The Analysis and Prevention of Risk Factors Determining Severe Ventricular Arrhythmias During Perioperative Period in Patients with Giant Left Ventricle Undergoing Cardiac Valve Surgery

Abstract:Objective To analyze the risk factors of severe ventricular arrhythmias and its prevention during perioperative period in patients with giant left ventricle undergoing cardiac valve surgery. Methods The clinical data of 153 patients with preoperative left ventricular end-systolic diameter (LVESD) over 5.0 cm and end-diastolic diameter (LVEDD) over 7.0 cm, who underwent cardiac valve surgery between Jan. 1991 and Dec. 2000, were studied retrospectively. The risk factors of severe ventricular arrhythmias were identified by using univariate and multivariate logistic regression analysis. Results The incidence of severe ventricular arrhythmias during operative and early postoperative period was 22.88%. The independent risk factors for frequent,couplets or multiform premature ventricular contractions were preoperative ventricular arrhythmias during operation, and preoperative ventricular arrhythmias, LVESD≥7.0 cm, NYHA functional class IV and post-operative low cardiac output syndrome after operation, respectively. The independent risk factors for ventricular tachycardia or fibrillation were left ventricular end-diastolic volume index≥200 ml/m2 and the dosage of lidocaine0.33 mg/min used during perioperative period,preoperative ventricular arrhythmias and LVESDE≥7.0 on after operation,respectively. The early mortality was higher in patients with severe ventricular arrhythmias (25.71%) than in those without (6.78% )(P0.01). Ventricular fibrillation during or after operation was one of the independent risk factors for early operative death. Conclusion The high-risk factors for perioperative severe ventricular arrhythmias in patients with giant left ventricle are related with preoperative ventricular arrhythmias, extreme left ventricular dilatation and low cardiac function. The keypoints of preventing severe ventricular arrhythmias perioperatively for high-risk patients are to maintain electrolytes and acid-base equilibrium, to strengthen myocardial protection, to use effective antiarrhythmic drugs or temporary pacing and intra-aortic balloon contrapulsation properly.
