Relationship between cagA vacA genotypes and antibiotic resistance of Helicobacter pylori strains

Abstract:AIM: To study the relationship between cagA vacA genotypes and incidences of antibiotic-resistant H. pylori strains. METHODS: MICs of antibiotics on 50 H. pylori strains were determined by agar dilution methods. MIC 50 and MIC 90 were also calculated. H. pylori vacA (s1a, s1b, s2, m1, m2) and cagA genotypes were classified by multiplex PCR method. RESULTS: MIC 30 and MIC 90 of clarithromycin, amoxicillin, tinidazole and metronidazole were < 0.016 mg·L -1, 0.016 mg·L -1; 0.016 mg·L -1, 0.047 mg·L -1; 16.000 mg·L -1, 52.000 mg · L -1 25.140 mg · L -1, 76.800 mg © L -1; respectively. The incidences of clarithromycin, amoxicillin, tinidazole or metronidazole-resistant H. pylori was 0%, 0%, 20% and 38%, respectively. In tinidazole-sensitive H. pylori strains, vacA s1a, s1b and s2 types occupied 50.0%, 42.5% and 7.5% respectively; vacA m1 and m2 types occupied 60.0% and 40.0%; vacA s1a/ m1, s1b/ m1, s1a/ m2, s1b/ m2 and s2/ m2 types occupied 32.5%, 27.5%, 17.5%, 15.0% and 12.5% respectively; cagA + and cagA - types were 87.5% and 12.5%. In tinidazole-resistant H. pylori strains, vacA s1a, s1b and s2 types occupied 50.0%, 40.0% and 10.0% respectively; vacA m1 and m2 types occupied 40.0% and 60.0%; vacA s1a/ m1, s1b/ m1, s1a/ m2, s1b/ m2 and s2/ m2 types occupied 20.0%, 20.0%, 30.0%, 20.0% and 10.0% respectively; cagA + and cagA - types were 80.0% and 20.0%. CONCLUSION: The incidence of metronidazole-resistant H. pylori strains was higher than the incidences of other antibiotics. Between tinidazole-sensitive and tinidazole-resistant H. pylori strains, the distributions of cagA or vacA genotypes were not significantly different.
