Experimental Study on Paradoxical Phenomenon in Type 1 Diabetic Heart:Increased Resistance to Ischemia/reperfusion Injury

Abstract:Objective To investigate the sensitivity of type 1 diabetic heart to ischemia/reperfusion injury and its possible mechanism. Methods Male SD rats were induced to be diabetic by peritoneal injection of streptozotocin (60 mg/kg). Three to four weeks later, the hearts were isolated and perfused in a modified Langendorff mode and subjected to 30 min ischemia and finally followed by 40 min reperfusion. Heart function, and electrocardiogram were recorded. Coronary creatine phosphate kinase (CPK) released, malondialdehyde (MDA) and lactate were measured. Results Baseline diabetic heart function was decreased as compared with that of normal hearts. However, systolic and diastolic heart function in diabetic hearts recovered rapidly and to a better level at the end of reperfusion after ischemia than that of normal hearts. Diabetic hearts also showed less severe reperfusional arrhythmias and myocardial CPK leakage than normal hearts. Lactate production was reduced and MDA production was increased in diabetes. Conclusion Type 1 diabetic hearts show decreased injury during ischemia and reperfusion. The depressed glycolysis may contribute to cardioprotection in type 1 diabetic hearts.
