Analysis of Cardiovascular Disease Burden on the Insured Population of Units Directly under Fujian Province

Abstract:This study analyzed the prevalence, medical expenses and reimbursement of major CVDs based on Fujian Provincial Claims Database, and investigated the budget risks that CVDs may bring. In this study major CVDs were identified as hypertension, diabetes, dyslipidemia, myocardial infarction and stroke. The study judged whether the patients met the diagnosis in accordance with ICD-10 and their medications, Patients' medical records from January 1, 2005 to December 31, 2014 were extracted. The drug code, surgery code, supplies, lab test and other items were thoroughly classified so as to eliminate the effects caused by treatment due to non-CVDs. The prevalence rates of hypertension, diabetes, dyslipidemia, MI and stroke were respectively 20.77%, 10.63%, 25.38%, 3.81% and 3.08%. The prevalence rate of CVDs showed a trend of sustained growth over the 10 years, especially dyslipidemia prevalence growth rate was very high (19.37%), and the growth was more faster in the population below the age of 60 years. Over the 10 years, the cumulative total CVD cost was 1.557 billion Yuan, in which outpatient costs and inpatient costs accounted for 45.03% and 54.97%, respectively. The average annual growth rate of CVD costs was about 12.98%, with inpatient costs growing faster than outpatient costs. In each year’s medical insurance total expenditures, CVDs accounted for more than 40%. In CVD inpatients costs, insurance covered for nearly 80%, while the out-of-pocket part for about 20%. The sustained growth of the prevalence of CVDs not only causes health damage of a large number of individuals, but also consumes a lot of insurance funds. Thus, implementing more policy incentives, attaching importance to the intervention of blood pressure, blood lipids and other risk factors, and preventing the occurrence or recurrence of serious CVD events may be the key initiatives to effectively reduce the social burden of CVDs.
