Neurofibromatosis Dystrophic Scoliosis:Imaging Feature and Its Clinical Significance

Abstract:Objective There is high risk of surgical complication for the scoliosis of neurofibromatosis, that the fusion rate,correction of deformity, and post-operative follow-ups are different from those of other spinal deformity mainly due to the dystrophic change. This study aims to analyze the imaging feature of neurofibromatosis scoliosis and to discuss its clinical significance. Methods There were 34 patients involving with the neurofibromatosis scoliosis, who had been admitted for surgical treatment in this spinal service from July, 2000 through Dec. 2002. A retrospective study of radiographic imaging data of these patients was made,additionally with the X ray、MRI、CT examination, in order to find out the most characteristic features. The pre-operative coronal Cobb angle was ranging from 50° to 164°, with a mean value of 87.6°, and the Cobb angle of thoracic kyphosis was ranging from 40° to 100° with the mean value of 72.9°. Results The radiographic features of scoliosis in neurofibromatosis were quite different from those of idiopathic scoliosis, among which the short angular curvature 100%、dural ectasia 100%、vertebral scalloping deformity 87%、vertebral wedging 85% on either coronal or sagittal plane and the spinal canal enlargement were the most characteristic dystrophic deformities.Conclusion The dystrophic deformity bears the potential risk of causing the intra-operative dura tear、damage of spinal cord、difficulty of performing instrumentation and the post-operative pseudo-arthritis and the lose of correction.Therefore,to identify the dystrophic changes in neurofibromatosis scoliosis is of great importance in clinical management of this special entity.
