Abstract:Objective:To study the molecular pathogenesis of protein deficiency which includes protein C,proteinS and antithrombin deficiency.Methods:The activities of protein C(PC:C),protein S(PS:C) and antithrombin (AT:C) were tested.All exons of protein C,proteinS and antithrombin gene in patients were amplified by polymerase chain reaction (PCR).The PCR products were sequenced directly to find the gene mutations.Results:The patient with protein C and protein S deficiency combined with antithrombin deficiency:Direct gene sequencing analysis showed:one mutation C4867T (NG_016323.1) was detected in promoter of PC gene,which is a polymorphism site,The sequencing analysis also showed that a heterozygous missense mutation G68395T (NG_009813.1) was detected in Exon4 of PS gene leading to the substitution of Arg90 by Leu (NP_000304.2) for the propositus.This had never been reported before thus it was a new mutation.The pedigree of inherited protein S deficiency:The sequencing analysis showed that a heterozygous (missense) mutation G68395T (NG_009813.1) was detected in Exon 4 ofPS gene.The same with that of the patient discussed above.Conclusions:The polymorphism site C4867T in PC gene and the novel missense mutation G68395T in PS gene caused the patient's deficiency of PC and PS.The novel mutation G68395T in PS gene might be the cause of deficiency of PS for the pedigree.

Abstract:OBJECTIVE:To identify the phenotype and gene mutation in two Chinese pedigrees with hereditary protein C deficiency.METHODS:The plasma level of protein C activity (PC: A) , protein C antigen (PC: Ag), protein S activity (PS: A), and antithrombin activity (AT: A) of the probands and their family members were detected using chromogenic assay and ELISA, respectively. All of the nine exons and intron-exon boundaries of protein C gene were amplified by PCR and analyzed by direct sequencing of the probands. Restriction enzyme site analysis was used to confirm the mutation.RESULTS:The plasma PC: A and PC: Ag for proband 1 was 1.2% and 0, respectively. Compound heterozygous mutations, C(TGC)64W (TGG) and F(TTC) 139V(GTC) , were identified in her, the former being inherited from the maternal side and the later the paternal side. Further genetic analysis showed that her husband ( II 8) had the heterozygous deletion mutation (K150 or 151 Del) in exon 7, her daughter had the same heterozygous deletion mutation and a F139V. The plasma PC: A and PC: Ag for proband 2 was 50. 3% and 1.9 mg/L, respectively. He had the heterozygous Lys150 or Lys151 deletion mutation, which was inherited from his father. Polymorphisms of C/T at position - 1654, A/G at - 1641 , and A/T at - 1476A/T in the promoter region of protein C were confirmed in all members of the two pedigrees, of which, proband 2 had homozygous CC/GG/TT. The F139V mutation was confirmed by restriction enzyme site analysis and polymorphism for this mutation was excluded. PS: A and AT: A were in normal range for all members.CONCLUSION:Compound heterozygous mutation C64W and F139V of protein C gene lead to type I hereditary protein C deficiency for proband 1. K150 or 151 deletion mutation and polymorphism of CC/GG/TT might lead to type I hereditary protein C deficiency for proband 2. C64W is a novel mutation for protein C gene. F139V and K150 or 151 deletion mutation are reported for the first time in China.

Abstract:BACKGROUND:We identified the gene mutations in two Chinese pedigree of type I hereditary protein C deficiency and type I hereditary antithrombin deficiency.METHODS:The plasma level of protein C activity (PC:A), protein C antigen (PC:Ag), protein S activity, antithrombin activity (AT:A) and antithrombin antigen (AT:Ag) of propositi and two family members were detected using ELISA and chromogenic assay, respectively. All exons and intron-exon boundaries of protein C gene and antithrombin gene were analyzed by direct sequencing of the corresponding amplified PCR products in DNA from the propositus.RESULTS:The plasma PC:A and PC:Ag of propositus 1 was 26% and 1.43 mg/dl, respectively. The PC:Ag and PC:A of his father were normal. The decreased PC:A level was seen in his mother and 4 of his maternal pedigree. PS:A and AT:A were all normal in pedigree 1 members. A C5498T heterozygous mutation in exon 3 of protein C gene, resulting in the substitution of Arg for Trp at the 15th amino acid, was identified in propositus 1 and 8 of his relatives. The plasma AT:A and AT:Ag of propositus 2 was 48.6% and 10.4 mg/dl, respectively. The reduced AT:A and AT:Ag levels were found in his father and 5 of paternal pedigree. PC:A, PC:Ag and PS:A were all in normal range. A heterozygous 13387-9G deletion in exon 6 of antithrombin gene was identified in propositus 2. This mutation introduced a frameshift and a premature stop at codon 426 and existed in 6 members of pedigree 2.CONCLUSION:The C5498T heterozygous mutation in exon 3 of protein C gene, first reported in China, leads to type I hereditary protein C deficiency. The 13387-9G deletion, a novel mutation, can cause antithrombin deficiency and thrombosis.
Gene Diagnosis Of Seven Patients With Hereditary Protein S Deficiencies

Abstract:Background: Hereditary protein S (PS) and protein C (PC) deficiencies are caused by gene mutations. The present research aimed to determine gene mutation sites in patients with thrombosis due to protein S deficiencies combined or not combined with protein C deficiencies. Methods: Patients with protein S deficiencies were enrolled. DNA was extracted from peripheral blood samples from these patients. All exons and their flanks of protein S genes (PROS) were amplified by polymerase chain reaction (PCR). After purification, the PCR products were sequenced directly and blastered to normal sequences, seeking out gene mutation sites. Polymorphism analysis was conducted by detecting the certain mutation in 50 normal people. Results: Seven patients were enrolled with thrombosis. They were diagnosed of protein S deficiencies combined or not combined with deficiencies of protein C. Polymorphism site G68395T on exon4 was identified in all seven patients. Non-sense mutation C68430T on exon4 was identified in patients No. 2 and No. 5. Missense mutation C86066T on exon10 was identified in patient No. 2. Missense mutation G82512C on exon9 was identified in patient No. 3. Searching PubMed, C86066T had been reported previously in a Hong Kong study, while C68430T and G82512C on PROS were the first ones reported worldwide. Conclusion: Polymorphism site G68395T and missense mutation C68430T, C86066T, and G82512C on PROS were identified and may be related to deficiencies of protein S. C68430T and G82512C were reported worldwide.

Abstract:OBJECTIVE:To explore the genetic basis for a pedigree affected with X-linked adrenoleukodystrophy presenting as spastic paraplegia of the lower limbs.METHODS:Genomic DNA was extracted from peripheral blood samples of the patient and his mother. Potential variant was detected with a panel for genes associated with spastic paraplegia. Candidate variant was verified by PCR and Sanger sequencing.RESULTS:Both the proband and his mother presented with walking difficulty. A previously known variant, c.623T to A (p.V208E), was identified in the ABCD1 gene mapped on chromosome X in both.CONCLUSION:X-link adrenoleukodystrophy should be taken into account as a possible diagnosis for this pedigree.

Abstract:We investigated the molecular mechanisms responsible for type I congenital antithrombin (AT) deficiency in two unrelated Chinese pedigrees manifesting multiple site venous thrombosis. Phenotype analysis showed both probands had almost 50% of normal AT levels. Direct sequencing of amplified DNA revealed 2757C > T in proband 1 and 13328G > A in proband 2, predicting a heterozygous Thr98Ile (T981) and Ala404Thr (A404T), respectively. No proband had 20210A allele or factor V Leiden mutation. Transient expression of complementary DNA coding for the mutations in COS-7 cells showed impaired secretion of the mutant molecules. Real-time quantitative PCR indicated that the mutant AT mRNA was transcribed at a similar or even higher level as that of wild-type (wt). Pulse-chase labeling studies suggested both AT variants did not accumulate, but degraded intracellularly. Immunohistochemical staining of the transfected cells revealed that CHO cells expressing the AT-198 mutant were stained diffusely without perinuclear enhancement and cells expressing AT-T404 mutant mainly in the whole cytoplasm with weaker perinuclear enhancement. We conclude that the impaired secretion of the mutant AT molecules, due to intracellular degradation, is the molecular pathogenesis of AT deficiency caused by T981 and A404T mutation for the two families, respectively.

Abstract:We report three heterozygous PROS1 mutations that caused type I protein S deficiency in three unrelated Chinese families. We measured protein S activity and antigen levels for all participants, screened them for mutations in the PROS1 gene. And we employed the calibrated automated thrombin generation (CAT) method to investigate thrombin generation. Numerous bioinformatics tools were utilized to analyze the conservation, pathogenicity of mutation, and spatial structure of the protein S. Phenotyping analysis indicated that all three probands exhibited simultaneous reduced levels of PS:A, TPS:Ag, and FPS:Ag. Genetic testing revealed that proband A harbored a heterozygous c.458_458delA (p.Lys153Serfs*6) mutation in exon 5, proband B carried a heterozygous c.1687C>T (p.Gln563stop) mutation in exon 14, and proband C exhibited a heterozygous c.200A>C (p.Glu67Ala) mutation in exon 2. Bioinformatic analysis predicted that the p.Lys153Serfs*6 frameshift mutation and the p.Gln563stop nonsense mutation in the protein S were classified as "disease-causing." The identification of the novel mutation p.Lys153Serfs*6 in PROS1 enriches the Human Genome Database. Our research suggests that these three mutations (p.Lys153Serfs*6, p.Gln563stop, and p.Glu67Ala) are possibly responsible for the decreased level of protein S in the three families. Furthermore, the evidence also supports the notion that individuals who are asymptomatic but have a family history of PSD can benefit from genetic analysis of the PROS1 gene.

Abstract:Objective To investigate the molecular etiology of protein C (PC) deficiency.Methods Routine diagnosis and genetic analysis were performed on four probands with PC deficiency.Results ①Case 1,female,40 years old,diagnosed of deep vein thrombosis in left lower limb.PC activity (PC∶ C) was 48％,PS activity (PS∶ C) was 26.3％,AT activity (AT∶ C) was 75.6％.Genetic analysis discovered heterozygous mutation C5156T on promoter of PC gene,together with heterozygous mutation A6578T on Exon2 of PC gene.After anticoagulant,thrombolysis and filter implantation therapies,the patient went home with improvement.②Case 2,female,32 years old,diagnosed of deep vein thrombosis in both lower limb,ischemia in both lower and upper limb,and skin infection in both lower limb.PC∶ C 27％,PS∶ C 22.9％,AT∶ C 86.7％.Genetic analysis identified heterozygous mutation C5156T,together with heterozygous mutation A5045T on promoter of PC gene.After anticoagulant and anti-infection therapy,the patient died of respiratory failure,septic shock and DIC.③Case 3,female,28 years old,diagnosed of vein thrombosis in right iliac and femoral vein.PC∶ C 58％,PS∶ C 57.3％,AT∶ C 80.8％.Genetic analysis disclosed heterozygous mutation C4867T on promoter of PC gene,AGA 12702-12704del or 12705-12707del on Exon7,the latter one lead to Arg192 or 193del.Heterozygous mutation G15240A on Exon9 was also found.After anticoagulant,thrombolysis and filter implantation therapies,the patient went home with improvement.④Case 4,male,30 years old,diagnosed of vein thrombosis in right iliac and femoral vein.PC∶C 50％,PS∶C 75.0％,AT∶C 89.1％.Genetic analysis found homozygous mutation C4867T and G4880A on promoter of PC gene,heterozygous mutation A5045T on promoter and heterozygous mutation T6589C on Exon2.After anticoagulant,thrombolysis and filter implantation therapies,the patient went home with improvement.⑤Polymorphism analysis revealed that heterozygous mutation C4867T,homozygous mutation G4880A,and heterozygous mutation C5156T were polymorphism sites of PC gene.Conclusions Polymorphism sites (G4880A,C4867T,C5156T),missense mutation A5045T,A6578T,G15240A,and deletion mutation AGA12702-12704del,12705-12707del may be related to deficiency of PC.Missense mutation A5045T,A6578T,G15240A,and deletion mutation AGA12702-12704,12705-12707del were first reported worldwide.

Abstract:Objective: To make genetic diagnosis and pedigree analysis for patients with recurrent venous thromboembolism due to inherited deficiency of protein C (PC), protein S (PS). Methods: The routine coagulation tests including activated partial thromboplastin time (APTT), prothrombin time (PT) and thrombin time (TT) were performed. Chromogenic substrate assay was used to detect the activities of Protein C (PC:C), total Protein S (PS:C) and antithrombin (AT:C). All exons and their flanks of PC and PS gene were amplified by polymerase chain reaction (PCR). The PCR products were sequenced directly and blastered to normal sequence of corresponding anti-coagulant protein to find the gene mutations. Results: Totally nine probands with DVT or PE were enrolled, their peripheral blood and medical histories collected after informed consents. Proband 1, 2 and 3 were with combined deficiency of PC and PS, while proband 4 was with PC deficiency. Sequencing of PC gene showed there were polymorphism sites G4880A, C4867T and A5054T in promoter region for all four probands with PC deficiency. PC:C and PS:C for proband 1 was 48% and 26.3%, respectively. PC gene sequencing showed that there was a heterozygous mutation A6578T in exon2 region, resulting in Thr18Ser. Sequencing of PS gene showed there was G68395T heterozygous mutation in exon4 region, leading to Arg90Leu. PC:C and PS:C of proband 2 was 27% and 22.9%, respectively. Heterozygous mutations of G68428A and C68430T in exon4 region of PS gene were found, leading to Arg100His and Gln101Stop, respectively. Proband 3 was with PC deficiency and PS deficiency, PC:C and PS:C were 58% and 57.3%. Heterozygous AGA12702-12704 or AGA12705-12707 deletion mutation was found in Exon2 of PC gene resulting in Arg192del or Arg193del, and heterozygous missense mutation A15240G in Exon9 resulting in His370Arg. Heterozygous mutation G68395T and G825512C was found in Exon4 and Exon9 region of PS gene, respectively, resulting in Arg90Leu and Ser321Thr. Proband 4 was with PC deficiency, PC:C50%. There was no other mutation detected except for polymorphism sites in promoter region. Proband 5 was with PS deficiency, PS:C 38.8%. Heterozygous mutation G68395T in exon4 region was detected, leading to Arg90Leu. Homozygous mutation C102102T was found in Exon14 region,leading to Gla616Val. Proband 6 was with PS deficiency, PS:C 35.2%. Heterozygous mutations G68395T andC68430T in Exon4 were found, leading to Arg90Leu and Gln101Stop, respectively. Proband 7 was with PS deficiency, PS:C 43.7%.Homozygous mutation C102102T in exon14 region was detected, resulting in Gla616Val. Proband 8 was with PS deficiency, PS:C43.6%. Heterozygous mutation G68395T and C68430T in exon4 region was found, leading to Arg90Leu and Gln101Stop. Proband 9 was with PS deficiency, PS:C 7.7%. Two of his family members were also with PS deficiency (II2，III2） with heterozygous mutation G68395T in Exon4 region（II1，II2，III1，III2）, leading to Arg90Leu. Conclusions: Polymorphisms of G4880A, C4867T and A5054T in promoter region, missense mutation A6578T, A15240G, AGA12702-12704 or 12705-12707 deletion mutation in PC gene，missense mutation G68395T, G68428A, C86066T, G82512C, C102102T, and nonsense mutation C68430T in PS gene might be the cause of reduced activities of corresponding anticoagulant proteins. All these mutations, except for C86066T in PS gene, which had been reported in Hongkong, are de novo ones.

Abstract:Antithrombin (AT) is the main physiological anticoagulant involved in hemostasis. Hereditary AT deficiency is a rare autosomal dominant thrombotic disease mainly caused by mutations in SERPINC1 , which was usually manifested as venous thrombosis and pulmonary embolism. In this study, we analyzed the clinical characteristics and screened for mutant genes in two pedigrees with hereditary AT deficiency, and the functional effects of the pathogenic mutations were evaluated.

Study on Rapid Diagnosis of Spondyloepiphyseal Dysplasia Tarda Family

Abstract:Objective:To develop a rapid diagnostic method for spondyloepiphyseal dysplasia tarda (SEDT).Methods:A four-generation SEDT family with 8 affected individuals was reported.The inheritance mode of the pedigree was X-linked recessive.On the basis of PCR and DNA sequencing of SEDL gene,total RNA was extracted from leukomonocytes of peripheral blood and RT-PCR was carried out to amply the coding region of SEDL directly.Results:RT-PCR and direct sequencing results revealed that there was a single nucleotide of A insertion (c.370-371 ins A ) in exon 6 of SEDL gene in all 8 patients.Also a presymptomatic persons carried the same mutation and the heterozygous mutation was identified in 6 female carriers.But 55 unaffected relatives and the 50 controls (unrelated healthy subjects,25 males and 25 females) were not found.Conclusion:RT-PCR and sequencing of cDNA is useful in rapid molecular diagnosis for SEDT.

Abstract:The multisystemic clinical characteristics of growth retardation, intellectual disability, joint contracture, and hepatopathy in humans are rare and there are no clear diagnoses of these conditions. However, previous studies using exome sequencing have suggested that they are caused by gene mutations, and some related pathogenic gene variants have been found. Here, we performed resequencing and genome-wide variation analysis of 3 individuals (an affected proband and unaffected parents) from a consanguineous family using Solexa sequencing technology to identify mutated genes. The following genetic features were identified: 3,586,775 single-nucleotide polymorphisms (SNPs), 583,416 insertion/deletion polymorphisms (InDels), and 8579 structural variations (SVs) in the genome of the father; 3,624,800 SNPs, 608,685 InDels, and 8,827 SVs in the genome of the mother; 3,574,431 SNPs, 571,196 InDels, and 8371 SVs in the genome of the proband. Variations between samples were determined by comparative analysis of authentic collections of SNPs and were functionally annotated. Variations in several important genes, including SEC22B, FLG, ZNF717, MUC4, TRIL, CTAGE4, FOXG1, LOC100287399, KRTAP1-3, and LRRC37A3, were surveyed by alignment analysis. The results present new evidence that mutations in 11 genes may be associated with characteristic clinical growth retardation, intellectual disability, joint contracture, and hepatopathy.

Abstract:Objective: To explore the pathogenesis of protein C deficiency in two pedigrees through mutation detection and model analysis. Methods: Chromogenic substrate method and enzyme linked immunosorbent assay (ELISA) were used to determine the plasma protein C activity (PC: A) and protein C antigen (PC: Ag) in the two probands and their family members. All of the 9 exons and intron-exon boundaries of the PROC gene were amplified by PCR and analyzed with Sanger sequencing after purification. Corresponding mutate sites of the family members were also amplified and sequenced. The PolyPhen-2 software was used to analyze the perniciousness of the mutations and Clustal X was to analyze the conservatism. The protein model and amino acids interaction of the mutations were analyzed by Swiss-PdbViewer software. Results: The PC: A and PC: Ag of proband 1 was 30% and 35%, while PC:A of his father, mother and aunt were all slightly under the reference range. Two heterozygous missense mutations were found in exons 7 and 5 of the PROC gene, namely c.565 C>T (p.Arg147Trp) and c.383 G>A (p.Gly86Asp). His father and aunt were carriers for c.565 C>T, while his mother had carried c.383 G>A. The PC: A of proband 2 and his son were 50% and 64%, respectively. And they were both positive for p.Arg147Trp. Analysis of PolyPhen-2 indicated that p.Arg147Trp was benign, while p.Gly86Asp was damaging. Clustal X analysis indicated that the p.Arg147Trp was non-conservative, while the p.Gly86Asp was highly conservative. Modeling for the mutant proteins revealed that the simple aromatic ring of Trp147 in p.Arg147Trp destroyed the two hydrogen bonds between Arg147-Lys146 and Arg147-Lys151, and steric hindranted with Arg178. The side chain of Asp86 extended and generated steric clash with Gln90 with the occurrence of p.Gly86Asp. The change of hydrogen bonds and steric effects has altered the spatial configuration of amino acids, which led to unstable mutate proteins and interfered with the secretion. Conclusion: Both probands had hereditary protein C deficiencies, for which their parents were all carriers. The heterozygous mutations p.Arg147Trp and p.Gly86Asp were the main cause for PC: A activity decrease. Among these, p.Gly86Asp was discovered for the first time.
Proteome and Metabolome Profiling of Anticoagulant Disorders Induced by Familial Protein S Deficiency

Abstract:Protein S deficiency (PSD) is an autosomal dominant disorder characterized by congenital thrombophilia. Studies on PSD are limited yet, resulting in a lack of clarity about molecular changes during abnormal coagulation. Proteomics and metabolomics analyses were conducted on the plasma of PSD patients based on liquid and gas chromatography-mass spectrometry (LC- and GC-MS). Differential proteins and metabolites of PSD were then filtered by univariate statistical analysis and subjected to network analysis using the ingenuity pathway analysis (IPA) platform. The proteome and metabolome of PSD were obviously disturbed, and the biological pathway of coagulation and complement cascades was the most affected. During PSD, overall levels of anticoagulant protein decreased and negative regulation of thrombin production was reduced, causing the formation of fibrin clots and platelet aggregation. Furthermore, 9 differential proteins correlated significantly with protein S, comprising A2M, AGT, APOE, FGG, GPLD1, IGHV1-69, CFHR5, CPN2, and CA1. The biological networks suggested that the pathways of acute phase response, FXR/RXR activation, serotonin receptor signaling, and p70S6K signaling were associated with PSD, indicating an interaction disorder of inflammatory immune and lipid metabolism. The findings may contribute to knowledge of available functional molecules and biological pathways of familial PSD and help with treatment improvement. Clinical Features and Molecular Genetic Analysis of a Pedigree of Limb Girdle Muscular Dystrophy

Abstract:Objective To investigate the clinical features and analyze the molecular genetics of a pedigree of limb girdle muscular dystrophy (LGMD).Methods Pedigree analysis and clinical examination were performed in one four-generation family with LGMD.Electrophysiology and muscle biopsy were done in the affected members.With an informed consent, gene mutation, genome screening and linkage analysis were conducted in 26 members of this pedigree.Results Seven patients were identified.Pedigree analysis was consistent with autosomal dominant inheritance.Affected members had early presentation.Main features included proximal muscle weakness without dysarthria nor spasticity; electrophysiology and muscle biopsy revealed myopathic changes.LGMD1 A, 1B, 1C and facioscapulohumeral dystrophy genes were not detected by gene mutation analysis.Genome screening and linkage analysis did not reveal any linkage with the disease-causing gene and the reported loci of LGMD1D and LGMD1F genes.Conclusions The clinical manifestations of this LGMD family are highly heterogeneous, and the disease-causing gene of this family is not linked to any of the reported sites, suggesting this may be a new disease-causing locus, or a new genetic type of LGMD.

Abstract:OBJECTIVE:To analyze the potential genetic cause of thrombocytopenia-absent radius (TAR) syndrome in a family and provide prenatal diagnosis for them.METHODS:Genetic mutation analysis of the sporadic family with TAR syndrome was performed with chromosome microarray analysis (CMA), quantitative polymerase chain reaction (qPCR) and Sanger sequencing. DNA samples were collected from 4 members of the family, including the proband, her parents and her sister. CMA, qPCR and Sanger sequencing were performed to determine the pathogenic mutation and prenatal diagnosis of the fetus was made accordingly.RESULTS:The proband had a 378 kb genomic heterozygous deletion in 1q21.1, which contained RBM8 A and other genes. c.-21G>A mutation was also found in the RBM8 A of the proband. The above-mentioned microdeletion and mutation were inherited from the mother and father, respectively. Prenatal CMA suggested that the fetus carried a 378 kb microdeletion in 1q21.1, and DNA testing did not find c.-21G>A mutation.CONCLUSION:The heterozygous deletion in 1q21.1 and RBM8 A: c.-21G>A is considered to be the genetic etiology of TAR syndrome in the family. The study provides information for subsequent family genetic counseling and prenatal diagnosis.

Abstract:Objective: To analyze the clinical features and gene mutations in four families with hereditary protein C (PC) deficiency and explore their association with vascular thromboembolism. Methods: The clinical data of four patients with PC deficiency were retrospectively analyzed. Venous blood samples were collected from the four affected patients and their family members, and relevant coagulation indexes and thrombin production and inhibition tests were performed. PCR was used to amplify and directly sequence the PROC gene of the probands. Software analysis was conducted to assess the conservativeness and pathogenicity of the mutated loci. Protein models were constructed to analyze the spatial structure before and after the mutation. Results: Thrombin generation and inhibition assays demonstrated impaired anticoagulation in all four probands. Proband 1 and 4 presented clinically with pulmonary embolism and lower extremity deep vein thrombosis (DVT), Proband 2 with cerebral infarction, and Proband 3 with DVT. Genetic analysis revealed the presence of the following mutations: c.541T > G heterozygous missense mutation, c.577-579delAAG heterozygous deletion mutation, c.247-248insCT heterozygous insertion mutation, c.659G > A heterozygous missense mutation, and a new variant locus c.1146_1146delT heterozygous deletion mutation in the four probands, respectively. In particular, c.1146_1146delT heterozygous deletion mutations not reported previously. Conservativeness and pathogenicity analyses confirmed that most of these amino acid residues were conserved, and all the mutations were found to be pathogenic. Analysis of protein modeling revealed that these mutations induced structural alterations in the protein or led to the formation of truncated proteins. According to the American College of Medical Genetics and Genomics (ACMG) classification criteria and guidelines for genetic variants, c.1146_1146delT was rated as pathogenic (PVS1 + M2 + PM4 + PP1 + PP3 + PP4). Conclusion: The identified mutations are likely associated with decreased PC levels in each of the four families. The clinical manifestations of hereditary PC deficiency exhibit considerable diversity.

Abstract:Background: Mutations in the F10 -coding gene can cause factor X (FX) deficiency, leading to abnormal coagulation activity and severe tendency for hemorrhage. Therefore, identifying mutations in F10 is important for diagnosing congenital FX deficiency. Methods: We studied a 63-year-old male patient with FX deficiency and 10 of his family members. Clotting and immunological methods were used to determine activated partial thromboplastin time (aPTT), prothrombin time (PT), thrombin time (TT), fibrinogen levels, FX activity, and FX antigen levels. The platelet count was determined. A mixing study was performed to eliminate the presence of coagulation factor inhibitors and lupus anticoagulant. Mutations were searched using whole-exome sequencing and certified by Sanger sequencing. Results: Genetic analysis of the proband identified two single-base substitutions: c.1085G>A (p.Ser362Asn) and c.1152C>A (p.Tyr384Ter, termination codon, caused by the DNA sequence TAA). His FX activity and antigen levels were 1.7% and 408.53 pg/mL, respectively; aPTT and PT were 52.3 and 48.0 s, respectively. One brother had the same compound heterozygous mutations, and his FX activity and antigen levels were 1.3% and 465.47 pg/mL, respectively; his aPTT and PT were 65.2 and 54.5 s, respectively. His mother, another brother, and one sister were heterozygous for c.1085G>A (p.Ser362Asn), and his daughter and grandson (6 years old) were heterozygous for c.1152C>A (p.Tyr384Ter). Conclusion: The heterozygous variants p.Ser362Asn or p.Tyr384Ter indicate mild FX deficiency, but the compound heterozygous mutation of the two causes severe congenital FX deficiency and bleeding. Genetic analysis of these two mutations may help characterize the bleeding tendency and confirm congenital FX deficiency.
