Perfusion MR Imaging in Evaluating the Histopathologic Grade of Cerebral Glioms

Abstract:Objective: To determine the value of perfusion MR imaging in assessing the histophathologic grade of cerebral gliomas. Methods:Twentry-eight patients with proven cerebral gliomas were studied preoperatively, using a first-pass Gd-DTPA T2*-weighted echoplanar perfusion sequence (GRE-EPI). Regional cerebral blood volume (rCBV) maps were calculated, and rCBV measurements of lesions were comparining with ROI values in contralateral normal white matter (rCBV2). At the same time, when the lesions was predominantly in cortical or in the basal ganglia or thalamus rCBV1 measurements of lesions were acquired with the ROI placed over the contralateral cortex or deep gray matter. The student t test was used to determine the statistically difference between the mean of rCBV in high-grade gliomas and that in low-grade gliomas. Results: The mean value of the rCBV1 was 1.38±0.36 in the low-grade gliomas and 4.32±3.41 in the high-grade gliomas. The mean value of the rCBV2 was 2.83± 0.76 in the low-grade gliomas and 9.71±6.23 in the high-grade gliomas. A significant difference in each rCBV1 or rCBV2 was found between the low- and high-grade gliomas. Additionally, there was a very close correlation between the rCBV1 and rCBV 2(r=0.736,P0.001). Conclusion: Perfusion MR imaging is a very useful tool in the evaluation of the histopathologic grade of cerebral gliomas.
