HSCT in the treatment of hematologic malignancies is reliable, with rapid hematopoietic reconstitution, low incidence of GVHD and virus reactivation.

Abstract:Abstract Four decades ago, Broxmeyer et al. demonstrated that umbilical cord blood (CB) contained hematopoietic stem cells (HSC) and hypothesized that CB could be used as a source of donor HSC for rescue of myeloablated bone marrow. In 1988, Gluckman et al. reported the first successful matched sibling cord blood transplant (CBT) in a child with Fanconi Anemia. In 1991, Rubinstein et al. established an unrelated donor CB bank, and in 1993, the first unrelated CBT used a unit from this bank. Since that time, >40 000 CBTs have been performed worldwide. Early outcomes of CBT were mixed and demonstrated the importance of cell dose from the CB donor. We hypothesized that improvements in CB banking and transplantation favorably impacted outcomes of CBT today and performed a retrospective study combining data from Eurocord and Duke University in 4834 children transplanted with a single unrelated CB unit (CBU) from 1993 to 2019. Changes in standard transplant outcomes (overall survival [OS], disease free survival [DFS], acute and chronic graft-versus-host disease [GvHD], treatment related mortality [TRM], and relapse) over 3 time periods (1: <2005; 2: 2005 to <2010; and 3: >2010 to 2019) were studied. Increased cell dose and degree of HLA matching were observed over time. OS, times to engraftment, and DFS improved over time. The incidence of TRM and GvHD decreased while the incidence of relapse remained unchanged. Relative contributions of cell dose and HLA matching to transplant outcomes were also assessed and showed that HLA matching was more important than cell dose in this pediatric cohort.

Abstract:OBJECTIVE:To analyse the efficacy and safety of co-transplantation of umbilical cord mesenchymal stem cell(UC-MSC) with haploidentical hematopoietic stem cell transplantation(hi-HSCT) in children with hematologic malignancy.METHODS:The clinical data of 47 children undergoing hi-HSCT were retrospectively analyzed from November 2003 to November 2014, among them 34 patients received UC-MSC from October 2011 to November 2014, and another 13 patients without UC-MSC from November 2003 to September 2011. The median follow-up time was 20(0.5-67) months.RESULTS:No adverse events were observed after the UC-MSC transplantation. The engraftment rate, the median neutrophils engraftment time and platelet engraftment time all were not significantly different between hi-HSCT and hi-HSCT+UC-MSCT(P>0.05). The three-years cumulative overall survival (70.6% vs 23.1%),(P=0.004), three-years cumulative disease-free survival(52.9% vs 0) (P=0), and early cytomegalovirus (CMV) viremia (91.2% vs 38.5%) (P=0) in UC-MSC+hi-HSCT group were statistically significantly higher than that in the conventional hi-HSCT group. The morbidity of aGVHD (44.1% vs 92.3%) (P=0.003), I-II aGVHD (26.5% vs 61.5%) (P=0.041) and transplantation-related mortality (11.8% vs 46.2%) (P=0.017) in UC-MSC+hi-HSCT group was statistically significantly lower than that in hi-HSCT group, however, the morbidity of III-IV aGVHD (17.6% vs 30.8%), cGVHD (26.5% vs 30.8%), HC (35.3% vs 7.7%), pulmonary infection (52.9% vs 46.2%) and relapse rate (32.4% vs 53.8%) were not statistically significantly different (P>0.05) between the 2 groups.CONCLUSION:The application of umbilical cord mesenchymal stem cell in children undergoing hi-HSCT is safe, the UC-MSC can improve the overall survival, disease-free survival and reduce transplantation-related mortality. UC-MSC can reduce the morbidity of aGVHD, but increase the early infection of CMV, however it is nothing for the pulmonary infection and relapse in the children after hi-HSCT.

Abstract:Objective To study the curative efficacy and safety of single-unit umbilical cord blood transplantation (sUCBT) for malignant hematologic diseases,which is provided by China's public cord blood bank.Methods We retrospectively analyzed 409 cases of malignant hematologic diseases who accepted myeloablative single-unit unrelated donor UCBT without ATG at our center between May 2008 and December 2016.A comparative analysis was made on the total nuclear cells (TNC) of the umbilical cord blood before freezing and after thawing,the cells of CD34+,the recovery rate of cells and the clinical effect of UCBT.Result 409 units of umbilical cord blood used in UCBT respectively came from eight China's public cord blood banks.The average TNC of 409 units of umbilical cord blood before freezing and after the tubular recovery were respectively 18.5 × 108 and 16.34 × 108 (p =0.000).The average recovery rate of the tubular recovery was 88.5％,and there was significant difference among cord blood banks (P =0.000).The average TNC of umbilical cord blood before freezing and transfusion were respectively 18.5 × 108 and 15.86 × 108 (p =0.000).The average recovery rate of umbilical cord blood transfusion was 85.9％,with the difference being significant among cord blood banks (P =0.000).The average number of CD34+ cells before freezing and after the tubular recovery was 11.18 × 106and 8.68 × 106 (p =0.000).The average recovery rate of CD34+ cells after the tubular recovery was 80.75 ％,with the difference being significant among the cord blood banks (P =0.000).At 42nd day after UCBT,the cumulative incidence of neutrophil engraftment was 95.4％,and the median time of the engraftment was 17 days (11-38 days).The cumulative incidence of platelet engraftment at 120th day was 84.6％,and the median time of the engraftment was 36 days (14-93 days).The cumulative incidence of erythrocyte engraftment at 60th day was 92％,and the median time of engraftment was 22 days (9d-60 days).After the umbilical cord blood provided by each bank was used in UCBT,it got the difference in cumulative incidence of engraftment.The P values for cumulative incidence of neutrophil,platelet and erythrocyte engraftment were respectively 0.004,0.01 and 0.000 2,with the differences being statistically significant.At 100th day after UCBT,the cumulative incidence of Ⅱ-Ⅳ and Ⅲ-Ⅳ degrees of acute graft-versus-host disease (aGVHD) was respectively 28.63％ and 15.7％.After umbilical cord blood provided by each bank was used in UCBT,it got the difference in cumulative incidence of aGVHD.There was no significant difference between Ⅱ-Ⅳ and Ⅲ-Ⅳ degrees (P =0.809 and 0.68 respectively).At 3rd year after UCBT,the cumulative incidence of relapse was 15.89％.After umbilical cord blood provided by each bank was used in UCBT,there was no significant difference in the cumulative incidence of relapse (P =0.898).At 3rd year after UCBT,the overall survival (OS) rate and disease free survival (DFS) rate were respectively 66.7％ and 59％.After umbilical cord blood provided by each bank was used in UCBT,it got the difference in OS and DFS.There was no significant difference in OS and DFS (P =0.566 and 0.703 respectively).At 3rd year after sUCBT,the rate of graft-versus-host diseases/relapse-free survival (GRFS) was 54.3％.After umbilical cord blood provided by each bank was used in UCBT,there was no significant difference in the rate of GRFS (P =0.449).Conclusion The umbilical cord blood provided by China's public cord blood bank was used in UCBT.It has a high safety and good efficacy in treating malignant hematologic diseases.But it needs to set up the standardized and normalized quality-control system of umbilical cord blood for China's public cord blood bank.
