Abstract:Background Over the years, video-assisted thoracic surgery (VATS) thymectomy has progressively replaced trans-sternotomy (TS) in early-stage thymoma (Masaoka stage I and stage II). This meta-analysis aimed to confirm the differences in the efficacies of VATS and TS approaches in early-stage thymoma patients. Methods A thorough literature search of the following online databases was performed: PubMed, Cochrane Library, Web of Science, and EMBASE. Appropriate search terms, such as "thymoma or thymus neoplasms or Thymic Carcinoma" and "Video-Assisted Thoracic Surgeries or Video-Assisted Thoracoscopic", were used with MeSH search methods. Heterogeneity was assessed first with the Q-test and inconsistency index and sensitivity analysis and subgroup analysis were then used to find the source of heterogeneity. Results We retrieved 1,228 articles, 11 articles were selected as the subjects of our research, and 1,222 patients were included in the research (666 VATS cases versus 556 TS cases). VATS caused less blood loss (P=0.02), and required shorter hospital stay (P<0.001), shorter duration of chest tube drainage (P=0.03) than TS. No obvious difference was found in operative time (P=0.14), postoperative recurrence (OR =0.81, 95% CI: 0.35-1.85, P=0.613), postoperative complications (OR =0.60, 95% CI: 0.31-1.16, P=0.129) and R0 resection (OR =0.35, 95% CI: 0.12-1.04, P=0.06), but the trend showed that more patients in the TS group achieved R0 resection. Conclusions For early-stage thymoma patients, VATS thymectomy seems to provide many advantages to be considered as a legitimate alternative to TS; however, when performing VATS, surgeons should pay special attention to ensure that R0 resection is achieved.
Clinical Analysis of Video-Assisted Thoracoscopic Surgery and Transsternal Thymectomy for Myasthenia Gravis Without Thymoma

Abstract:Objective To compare the surgical procedures and long-term clinical effects of thymectomy for myasthenia gravis without thymoma between video-assisted thoracoscopic surgery(VATS) and transsternal surgery (median sternotomy and transverse sternotomy). Methods Between January 1st 2000 and December 31st 2013, 355 patients with myasthenia gravis combined with thymic hyperplasia underwent surgery in Huashan Hospital, and retrospective study was conducated in 272 of those with follow-up results. Among these 272 patients, 58 received VATS(VATS group), 23 received median sternotomy(median sternotomy group) and the other 191 received transverse sternotomy(transverse sternotomy group). Results Compared with median sternotomy group, there was less blood loss[(96.90±47.69)ml vs (152.30±93.19)ml, χ2=3.497, P<0.001], shorter time of postoperative drainage [(1.93±0.97)d vs (2.61±0.94)d, t=2.857, P=0.006] and shorter postoperative hospital stay in VATS group[(3.48±1.68)d vs (8.04±4.23)d, t=6.993, P<0.001]. Compared with transverse sternotomy group, there was shorter time of postoperative drainage [(1.93±0.97)d vs (2.75±0.97)d, t=5.612, P<0.001] and shorter postoperative hospital stay in VATS group[(3.48±1.68)d vs (6.87±4.16)d, t=6.042, P<0.001]. Patients were followed up for 12 to 76 months, and the mean time of follow up was 55 months. The overall remission rates of VATS group, median sternotomy group and transverse sternotomy group were 79.3%, 86.9% and 85.3%, respectively, and there was no significant difference among three groups(χ2=0.638, P=0.424; χ2=1.200, P=0.273). Conclusions Thymectomy for myasthenia gravis without thymoma by VATS has the advantages of small incision, less blood loss, fast postoperative recovery and shorter postoperative hospital stay, and also has the same clinical effect as transsternal surgery. Therefore, VATS is preferable for the treatment of myasthenia gravis without thymoma. Abstract:Objective To compare the prognosis of thymectomy by video-assisted thoracoscopy (VATS) or by OPEN thoracotomy and its influence on myasthenia gravis (MG). Methods Between June 1990 and December 2010, 173 patients were diagnosed with MG and performed with thymectomy in our department, including 71 patients in the VATS group and 102 patients in open thoracotomy group. Results In the VATS group, MG disappeared in 9 (12.6%) cases. Twenty-three (32.4%) cases tapered medication for MG, 38 (54.5%) cases had their MG symptoms alleviated, and 1 case the symptom of MG got worse. In open thoracotomy group, MG disappeared in 11 (10.8%) cases. Forty (39.2%) cases ta-pered medications for MG, 48 (47.1%) cases had their MG symptom alleviated, and for 3 case still had symptoms of MG without improvement. Conclusion VATS thymectomy and thoracotomy can get similar remission rate. But VATS has less trauma and shorter hospital stay. It could serve as a safe alternative for thoracotomy for selected MG patients.

Abstract:Objective To investigate the value of video-assisted thracoscopic surgery(VATS) in the treatment of thymoma with myasthenia gravis(MG) by comparing with trans-sternal(TS) extended thymectomy.Methods A retrospective review of 50 thymoma with MG patients from May 2006 to September 2011,22 cases were treated by VATS extended thymectomy,28 cases were treated by transsternal extended thymectomy.Results No patient had myasthenia crisis in VATS group,but 2 patients developed myasthenia crisis in TS group.There was no mortality in the two groups.When VATS group was compared with TS group,the blood loss during operation,the time of postoperative drainage tube left in chest,postoperative hospitalization days and length of stay in ICU were decreased,and there were significant differences between two groups(P<0.01 or P<0.05),but the operative time between two groups was no significant difference(P>0.05).Conclusion Video-assisted thoracoscopic extended thymectomy is feasible for thymoma with MG,the operation was showed as smaller traumas,rapid recovery and less complication.

Abstract:INTRODUCTION: Thymectomy is the preferred standard treatment in younger non-thymoma patients with myasthenia gravis as well as in patients with early stage thymoma. Total thymectomy by median sternotomy has been the surgical approach since resection of the thymus with video-assisted thoracoscopic surgery (VATS).AIM: To compare the clinical outcomes of VATS thymectomy with conventional open thymectomy for neoplastic and non-neoplastic thymic diseases.MATERIAL AND METHODS: Forty patients underwent thymectomy between October 2012 and January 2016. Fifteen patients were male and 25 patients were female. The mean age was 40.3 ±17.7 years. Seventeen (55%) patients underwent VATS thymectomy and 23 (45%) patients underwent an open procedure. We retrospectively reviewed the data of the patients and compared these two techniques.RESULTS: The mean tumor size was 5.17 ±3.2 cm in the thymoma group (VATS 2.5 ±2.4 cm vs. open access 4.7 ±3.7 cm). None of the patients experienced a myasthenic crisis. Conversion to thoracotomy was required in 1 patient in the VATS group due to bleeding from the right internal mammary artery; therefore, the conversion rate was 2.5% among all the patients. No mortality occurred in either group. No significant difference was found in the perioperative blood loss, operative time or pain visual analogue scale scores. On the other hand, regarding postoperative drainage, duration of chest tube drainage and length of hospital stay, VATS thymectomy yielded better results and the differences were significant.CONCLUSIONS: Video-assisted thoracoscopic surgery thymectomy can be performed for both neoplastic and non-neoplastic thymic diseases with minimal morbidity and mortality.

Abstract:Objective To investigate the value of video-assisted thoracoscopic surgery (VATS) by comparing the clinic effect of VATS and thymectomy in the treatment of myasthenia gravis(MG). Methods Twenty-two patients who were confirmed to be MG preoperatively from February 2002 to July 2006 were divided into VATS group (12 cases) and thoracotomy group (10 cases). Thymectomy and dissection of all faity tissue anterior to pericardium were performed in both groups. To compare the operative time, blood loss during operation, the time of postoperative drainage tube left in chest, postoperative hospitalization days, MG crisis after operation and the curative effect in two groups.Results All indexes of VATS group were super to those of thoracotomy group (P0.05) . In VATS group, one case confirmed to be malignant thymoma during operation received thoracotomy instead. One case complicated with MG crisis was found in thoracomy group. There was no mortality in two groups. During a follow-up of 6-24 months, symptom was improved in 83.3% of patients in VATS group and 80.0% in thoracotomy group (P0.05).Conclusion Maximal thymectomy with VATS is safe and feasible with the advantages of less invasion, quick recovery and good curative effect. It is an ideal choice for the surgical treatment of MG without complication of malignant thymoma .
Efficacy And Safety Of Video-Assisted Thoracic Surgery And Median Sternotomy In The Treatment Of Thymoma-Associated Myasthenia Gravis

Abstract:Objective: Our study aims to explore the efficacy and safety of video-assisted thoracic surgery (VATS) and median sternotomy in the treatment of thymoma-associated myasthenia gravis (MG). Methods: The clinical data of 73 patients with thymoma-associated MG from January 2004 to January 2010 in our hospital were analyzed retrospectively. The clinical curative effect, postoperative MG crisis and quality of life of patients with thymomaassociated MG after treated by VATS and median sternotomy were compared. Results: Patients with thymomaassociated MG after treated by VATS and median sternotomy both obtained well curative effects. The intraoperative bleeding volume, postoperative thoracic drainage time, postoperative hospitalization time and intensive care unit (ICU) monitoring time in the VAST group were shorter than that in the median sternotomy group (all P<0.05). The quantitive myasthenia gravis (QMG) score and 15-item myasthenia gravis quality of life scale (MG-QOL15) score of the two groups were significantly improved compared with that before operation (all P<0.05). With the growth of postoperative time, Delta QMG score and Delta MG-QOL15 score were first increased and then decreased in two groups. One year after operation in the VATS group, Delta QMG score and Delta MG-QOL15 score were significantly higher than that in the median sternotomy group (all P<0.05). The occurrence rate of complications especially postoperative MG crisis were significantly higher (P<0.05) while the 5-year mortality rate was slightly higher (P>0.05) in the median sternotomy when compared to that in the VATS group. Conclusion: VATS is more secure than the median sternotomy in patients with thymoma-associated MG; it can also reduce the occurrence of postoperative MG crisis.

Abstract:BACKGROUND:Surgical resection of the thymus is indicated in the presence of primary thymic diseases such as thymoma. Video-assisted thoracoscopic surgery (VATS) and robot-assisted thoracic surgery (RATS) offer a minimally invasive approach to thymectomy. However, there is no clear conclusion whether RATS can achieve an equal or even better surgical effect when compared with VATS in treatment of thymoma. We performed this meta-analysis to explore and compare the outcomes of RATS versus VATS for thymectomy in patients with thymoma.METHODS:PubMed, Cochrane Library, EMBASE, China National Knowledge Infrastructure (CNKI), Medline, and Web of Science databases were searched for full-text literature citations. The quality of the articles was evaluated using the Newcastle-Ottawa Scale and the data analyzed using Review Manager 5.3 software. Fixed or random effect models were applied according to heterogeneity. Subgroup analysis was conducted.RESULTS:A total of 11 studies with 1418 patients, of whom 688 patients were in the RATS group and 730 in the VATS group, were involved in the analysis. Compared with VATS, RATS was associated with less blood loss in operation, lower volume of drainage, fewer postoperative pleural drainage days, shorter postoperative hospital stay, and fewer postoperative complications. There was no significant difference in operative time and patients with or without myasthenia gravis between the two groups.CONCLUSIONS:RATS has more advantages over VATS, indicating that RATS is better than VATS in terms of postoperative recovery. We look forward to more large-sample, high-quality randomized controlled studies published in the future.
Comparison of the Early Outcomes of Extended Thymectomy for Myasthenia Gravis Between Video-assisted Thoracoscopic Surgery and Median Sternotomy

Abstract:Objective To investigate the value of video-assisted thoracoscopic surgery(VATS)in the treatment of myasthenia gravis(MG)by comparing the early clinical outcomes of extensive thymectomy of VATS and median sternotomy.Methods 195 patients who received extended thymectomy for MG from July 1998 to May 2007 in our department were divided into two groups by operative approach,83 patients in the VATS group(from April 2002 to May 2007)and 112 patients in the full median sternotomy group(from July 1998 to May 2007).The clinical features,such as operative time,operative blood loss,postoperative drainage,the incidence of crisis,duration of crisis(time of mechanical ventilation),were retrospectively analysed by independent samples t test or chi-square test to evaluate the early outcomes.Results The postoperative drainage in the VATS group was more than that in the median sternotomy group(164.65±38.19ml vs.98.26±26.84ml,P=0.023),and the operative blood loss in the VATS group was less than that in the median sternotomy group(53.24±11.69ml vs.97.37±24.61ml,P=0.036).The incidence of crisis in the VATS group was 4.82%(4/83),which was less than that in the median sternotymy group [13.39%(15/112),P=0.046,OR=3.054].And the persistence time of mechanical ventilation for post-operative crisis in the VATS group was much shorter than that in median sternotomy group(75.33±39.31h vs.189.20±89.74h,P=0.012).Conclusion VATS extended thymectomy for myasthenia gravis is safe and less invasive.It can decrease the incidence of crisis and the time of mechanical ventilation of crisis,as well as decreasing operative blood loss.

Abstract:Complete resection could be achieved in virtually all myasthenic patients with Masaoka stage I and II thymoma using the trans-sternal technique. Whether this is appropriate for minimally invasive approach is not yet clear. We evaluated the feasibility of complete video-assisted thoracoscopic surgery (VATS) thymectomy for the treatment of Masaoka stage I and II thymoma-associated myasthenia gravis, compared to conventional trans-sternal thymectomy. We summarized 33 patients with Masaoka stage I and II thymoma-associated myasthenia gravis between April 2006 and September 2011. Of these, 15 patients underwent right-sided complete VATS (the VATS group) by using adjuvant pneuomomediastinum, comparing with 18 patients using the trans-sternal approach (the T3b group). No intraoperative death was found and no VATS case required conversion to median sternotomy. Significant differences between the two groups regarding duration of surgery and volume of intraoperative blood loss (P = 0.001 and P < 0.001, respectively) were observed. Postoperative morbidities were 26.7% and 33.3% for the VATS and T3b groups, respectively. All 33 patients were followed up for 12 to 61 months in the study. The cumulative probabilities of reaching complete stable remission and effective rate were 26.7% (4/15) and 93.3% (14/15) in the VATS group, which had a significantly higher complete stable remission and effective rate than those in the T3b group (P = 0.026 and P = 0.000, respectively). We conclude that VATS thymectomy utilizing adjuvant pneuomomediastinum for the treatment of stage I and II thymoma-associated myasthenia gravis is technically feasible but deserves further investigation in a large series with long-term follow-up.

Abstract:Purpose: This study was designed to evaluate the feasibility and safety of video-assisted thoracoscopic surgery (VATS), and to compare the surgical results of VATS with the standard median sternotomy (MS) approach. Materials and Methods: Between April 2010 and April 2012, the data of 245 patients who underwent thymectomy for thymic tumors were prospectively collected. Among them, 93 patients with clinical stage I-II disease were retrospectively reviewed. Results: Resection was planned for VATS in 49 cases, and for MS in 44 cases. During operation, there were 3 conversions to open surgery because of local invasion (conversion to thoracotomy in 1 patient, and sternotomy in 2). No transfusion was required in any patient. There was no significant difference in duration or amount of postoperative chest tube drainage between the 2 groups (P>0.05). Operative time, blood loss during operation, average length of the intensive care unit stay, and length of hospital stay were significantly less in the VATS group than the MS group (P<0.05). There were no major perioperative complications or mortality. No recurrence was detected during a median follow-up of 27 months (range, 12 to 36 mo). Conclusions: VATS thymectomy for early-stage thymic tumors is safe and feasible. In comparison with standard MS, the VATS approach was associated with a shorter intensive care unit stay and hospital stay. Prospective randomized multi-institutional trials with long-term follow-up are needed to compare the oncological outcomes.

Abstract:BACKGROUND: Pulmonary metastasectomy via a traditional open thoracotomy is effective for the treatment of patients with pulmonary metastases. However, whether video-assisted thoracoscopic surgery (VATS) achieves similar clinical effects in terms of survival rates remains unclear. We conducted this meta-analytic study to compare VATS with open thoracotomy for pulmonary metastasectomy.METHODS: Relevant studies were identified by searching PubMed from inception to December 2014. The overall and recurrence-free survival rates were measured in terms of hazard ratios (HRs) with VATS and open thoracotomy as the main outcomes.RESULTS: Eight studies were included in our meta-analysis, which included 337 patients in the VATS group and 485 patients in the open thoracotomy group. The overall survival rate in the VATS group was statistically significantly higher than that in the open thoracotomy group [HR, 0.69; 95% confidence interval (CI), 0.55-0.87; P = 0.002]. The VATS group still had a higher overall survival rate after exclusion of the one low-quality study (HR, 0.78; 95% CI, 0.59-1.03), but the result was not statistically signiinot signcant (P = 0.075). No difference in the recurrence-free survival rate was found between the two groups (HR, 0.86; 95% CI, 0.69-1.08). The subgroup analysis showed no difference between the two groups in either the overall survival rate (HR, 0.72; 95% CI, 0.50-1.04) or the recurrence-free survival rate (HR, 0.79; 95% CI, 0.59-1.08) in patients with metastases from colorectal cancer.CONCLUSIONS: VATS is an alternative surgical approach for pulmonary metastasectomy in patients with resectable pulmonary metastases. Further large prospective studies are needed to identify the indications for VATS in patients with pulmonary metastases.
Perioperative and Long-Term Outcome of Thymectomy for Myasthenia Gravis: Comparison of Surgical Approaches and Prognostic Analysis.

Abstract:BACKGROUND:Thymectomy is an established treatment for myasthenia gravis (MG), and video-assisted thoracoscopic surgery (VATS) thymectomy has become an acceptable surgical procedure. This study aimed to compare the results of VATS thymectomy and open thymectomy and to identify the prognostic factors after thymectomy.METHODS:The clinical data of 187 consecutive thymectomies performed between July 2000 and December 2009 were retrospectively reviewed; 75 open thymectomies and 112 VATS thymectomies. Clinical efficacy and variables influencing outcome were assessed by Kaplan-Meier survival curves and Cox proportional hazards regression analysis.RESULTS:The operative blood loss in the VATS group was significantly less than that in the open group ((62.14 ± 55.43) ml vs. (137.87 ± 165.25) ml, P < 0.05). The postoperative crisis rate increased with the severity of preoperative MG and the prescription dose of anticholinesterase. Complete follow-up information of patients more than 12 months after the thymectomy was obtained on 151 cases, 89 cases from the VATS group and 62 cases from the open group, with a mean follow-up period of 59.3 months, range from 12 to 117 months. Complete stable remission (CSR) was the end point for evaluation of the treatment results. The overall five-year CSR rate was 57.5%. Two good prognostic factors were identified; preoperative prescription of anticholinesterase alone (P = 0.035) and non-thymomatous MG (P = 0.003). The five-year CSR rate of the ocular type of MG reached a high level of 67.4%.CONCLUSIONS:Thymectomy can achieve good long-term CSR in MG, and VATS is an ideal alternative method. High-dose prescription of anticholinesterase and the advanced stage by Myasthenia Gravis Foundation of America (MGFA) classification have higher risks of postoperative crisis. Preoperative prescription of anticholinesterase alone and non-thymomatous MG are good prognostic factors. Thymectomy should also be considered for the ocular type of MG.

Abstract:Objective To investigate the clinical outcomes of subxiphoid video-assisted thoracoscopic thymectomy for myasthenia gravis. Methods The clinical data of the 85 patients undergoing video-assisted thoracoscopic thymectomy for myasthenia gravis in Department of Cardiothoracic Surgery, Huashan Hospital affiliated to Fudan University between January 2014 and July 2016 were studied. Subxiphoid approach video-assisted thoracoscopic thymectomy (SXVT) and through traditional unilateral approach video-assisted thymectomy (TVAT) were compared.The clinical outcomes of SXVT and TVAT were compared. Results There was no surgical death and no statistical difference between the two groups in drainage time, postoperative volume of drainage, postoperative hospital stay and bleeding volume during operation (P＞0.05). However, the acute chest pain after surgery, as well as the postoperative chest pain, and operative time were less in the the SXVT group than that in the TVAT group (P＜0.05). Conclusion SXVT for myasthenia gravis is safe and executable. It can alleviate intercostal neuralgia and abnormal chest wall feeling. And it should be considered in the treatment of myasthenia gravis.

Abstract:The efficacy and safety of subxiphoid thoracoscopic thymectomy (SVATS) for early thymoma are unknown. The purposes of this meta-analysis were to evaluate the effectiveness and safety of SVATS for early thymoma, to compare it with unilateral intercostal approach video thoracoscopic surgery (IVATS) thymectomy, and to investigate the clinical efficacy of modified subxiphoid thoracoscopic thymectomy (MSVATS) for early anterior mediastinal thymoma.

Abstract:From June 2008 to December 2012,video-assisted thoracoscopic surgery (VATS group) was performed for 24 cases of thymoma.Their operative procedures,postoperative complications,remission of myasthenia gravis (MG),perioperative mortality and oncological outcomes were compared with 19 cases of trans-sternal open thymectomy (OT group).Compared with OT group,VATS group had significantly shorter operation duration,less blood loss,shorter postoperative hospital stay,shorter chest tube duration and less chest drainage amount (P ＜ 0.05).The total rate of postoperative complications of VATS group was 4％ (1/24) and it was less than OT group (4/19) without significant difference (P ＞0.05).The effective remission rate of thymomatous MG was 7/9 in VATS group and it was similar to that in OT group (6/8,P ＞ 0.05).VATS group had a recurrent or metastatic rate of 0％ (0/23) and it was better than OT group with 1/17.And there was no significant inter-group difference (P ＞ 0.05).The total survival rate of VATS group was 100％ and it was similar to that of OT group (16/17,P ＞ 0.05).Thoracoscopic thymectomy is safe,feasible,efficacious and mini-invasive for thymoma.With fewer complications and a quicker recovery,it has similar short-term outcomes to conventional open thymectomy.
Surgical Techniques for Early-Stage Thymoma: Video-Assisted Thoracoscopic Thymectomy Versus Transsternal Thymectomy.

Abstract:Objective: The present study compared the outcomes between patients who had undergone video-assisted thoracoscopic surgery (VATS) thymectomy and transsternal (TS) thymectomy for Masaoka stage I and II thymoma.Methods: The outcomes of 262 patients without myasthenia gravis who had undergone surgery for Masaoka stage I and II thymoma from January 2008 to December 2012 at our center were retrospectively evaluated. The study included 125 patients who had undergone unilateral VATS thymectomy (VATS group) and 137 patients who had undergone TS thymectomy (TS group).Results: The VATS group had a shorter operative time than the TS group (170 vs 210 minutes, P<. 001). The VATS group also had a smaller intraoperative blood loss (200 vs 450 mL, P<. 001), smaller pleural drainage volume in the first 24 hours postoperatively (300 vs 500 mL, P<. 0010), shorter postoperative pleural drainage duration (3 vs 5 days, P<. 001), and shorter postoperative hospital stay (8 vs 10 days, P<. 001). Four patients in the VATS group underwent conversion to open surgery because of injury to the innominate vein. The postoperative complication rate was similar between the 2 groups. One patient in the VATS group developed pleural recurrence, and one in the TS group developed local recurrence.Conclusions: Unilateral VATS thymectomy for Masaoka stage I and II thymoma is technically feasible and safe and is less invasive than TS thymectomy, with a shorter duration of surgery, less intraoperative blood loss, less postoperative pleural drainage, shorter postoperative pleural drainage duration, and shorter postoperative hospital stay. We have concluded that it is preferable to perform VATS thymectomy, although perhaps under certain circumstances sternotomy might be preferred. The oncologic outcomes were comparable between the 2 procedures. Additional follow-up is required to evaluate the long-term outcomes.

Abstract:Objective Cost-effetiveness analysis was made and compared between esophagogastric mechanical anastomosis and esophagogastric demixing anastomosis with the use of three leaf clamp.Methods 133 patients who received extended thymectomy for MG during May 2000 to July 2007 in our department were divided into two groups by operative approach,which 53 patients in VATS group and 80 patients in sternotomy group.The clinical features,such as operative time,operative blood loss,postoperative drainage quantity,the incidence of crisis,duration of crisis(time of mechanical ventilation),were analysed to evaluate the early efficacy.Results The postoperative drainage quantity in VATS group was more than that in sternotomy group,and the operative blood loss in VATS group was less than that in sternotomy group.The incidence of crisis in VATS group was 3.77%(2/53),which was less than that in sternotymy group 13.75%(11/80)(P=0.042).Conclusion Video-assisted thoracoscopic extended thymectomy for myasthenia gravis is safe and minimally invasive,can decrease the incidence of crisis and the time of mechanical ventilation of crisis,as well as operative blood loss.

Abstract:Objective: To examine the efficacy and methodology of video-assisted thoracoscopic thymectomy for thymoma and MG. Methods: Twenty-two patients underwent either video-assisted thoracoscopic thymectomy or extended thymectomy between Mar, 1996 and Dec, 2002. There were 16 males and 6 females, aged 17 to 71, mean 44 1 years. 12 thymectomies for thymoma and 10 extended thymectomies for MG were performed. Results: Mean chest tube drainage time was 1 95 days and mean postoperative hospital stay was 4 55 days. 3 of the 10 patients with MG needed temporary mechanical ventilation (less than 24 hrs), and 1 reintubation. There was no mortality. Conclusion: Video-assisted thoracoscopic thymectomy for gradeI thymoma has the superiority of less traumatic and quicker recovery comparing with that through transsternal procedures. Extended thymectomy through VAT is effective and reliable. This technique may be of some value in decreasing postoperative complications and shortening hospital stay.

Abstract:Objective: To evaluate the results of video-assisted thoracoscopic extended thymectomy for myasthenia gravis. Methods: We retrospectively reviewed data from 107 patients received thoracoscopic extended thymectomy from June 1995 to June 2004. All patients had confirmed diagnosis of myasthenia gravis by clinical manifestation and electromyogram. Thoracoscopic extended thymectomy as well as dissection of all fatty tissue anterior to the pericardium was performed. Results: During a follow-up of 1-98 months, symptom was significantly improved in 83% of patients, including 34 patients experienced complete remission. There was no postoperative mortality. Conclusion: Favorable results of video-assisted thoracoscopic extended thymectomy can be achieved in patients with myasthenia gravis. The technique is safe and minimally invasive.
