Risk factors associated with liver damage induced by high-dose methotrexate treatment of osteosarcoma

Abstract:Objective: To investigate the risk factors associated with liver damage induced by high-dose methotrexate treatment of osteosarcoma. Methods: Sixty patients with osteosarcoma received a total of 186 cycles of high-dose methotrexate chemotherapy. The serum alanine aminotransferase (ALT) activity was examined to determine the liver function grades. The potential factors associated with liver damage were evaluated by multiple regression analysis. Results: Of 186 cycles of high-dose methotrexate chemotherapy, 96 cycles induced liver damage with a liver damage rate of 51.6%. The age (P = 0.001), gender (P = 0.008), methotrexate concentration at 24 h after chemotherapy (P = 0.001) and the cycles of chemotherapy (P = 0.007) were associated with the rate of liver damage. The age (P = 0.001), gender (P = 0.000) and methotrexate concentration at 0 h after chemotherapy (P = 0.029) were associated with the severity of liver damage. The age (P = 0.041) and serum alanine aminotransferase level (P = 0.029) were associated with the recovery time of liver function. Conclusion: This study reveals the several risk factors related to the liver damage induced by high-dose methotrexate and raises the awareness of these risk factors which may help to avoid or reduce the rate of liver damage.
