Testicular tissue bleeding on the assessment of testicular viability in testicular torsion surgery

Abstract:Objective To investigate the reliability of testicular tissue bleeding to assesstesticular viability during surgery.MethodsThe affected testes of 15 cases with testiculartorsion were incised for obtaining a wedge biopsy and then made a deep incision throughmedulla.After waiting up to 10 minutes to assess any fresh arterial bleeding from the cutsurface,the bleeding were categorized using three grades:grade Ⅰ(bleeding or oozing when thebiopsy was obtained),grade Ⅱ(no bleeding immediately after the incision but starting within10 minutes),and grade Ⅲ(no bleeding within 10 minutes).Testes with grade Ⅰ and Ⅱ weresaved,and grade Ⅲ testes were removed.The histopathological diagnoses were used asreference standard.Results15 cases with testicular torsion were categorized by bleedinggrading.Orchidectomy were carried out in 5 cases with grade Ⅲ,while orchidopexy wereperformed in 6 cases with grade Ⅰand 4 cases with grade Ⅱ.Testicular atrophy was detectedon postoperative 1st and 3rd month in 2 patients with grade Ⅱ,which underwent a secondoperation to remove testis.The histopathological results were necrotic in all 5 cases with gradeⅢ and 2 cases with grade Ⅱ,indeterminate(tissue haemorrhage with small area of necrosis)in other 2 cases with grade Ⅱ and haemorrhage in all 6 cases with grade Ⅰ.ConclusionTesticular tissue bleeding is not only a accurate predictor of a twisted testis, butalso the indicator of the final surgical decision on whether to save the testis or to incise itduring surgery.[
