Efficacy of inhalation of NO on endotoxin-induced acute respiratory distress syndrome in dogs

Abstract:Objective To investigate the effects of inhalation of nitric oxide (NO) on endotoxin-induced acute respiratory distress syndrome (ARDS) . Methods Twelve Beangles dogs of both sexes weighing 8.0-12. 5kg were anesthetized with intravenous 1 % pentobarbital 15-20 mg ·kg-1 . The animals were mechanically ventilated after tracheal intubation. Swan-Ganz catheter was placed via femoral vein for hemodynamic monitoring. Femoral artery was cannulated for direct BP monitoring. Septic shock was induced with iv endotoxin 0.55mg·kg-1 (055:B5, No.L263, Sigma) . The animals were randomly allocated to one of two groups: control group in which the animals were only mechanically ventilated ( n = 6); NO inhalation group received NO inhalation in addition to mechanical ventilation ( n - 6) . The concentration of NO inhaled was gradually increasing from 10,20,40 to 80 ppm. Each concentration was maintained for 1h. Then the NO concentration was reduced to 10 ppm and maintained for 6h. The efficacy of NO inhalation was evaluated according to following parameters: (1) systemic and pulmonary hemodynamic parameters; (2) PaO2/FiO2 , Qs/Qt, dynamic compliance (Cdyn), VD/VT and airway resistance (Rrs); (3) peripheral white blood cell count; (4) blood and urine nitrite/nitrate (NO2- /NO3- ) ratio; (5) methemoglobin content and (6) survival rate. Results (1) Inhalation of 10,20,40 and 80 pom NO resulted in increase in PaO:/FiO2 by 50% , a reduction in VD/VT to 0.36, decrease in PVRI by 50% , decrease in Qs/Qt 25 % and increase in Cdyn. There were significant differences in these parameters between the two groups (P 0.01) . The survival time was (10.2± 1.7) h in NO group, significantly longer than that in control group (4.4 ± 1.2) h ( P 0.01) . (2) When NO concentration was reduced to 10 ppm PaO2/FiO2 , VD/VT, Qs/Qt, Cdyn and PVRI deteriorated again in the NO group. (3) NO inhalation reduced the level of decrease in peripheral WBC count ( P 0.01). Conclusions Inhalation of 10-80 ppm NO can produce a certain efficacy on the endotoxin-induced ARDS in dogs through improving pulmonary circulation and ventilation/perfusion ratio.
