Abstract:Upper cervical spine injury is a common spine injury in the clinic,which mainly refers to the fracture,ligament tear,and dislocation of atlantoaxial and its accessory structures due to violence.The injury of upper cervical ligament complex is the main cause of instability after injury.The controversy lies in how to evaluate the effect of ligaments on stability.The injuries of different sports units may involve different ligament composite structures which show different instability characteristics and types,requiring special attention.In addition to the shape and location of the fracture,fixation method and range of the surgical treatment should be selected mainly based on whether there are stable or unstable types.The author mainly expounds the characteristics of upper cervical injury,clinical classification,diagnosis and treatment measures to discusses the difficulties and focus of the treatment of upper cervical injury,so as to provide reference for clinical treatment.

Abstract:Objective To study the diagnosis and treatment for distractive extension injuries of the cervical vertebrae.Method From 2000 to 2005.the clinical and image data of fifty-six patients in the Second Affiliated Hospital of Zhejiang University College of Medicine with distractive extension injuries of the cervical vertebrae were studied and treated with anterior discectomy or subtotal vertebrectomy in the second affiliated Hospital of zhejiang university collegeof medicine,bone grafting and internal fixation with plate.Results The follow-up period was six to seventeen months in fourty-eight patients.The neurological recovery was found after operation in five of nine case with complete spinal cord injuries and thirteen patient had complete recovery.Neck pain completely disappeared in 6 patients with oboslete injury and neurological recovery was improved in 4 patients.Condusions MRI examination is the essential approach to diagnoze distractive extension injuries of the cervical vertebran,and early operation,including anterior discectomy,bone graftillg and internal fixiation with plate,is a best choice of surgical interrention to achieve cervical stabilization and neurological improvement.
Posterior Atlantoaxial Transpedicular Screw Fixation of Unstable Atlas Fractures Combined with Rupture of Transverse Ligament

Abstract:Objective To determine the outcome of unstable atlas fractures combined with rupture of transverse ligament treated by posterior atlantoaxial transpedicular screw fixation.Methods A retrospective case series study was made on 17 patients with unstable atlas fractures combined with rupture of transverse ligament treated by posterior atlantoaxial transpedicular screw fixation and fusion from January 2008 to December 2015.There were 13 males and 4 females,with age range of 34-69 years (mean,47.8 years).All atlas fractures were Jefferson fractures (Levine-Edwards type Ⅲ).Classification of transverse ligament rupture was type Ⅰ in 12 patients and type Ⅱ in 5 patients.No patients had neurologic deficit [American spinal injury association (ASIA) classification grade E].Operation time,blood loss,implant failure,bone fusion and visual analogue scale (VAS) were recorded after operation.Results Operation time was 85-120 min (mean,102 min).Blood loss was 90-150 ml (mean,115 ml).All patients were followed up for 10-20 months (mean,17.8 months).At the final follow-up,all patients achieved bone union,with no implant loosening or breakage happened.VAS was improved from preoperative (5.5 ± 1.8) points to (2.4 ± 1.5) points at tbe final follow-up (P ＜ 0.05).ASIA Grade E remained in all patients.Conclusion Posterior atlantoaxial transpedicular screw fixation of unstable atlas fractures combined with rupture of transverse ligament is a safe and effective surgical procedure that is able to restore the atlanto-axial vertebral stability and relieve pain.

Abstract:Posterior screw fixation on atlas includes posterior screw fixation within lateral mass of atlas and in atlas via posterior arch and lateral mass.Atlas is anatomically capable for screw fixation in its lateral mass and via its posterior arch and lateral mass.But it is necessary to pay attention to the thickness of the posterior arch at the thinnest part of the groove and avoid injuring the vertebral artery and the internal carotid artery.Routine lateral radiographs and CT scans of the cervical spines of patients must be performed before surgery,which can be of definite help in locating the entry point and regulating the screw direction.The screws have good pullout strength(even when placed unicortically),and constructs using C_1 lateral mass screws and C_2 pars screws connected with rods are as stable as transarticular screws with posterior wire-graft constructs.At present there are three popular techniques:the Harms technique,as well as Xia and Gupta,which is screw fixation within lateral mass of atlas,and the Tan and Ma technique,which are both screw fixation in atlas via posterior arch and lateral mass.Clinical results showed that posterior screw fixation on atlas could relieve spinal and neural oppression immediately,improve clinical symptoms,have good immediate postoperative stability,keep the movement of occipitocervical region,and showed its advantages in very high successful fusion rate,and very few instrument failure,morbidity and complications.

Abstract:Background: Multiple fractures of the atlas and axis are rare. The management of multiple fragment axis fractures and unstable atlas fractures is still challenging for the spinal surgeon. There are no published reports of similar fractures with 3-part fracture of axis associated with an unstable atlas fracture.Case summary: We present a patient with concurrent axis and atlas fractures, which have not been reported. The patient suffered hyperextension injury with neck pain and numbness of the bilateral upper extremity associated with weakness after a 2-m fall. The axis fractures included an odontoid type IIA fracture and traumatic spondylolisthesis of C2-C3. The atlas fracture was unstable. The neurological examination manifested as central canal syndrome, which was due to the hyperextension injury of cervical spine and spondylolisthesis of C2-C3. The patient was diagnosed as multiple unstable upper cervical fractures with spinal cord compromise. We performed posterior arthrodesis of C1-C3. Postoperatively, the patient showed neurological improvement, and C1-C3 had fused at the 3-month follow-up.Conclusion: Posterior arthrodesis of C1-C3 could provide a stable fixation for the 3 parts of axis (an odontoid type IIA fracture and traumatic spondylolisthesis of C2-C3) combined an unstable atlas fracture. Both the patient and the doctor were satisfied with the results of the treatment. So posterior arthrodesis of C1-C3 is a suitable treatment option for the treatment of a concurrent unstable atlas fracture and multiple fractures of the axis.

Abstract:Objective:To explore the methods and the reasons to the treatment of atlas fracture. Method:28 consecutive patients were investigated.25 received conservative treatment,15 were immobilized by Halo-vest and 10 by cervical collar.3 received surgery. Result:The 25 patients who received conservative therapy were followed 4~48 months,no atlantoaxial instability occurred.The 3 patients who received operation achieved arthrodesis. Conclusion:Atlas fracture should received conservative therapy,operation should be performed in rare cases with special reasons.

Abstract:Objective To study the clinical and radiographic characteristics of complicated axis fractures combined with adjacent segment instability and explore reasonable surgical treatment strategy. Methods A retrospective study was performed on 21 patients with axis fractures treated from August 2003 to June 2009. There were 14 males and 7 females at mean age of 34 years. The treatment strategy was based on the fracture type and the stabilities of adjacent atlantoaxial joint and intervertebral C2/3.Treatment strategies included anterior C2/3 interbody discectomy and fusion, anterior cervical plate internal fixation, odontoid screw fixation, posterior C1-2 pedicle screw fixation, cervical lateral mass screw fixation or combined anteroposterior approach. Results All patients were immobilized in a hard collar for thee months and followed up for 6-36 months (average 12 months), which showed bony fusion and cervical stability, with no intraoperative surgery-related complications such as loosening, extrusion or breakage of fixation, vertebral artery injury, nerve damage, cerebrospinal fluid leakage or wound infection. Neurological recovery was observed in five patients. Conclusions For complicated atlas fractures, correct identification of fracture type and instability disturbance of adjacent atlantoaxial joint and C2/3 as well as active treatment can conduce to better effect.

Abstract:Upper cervical fracture-dislocations are series of pathological injuries occurred in occipital, atlas and axis caused by traumatic impact, which often results in severe clinical consequences, such as paraplegia, quadriplegia, and even death. In light of the potential severe clinical outcomes, it is important to define the type of upper cervical fracture-dislocations depending on which the clinician can deal with the disease properly. Owing to its specific anatomical structures, the upper cervical fractures occurr to the unique osseous structures or ligamentous connections always present in predictable patterns. Common fracture include occipital condylar fracture, atlanto-occipital dislocation, atlas fracture, and the three types of fractures of axis, including odontoid fracture, Hangman's fracture and axis body fracture. In this paper we will review the literatures about the classification and management of upper cervical fracture, to help the clinicians to treat their patients better.

Abstract:The thoracolumbar junction is the junction of the thoracic and lumbar spine and is subjected to high biomechanical stress. With the rapid development of the society, the morbidity of thoracolumbar spine increases every year. Thoracolumbar fracture may affect the stability of the spine and even result in paralysis via the compression of the spinal cord. As a result of advancements in modern medicine, the theoretical and practical aspects concerning thoracolumbar fracture have made great progress. However, due to the complexity of thoracolumbar fracture, controversy still exists in the treatment strategies and methods for thoracolumbar fractures. In order to provide reference for clinical practice, the research progress in the treatment of thoracolumbar fracture is reviewed in this paper.

Abstract:Osteoporotic fractures are the most common bone disease in the elderly. The spine is the most common site for osteoporotic fractures, while osteoporotic fractures in the thoracolumbar segment of the spine account for more than 90% of all spinal fractures. In order to standardize the diagnosis and treatment of osteoporotic thoracolumbar fracture, the authors analyze the diagnosis, treatment, surgical methods and related hot issues of osteoporotic thoracolumbar fractures with a review of the current literatures in order to provide references for the standardized clinical diagnosis and treatment.

Abstract:A 29-year-old man was admitted to our department after a road traffic crash. He complained about severe neck pain and limitation of cervical spine. No neurologic deficit was found. The patient underwent a computed tomography scanning and a magnetic resonance imaging scanning of the cervical spine. Computed tomography images showed the fracture of anterior arch of atlas on the left side (Fig. 1), and the coronal reconstructed image showed that there was no lateral mass displacement of C1 over C2 (Fig. 2). Magnetic resonance imaging showed that the transverse atlantal ligament was intact (Fig. 3). Therefore, the patient was asked to wear a collar for 3 months. The patient returned to his normal work free of symptoms at 3 months after injury. Fig. 2Coronal reconstructed image showed that there was no lateral mass displacement of C1 over C2. Management of Combination Fractures of the Atlas and Axis: a Report of Four Cases and Literature Review.

Abstract:Four cases of combination fractures of the atlas and axis are presented. Three types of management were performed: plaster immobilization, odontoid screw fixation combined with atlantoaxial pedicle screw fixation, occipito-cervical fusion with anterior operation by staged. Based on a literature review and our experience, treatment strategies is discussed according to the stability of the upper cervical spine and neurological involvement, with a reminder that combined injuries in the upper cervical spine should be sought in any patient with a cervical injury and early surgical solution may bring benefits once injury attack.

Abstract:OBJECTIVE:To study the clinical and radiographic characteristics of axial fracture with adjacent section instability and the diagnosis and surgical treatment thereof.METHODS:Seventeen patients of axial fracture with adjacent section instability, 11 males and 7 females, aged 34 (23-56) underwent different treatment patterns depending on the fracture type and stability of the atlanto-axial joint and C2-3: resection of C2-3 intervertebral disk and bone grafting, anterior interbody fusion and plate internal fixation, odontoid screw fixation, posterior C2 pedicle screw fixation, odontoid screw fixation combined with bilateral C-2 pedicle screw fixation. Follow-up was conducted for 12 (6-36) months.RESULTS:Postoperatively all patients were immobilized in a hard collar for 3 months. Bony fusion was obtained in all patients in 3 months. There were no operation-related complications, such as spinal cord injury, cerebrospinal fluid leakage, vertebral artery injury, etc. No re-dislocation was found. Neurological recovery was observed in the 3 patients who presented neurological deficits.CONCLUSION:The type of atlas fracture with adjacent section instability and the impact thereof on the stability of the atlanto-axial joint and C2/3 neck joint should be identified early. Surgical treatment results in better outcome.

Abstract:Objective To investigate the effect of Atlas fixator system in treating thoracolumbar spine fracture with spine cord injury.Methods The clinical date of 45 cases of thoracolumbar fracture were analysed retrospectively.All patients were operated by AF internal fixation in posterior.25 patients of those aminectory was done,spine canal was effectively limited decompressed in these cases the fracture blocks entered into the vertebral canal over 33%.Results All fractures and dislocation acquired good reduction.The main posterior convex angle of all cases was corrected from 26° preoperatively to 5° postoperatively.The patients of Frankel's were improved by 1～2 grade after operation.Conclusion The AF fixation is effective procedure for treatment of thoraclumbar spine fracture with spine cord injury.The operation is characteristic of little thauma,less bleeding,less postoperative complication and good recovery.It is easy to operate.It provides exact reduction and solid fixation.

Abstract:The acute combination fractures of the atlas and axis in adults have a higher rate of neurological injury and early death compared with atlas or axial fractures alone. Currently, the diagnosis and treatment choices of acute combination fractures of the atlas and axis in adults are controversial because of the lack of standards for implementation. Non-operative treatments have a high incidence of bone nonunion and complications, while surgeries may easily lead to the injury of the vertebral artery, spinal cord and nerve root. At present, there are no evidence-based Chinese guidelines for the diagnosis and treatment of acute combination fractures of the atlas and axis in adults. To provide orthopedic surgeons with the most up-to-date and effective information in treating acute combination fractures of the atlas and axis in adults, the Spinal Trauma Group of Orthopedic Branch of Chinese Medical Doctor Association organized experts in the field of spinal trauma to develop the Evidence-based guideline for clinical diagnosis and treatment of acute combination fractures of the atlas and axis in adults ( version 2023) by referring to the "Management of acute combination fractures of the atlas and axis in adults" published by American Association of Neurological Surgeons (AANS)/Congress of Neurological Surgeons (CNS) in 2013 and the relevant Chinese and English literatures. Ten recommendations were made concerning the radiological diagnosis, stability judgment, treatment rules, treatment options and complications based on medical evidence, aiming to provide a reference for the diagnosis and treatment of acute combination fractures of the atlas and axis in adults.
Diagnosis and Treatment of Odontoid Fracture in Children

Abstract:Objective To explore the clinical characteristics and treatment strategies of odontoid fractures in children.Methods A total of 7 pediatric patients with odontoid fractures were analyzed retrospectively.There were 5 males and 2 females with an average age of 5.6 (3-12) years.The causes were traffic accident (n =2) and fall (n =5).All of them belonged to type Ⅱ fracture according to the Anderson-DAlonzo classification.And the factures were synchondrosis (n =5) and real (n =2).Two patients had neurologic involvements of unilateral extremity paresthesia (n =1) and high paraplegia (n =1).Five patients underwent closed reduction and Minerva cast immobilization.One senior boy had atlantoaxial fixation and fusion while another patient with unilateral extremity paresthesia temporary atlantoaxial fixation.Results The patient with high paraplegia achieved fair reduction after Minerva cast immobilization,but died 1 month later from severe pulmonary infection.The reminder was followed up for a median period of 34(7-84) months.Complete fracture union was achieved.The median healing time was 2.6 (2-4) months.Four patients with Minerva cast immobilization regained excellent cervical motion.The cervical rotation was merely 30 for patients of atlantoaxial fixation and fusion and 65 for those of temporary atlantoaxial fixation.The patient with unilateral extremity paresthesia achieved complete neurologic recovery at 3 months post-operation.Conclusions Most odontoid fractures in children are synchondrosis type.Closed reduction and Minerva cast immobilization is an effective first option for pediatric odontoid fractures.Open reduction and atlantoaxial fixation results in cervical rotation limitation.And it may be used for some select patients.Internal fixation should removed after fracture union.

Abstract:The majority of cervical spine injuries in children occur in the upper cervical spine, of which odontoid fracture is the most common. Odontoid fracture in children is a very insidious injury. Due to the unclear language and incompatible physical examination, the disease is often missed diagnosis. Because the child axis is still in the developmental segment, including 4 synchondrosis and 6 ossification centers, there are obvious anatomical and biological differences between the child odontoid fracture and the adult. Therefore, the choice of treatment is different from that of adults. This article will introduce the development of odontoid in children, and summarize the injury characteristics, clinical classification and treatment of odontoid fracture in children.
