The Role of TIMI Risk Score in Risk Stratification in Patients with Acute Coronary Syndromes Without Persistent ST Elevation

Abstract:Objectives To investigate the role of TIMI risk score in risk stratification and predicting outcomes in patients with acute coronary syndromes (ACS) without persistent ST elevation.Methods From July 2000 to June 2001, 248 consecutive patients with non-ST elevation ACS were examined by inquiry of history,physical examination and ECG or examination of cardiac injury markers. TIMI risk scores were calculated for 7 variables and risk stratification of patients was done according to scores. The effects of TIMI risk score on combined cardiac events during hospital stay and at follow-up of 7.2±3.8 months were analyzed.Results The incidences of combined cardiac events increased progressively by 6.7% to 44.4% with ascending TIMI risk scores.When the capacity for predicting combined cardiac events was compared between the TIMI risk score and troponin I(cTnI) level,out of 41 cases with cardiac events,38 cases were distributed in patients with cTnI-positive and among the patients with cTnI-positive, the incidence of combined cardiac events increased significantly with the rise in TIMI risk score.Conclusion TIMI risk score of patients with ACS without persistent ST elevation is a simple, practical and effective tool, and may provide more information of quantitative characteristics than single test for cTnI in risk stratification and prediction of clinical outcomes.
