Abstract:Background: Body pain was associated with many individual somatic and mental chronic conditions, but the association of body pain with somatic and mental multimorbidity (SMM) remains unclear. We aimed to evaluate the association of body pain with the prevalence and onset of SMM and SMM patterns.Methods: Participants aged ≥45 years at baseline (2011) from the China Health and Retirement Longitudinal Study were included. Body pain included pain sites in the head, neck/shoulder, upper limb, torso, and lower limb. Five SMM patterns were categorized by the number of mental condition (MC, depression) and somatic conditions (SC): 1 MC, 1 SC, SC multimorbidity (≥2 SC), mixed multimorbidity (1 MC and 1 SC), and complex multimorbidity (1MC and ≥2 SC). Multinomial logistic regressions were used to link body pain, specific pain sites, and the number of pain sites with SMM patterns in cross-sectional (2011) and longitudinal (2011-2018) analyses.Findings: Body pain was associated with higher odds of SMM and SMM patterns among 10,984 participants in the cross-sectional analysis; it was associated with 1 SC (odds ratio [OR] 1.62, 95% confidence interval [CI] 1.10-2.40), SC multimorbidity (OR 2.90, 95% CI 1.94-4.34), mixed multimorbidity (OR 3.27, 95% CI 1.95-5.48), and complex multimorbidity (OR 4.90, 95% CI 2.85-7.92) among 1,914 participants in the longitudinal analysis. Pain in the head, neck/shoulder, torso, and lower limb were associated with increased odds of SMM patterns, including SC multimorbidity, mixed multimorbidity, and complex multimorbidity. Moreover, participants with ≥4 pain sites would experience higher odds of most SMM patterns.Interpretation: Body pain was significantly associated with higher odds of SMM and SMM patterns, and different pain sites were associated with the onset of different SMM patterns, and so do pain site numbers. Abstract:The scientific investigation of pain faces many challenges. I argue that one of the central challenges posed by pain – its inherent complexity and supposed idiosyncrasy – is best understood as an instance of a more general issue in the mind sciences: the problem of psychological integration. This label is not yet an explanation or account of the underlying challenge, but rather a more precise, empirically tractable formulation of the worry as it arises in multiple domains. In this paper I first lay out a more substantive account of psychological integration, focusing on both the informational and regulative aspects of psychological coordination and control, and then apply this conception to the peripheral and central elements of sensory pain processing.

The dual facilitatory and inhibitory effects of social pain on physical pain perception

Abstract:Pain is a multi-dimensional phenomenon that encompasses both physical pain experienced physiologically and social pain experienced emotionally. The interactions between them are thought to lead to increased pain load. However, the effect of social pain on physical pain perception during interactions remains unclear. Four experiments were conducted merging physical and social pains to examine the behavioral pattern and neural mechanism of the effect of social pain on physical pain perception. Seemingly paradoxical effects of social pain were observed, which both facilitated and inhibited physical pain perception under different attention orientations. Brain imaging revealed that the posterior insula encoded the facilitatory effect, whereas the frontal pole engaged in the inhibitory effect. At a higher level, the thalamus further modulated both processes, playing a switch-like role under different concern statuses of social pain. These results provide direct evidence for the dual-pathway mechanism of the effect of social pain on physical pain.

Abstract:The coexistence of physical pain and psychological pain,two common types of pain associated with depression,complicates the timely diagnosis of depression,alleviation of symptoms,and improvement of patients'quality of life.In-depth research into the distinct clinical manifestations and neural mechanisms of these two types of pain is of great significance for the diagnosis of depression and treatment of patients with depression.A review of the literature reveals that,in terms of behavioral indicators,individuals with depression exhibit inconsistent sensitivity to different types of physical pain and characteristics,such as a low pain threshold,long duration,and intense suffering from psychological pain,compared to healthy subjects.With respect to neural mechanisms,the patterns of physical and psychological pain in patients with depression are similar to those of healthy subjects.In the future,researchers should concentrate on the features of these two types of comorbid pain in depression,clarify the influencing factors in pain processing in depression,compare the commonalities and differences between the processing of physical and psychological pain in depression,and elucidate the functional neuroscientific changes associated with pain in depression,thus providing a basis for more accurate diagnosis and more effective treatment.
The consequences of pain: The social and physical pain overlap on psychological responses

Abstract:Current theories suggest that social and physical pain overlap in their neurological and physiological outcomes. We investigated how social and physical pain overlap in their psychological responses by testing the hypothesis that both social and physical pain would thwart satisfaction on four human needs, worsen mood, and increase desire to aggress. In Experiment 1, recalling an experience of social or physical pain produced overlapping effects in the form of thwarted self-esteem and control needs and increased negative affect and desire to aggress. In Experiment 2, we induced social (Cyberball ostracism) or physical pain (cold pressor) within the laboratory session, and found that both pain types produced feelings of being ignored and excluded, and thwarted belonging, self-esteem, control, and meaningful existence. Our results provide further support to pain overlap theories and indicate that social and physical pain cause common psychological consequences, resulting in new ways to understand and manage pain. There is growing evidence in the literature showing that physical pain, the pain caused by a physical injury, and social pain, the pain caused by the threat or actual loss of a social connection (MacDonald & Leary, 2005), share a common neural system. Yet, it is still unclear whether the physiological overlap between the pain types also implies an overlap in their psychological responses. The present paper aims to fill this gap. We examined whether the social/physical pain overlap results in common psychological responses as well. The physiological overlap underlying physical and social pain was initially proposed by Panksepp (1998). Panksepp found that, besides reducing the experience of pain, the administration of an opiate also reduced crying behavior in animals that were socially isolated (Panksepp, Herman, Conner, Bishop, & Scott, 1978). Following Panksepp and colleagues’ early suggestion, several authors investigated whether similar neural brain circuitries are delegated to detect, process, and regulate both social and physical pain in social animals and humans (DeWall et al., 2010; Eisenberger, Lieberman, & Williams, 2003).

Abstract:Pain is a multifaceted process that encompasses unpleasant sensory and emotional experiences. The essence of the pain process is aversion, or perceived negative emotion. Central sensitization plays a significant role in initiating and perpetuating of chronic pain. Melzack proposed the concept of the "pain matrix", in which brain regions associated with pain form an interconnected network, rather than being controlled by a singular brain region. This review aims to investigate distinct brain regions involved in pain and their interconnections. In addition, it also sheds light on the reciprocal connectivity between the ascending and descending pathways that participate in pain modulation. We review the involvement of various brain areas during pain and focus on understanding the connections among them, which can contribute to a better understanding of pain mechanisms and provide opportunities for further research on therapies for improved pain management.
Soreness Reminds Me of Grief: Patients with Chronic Pain Show Less Differentiated Representations of Emotional Feelings and Bodily States

Abstract:People experience similarities between emotional feelings and bodily states on a daily basis, but both the magnitude and pervasiveness of this experiential similarity varies across individuals. Inspired by previous findings that chronic pain (CP) is characterized by strengthened pain-affect coupling and a reduced interoceptive accuracy, we conducted two cross-sectional studies to examine whether patients with CP would exhibit less differentiated perception and mental representation of emotional feelings and bodily states. In Study 1 (N=500), patients with CP and healthy controls (HCs) completed a self-report questionnaire that asked explicitly about the perceived similarity between five basic emotion categories and a series of bodily states. In Study 2 (N=73), a specially designed false memory test was administered to examine whether patients with CP would have reduced differentiation of concepts of negative emotion and somatic distress. We found that patients with CP perceived greater and more pervasive similarities between emotional feelings and bodily states, as indicated by higher questionnaire scores and denser, less specialized bipartite emotion-body networks, both associated with lower subjective interoceptive accuracy. Furthermore, patients with CP formed false memories for negative emotion words (e.g., grief) more readily than HCs after memorizing somatic distress words (e.g., soreness), as if they represented negative emotion and somatic distress as a single, enmeshed semantic category. Our findings extend previous literature by demonstrating reduced discrimination between emotional and bodily experiences in CP that is not restricted to pain-related emotional and sensory experiences and may be related to a fundamentally less differentiated interoception. Perspectives This study shows that patients with chronic pain have a profoundly less differentiated perception and implicit conceptualization of emotional feelings and bodily states, which appears to be associated with altered interoception. These findings may provide new perspectives on why they often experience a stronger pain-affect coupling.

Abstract:Characterizing the relationship between the brain and the mind is essential, both for understanding how we experience sensations and for attempts to create machine-based artificial intelligence. Materialists argue that the brain and the mind are both physical/material in nature whereas Cartesian dualists posit that the brain is material, the mind is non-material, and that they are separate. Recent investigations into the mechanisms responsible for pain can resolve this issue. Pain from an injury requires both the induction of a long-term potentiation (LTP) in a subset of pyramidal neurons in the anterior cingulate cortex and the creation of electromagnetic waves in the surrounding area. The LTP sensitizes synaptic transmission and, by activating enzyme cascades, changes the phenotype of the pyramidal neurons. The changes sustain the generation of the waves and the pain. The waves rapidly disseminate information about the pain to distant areas of the brain and studies using Transcranial Stimulation show that EM waves can influence the induction of LTP. According to leading contemporary theories, the waves will communicate with the mind, which is where the painfulness is experienced. The material brain and immaterial mind are therefore separate and we can no longer attribute painfulness solely to the activities of the brain. This is a radical departure from the contemporary view of brain functions and supports Cartesian Dualism. Consequently, consciousness and higher mental functions cannot be duplicated by mimicking the activities of the brain.

Abstract:Mental disorders such as anxiety and depression induced by chronic pain are common in clinical practice, and there are significant sex differences in their epidemiology. However, the circuit mechanism of this difference has not been fully studied, as preclinical studies have traditionally excluded female rodents. Recently, this oversight has begun to be resolved and studies including male and female rodents are revealing sex differences in the neurobiological processes behind mental disorder features. This paper reviews the structural functions involved in the injury perception circuit and advanced emotional cortex circuit. In addition, we also summarize the latest breakthroughs and insights into sex differences in neuromodulation through endogenous dopamine, 5-hydroxytryptamine, GABAergic inhibition, norepinephrine, and peptide pathways like oxytocin, as well as their receptors. By comparing sex differences, we hope to identify new therapeutic targets to offer safer and more effective treatments.

Abstract:Jens Agerström, Kent Stening, Olof Axman Department of Psychology, Linnaeus University, 391 82 Kalmar, Sweden Background: The ability to traverse psychological distance by going beyond the experienced reality of the self, here and now, is fundamental for effective human functioning. Yet, little is known about how physical pain affects transcendence of psychological distance. Using a construal level theory framework of psychological distance, the current research examines the hypothesis that pain impairs people's ability to traverse any kind of psychological distance whether it be temporal, social, and spatial distance, or the hypothetical. Methods: Using the cold pressor test, 151 participants participated in an experiment where they were either induced with acute pain (treatment group) or no pain (control group) while completing a battery of questions measuring to what extent their current thoughts were transcending psychological distance. Results: The results were largely consistent with the hypothesis. Relative to the control group, pain induced participants showed significantly less transcendence of past temporal distance, social distance, spatial distance, and the hypothetical. Furthermore, greater self-reported pain intensity was significantly associated with less transcendence of temporal (past and future), social, and spatial distance. Conclusion: Physical pain impairs the ability to traverse psychological distance. The research has practical implications for the pain clinic and for pain-afflicted individuals in everyday life. The Effect of Electrical Stimulation–Induced Pain on Time Perception and Relationships to Pain-Related Emotional and Cognitive Factors: A Temporal Bisection Task and Questionnaire–Based Study

Abstract:Pain has not only sensory, but also emotional and cognitive, components. Some studies have explored the effect of pain on time perception, but the results remain controversial. Whether individual pain-related emotional and cognitive factors play roles in this process should also be explored. In this study, we investigated the effect of electrical stimulation–induced pain on interval timing using a temporal bisection task. During each task session, subjects received one of five types of stimulation randomly: no stimulus and 100 and 300 ms of non-painful and painful stimulation. Pain-related emotional and cognitive factors were measured using a series of questionnaires. The proportion of “long” judgments of a 1,200-ms visual stimulus duration was significantly smaller with 300 ms painful stimulation than with no stimulus ( P < 0.0001) and 100 ms ( P < 0.0001) and 300 ms ( P = 0.021) non-painful stimulation. The point of subjective equality (PSE) did not differ among sessions, but the average Weber fraction (WF) was higher for painful sessions than for no-stimulus session ( P = 0.022). The pain fear score correlated positively with the PSE under 100 ms non-painful ( P = 0.031) and painful ( P = 0.002) and 300 ms painful ( P = 0.006) stimulation. Pain catastrophizing and pain anxiety scores correlated significantly with the WF under no stimulus ( P = 0.005) and 100 ms non-painful stimulation ( P = 0.027), respectively. These results suggest that electrical stimulation–induced pain affects temporal sensitivity, and that pain-related emotional and cognitive factors are associated with the processing of time perception.

Abstract:Objective:To evaluate the relationship among quality of life,emotion mental status and social support of patients with chronic soft tissue pain.Methods:210 patients with chronic soft tissue pain (CSTP)were investigated with face to face interview.All patients were assessed with short form-36 health survey(SF-36),medical outcomes study pain measurement(MOSPM),hospital anxiety and de- pression scale(HAD)and brief system inventory(BSI).All patients were classified into three groups depending on HAD scale.Related factors among three groups were compared.Results:The quality of life of CSTP was significantly correlated with the patients'education,economic status and social support,and it was negatively correlated with emotion and mental status.Conclusions:To improve the quality of life of CSTP,besides treatment of biotic facts,better humanity,service should regarded and concordant system- atie of social supports should established for CSTP.

Abstract:Recent approaches for understanding the neural basis of pain empathy emphasize the dynamic construction of networks underlying this multifaceted social cognitive process. Inter-subject phase synchronization (ISPS) is an approach for exploratory analysis of task-fMRI data that reveals brain networks dynamically synchronized to task-features across participants. We applied ISPS to task-fMRI data assessing vicarious pain empathy in healthy participants (n = 238). The task employed physical (limb) and affective (face) painful and corresponding non-painful visual stimuli. ISPS revealed two distinct networks synchronized during physical pain observation, one encompassing anterior insula and midcingulate regions strongly engaged in (vicarious) pain and another encompassing parietal and inferior frontal regions associated with social cognitive processes which may modulate and support the physical pain empathic response. No robust network synchronization was observed for affective pain, possibly reflecting high inter-individual variation in response to socially transmitted pain experiences. ISPS also revealed networks related to task onset or general processing of physical (limb) or affective (face) stimuli which encompassed networks engaged in object manipulation or face processing, respectively. Together, the ISPS approach permits segregation of networks engaged in different psychological processes, providing additional insight into shared neural mechanisms of empathy for physical pain, but not affective pain, across individuals.

Abstract:The socio-emotional condition during the COVID-19 pandemic subsidises the (re)modulation of interactive neural circuits underlying risk assessment behaviour at the physical, emotional, and social levels. Experiences of social isolation, exclusion, or affective loss are generally considered some of the most “painful” things that people endure. The threats of social disconnection are processed by some of the same neural structures that process basic threats to survival. The lack of social connection can be “painful” due to an overlap in the neural circuitry responsible for both physical and emotional pain related to feelings of social rejection. Indeed, many of us go to great lengths to avoid situations that may engender these experiences. Accordingly, this work focuses on pandemic times; the somatisation mentioned above seeks the interconnection and/or interdependence between neural systems related to emotional and cognitive processes such that a person involved in an aversive social environment becomes aware of himself, others, and the threatening situation experienced and takes steps to avoid daily psychological and neuropsychiatric effects. Social distancing during isolation evokes the formation of social distress, increasing the intensity of learned fear that people acquire, consequently enhancing emotional and social pain.
Time-varying phase synchronization of resting-state functional magnetic resonance imaging reveals a shift toward self-referential processes during sustained pain

Abstract:Abstract Growing evidence has suggested that time-varying functional connectivity between different brain regions might underlie the dynamic experience of pain. This study used a novel, data-driven framework to characterize the dynamic interactions of large-scale brain networks during sustained pain by estimating recurrent patterns of phase-synchronization. Resting-state functional magnetic resonance imaging signals were collected from 50 healthy participants before (once) and after (twice) the onset of sustained pain that was induced by topical application of capsaicin cream. We first decoded the instantaneous phase of neural activity and then applied leading eigenvector dynamic analysis on the time-varying phase-synchronization. We identified 3 recurrent brain states that show distinctive phase-synchronization. The presence of state 1, characterized by phase-synchronization between the default mode network and auditory, visual, and sensorimotor networks, together with transitions towards this brain state, increased during sustained pain. These changes can account for the perceived pain intensity and reported unpleasantness induced by capsaicin application. In contrast, state 3, characterized by phase-synchronization between the cognitive control network and sensory networks, decreased after the onset of sustained pain. These results are indicative of a shift toward internally directed self-referential processes (state 1) and away from externally directed cognitive control processes (state 3) during sustained pain.

Signal Transduction and Targeted Therapy

Abstract:Pain is estimated to affect more than 20% of the global population, imposing incalculable health and economic burdens. Effective pain management is crucial for individuals suffering from pain. However, the current methods for pain assessment and treatment fall short of clinical needs. Benefiting from advances in neuroscience and biotechnology, the neuronal circuits and molecular mechanisms critically involved in pain modulation have been elucidated. These research achievements have incited progress in identifying new diagnostic and therapeutic targets. In this review, we first introduce fundamental knowledge about pain, setting the stage for the subsequent contents. The review next delves into the molecular mechanisms underlying pain disorders, including gene mutation, epigenetic modification, posttranslational modification, inflammasome, signaling pathways and microbiota. To better present a comprehensive view of pain research, two prominent issues, sexual dimorphism and pain comorbidities, are discussed in detail based on current findings. The status quo of pain evaluation and manipulation is summarized. A series of improved and innovative pain management strategies, such as gene therapy, monoclonal antibody, brain-computer interface and microbial intervention, are making strides towards clinical application. We highlight existing limitations and future directions for enhancing the quality of preclinical and clinical research. Efforts to decipher the complexities of pain pathology will be instrumental in translating scientific discoveries into clinical practice, thereby improving pain management from bench to bedside.

Abstract:Acute pain is a physiological response that causes an unpleasant sensory and emotional experience in the presence of actual or potential tissue injury. Anatomically and symptomatically, chronic pathological pain can be divided into three distinct but interconnected pathways, a lateral "painfulness" pathway, a medial "suffering" pathway and a descending pain inhibitory circuit. Pain (fullness) can exist without suffering and suffering can exist without pain (fullness). The triple network model is offering a generic unifying framework that may be used to understand a variety of neuropsychiatric illnesses. It claims that brain disorders are caused by aberrant interactions within and between three cardinal brain networks: the self-representational default mode network, the behavioral relevance encoding salience network and the goal oriented central executive network. A painful stimulus usually leads to a negative cognitive, emotional, and autonomic response, phenomenologically expressed as pain related suffering, processed by the medial pathway. This anatomically overlaps with the salience network, which encodes behavioral relevance of the painful stimuli and the central sympathetic control network. When pain lasts longer than the healing time and becomes chronic, the pain- associated somatosensory cortex activity may become functionally connected to the self-representational default mode network, i.e., it becomes an intrinsic part of the self-percept. This is most likely an evolutionary adaptation to save energy, by separating pain from sympathetic energy-consuming action. By interacting with the frontoparietal central executive network, this can eventually lead to functional impairment. In conclusion, the three well-known pain pathways can be combined into the triple network model explaining the whole range of pain related co-morbidities. This paves the path for the creation of new customized and personalized treatment methods.

Abstract:The International Association for the Study of Pain (IASP) revised its definition of pain to "an unpleasant sensory and emotional experience." Three recent recommendations for understanding pain if there are no clear brain correlates include eliminativism, multiple realizability, and affordance-based approaches. I adumbrate a different path forward. Underlying each of the proposed approaches and the new IASP definition is the suspicion that there are no specific correlates for pain. I suggest that this basic assumption is misguided. As we learn more about brain function, it is becoming clear that many areas process many different types of information at the same time. In this study, I analogize how animal brains navigate in three-dimensional space with how the brain creates pain. Underlying both cases is a large-scale combinatorial system that feeds back on itself through a diversity of convergent and divergent bi-directional connections. Brains are not like combustion engines, with energy driving outputs via the structure of the machine, but are instead more like whirlpools, which are essentially dynamic patterns in some substrates. We should understand pain experiences as context-dependent, spatiotemporal trajectories that reflect heterogeneous, multiplex, and dynamically adaptive brain cells.
