The Assessment of Clinicopathological Features, Therapy Pattern and Prognostic Factors of 146 Gastric Cancer Patients with Bone Metastasis

Abstract:Gastric cancer with bone metastasis represents an increasing clinical problem with poor survival. This study investigated the influence of clinicopathological characteristics and therapy patterns on overall survival (OS) of gastric cancer with bone metastasis. From December 1996 to December 2014, 146 gastric cancer patients with bone metastasis were enrolled. Univariate and multivariate analyses were performed to assess the relation between clinicopathological characteristics, treatment methods and overall survival. Of all patients, 51 (34.9%) patients had synchronous metastasis and 95 (65.1%) had metachronous metastasis. Thirty-five (24.0%) patients only had bone metastasis, while 111 (76.0%) combined with other organ metastasis, including liver (30.0%), peritoneal (24.0%), lung (15.1%) and bone marrow (7.5%). Bisphosphonates and bone radiotherapy were applied in 99 (67.8%) patients and 34 (23.3%) patients. After diagnosis of bone metastasis, 96 (65.6%) patients received palliative chemotherapy. The median OS was 5.8 months (95% CI 4.284-7.316). Karnofsky Performance Status (KPS), bone marrow metastasis, lactate dehydrogenase (LDH), carbohydrate antigen 199 (CA199), cancer embryo antigen, combined with other organ metastasis, bisphosphonates used, palliative chemotherapy after bone metastasis were correlated with survival by univariate analysis (all p<0.05). Multivariate analysis revealed KPS<80 (p=0.030), bone marrow metastasis (p=0.000), elevated CA199 (p=0.000) and without palliative chemotherapy (p=0.000) were independent poor prognostic factors. The outcome of gastric cancer with bone metastasis is poor, especially in patients with bone marrow metastasis, worse KPS and elevated CA199. Palliative chemotherapy may have benefit for survival.
