Evaluation of Gastric Carcinoma with spiral CT: Comparison with Upper Gastrointestinal Series, Fiberoptic Gastroscopy

Abstract:Objective To assess the role of spiral CT in the evaluation of gastric carcinoma compared with upper gastrointestinal series (UGI) and fiberoptic gastroscopy (FG).Methods In 60 gastric carcinoma patients proved histologically, the results of SCT imaging were compared to those with UGI and FG. Two observers blindly evaluated images with all three techniques in terms of image quality, artifacts, and diagnostic confidence. Sensitivities of lesion detection and classification based on Borrmann's classification of SCT, UGI and FG, as well as the accuracy of staging of SCT were determined by comparing them to surgical and histological findings. Results Overall image quality of SCT, UGI, FG was considered similar by both observers (Ρ 0.05). Excess fluid remains in the stomach and patient respiratory movement during breath holds was the reason causing severe artifacts (5.8%) that influenced the diagnostic evaluation. There was no statistically significant difference of lesion detection sensitivity between SCT, UGI and FG(Ρ0.05, Chi-square test). Six cases of early gastric carcinoma were all detected by SCT. Both observers considered the sensitivity of Borrmann's classification of SCT was statistically significant higher than that of UGI (Ρ 0.05) but similar to FG(Ρ0.05).Conclusion The performance of SCT is equivalent to UGI and FG in the detection of gastric carcinoma, superior to UGI but similar to FG in the classification. SCT is capable of providing staging information of gastric carcinoma.
