Methotrexate reverses the drug resistance induced by PRPS1 gene mutation in acute lymphoblastic leukemia cells to 6-mercaptopurine

Abstract:Objective: To investigate the reversal effect of methotrexate (MTX) on phosphoribosyl pyrophosphate synthetase 1 (PRPS1) gene mutant-driven 6-mercaptopurine (6-MP) resistance in acute lymphoblastic leukemia cells, and to explore its possible mechanism.Methods: The recombinant lentivirus carrying wide type (WT)-PRPS1 or mutant-PRPS1 (A190T and S103T) was infected into human acute lymphoblastic leukemia Reh cells (the one infected with the empty lentivirus GV303 was used as the blank control). The successful construction of the four cell lines were verified by Western blotting and cell viability assay. All cells were treated with MTX and 6-MP single or in combination. Then the proliferation and apoptosis of all cells were detected by trypan blue exclusion assay and AnnexinⅤ/7-AAD double staining assay, respectively. The expression levels of DNA damage- and apoptosis-associated proteins were examined by Western blotting. The effects of MTX on half maximal inhibitory concentration (IC50) of 6-MP as well as the levels of hypoxanthine and 6-MP metabolites in above cells were detected by cell viability assay and liquid chromatography-mass spectrometry, respectively.Results: The exogenous PRPS1 protein was overexpressed in Reh cells constructed successfully. As compared with the control cells (GV303 and WT), the cells infected with mutated PRPS1 (A190T and S103T) were resistant to 6-MP and MTX (all P u003c 0.001). After treatment with the combination of MTX and 6-MP, as compared with 6-MP or MTX monotherapy group, the proliferation of A190T and S103T cells was significantly suppressed (all P u003c 0.001), the apoptosis was significantly induced (P u003c 0.01, P u003c 0.001), and the expression levels of phosphorylated H2AX (γH2AX), phosphorylated checkpoint kinase 2 (p-Chk2) and the cleaved poly (ADP-ribose) polymerase (PARP) were significantly up-regulated (P u003c 0.05, P u003c 0.01, P u003c 0.001). In addition, both the IC50 values of 6-MP and hypoxanthine level in A190T and S103T cells treated with MTX were significantly reduced (all P u003c 0.001), while the levels of thioinosine monophosphate (TIMP) and thioguanosine monophosphate (TGMP) were significantly up-regulated (all P u003c 0.001) as compared with those in MTX-untreated cells.Conclusion: MTX can reverse PRPS1 mutant-driven resistance of acute lymphoblastic leukemia cells to 6-MP, and its possible mechanism is related to inhibiting the abnormal accumulation of hypoxanthine.
